Consolidation therapy of ovarian cancer
U większości chorych na zaawansowanego raka jajnika stwierdza się bardzo dobrą odpowiedź na pooperacyjną
chemioterapię opartą na związkach platyny. Niestety efekt jest krótkotrwały i dochodzi do
nawrotu choroby. Założeniem leczenia konsolidacyjnego jest zastosowanie środków, które wydłużą czas
do progresji choroby, ale przede wszystkim przedłużą całkowity czas przeżycia. Stosowano wiele różnych
metod leczenia, takich jak chemioterapię dożylną związkami platyny, paklitakselem oraz innymi
chemioterapeutykami, a także chemioterapię dootrzewnową, chemioterapię doustną, chemioterapię wysokodawkową
z przeszczepem allogenicznym komórek szpiku, teleterapię radioterapię jamy brzusznej,
dootrzewnowe infuzje fosforu 32P oraz różne rodzaje tak zwanych terapii biologicznych (głównie przeciwciał
monoklonalnych). Mimo to nie stwierdzono ewidentnej ich skuteczności klinicznej. Wyniki badań
klinicznych są często sprzeczne, nieprowadzące do konkretnych wniosków i niespełniające oczekiwań.
Ostatnio opublikowane wyniki amerykańskiego badania z randomizacją, dotyczące przedłużonego stosowania
paklitakselu (175 mg/m2 co 3 tygodnie), wykazały znaczące wydłużenie czasu wolnego do progresji
u chorych z całkowitą remisją po chemioterapii I rzutu opartej na paklitakselu i związkach platyny.
Różne alternatywne schematy konsolidacyjnej chemioterapii mogą być bardziej korzystnie zrównoważone
między ich działaniem przeciwnowotworowym i wpływem na zapewnienie dobrej jakości życia.Most patients with advanced epithelial ovarian cancer experience objective responses to paclitaxel/platinum-
based chemotherapy, but responses are generally short-term and the clinical outcome is still unsatisfactory.
Therefore, the strategy to consolidate and to prolong the duration of response is very attractive.
Different consolidation or maintenance treatments have been attempted, such as whole abdomen radiotherapy,
intraperitoneal chromic phosphate, radioimmunotherapy, intraperitoneal chemotherapy, highdose
chemotherapy with haematopoietic support, prolonged administration of the first-line regimen, second-
line single-agent chemotherapy, and biological agents. Clinical studies have given conflicting, inconclusive,
and generally disappointing results. A recent US randomised trial appeared to show that the
prolonged administration of single-agent paclitaxel (175 mg/m2 every 3 weeks) significantly improved the
progression-free survival of complete responders to paclitaxel/platinum-based chemotherapy. Alternative
less toxic and probably more effective schedules of administration of chemotherapy (i.e. weekly
paclitaxel) might assure a better balance between quality of life and anti-tumor activity in patients previously
exposed to chemotherapy

Ground fortifications in the development of the park behind the marine school in Darłowo
The article presents problems related with designing public space, i.e. recreational areas and park green space in the context of existing fortifications with a case study of the town of Darłowo. It shows examples of the development of historical areas of ground fortifications as part of the project ‘A Plan of Development of the Park Behind the Marine School in Darłowo’. The article also discusses selected issues related with the composition, spatial functionality, landscape and execution of the plan, included in the park design documentation. It presents the results of design work, which was used to prepare complex technical documentation. This study includes the arrangements thatcan be used to make a town park with scenic values which will be similar a natural scenery.W artykule przedstawiono zagadnienia związane z projektowaniem przestrzeni publicznych tj. trenów rekreacji i zieleni parkowej w kontekście istniejących fortyfikacji na przykładzie miasta Darłowa. Zaprezentowano przykładowe propozycje zagospodarowania obszarów historycznych fortyfikacji ziemnych w ramach opracowania realizacyjnego pt. Projekt zagospodarowania Parku za Szkołą Morską w Darłowie. Omówiono również wybrane zagadnienia kompozycyjne, funkcjonalno-przestrzenne, krajobrazowe i realizacyjne zawarte w dokumentacji projektowej parku. Przedstawiono wyniki prac projektowych, na postawie których przygotowano kompleksową dokumentacje techniczną. W opracowaniu zawarto ustalenia, na podstawie których istnieje możliwość zrealizowania parku miejskiego o zbliżonych, do naturalnych walorach krajobrazowych
Effect of chemotherapeutic drugs on caspase-3 activity, as a key biomarker for apoptosis in ovarian tumor cell cultured as monolayer : a pilot study
We aimed to develop a cost-effective and robust method to predict drug resistance in individual patients. Representative tissue fragments were obtained from tumors removed from female patients, aged 24-74 years old. The tumor tissue was taken by a histopathology’s or a surgeon under sterile conditions. Cells obtained by enzymatic dissociation from tumor after surgery, were cultured as a monolayer for 6 days. Paclitaxel, doxorubicin, carboplatin and endoxan alone or in combination were added at the beginning of culture and after 6 days, Alamar blue test was used for showing action on cell proliferation why caspase- 3 activity assays for verifying action on apoptosis. Inhibitory action on cell proliferation was noted in 2 of 12 patients tumor treated with both single and combined drugs. Using caspase-3 assay we showed that 50% of tumor cells was resistant to single chemotherapeutic drugs and 40% for combined. In 2 of 12 tumors, which did not reacted on single drugs, positive synergistic action on cell proliferation was observed in combination of D + E and C + E. This pilot study suggests: 1) monolayer culture of tumor cells, derived from individual patients, before chemotherapy could provide a suitable model for studying resistance for drugs; 2) caspase-3 activity is cheap and useful methods; 3) Alamar blue test should be taken into consideration for measuring cell proliferation
Hereditary ovarian cancer. The role of BRCA1 gene dysfunction in response to chemotherapy
Summary Hereditary ovarian cancer is often believed to be as a distinct disease. It is diagnosed earlier than its sporadic type; serous subtypes and more advanced stages are usually observed. Mutations of genes like BRCA1, BRCA2, MMR (MLH1, MSH2, PMS1, PMS2) are strictly associated with the heredity of ovarian and also breast cancer. Systematic controls and specific procedures to lower the risk of those tumors are required for mutation carriers. Most authors emphasize better prognosis for patients with inherited type of ovarian cancer when comparing to sporadic one. It probably results from dysfunction of BRCA1 gene, inducing better response to platinum-based cytostatic drugs. This phenomenon, called “BRCAness profile”, is also observed in non-hereditary ovarian cancers and it arises from somatic mutation or hypermetylation of BRCA1 promoter. Thus, the process of DNA repair is defective. Currently new groups of drugs using the BRCA1 dysfunctions are being introduced into clinical practice
Role of cyclooxygenase-2 in cervical cancer
Concurrent radiation and chemotherapy is a standard treatment of patients with
cervical cancer in FIGO stages IB2-IVA. Chemotherapy given in the same time as
irradiation improved survival significant, but not involves all patients. Cancer
recurrence is the most common treatment failure in patients with advanced
tumor. Further improvement in the treatment of advanced cases is essential.
The increasing knowledge about new biologic markers and better ability to predict
risk of cancer recurrence is very important for construction of more effective
treatment strategies. Currently, there is considerable interest of role of cyclooxgenase-2 in cervical neoplasia. A lot of cancers in different localizations appear
to overexpress the cyclooxygenase-2, which may have an important role in
carcinogenesis. Ongoing clinical trials, clinical studies have shown the positive
therapeutic effect of COX-2 inhibitors and may allow to increase understanding
of this novel targeted approach for cervical cancer control
Mięsak popromienny trzonu macicy – opis przypadku
StreszczenieNowotwory indukowane napromienianiem są rzadkim powikłaniem odległym radioterapii. Histologicznie są to w ogromnej większości mięsaki tkanek miękkich lub kości. Przypadki zlokalizowane w miednicy mniejszej są zwykle późno rozpoznawane i rokują źle. Opisujemy przypadek popromiennego mięsaka trzonu macicy u chorej leczonej 7 lat wcześniej napromienianiem z powodu raka szyjki macicy, która pomimo przeprowadzonego chirurgicznego leczenia radykalnego zmarła wkrótce z powodu gwałtownej progresji. Przedstawiamy krótki przegląd aktualnego piśmiennictwa o częstości występowania, najczęstszych typach histologicznych, a także możliwych mechanizmach powstania wtómych nowotworów popromiennych. Kontrola chorych po skutecznym leczeniu napromienianiem powinna być prowadzona przez wiele lat. Wiedza o możliwości rozwoju wtórnych nowotworów upowszechniona wśród lekarzy pierwszego kontaktu może pozwolić na wcześniejsze rozpoznanie i skuteczniejsze leczenie tych nowotworów.SummaryPostirradiation tumors are rare kind of late complication of radiation therapy. The most of them are histologicaly confirmed sarcomas of bones or soft tissues. The pelvic location of malignance are usualy recognised late and has poor prognosis. We report a case of the postirradiation sarcoma developing in uterus 7 years after radiation therapy for cervical cancer. The rapid progresion was cause of death of observed patient, nevertheless radical surgery treatment. A short references review, concerning frequency, the most common histological types and possible mechanisms of secondary postirradiation malignancies has been presented. According to reccurence, dissemination and secondary malignancies risks the follow-up after radiation therapy should last for many years. Acknowledgement of the nature of postirradiation malignancies among primary care doctors may result in earlier diagnosis and better treatmant efficacy
Czynniki prognostyczne u chorych na pierwotnego inwazyjnego raka pochwy
Aim of the study: Aim of the study was the assessment of prognostic factors in the group of primary invasive vaginal carcinoma (PIVC) patients subjected to radical radiation therapy. Material and methods: The analysis was performed for the group of 152 PIVC patients treated with intracavitary brachytherapy alone (16.5%), the combination of brachytherapy and external radiotherapy (78.9%), or external radiotherapy alone (4.6%). The relationship was investigated between treatment outcome and the following demographic, clinical and histopathological features: age, duration of pathological symptoms, number of births given, prior hysterectomy, haemoglobin level, Karnofsky performance status score, primary tumour location in vagina, length of vagina involved, FIGO stage, gross appearance, histological type, and tumour grade. Results: Five-year disease-free survival was observed in 46.1% of the patients (70/152). Patients below 60 years of age, with Karnofsky score of 80-90, diagnosed with PIVC in stage I0 or II0, and with tumour of grade G1 or G2 had significantly higher 5-year disease-free survival. Multifactoral analysis showed that age below 60 and FIGO stage I0 and II0 are independent favourable prognostic factors. Conclusions: The independent prognostic factors in PIVC patients treated with radical radiotherapy are patient age and FIGO stage.Cel pracy: Celem pracy była ocena czynników prognostycznych w grupie chorych na pierwotnego inwazyjnego raka pochwy (PIVC) poddanych radykalnej radioterapii. Materiał i metody: Przedmiotem analizy była grupa 152 chorych na PIVC poddanych: samodzielnej brachyterapii dojamowej (16,5%), brachyterapii dojamowej skojarzonej z teleradioterapią (78,9%) lub samodzielnej teleradioterapii (4,6%). Przeprowadzono analizę zależności pomiędzy wynikami leczenia, a następującymi cechami populacyjnymi, klinicznymi i mikroskopowymi: wiek, czas trwania objawów chorobowych, liczba porodów, uprzednio wykonana histerektomia, poziom hemoglobiny, stopień sprawności wg skali Karnofskiego, punkt wyjścia raka w obrębie pochwy, długość pochwy zajętej przez raka, zaawansowanie raka wg FIGO, postać makroskopowa guza, postać mikroskopowa i zróżnicowanie raka. Wyniki: 5 lat bez objawów nowotworu przeżyło 46,1% chorych (70/152). Statystycznie znamiennie wyższe bezobjawowe przeżycie 5-letnie uzyskano u chorych poniżej 60 roku życia, w stopniu sprawności Karnofskiego 80-90, chorych na PIVC w I0 i II0 zaawansowania oraz chorych na PIVC G1 i G2. W analizie wielocechowej niezależnymi, korzystnymi czynnikami prognostycznymi były: wiek poniżej 60 lat oraz I0 i II0 zaawansowania raka wg FIGO. Wnioski: Niezależnymi czynnikami prognostycznymi u chorych na PIVC poddanych radykalnej radioterapii są wiek i stopień zaawansowania raka wg FIGO
Endometrial carcinoma in women under 40 years of age
W oparciu o dostępną literaturę podjęto próbę oceny skuteczności i bezpieczeństwa hormonalnego leczenia
niezaawansowanego raka błony śluzowej macicy u młodych kobiet. Około 1,5 do 15% przypadków raka błony śluzowej
macicy dotyczy kobiet w wieku prokreacyjnym. Standardowe leczenie polegające na usunięciu macicy z przydatkami z lub
bez węzłów chłonnych jest przez wiele z tych kobiet nie do zaakceptowania z uwagi na życzenie urodzenia dziecka. Leczenie
gestagenami, poprzedzone szczegółową diagnostyką, pozwala na uzyskanie całkowitej remisji zmian nowotworowych
u większości z tej grupy chorych. Gdy po zakończonej terapii nie dochodzi do spontanicznej ciąży, stosowane są
techniki wspomaganego rozrodu. Z uwagi na ryzyko nawrotu nowotworu po zakończeniu prokreacji zalecany jest zabieg
operacyjny.We evaluate the efficacy and safety of hormonal treatment of non-advanced endometrial cancer in young women.
Approximately 1.5-15% of the cases of endometrial cancer affect women of child-bearing potential. Standard treatment
involving hysterectomy with adnexectomy, with or without lymphadenectomy, is not accepted by quite a number of these
patients due to their desire to preserve child-bearing potential. Gestagen therapy preceded by thorough diagnostics allows to
achieve complete remission of neoplastic lesions in the majority of women in this group. If spontaneous pregnancy cannot
be achieved after the completion of the therapy, assisted reproduction techniques are applied. Owing to the risk of neoplastic
recurrence surgery is recommended after procreation is finished
