A preliminary effort was made to identify the influence of consumers’ socio-demographic backgrounds on the variations in their responses. Extensor-tendons reconstruction using autogenous palmaris longus tendon grafting for rheumatoid arthritis patients
<p>Abstract</p> <p>Background</p> <p>The purpose of the study is to retrospectively review the clinical outcome of our study population of middle-aged RA patients who had suffered extensor-tendon rupture. We reported the outcome of autogenous palmaris tendon grafting of multiple extensor tendons at wrist level in 14 middle-aged rheumatoid patients.</p> <p>Methods</p> <p>Between Feb. 2000 to Feb. 2004, thirty-six ruptured wrist level extensor tendons were reconstructed in fourteen rheumatoid patients (11 women and three men) using autogenous palmaris longus tendon as a free interposition graft. In each case, the evaluation was based on both subjective and objective criteria, including the range of MCP joint flexion after surgery, the extension lag at the metacarpophalangeal joint before and after surgery, and the ability of the patient to work.</p> <p>Results and Discussion</p> <p>The average of follow-up was 54.1 months (range, 40 to 72 months). The average range of MCP joint flexion after reconstruction was 66°. The extension lag at the metacarpophalangeal joint significantly improved from a preoperative mean of 38° (range, 25°–60°) to a postoperative mean of 16° (range, 0°–30°). Subjectively all patients were satisfied with the clinical results, and achieved a return to their level of ability before tendon rupture. We found good functional results in our series of interposition grafting using palmaris longus to reconstruct extensor tendon defects in the rheumatoid patients.</p> <p>Conclusion</p> <p>Reconstruction for multiple tendon ruptures is a salvage procedure that is often associated with extensor lag and impairment of overall function. Early aggressive treatment of extensor tendon reconstruction using autogenous palmaris longus tendon as a free interposition graft in the rheumatoid wrist is another viable option to achieve good clinical functional result.</p
Promoter methylation of the hMLH1 gene and protein expression of human mutL homolog 1 and human mutS homolog 2 in resected esophageal squamous cell carcinoma
ObjectiveAberrant expression of mismatch repair genes, such as human mutL homolog 1 (hMLH1) and human mutS homolog 2 (hMSH2), are common in some human cancers, and promoter methylation is believed to inactivate expression of hMLH1. We investigated whether promoter methylation is involved in loss of hMLH1 protein and whether aberrant expression of hMLH1 and hMSH2 protein is related to prognosis after resection for esophageal squamous cell cancer.MethodsWe analyzed promoter methylation of hMLH1 using methylation-specific polymerase chain reaction and hMLH1 and hMSH2 protein by using immunohistochemistry in 60 resected tumor specimens. The Pearson χ2 test was used to compare expression of hMLH1 and hMSH2 protein among patients with different clinicopathologic parameters. Concordance analysis was performed between hMLH1 methylation and its protein expression.ResultsLoss of hMLH1 and hMSH2 protein was found in 43 (72%) and 39 (65%, P = .06) of 60 resected specimens, respectively. hMLH1 protein correlated well with tumor staging (P < .0001), depth of tumor invasion (P = .008), and nodal involvement (P < .0001) but not with distant metastasis, whereas hMSH2 did not show correlation with any of these parameters. A concordance rate of 83.3% was present between expression of hMLH1 protein and its promoter methylation (P < .001).ConclusionsAberrant expression of hMLH1 and hMSH2 protein is frequently associated with the presence of esophageal squamous cell carcinoma, and expression of hMLH1 protein is a better prognostic predictor than is expression of hMSH2 protein. Promoter methylation is one of the mechanisms responsible for loss of hMLH1 protein in esophageal squamous cell cancer
Optimization of the optical parameters in Fabry-Perot interferometer
Due to insensitivity to the environmental disturbances, Fabry-Perot interferometers are suitable for displacement measurements under ordinary conditions. In the structure of folded Fabry-Perot interferometer, the results of the signal subdivision are affected by the optical parameters in the resonant cavity. In this paper, the analysis of the Fabry-Perot interferometer for the measurement of the micro-displacement and the long-distance are investigated. By considering the reflectance of the planar mirror and the intensity loss in the resonant cavity, the parameters of systematic optimization which are suitable for the measurement of the micro-displacement and the long-distance are proposed. The experimental and simulated results reveal that the intensity loss in the resonant cavity is 86% and the optimized reflectance of the planar mirror is 12%

Pilot Scale Production of Highly Efficacious and Stable Enterovirus 71 Vaccine Candidates
BACKGROUND: Enterovirus 71 (EV71) has caused several epidemics of hand, foot and mouth diseases (HFMD) in Asia and now is being recognized as an important neurotropic virus. Effective medications and prophylactic vaccine against EV71 infection are urgently needed. Based on the success of inactivated poliovirus vaccine, a prototype chemically inactivated EV71 vaccine candidate has been developed and currently in human phase 1 clinical trial. PRINCIPAL FINDING: In this report, we present the development of a serum-free cell-based EV71 vaccine. The optimization at each step of the manufacturing process was investigated, characterized and quantified. In the up-stream process development, different commercially available cell culture media either containing serum or serum-free was screened for cell growth and virus yield using the roller-bottle technology. VP-SFM serum-free medium was selected based on the Vero cell growth profile and EV71 virus production. After the up-stream processes (virus harvest, diafiltration and concentration), a combination of gel-filtration liquid chromatography and/or sucrose-gradient ultracentrifugation down-stream purification processes were investigated at a pilot scale of 40 liters each. Although the combination of chromatography and sucrose-gradient ultracentrifugation produced extremely pure EV71 infectious virus particles, the overall yield of vaccine was 7-10% as determined by a VP2-based quantitative ELISA. Using chromatography as the downstream purification, the virus yield was 30-43%. To retain the integrity of virus neutralization epitopes and the stability of the vaccine product, the best virus inactivation was found to be 0.025% formalin-treatment at 37 °C for 3 to 6 days. Furthermore, the formalin-inactivated virion vaccine candidate was found to be stable for >18 months at 4 °C and a microgram of viral proteins formulated with alum adjuvant could induce strong virus-neutralizing antibody responses in mice, rats, rabbits, and non-human primates. CONCLUSION: These results provide valuable information supporting the current cell-based serum-free EV71 vaccine candidate going into human Phase I clinical trials

measurement by using digital image correlation (DIC) technique in micro-scale
test. In this study, water immersion is firstly proposed to combine with DIC
technique to reduce the influence of light reflection on the surfaces of
measured sample in a micro-scale experiment. Then, all test images are
transformed into two dimensional K-space
to compare the influence degree of light reflection on the various
configurations of test specimens. Furthermore, the effects of light
projection direction and the angle of light source are investigated into the
analysis accuracy of strain measurement for the various configurations of test
specimens. The test and analysis results from <i style="mso-bidi-font-style:
normal">K-space indicate that the frequency signals of images, measured
from test samples, observed in air are higher than the results from this
proposed method. Otherwise, no matter how the variation of the light
projection direction and the angle of light source are, the standard deviations
of strain from this proposed method are diminished, reduced to below 0.001. The analysis accuracies of
strain analysis for using water immersion method have been verified.
