    Spectral studies of the dithiacrown ether derivative in the presence of metal cations and modeling their possible complexes

The preparation of compositions comprising multifunctional compound is one of the key directions for design of chemosensoring materials that are important for the determination of particular ions in solutions. These compounds are characterized by ionophoric and photosensitive parts with reliable optical response and ion-selectivity. Their structures suggest the possibility of the complex formation with particular metal cations. The main goals of this study are as following: a) to investigate the absorption spectra of the novel derivative of the dithiacrown ethers (DTCE), synthesized in the laboratory of Professor S.P. Gromov (Photochemistry Center RAS), both: in salt solutions and in monolayers; b) to study the DTCE interaction with various metal cations, including mercury, as well as to carry out quantum-chemical calculations of their possible complexes. Spectral characteristics were examined in the DTCE acetonitrile solutions and their changes in the presence of some cations were found. The pronounced changes were obtained in the case of Hg2+ which are expressed as a hypsochromic shift on 22 nm. Absorption spectra of aqueous solutions of DTCE and perchlorates of alkali, alkaline earth, and heavy metals (lithium, sodium, potassium, cesium, magnesium, calcium, strontium, barium, copper, zinc, lead, cadmium, and mercury) at various concentrations were obtained. When the DTCE concentration equals to 10–5 M, the absorption intensity is low, and DTCE absorption maximum shifts in the presence of all the salts are rather small. The greatest shift (by 6 nm) of the DTCE absorption maximum to shorter wavelengths was observed for the mercury(II) perchlorate only. In order to select the DTCE concentration for further research the spectral dependence of the optical density vs. DTCE concentration was obtained (10 M was the most appropriate value of the DTCE concentration). Based on these results the DTCE extinction coefficient in aqueous solutions was determined (ε = 9329 M–1cm–1). Assuming the formation of the DTCE-cation complexes as 1:1, the metal perchlorates with lack and excess of the cations in the aqueous solutions were chosen. The absorption spectra had minor differences at DTCE concentration of 10–4 M and metal perchlorates concentration of 10–5 M (lack of the metal cations). Maximum hypsochromic shift by 4 nm was observed in the presence of mercury(II) perchlorate. It appeared that the lack of concentration of the metal salt does not allow the formation of complexes between the crown ether ring and a cation. The absorption intensity in the presence of most of salts was slightly reduced up to 85–95 % from baseline. The absorption spectra showed significant shifts of the absorption maxima to shorter wavelengths by 29 nm only in the presence of mercury(II) perchlorate at high concentrations (10–3 M). This fact proves a DTCE selectivity for mercury(II) cation. Moreover, the value of the shift depends on the salt concentration. An additional confirmation of these results was obtained in the study of the absorption spectra of the DTCE monolayers transferred from bidistilled water and aqueous solutions of Hg(ClO4)2 at constant surface pressure (about 10 mN/m). In all cases, the broad absorption maxima in the region of 350–450 nm were found. The main feature was that the maximum absorption for DTCE monolayer transferred to 10–5 M solution of Hg(ClO4)2 was 0.00575 a.u. at 406 nm. This is by 209% higher than those for the DTCE monolayer transferred from the water. Thus, the shift of the absorption maximum of DTCE monolayer in the presence of mercury salt was 23 nm to shorter wavelengths. This is further evidence of complex formation between DTCE (in monolayer) and mercury(II) cations from the aqueous subphase. For visualization and confirmation of the possibility of complex formation between DTCE and mercury(II) cation the quantum-chemical calculations of their structural models were carried out. The total energy of the optimized structure of the complex between DTCE cation and mercury(II) (with 2 perchlorate counter-anions) is set to E0 = –565.1231 a.u. It can be compared with the energies of individual species: the DTCE cation (E0 = –250.5477 a.u.) and Hg(ClO4)2 (E0 = –314.4877 a.u.). Thus, the change in energy (ΔE) upon complex formation is about –0.0877 a.u. (–230 kJ/mol). High energy of complex formation and high covalency of the Hg–S bonds explain the significant blue shift of the maximum in the absorption spectra of the solutions of DTCE with Hg(ClO4)2

    The article presents the results of a retrospective study of case histories of children with pulmonary disease in children with COVID-19 and community-acquired pneumonia (CAP). The study is based on the problem of interpreting the manifestations of COVID-19 in children and the adequacy of the appointment of antimicrobial therapy. Based on the results of the analysis, it was established that antibiotic therapy was inappropriately prescribed in a group of children with COVID-19.В статье представлены результаты ретроспективного исследования историй болезни детей с поражением легких при COVID-19 и внебольничной пневмонии (ВП). В основе исследования лежит проблема интерпретации проявлений COVID-19 у детей и адекватности назначения антимикробной терапии.     Programming of chronic diseases in children

    The purpose of this study is to evaluate the association of excessive body weight gain at an early age in full – term children with chronic diseases in the adolescent period. The study included 51 obese adolescents (average age 13.84±1.97 years) and 49 healthy children (average age 13.58±2.029 years), comparable by gender and age. The selection criteria for inclusion in the main group also included excessive weight gain from birth to 18 months. We have found that the excessive rate of body weight gain at an early age is associated with the development of obesity and arterial hypertension in adolescence. Every extra kilogram of body weight under the age of 1 year 6 months increases the risk of obesity in the adolescent period by 3 times; there was no data indicating a direct connection with atherogenesis. The smaller the child’s body weight at birth and the longer the length, the earlier the complementary feeding was introduced and exclusively breastfeeding was completed, the more excessive the child’s body weight gain in the first 18 months of life.Цель настоящего исследования – оценить связь избыточного набора массы тела в раннем возрасте у доношенных детей с хроническими заболеваниями в подростковом периоде. В исследование был включен 51 подросток (средний возраст 13,84±1,97 лет), страдающий ожирением, и 49 здоровых детей (средний возраст 13,58±2,029 лет), сравнимых по полу и возрасту. К критериям отбора для включения в основную группу относилась также избыточная прибавка массы тела от рождения до 18 мес. Нами установлено, что избыточная скорость набора массы тела в раннем возрасте связана с развитием ожирения и артериальной гипертензии в подростковом возрасте. Каждый лишний килограмм массы тела в возрасте до 1 года 6 мес. увеличивает риск возникновения ожирения в подростковом периоде в 3 раза; данных, свидетельствующих о прямой связи с атерогенезом, получено не было.     Поражение легких при COVID-19 и внебольничной пневмонии у детей: сравнительный клинико-лабораторный анализ

    The study is devoted to the challenge of interpreting the lung damage associated with COVID-19 in children and the necessity for antimicrobial therapy in this disease. The aim of the research was a comparative analysis of clinical, radiological and laboratory signs in children with COVID-19 and community-acquired pneumonia (CAP). Methods. The observational comparative study included medical records of 53 children with the lung damage at the age of 1 year 4 months up to 17 years old, hospitalized for the period from June to August 2020. All children were tested for Severe acute respiratory syndrome-related coronavirus-2 (SARS-CoV-2) by polymerase chain reaction (PCR). 34 patients with SARS-CoV-2+ were diagnosed with COVID-19 and 19 children with SARS-CoV-2– were diagnosed with CAP. The assessment included medical history, clinical, laboratory and radiological changes. Results. Distinctive reliable clinical, laboratory, and X-ray signs of lung damage in COVID-19 compared to CAP were olfactory impairment (26%), bilateral lung damage (23%), erythrocytosis, leukopenia (20.6%), granulocyto- and monocytopenia, lower levels of reactive protein (CRP). Irrational prescribing of antibiotics (94%) was detected in the group of patients with COVID-19. Conclusion. We established the reliable clinical, laboratory and radiological features of COVID-19 in children, which indicate the viral nature of lung damage.Основу исследования составила проблема интерпретации педиатрами поражения легких у детей при COronaVIrus Disease-2019 (COVID-19) и необходимости назначения при этом антибактериальной терапии. Целью исследования явился сравнительный анализ клинических, рентгенографических (РГ) и лабораторных признаков COVID-19 поражения легких и внебольничной пневмонии (ВП) у детей. Материалы и методы. В рамках наблюдательного сравнительного исследования проанализированы данные историй болезни детей (n = 53) в возрасте от 1 года 4 мес. до 17 лет с поражением легких, госпитализированных за период с июня по август 2020 г. У всех детей методом полимеразной цепной реакции (ПЦР) проведено исследование на Severe acute respiratory syndrome-related coronavirus-2 (SARS-CoV-2), по результатам которого пациентам с SARS-CoV-2+ (n = 34) установлен диагноз СOVID-19, при SARS-CoV-2– (n = 19) диагностирована ВП. Проанализированы также анамнез заболевания, клинические, лабораторные показатели и данные РГ. Результаты. Отличительными достоверными клинико-лабораторными и РГ-признаками поражения легких при COVID-19 по сравнению с ВП являлись нарушение обоняния (26 %), двустороннее поражение легких (23 %), эритроцитоз, лейкопения (20,6 %), грануло- и моноцитопения, низкий уровень С-реактивного белка. В группе пациентов с COVID-19 выявлено нерациональное назначение антибактериальных препаратов (94 %). Заключение.
