    ANALYSIS OF AGOMELATINE TREATMENT WITH DEPRESSIVE SYMPTOMS

    Objective: Agomelatine is a new mechanism of antidepressants, which is approved by Taiwan Food and Drug Administration and available in Taiwan.Agomelatine behaves both as a potent agonist at melatonin MT1 and MT2 receptors and as a neutral antagonist at 5-HT2C receptors. The structuresof agomelatine are similar to melatonin with not only the effects to maintain depression symptoms but also can help patients who have insomnia.Methods: This is a retrospective study. In a mental hospital in Taoyuan, we analyzed the prescriptions of the outpatients who were prescribedagomelatine to realize the effects of agomelatine and whether the prescriptions were prescribed appropriately.Results: Catastrophic illnesses were found to be associated with significantly used multiple hypnotics (Ļ‡2 =22.02, p<0.001). When patientsā€™ ageincreased by 1 year, multiple hypnotics used increased by 1.013 times (Exp(B)=1.013, p<0.01). Catastrophic illnesses were found to be associatedwith significantly used augmentation with other antidepressants (Ļ‡2=54.07, p<0.001).Conclusions: Doctors should be evaluating the benefits and risks when they prescribe a medicine to patients, and they should be written in medicalrecord. This study is the hope to provide relevant units as a reference for formulating health policies

    Physiological indexes, psychological resilience, sensory functions, and sleep quality on the cognitive function of older adults with pre-frailty: a predictive study

    Population ageing has increased the prevalence of prefrailty comorbid with cognitive impairment among older adults. However, few studies have explored the risk factors common to both prefrailty and cognitive impairment. This study determined the predictive accuracy of demographic characteristics, physiological indexes, psychological resilience, sensory function, and sleep quality on the cognitive function of older adults with prefrailty. In this cross-sectional study, the physiological indexes, psychological resilience, sensory function, sleep quality, and cognitive function of 167 community-dwelling older adults with prefrailty recruited through purposive sampling were measured. SPSS software was used for data coding and compilation. Data analysis involved the use of descriptive statistics, the independent samples t test, the chi-square test, and logistic regression. Overall, in cognitive function, there was no difference in gender but were in age, were incapable of text messaging, had a greater number of chronic diseases, were less able to perform activities of daily living, had low psychological resilience, and had depressive tendencies. In addition, Text messaging capability and depression status can all predict the cognitive impairment state of prefrail older elderly. Physiological indexes, psychological resilience, sensory function, and sleep quality can affect cognitive function in older adults with prefrailty. Meanwhile, depressive tendencies and the inability to send text messages on a mobile device constituted critical predictors of cognitive function in the participants

    Effects of Jia-Wei-Xiao-Yao-San on the Peripheral and Lymphatic Pharmacokinetics of Paclitaxel in Rats

    Paclitaxel is effective against breast cancer. The herbal medicine, Jia-Wei-Xiao-Yao-San (JWXYS), is the most frequent prescription used to relieve the symptoms of breast cancer treatments. The aim of the study was to investigate the herb-drug interaction effects of a herbal medicine on the distribution of paclitaxel to lymph. A validated ultraperformance liquid chromatography with tandem mass spectrometry (UPLC-MS/MS) method was used to determine the paclitaxel levels in rat plasma and lymph after intravenous infusion of paclitaxel alone with or without 7 days of JWXYS pretreatment. The pharmacokinetic results indicate that paclitaxel concentrations in plasma exceeded those in lymph by approximately 3.6-fold. The biodistribution of paclitaxel from plasma to lymph was 39Ā±5%; however, this increased to 45Ā±4% with JWXYS pretreatment. With JWXYS pretreatment, the AUC and Cmax of paclitaxel in plasma were significantly reduced by approximately 1.5-fold, compared to paclitaxel alone. Additionally, JWXYS decreased the AUC and Cmax of paclitaxel in lymph. However, the lymph absorption rate of paclitaxel with or without JWXYS pretreatment was not significantly changed (27Ā±3 and 30Ā±2%, resp.). Our findings demonstrate that when paclitaxel is prescribed concurrently with herbal medicine, monitoring of the blood pharmacokinetics of paclitaxel is recommended

    Seizure After Local Anesthesia for Nasopharyngeal Angiofibroma

    We report a young male patient who experienced seizure after local injection of 3 mL 2% lidocaine with epinephrine 1:200,000 around a recurrent nasal angiofibroma. After receiving 100% oxygen via mask and thiamylal sodium, the patient had no residual neurologic sequelae. Seizure immediately following the injection of local anesthetics in the nasal cavity is probably due to injection into venous or arterial circulation with retrograde flow to the brain circulation. Further imaging study or angiography should be done before head and neck surgeries, especially in such highly vascular neoplasm

    Serotonin receptor HTR6-mediated mTORC1 signaling regulates dietary restriction-induced memory enhancement

    Dietary restriction (DR; sometimes called calorie restriction) has profound beneficial effects on physiological, psychological, and behavioral outcomes in animals and in humans. We have explored the molecular mechanism of DR-induced memory enhancement and demonstrate that dietary tryptophan-a precursor amino acid for serotonin biosynthesis in the brain-and serotonin receptor 5-hydroxytryptamine receptor 6 (HTR6) are crucial in mediating this process. We show that HTR6 inactivation diminishes DR-induced neurological alterations, including reduced dendritic complexity, increased spine density, and enhanced long-term potentiation (LTP) in hippocampal neurons. Moreover, we find that HTR6-mediated mechanistic target of rapamycin complex 1 (mTORC1) signaling is involved in DR-induced memory improvement. Our results suggest that the HTR6-mediated mTORC1 pathway may function as a nutrient sensor in hippocampal neurons to couple memory performance to dietary intake

    Modeling meteorite craters by impacting melted tin on sand

    To simulate the heated exterior of a meteorite, we impact a granular bed with melted tin. The morphology of tin remnant and crater is found to be sensitive to the temperature and solidification of tin. By employing deep learning and convolutional neural network, we can quantify and map the complex impact patterns onto network systems based on feature maps and Grad-CAM results. This gives us unprecedented details on how the projectile deforms and interacts with the granules, which information can be used to trace the development of different remnant shapes. Furthermore, full dynamics of granular system is revealed by the use of Particle Image Velocimetry. Kinetic energy, temperature and diameter of the projectile are used to build phase diagrams for the morphology of both crater and tin remnant. In addition to successfully reproducing key features of simple and complex craters, we are able to detect a possible artifact when compiling crater data from field studies. The depth of craters from high-energy impacts in our work is found to be independent of their width. However, when mixing data from different energy, temperature and diameter of projectile, a bogus power-law relationship appears between them. Like other controlled laboratory researches, our conclusions have the potential to benefit the study of paint in industry and asteroid sampling missions on the surface of celestial bodies.Comment: 6 pages, 5 figure

    Genomic diversity of citrate fermentation in Klebsiella pneumoniae

    <p>Abstract</p> <p>Background</p> <p>It has long been recognized that <it>Klebsiella pneumoniae </it>can grow anaerobically on citrate. Genes responsible for citrate fermentation of <it>K. pneumoniae </it>were known to be located in a 13-kb gene cluster on the chromosome. By whole genome comparison of the available <it>K. pneumoniae </it>sequences (MGH 78578, 342, and NTUH-K2044), however, we discovered that the fermentation gene cluster was present in MGH 78578 and 342, but absent in NTUH-K2044. In the present study, the previously unknown genome diversity of citrate fermentation among <it>K. pneumoniae </it>clinical isolates was investigated.</p> <p>Results</p> <p>Using a genomic microarray containing probe sequences from multiple <it>K. pneumoniae </it>strains, we investigated genetic diversity among <it>K. pneumoniae </it>clinical isolates and found that a genomic region containing the citrate fermentation genes was not universally present in all strains. We confirmed by PCR analysis that the gene cluster was detectable in about half of the strains tested. To demonstrate the metabolic function of the genomic region, anaerobic growth of <it>K. pneumoniae </it>in artificial urine medium (AUM) was examined for ten strains with different clinical histories and genomic backgrounds, and the citrate fermentation potential was found correlated with the genomic region. PCR detection of the genomic region yielded high positive rates among a variety of clinical isolates collected from urine, blood, wound infection, and pneumonia. Conserved genetic organizations in the vicinity of the citrate fermentation gene clusters among <it>K. pneumoniae</it>, <it>Salmonella enterica</it>, and <it>Escherichia coli </it>suggest that the13-kb genomic region were not independently acquired.</p> <p>Conclusion</p> <p>Not all, but nearly half of the <it>K. pneumoniae </it>clinical isolates carry the genes responsible for anaerobic growth on citrate. Genomic variation of citrate fermentation genes in <it>K. pneumoniae </it>may contribute to metabolic diversity and adaptation to variable nutrient conditions in different environments.</p
