    Bilateral Implantation of Scleral-Fixated Cionni Endocapsular Rings and Toric Intraocular Lenses in a Pediatric Patient with Marfan's Syndrome

    The management of ectopia lentis in Marfan's syndrome is challenging. Multiple disease-associated factors conspire to deprive these patients of adequate vision. While optical correction with glasses and contact lenses is usually advocated early on, the irregular astigmatism and even partial aphakia that accompanies advanced cases generally warrant surgical intervention. Several surgical strategies have been devised to manage these challenging cases, including the combination of endocapsular or pars plana lensectomy and iris or scleral fixation of the intraocular lens (IOL) or IOL-bag complex. All of the reported cases have been implanted with IOLs that correct for myopia only. With toric lenses, it is now possible to correct for corneal astigmatism in these patients as well, provided that the capsular bag is maintained and can be properly centered. We report the combination of scleral-fixated Cionni endocapsular rings and toric IOLs in a pediatric patient with bilateral ectopia lentis secondary to Marfan's syndrome

    Sensitivity to temporal structure facilitates perceptual analysis of complex auditory scenes

    The notion that sensitivity to the statistical structure of the environment is pivotal to perception has recently garnered considerable attention. Here we investigated this issue in the context of hearing. Building on previous work (Sohoglu and Chait, 2016a; elife), stimuli were artificial 'soundscapes' populated by multiple (up to 14) simultaneous streams ('auditory objects') comprised of tone-pip sequences, each with a distinct frequency and pattern of amplitude modulation. Sequences were either temporally regular or random. We show that listeners' ability to detect abrupt appearance or disappearance of a stream is facilitated when scene streams were characterized by a temporally regular fluctuation pattern. The regularity of the changing stream as well as that of the background (non-changing) streams contribute independently to this effect. Remarkably, listeners benefit from regularity even when they are not consciously aware of it. These findings establish that perception of complex acoustic scenes relies on the availability of detailed representations of the regularities automatically extracted from multiple concurrent streams

    Development of the Guernsey Community Participation and Leisure Assessment – Revised (GCPLA-R).

    A sufficiently psychometrically robust measure of community and leisure participation of adults with intellectual disabilities was not in existence, despite research identifying this as an important outcome and a key contributor to quality of life. The current study aimed to update the Guernsey Community Participation and Leisure Assessment (GCPLA). Adults with intellectual disabilities, carers and experts were consulted in creating a revised pool of 46 items. These were then tested and data from 326 adults with intellectual disabilities were analysed for their component structure and psychometric properties. Principal Component analysis discovered a stable set of components describing seven different clusters. This revised measure (the GCPLA-R) was demonstrated to have satisfactory reliability, and scores were related to challenging behaviour and adaptive behaviour in theoretically consistent ways and were correlated with scores on comparable measures

    STUDY AND DESIGN OF PORTABLE ANTIMICROBIAL WATER FILTER

      Objective: This study was conducted to design a portable antimicrobial water filter which is both economic and easy to use.Methods: A prototype following the designing of the water filter was constructed. Layers of sand, cloth, activated charcoal, and cotton containing compartments were built for carrying out water analysis.Results: Most probable number index of Vellore Institute of Technology lake water was compared with that of filtered water. Complete water analysis was done, and the sand filter layer was observed to be responsible for a maximum of the antimicrobial action of the filter.Conclusion: The study demonstrated that the proposed design of water filter is efficient in removal of turbidity, odor, and microbial content of lake water along with decreasing the acidity of water

    Necrotizing Keratitis after Laser Refractive Surgery in Patients with Inactive Inflammatory Bowel Disease

    Two cases of necrotizing keratitis following laser refractive corneal surgery, with stable and controlled Crohn's disease are described. A 40-year-old woman developed bilateral stromal inflammation and inferior thinning in the right eye along the flap edge within 1 day of uneventful bilateral IntraLase laser-assisted in situ keratomileusis. The other case is a 30-year-old man who also developed bilateral inferior stromal infiltrates 2 days following photorefractive keratectomy. Both cases were aggressively treated with systemic and topical corticosteroids. The infiltrates in both patients gradually resolved, with one relapse during the 7 months period of follow-up in the first case. These cases highlight the importance of taking precautions considering this and similar autoimmune conditions as a relative contraindication to refractive surgery

    An Exact Fluctuating 1/2-BPS Configuration

    This work explores the role of thermodynamic fluctuations in the two parameter giant and superstar configurations characterized by an ensemble of arbitrary liquid droplets or irregular shaped fuzzballs. Our analysis illustrates that the chemical and state-space geometric descriptions exhibit an intriguing set of exact pair correction functions and the global correlation lengths. The first principle of statistical mechanics shows that the possible canonical fluctuations may precisely be ascertained without any approximation. Interestingly, our intrinsic geometric study exemplifies that there exist exact fluctuating 1/2-BPS statistical configurations which involve an ensemble of microstates describing the liquid droplets or fuzzballs. The Gaussian fluctuations over an equilibrium chemical and state-space configurations accomplish a well-defined, non-degenerate, curved and regular intrinsic Riemannian manifolds for all physically admissible domains of black hole parameters. An explicit computation demonstrates that the underlying chemical correlations involve ordinary summations, whilst the state-space correlations may simply be depicted by standard polygamma functions.
