    Portable instrument for in-vivo infrared oxymetry using spread-spectrum modulation

    Near Infrared Spectroscopy (NIRS) can be employed to monitor noninvasively and continuously local changes in hemodynamics and oxygenation of human tissues. A portable NIRS research-grade acquisition system, dedicated to measurements during muscular exercise, is presented. The instrument is able to control up to eight LED sources and two detectors. A digital correlation technique, implemented on a single-chip RISC microcontroller, performs source-to-detector multiplexing. Such algorithm is highly optimized for computational efficiency and ambient noise rejection. Software-configurable input stages allow for flexibility in instrument setup. As a result of the specific correlation technique employed, the instrument is compact, lightweight and efficient. Clinical tests on oxygen consumption show excellent performance

    GPU Accelerated Time-Domain Discrete Geometric Approach Method for Maxwell's Equations on Tetrahedral Grids

    A recently introduced time-domain method for the numerical solution of Maxwell's equations on unstructured grids is reformulated in a novel way, with the aim of implementation on graphical processing units (GPUs). Numerical tests show that the GPU implementation of the resulting scheme yields correct results, while also offering an order of magnitude in speedup and still preserving all of the main properties of the original finite-difference time-domain algorithm. © 2017 IEEE

    This report presents the results of the implementation of the Media Pluralism Monitor for the year 2021 (MPM2022) in Italy. The MPM is a holistic tool geared at assessing the risks to media pluralism in EU member states and selected candidate countries (32 European countries in total, including Albania, Montenegro, the Republic of North Macedonia, Serbia, and Turkey). The MPM takes into account legal, political and economic variables that are relevant to analysing the levels of plurality of media systems in a democratic society.     Case Report: Simil-Appendicitis Presentation May Precede Cardiac Involvement in MIS-C Patient

    Introduction: Multisystem inflammatory syndrome in children (MIS-C) is a new clinical entity characterized by a systemic hyperinflammation triggered by SARS-CoV-2 infection in children and adolescents. This condition could potentially involve all organs with main complications concerning cardiovascular system. Despite up to 90% of patients complain gastrointestinal symptoms (nausea, vomit, and diarrhea), a presentation mimicking acute appendicitis has rarely been reported, and can be the presenting feature of the disease, potentially leading to misdiagnosis and delayed treatment. Case Description: A 15-year-old boy presented to the Emergency Department for a 2-day history of fever, vomiting, and mild abdominal pain. One month before, the patient complained ageusia and anosmia while his mother tested positive for Sars-CoV2 nasopharyngeal swab. At admission, laboratory tests showed leukocytosis with lymphopenia and elevation of inflammatory markers, while cardiac enzymes, electrocardiogram and echocardiography were unremarkable. An abdominal ultrasound displayed a thickening of terminal ileus and cecum with ascites. Because of the worsening abdominal pain and a physical examination suggestive of acute appendicitis, a laparoscopy was performed but no surgical condition was found. After surgery, fever and generalized malaise persisted, so a cardiac evaluation was repeated, showing a relevant increase in inflammatory markers and cardiac enzymes. Electrocardiogram demonstrated a QTc prolongation with mild decrease in left ventricular ejection fraction at echocardiogram. A MIS-C was diagnosed and intravenous immunoglobulin along with a steroid treatment started. After 36 h, the patient presented a complete clinical recovery with fever cessation. Cardiac anomalies normalized in 3 weeks. Conclusion: MIS-C has been defined as a systemic inflammation, involving at least two organs, after a previous SARS-CoV2 infection in children and adolescents. Physicians should be aware that while gastrointestinal manifestations are common, a pseudo appendicitis presentation may also occur, leading to misdiagnosis and delayed treatment. This report suggests that in patients with symptoms suggestive of an acute appendicitis, the presence of lymphopenia, hypoalbuminemia and ultrasound images of terminal ileus inflammation, should raise the suspect for MIS-C even without initial overt signs of cardiac involvement

    Exposure of the Main Italian River Basin to Pharmaceuticals

    This study give a preliminary survey of pharmaceutical contamination and accumulation in surface waters and sediments along the river Po basin (74,000 km2, the largest in Italy), a strategic region for the Italian economy: it collects sewage from a vast industrialized area of Italy (Autorità di Baciono del fiume Po, 2006, 2009). 10 pharmaceuticals (atenolol, propanolol, metoprolol, nimesulide, furosemide, carbamazepine, ranitidine, metronidazole, paracetamol, and atorvastatin) from several therapeutic classes were searched in 54 sampling points along the river Po from the source to the delta, and at the mouth of its major effluents. In water samples were found pharmaceuticals in the range of 0.38–0.001 μg/L, except for furosemide (max conc. 0.605 μg/L), paracetamol (max conc. 3.59 μg/L), metoprolol (never detected) and for atenolol (not analysed). In sediment samples, only paracetamol was not detected, while the others were generally found in the range of 0.4–0.02 μg/kg ww with high concentrations for atenolol (max conc. 284 μg/kg ww) and furosemide (max conc. 98.4 μg/kg ww). The findings confirm also STPs as point sources of contamination. Despite of the much evidence for the adverse effects of pharmaceuticals in the aquatic environment, the observed low levels cannot be considered to pose a serious risk to human health; further studies are necessary for a comprehensive risk assessment

    The GENESIS project set out, principally, to review and develop scientific knowledge regarding groundwater systems, and to develop tools for better integrated management of these systems with related aquatic and terrestrial groundwater dependent ecosystems. The objectives of the project over its five year duration also included development of indicator methods, and of integrated model simulations applied to a series of representative European groundwater systems that incorporate new components on climate, land-use and pollution input changes. Special efforts have been made to link the project research to the ongoing process of implementing the Water Framework and Groundwater Directives (WFD and GWD respectively) – for example, examining the role of biogeochemical processes in pollutant degradation and the vulnerability of groundwater systems in the context of the GWD art.4(c) “appropriate investigation”. In addition, new methods were to be developed for assessing cost-effectiveness and the economic impacts resulting from changes in groundwater management practices across a range of the project case areas.This report aims to set out the main conclusions from each of the constituent work packages under which work has been done for the project. It will then go on to detail those conclusions that have relevance to policy making at the EU level, and those that are most relevant to decision makers at the Member State level as they seek to implement the WFD and GWD. Work Packages 1 and 7 have been excluded from this report as they were not concerned with substantive research work.<br/

    "Shoulder pain and limitation of motion in a young girl: think different"

    Background Primary Synovial Chondromatosis (PSC) is a rare benign tumor of the synovial membrane in which cartilage metaplasia produces calcific loose bodies within the articular space. Only a few cases are reported in the pediatric population and its etiology remains unknown. This condition typically affects large weight-bearing joints with pain, swelling and decrease range of motion. Due to its slow progressions, delayed diagnosis is frequent and differential diagnosis should consider other chronic arthritis and malignancies. While arthroscopic removal of loose bodies is the current treatment up to now, the association of partial or complete synovectomy is debated. Case presentation We report about a 14-year-old girl with a long-lasting right shoulder pain, especially during movements or exercise, localized tenderness and hypotonia of the glenohumeral joint. No previous trauma was mentioned. Blood exams, Mantoux test and plain radiography of the right shoulder were unremarkable. Ultrasound imaging revealed echogenic and calcified bodies stretching the glenohumeral joint and dislocating the long head of biceps tendon. Magnetic resonance showed a "rice-grain" pattern of the right shoulder. From an arthroscopic surgery, multiple loose white bodies were removed within the synovial membrane, and synovial chondromatosis was confirmed by histological analysis. At one month follow up visit, the patient completely recovered without pain. Conclusion Synovial chondromatosis is a very uncommon cause of mono articular pain in children, especially when it affects shoulder. Pediatricians should keep in mind this condition to avoid delayed diagnosis and treatment, even in consideration of the low risk of malignant transformation.
