    Cross-sectional study to evaluate various cardiovascular manifestations in pregnant women with preeclampsia using 2d echocardiogram

    Preeclampsia is one of the most frequent complications found in pregnancy. Preeclampsia was initially thought to cause poor long-term cardiovascular outcomes, but recent studies have shown its effect to be more early and severe. Aims: To reduce the morbidity and mortality for all patients with hypertension in pregnancy through any cardiovascular problem that is directly caused by hypertension in pregnancy Methods: it is a cross-sectional and observational study of pregnant women with preeclampsia in the reproductive age group (15–44 years) diagnosed with preeclampsia in the antenatal ward/HDU/MICU at Niloufer hospital were evaluated with 2d echo on diagnosis and a follow up 2d echo after delivery was done and changes in 2d echo were noted, patients with prior changes in 2d echo have been excluded from the study. About 113 preeclamptic patients were studied and further classified as non-severe and severe preeclampsia, and their results were compared. Results: This study shows significant cardiovascular dynamics changes in subjects with preeclampsia (both severe and non-severe) which can be studied by 2 D echo. Non-severe preeclampsia was associated with more normal birth weight. This was found statistically significant in our study. IUD and early neonatal death were more associated with severe preeclampsia when compared to non-severe preeclampsia, which was found statistically significant. Conclusions: Early identification of preeclamptic patients at higher risk of developing cardiovascular complications later in life by undergoing timely echocardiography

    Perinatal outcome in abnormal placental villus proliferation and mesenchymal dysplasia

    Placental mesenchymal dysplasia (PMD) is a rare vascular anomaly which is characterized by mesenchymal stem villous hyperplasia and placentomegaly. Since the modality of treatment changes it is necessary to distinguish PMD from molar pregnancy, placenta mosaicism, chorioangioma, twin pregnancy with co-existent molar pregnancy. On reviewing cases of abnormal placental villus proliferation having features of placental mesenchymal hyperplasia placentomegaly and cystic appearance of placenta in database of our hospital from 2015-2019, we reported 4 cases of abnormal placental villous proliferation. And performed systematic review of existing literature. Provisional   diagnosis of PMD was made as USG and placental morphology showed 30-60% of the placenta with cystic vesicles, placentomegaly with a normal growing fetus. PMD an uncommon vascular anomaly which resembles molar pregnancy but prognosis is different. The fetus was normal in majority of the cases. This clinical entity should be kept in mind to avoid unnecessary termination of pregnancy

    Image Segmentation and Classification for Medical Image Processing

    Segmentation and labeling remains the weakest step in many medical vision applications. This paper illustrates an approach based on watershed transform which are designed to solve typical problems encountered in various applications, and which are controllable through adaptation of their parameters. Two of these modules are presented: the lung cancer detection, a method for the segmentation of cancer regions from CT images, a watershed algorithm for image segmentation and brain tumor detection from MRI images. Various GLCM features along with some statistical features are used for classification using Neural network and Support Vector Machine (SVM). We describe the principles of the algorithms and illustrate their generic properties by discussing the results of both applications in 2D MRI images of Brain tumor and CT images of lung cancer

    Recurrent implantation failure is most distressing condition to both couple as well as doctor in the field of reproductive medicine. Patients have already undergone various ART treatments with no favourable outcome and are drained emotionally as well as economically. To overcome this obstacle a comprehensive approach is needed. In this case report, we are addressing a couple who came to us with primary infertility of 15 yrs, with recurrent IVF failures (four cycles of IUI, seven cycles of ICSI both fresh and frozen and with ovum donation in last two cycle), with poor ovarian reserve and male factor infertility for surrogacy as a last option. After complete evaluation of couple, recurrent Implantation failure is thought to be due to poor endometrium and bad embryo quality, and we counselled couple to go for ovum donation and ICSI before considering surrogacy. To improve endometrial receptivity we performed local endometrial injury, oral estradiol therapy, intrauterine G-CSF instillation and IVIG etc. Simultaneously for better quality of embryo we decided to choose ovum donation due to poor ovarian reserve and advance age of the patient and ICSI was done with husband sperm after treatment of male factor infertility and ET was performed. Patient conceived in first cycle with single live intrauterine pregnancy, her antenatal course was uneventful and delivered a healthy baby at term without any complication

    Association of plasma fibrinogen and development of complications in type 2 diabetes mellitus

    Background: Fibrinogen is a glycoprotein produced by the liver that plays a crucial role in blood clotting. Elevated levels of fibrinogen have been associated with an increased risk of cardio vascular disease, stroke and other chronic conditions. Several studies have investigated the relationship between fibrinogen and diabetes mellitus. Furthermore, fibrinogen has been suggested to play a role in pathogenesis of diabetes and its complications by promoting inflammation and endothelial dysfunction. The aim of the study was done to study association of fibrinogen levels and development of complications of diabetes mellitus. Methods: A cross sectional observational study was conducted at medicine department of Dhiraj Hospital, Vadodara. Total of 114 diabetes mellitus patients were studied. The level of fibrinogen as well as presence of various risk factors like smoking, hypertension, obesity, dyslipidemia and different microvascular and macro-vascular complications were assessed. There were no conflict of interest. Results: Of 114 patients smoking, overweight, hypertension, uncontrolled diabetes and dyslipidemia was reported in 54.4%, 30.7%, 41.2%, 28.1% and 60.5% respectively. Microvascular complication like retinopathy, nephropathy and neuropathy was reported in 34.2%, 40.4% and 21.1% patients respectively. Macrovascular complications like coronary artery disease and stroke was reported in 20.2% and 16.7% patients respectively. Average level of fibrinogen was found higher amongst diabetic patients with microvascular as well as macrovascular complications. Conclusions: Serum fibrinogen level was found to be higher among patients with poor glycemic control, dyslipidemia, hypertension and higher BMI. A positive correlation was found between the level of fibrinogen and various complications of diabetes mellitus.

    Black cotton soil is a soil with low bearing capacity, swelling and shrinkage characteristics. Due to its peculiar characteristics, it forms a very poor foundation material. As black cotton soil is an expansive soil; it creates problem for lightly loaded structure than moderately loaded structure. Through this project, we are trying to study the improvements in the properties of soil by adding coconut fiber of varying percentages. Tests will be conducted to determine liquid limit and unconfined compression strength. Stabilization of soil is an effective method for improving the properties of soil. It has great significance in the future projectsEl suelo de algodón negro es un suelo con baja capacidad de carga, características de hinchamiento y contracción. Por sus características peculiares, forma un material de cimentación muy pobre. Como el suelo de algodón negro es un suelo expansivo; crea un problema para la estructura con una carga ligera que la estructura con una carga moderada. A través de este proyecto, estamos tratando de estudiar las mejoras en las propiedades del suelo agregando fibra de coco en diferentes porcentajes. Se realizarán pruebas para determinar el límite de líquido y la resistencia a la compresión no confinada. La estabilización del suelo es un método eficaz para mejorar las propiedades del suelo. Tiene una gran trascendencia en los proyectos futuros
