    Extent of liver inflammation in predicting response to interferon alpha & Ribavirin in chronic hepatitis C patients: a cohort study

    Background: Liver inflammation due to HCV infection leads to fibrosis, which is an independent predictor of treatment response to interferon therapy in Chronic Hepatitis C (CHC) patients. This relationship has not been studied for liver inflammation on pretreatment liver biopsy and End of Treatment Response (ETR). ALT is a less invasive test than liver biopsy for measuring liver inflammation. Aim of this study was to compare ETR to Interferon alpha (recombinant Interferon) & Ribavirin in CHC patients having higher and lower grades of liver inflammation and to determine the diagnostic accuracy of pretreatment ALT for grades of liver inflammation.Methods: A retrospective cohort of 876 naive CHC patients, who completed Interferon alpha & Ribavirin for 24weeks, was studied for ETR. Pretreatment grade of inflammation on liver biopsy was taken as the exposure variable. It was classified as high if there was moderate or severe and low if there was minimal or mild. Multivariable logistic regression modeling was performed. Diagnostic accuracy of pretreatment ALT for liver inflammation grades was determined by computing Area Under the Receiver Operator Curve (AUROC).Results: Of all patients, 672 having diagnostic liver biopsy and ETR available were analyzed. Among them, 103 had high and 569 had low grades of liver inflammation. Mean age was 36.9 (SD 9.1) years, with patients with high grades being older than those with low grades inflammation (p=0.03). High grades of liver inflammation was associated with ETR (RR 1.17, 95% CI 1.12-1.18) adjusting for age, Total Leukocyte count (TLC) and pretreatment levels of ALT, irrespective of liver fibrosis. This relation remained significant for \u27bridging fibrosis and cirrhosis\u27 and not for \u27no\u27 or \u27portal fibrosis\u27. AUROC of pretreatment ALT for males and females was moderately accurate for severe inflammation compared to minimal inflammation and less accurate for high grades compared to low grades.CONCLUSIONS: ETR in patients with higher grades of liver inflammation was 17% higher than those with lower grades irrespective of fibrosis and 9% higher for bridging fibrosis and cirrhosis. Pretreatment ALT was moderately accurate for severe inflammation only on liver biopsy in both males and females

The purpose of this study was to examine the management of the distribution of educational scholarships at Baznas Pohuwato and to examine the obstacles in the distribution of academic scholarships at Baznas Pohuwato. This research is a qualitative descriptive method to answer questions about existing phenomena—data collection techniques through observation, interviews and documentation from primary and secondary data sources. The data obtained is then processed to produce conclusions from the phenomena found. The results of this study are the scholarship program at Baznas Pohuwato to improve achievement and educate the nation's children with a budget that will be allocated according to the expected student targets by analyzing the circumstances and needs of students. Baznas hopes that scholarship recipients can convey and introduce this program to other communities, and the community will be aware of the obligation to pay ZIS through Pohuwato to manage the funds collected. The Baznas Pohuwato can carry out routine socialization to people experiencing poverty so that the community will be aware of the activities and programs managed by the National Amil Zakāt  Agency (BAZNAS), especially the scholarship assistance program for poor children at all levels of education. In distributing scholarship assistance, the National Amil Zakāt Agency (BAZNAS) would pay attention to the timeliness of scholarship distribution so that the waiting time for recipients is not too long

    Locking Versus Non-Locking Miniplates in the Treatment of Mandibular Fractures in Above 35 Years Age Patients

    Objective: To compare the frequency of short term post-operative infection of locking plates versus non-locking plate in mandibular fractures in patients above 35 years of age. Methodology: A randomized controlled study was conducted at the Department of Oral and Maxillofacial Surgery, Pakistan Institute of Medical Sciences, Islamabad from January 2018 to January 2019, involving a 100 patients (50 patients in each group). On the basis of history, clinical features, orthopantomogram (OPG) and intraoperative findings, the diagnosis of mandibular fractures was established. Group A patients were treated with reduction and fixation of fracture with locking miniplates and miniscrews following champy’s principle of osteosynthesis. Whereas in group B patients, fractures were reduced and fixed with non-locking miniplates and miniscrews following champy’s principle of osteosynthesis. Results: Effect modifiers such as age, gender, and type of fracture were controlled by stratification. A post-stratification chi-square test was applied. The 1 patient that had infection in group A had an age range 50- 65 years (53 years) (P value: 0.197) while the 2 patients that had infection in group B were younger in age range of 35-50 years (36 years and 38 years) (P value: 0.322). The 1 patient who had infection in group A was female (P value: 0.043) while the 2 patients of group B who had infection were also females (P value: 0.027). In group A, only 1 patient had infection at the para-symphysis of mandible according to site (P value: 0.494). In group B, 1 patient had infection at para-symphysis of mandible and 1 at the body of the mandible site (P value: 0.599).  The results were not statistically significant in terms of infection by the end of 3rd week. Conclusion: The study concludes that insignificant post-operative infection rates while managing mandibular fractures either by locking plates or non-locking plates.

    Parametric analysis of turning HSLA steel under minimum quantity lubrication (MQL) and nanofluids-based minimum quantity lubrication (NF-MQL) : a concept of one-step sustainable machining

    Abstract: The requirement of cost-effective and ecological production systems is crucial in the competitive market. In this regard, the focus is shifted towards sustainable and cleaner machining processes. Besides the clean technologies, effective parametric control is required for machining materials (such as High Strength Low Alloy Steels) specifically designed for high strength applications having superior physio-chemical properties. Therefore, the machinability complexities require optimized solutions to reduce temperature elevation and tooling costs and improve machining of these materials. Complying to the market needs, this research examines the effectiveness of nanofluid on tool life, wear mechanisms, surface roughness (Ra), surface morphology, and material removal rate (MRR) in turning of 30CrMnSiA (HSLA) using minimum quantity lubrication (MQL) and SiO2-H2O nanofluids (NF-MQL). A systematic investigation based on physical phenomena involved is carried out considering four process parameters (cutting speed (VC), feed rate (Fr), depth of cut (DOC), and mode of lubrication for machining. Fr is found as the vital parameter for surface roughness while MRR is highly influenced by DOC regardless of lubrication approach. One-step sustainability technique is applied, in which process variables used for roughing conditions are analogous to attain surface comparable to finished machining without compromising process efficiency and demonstrate its feasibility through optimal settings under NF-MQL. Multi-response optimization proved the NF-MQL machining condition as the best alternative which result in 28.34% and 5.09% improvements for surface roughness and MRR, respectively. Moreover, the use of SiO2 is recommended over MQL due to lower energy consumption, low tool wear, and better surface integrity, sustainable liquid, and related costs

    Detection of Paracetamol as substrate of the gut microbiome

    Gut microbiome, a new organ; represent targets to alter pharmacokinetics of orally administered drugs. Recently, in vitro trials endorsed the idea that orally administered drugs interact and some of their quantity may be taken up by normal microbiome during transit through gut. Such transport mechanisms in microbiome may compete for drug with the host itself. Currently, no data confirms specific transport system for paracetamol uptake by gut microbiome. In vivo trial was conducted in normal healthy male rats (n=36). Paracetamol was administered orally in a single dose of 75mg/kg to isolate microbial mass after transit of 2, 3, 4, 5 and 6 hours post drug administration. Paracetamol absorbance by microbiome was pursued by injecting extracted microbial lysate in RP-HPLC-UV with C18 column under isocratic conditions at 207nm using acetonitrile and water (25:75 v/v) pH 2.50 as mobile phase. Paracetamol absorbance (14.10±0.75μg/mg of microbial mass) and percent dose recovery (13.16±0.55%) seen at transit of 4 hours was significantly higher (P<0.05) compared to other groups. Study confirms the hypothesis of homology between membrane transporters of the gut microbiome and intestinal epithelium. Orally administered drugs can be absorbed by gut microbes competitively during transit in small intestine and it varies at various transit times

    Revealing the microstructure and mechanical attributes of pre-heated conditions for Gas Tungsten Arc Welded AISI 1045 steel joints

    Gas tungsten arc welding (GTAW) is considered a well-established process in the manufacturing industry. Despite, certain challenges associated with high hardness of heat affected zone and cold cracking susceptibility of joints, are the main barriers for this process to be implemented successfully within high integrity structure. By using a combined procedure of experiments and modelling (response surface methodology (RSM) and multi-objective optimization: multi-objective genetic algorithm (MOGA)) allows obtaining good enhancement over uniform heating, cooling and the heat-affected zone which enable major progress in obtaining high quality welded parts. Therefore, this research study combines the experiments and modelling in a systematic manner considering for the first type the pre-heated treatment and without- pre-heating conditions of GTAW manufacturing. It leads to optimizing the process parameters of GTAW when manufacturing AISI 1045 medium carbon steel. The effects of critical parameters i.e. welding current: WC, welding speed: WS, and gas flow rate: GFR on the mechanical properties (ultimate tensile strength (UTS) and hardness) were investigated and evaluated against the microstructure of weld fracture. The multi-objective genetic algorithm corroborated with experimental observation enables to obtain a maximum UTS of approx. 625 MPa and hardness of 80.19 HRB for preheat condition. The results highlight an improvement in UTS of 0.2% to 6.7% and a decrease in hardness of 0.1% to 21.5% by implementing the preheating conditio
