    Appendicular peritonitis in situs inversus totalis: a case report

    <p>Abstract</p> <p>Introduction</p> <p><it>Situs inversus </it>is a congenital anomaly characterized by the transposition of the abdominal viscera. When associated with dextrocardia, it is known as <it>situs inversus totalis</it>. This condition is rare and can be a diagnostic problem when associated with appendicular peritonitis.</p> <p>Case presentation</p> <p>We report the case of a 20-year-old African man who presented to the emergency department with a 4-day history of diffuse abdominal pain, which began in his left iliac region and hypogastrium. After examination, we initiated a surgical exploration for peritonitis. We discovered a <it>situs inversus </it>at the left side of his liver, and his appendix was perforated in its middle third. A complementary post-operative thoracic and abdominal tomodensitometry revealed a <it>situs inversus totalis</it>.</p> <p>Conclusion</p> <p>Appendicular peritonitis in <it>situs inversus </it>is a rare association that can present a diagnostic problem. Morphologic exploration methods such as ultrasonography, tomodensitometry, magnetic resonance imaging, and laparoscopy may contribute to the early management of the disease and give guidance in choosing the most appropriate treatment for patients.</p

    Dynamics of multiple insecticide resistance in the malaria vector Anopheles gambiae in a rice growing area in South-Western Burkina Faso

    <p>Abstract</p> <p>Background</p> <p>Insecticide resistance of the main malaria vector, <it>Anopheles gambiae</it>, has been reported in south-western Burkina Faso, West Africa. Cross-resistance to DDT and pyrethroids was conferred by alterations at site of action in the sodium channel, the Leu-Phe <it>kdr </it>mutation; resistance to organophosphates and carbamates resulted from a single point mutation in the oxyanion hole of the acetylcholinesterase enzyme designed as <it>ace-1</it><sup><it>R</it></sup>.</p> <p>Methods</p> <p>An entomological survey was carried out during the rainy season of 2005 at Vallée du Kou, a rice growing area in south-western Burkina Faso. At the Vallée du Kou, both insecticide resistance mechanisms have been previously described in the M and S molecular forms of <it>An. gambiae</it>. This survey aimed i) to update the temporal dynamics and the circumsporozoite infection rate of the two molecular forms M and S of <it>An. gambiae </it>ii) to update the frequency of the Leu-Phe <it>kdr </it>mutation within these forms and finally iii) to investigate the occurrence of the <it>ace-1</it><sup><it>R </it></sup>mutation.</p> <p>Mosquitoes collected by indoor residual collection and by human landing catches were counted and morphologically identified. Species and molecular forms of <it>An. gambiae</it>, <it>ace-1</it><sup><it>R </it></sup>and Leu-Phe <it>kdr </it>mutations were determined using PCR techniques. The presence of the circumsporozoite protein of <it>Plasmodium falciparum </it>was determined using ELISA.</p> <p>Results</p> <p><it>Anopheles gambiae </it>populations were dominated by the M form. However the S form occurred in relative important proportion towards the end of the rainy season with a maximum peak in October at 51%. Sporozoite rates were similar in both forms. The frequency of the Leu-Phe <it>kdr </it>mutation in the S form reached a fixation level while it is still spreading in the M form. Furthermore, the <it>ace</it>-<it>1</it><sup><it>R </it></sup>mutation prevailed predominately in the S form and has just started spreading in the M form. The two mutations occurred concomitantly both in M and S populations.</p> <p>Conclusion</p> <p>These results showed that the Vallée du Kou, a rice growing area formerly occupied mainly by M susceptible populations, is progressively colonized by S resistant populations living in sympatry with the former. As a result, the distribution pattern of insecticide resistance mutations shows the occurrence of both resistance mechanisms concomitantly in the same populations. The impact of multiple resistance mechanisms in M and S populations of <it>An. gambiae </it>on vector control measures against malaria transmission, such as insecticide-treated nets (ITNs) and indoor residual spraying (IRS), in this area is discussed.</p

    Prévalence de la démence dans une population de personnes âgées sénégalaises

    Description Avec le vieillissement de la population, survient la démence dans la population de personnes âgées. Objectif. L’objectif de cette étude était d’estimer la prévalence de cette affection dans une population de personnes âgées sénégalaises. Méthodes Par une étude transversale qui s’est déroulée du 01 mars 2004 au 31 décembre 2005, des personnes âgées de 55 ans et plus venant consulter pour un problème de santé au Centre Médico-Social et Universitaire de l’IPRES (Sénégal) ont été évaluées sur le plan clinique et neuropsychologique. Des données sur les caractéristiques sociodémographiques, les antécédents médico-chirurgicaux et familiaux, le mode de vie, le réseau social ont été collectées à l’aide d’un questionnaire structuré avec des réponses fermées. Résultat La population à l’étude était composée de 872 personnes. Elles avaient un âge moyen de 67,2 ans ± 7,5.Elles étaient de sexe masculin (62,6%), mariées (79%), non instruites (50,7%). Dans les antécédents, l’HTA, le diabète, les affections respiratoires, les affections rhumatismales, la cataracte et les troublesdigestifs étaient les plus fréquents. Le tabagisme et l’alcoolisme était faible alors que la marche constituait la principale activité physique. Les personnes âgées vivaient en famille avec un bon réseau social.Cinquante huit (58) personnes ont présenté une démence (6,6%). La prévalence de la maladie variait de manière significative avec l’âge et l’instruction. Conclusion Ce résultat confirme que la prévalence de la démence varie en fonction de l’âge et de l’instruction de la personne âgée

    Genetic Diversification and Selection Strategies for Improving Sorghum Grain Yield Under Phosphorous-Deficient Conditions in West Africa

    Sorghum, a major crop for income generation and food security in West and Central Africa, is predominantly grown in low-input farming systems with serious soil phosphorus (P) deficiencies. This study (a) estimates genetic parameters needed to design selection protocols that optimize genetic gains for yield under low-phosphorus conditions and (b) examines the utility of introgressed backcross nested association mapping (BCNAM) populations for diversifying Malian breeding materials. A total of 1083 BC1F5 progenies derived from an elite hybrid restorer “Lata-3” and 13 diverse donor accessions were evaluated for yield and agronomic traits under contrasting soil P conditions in Mali in 2013. A subset of 298 progenies were further tested under low-P (LP) and high-P (HP) conditions in 2014 and 2015. Significant genetic variation for grain yield was observed under LP and HP conditions. Selection for grain yield under LP conditions was feasible and more efficient than the indirect selection under HP in all three years of testing. Several of the BCNAM populations exhibited yields under LP conditions that were superior to the elite restorer line used as a recurrent parent. The BCNAM approach appears promising for diversifying the male parent pool with introgression of diverse materials using both adapted Malian breed and unadapted landrace material from distant geographic origins as donors

    Schistosomiasis — Assessing Progress toward the 2020 and 2025 Global Goals

    BACKGROUND: With the vision of "a world free of schistosomiasis," the World Health Organization (WHO) set ambitious goals of control of this debilitating disease and its elimination as a public health problem by 2020 and 2025, respectively. As these milestones become imminent, and if programs are to succeed, it is important to evaluate the WHO programmatic guidelines empirically. METHODS: We collated and analyzed multiyear cross-sectional data from nine national schistosomiasis control programs (in eight countries in sub-Saharan Africa and in Yemen). Data were analyzed according to schistosome species (Schistosoma mansoni or S. haematobium), number of treatment rounds, overall prevalence, and prevalence of heavy-intensity infection. Disease control was defined as a prevalence of heavy-intensity infection of less than 5% aggregated across sentinel sites, and the elimination target was defined as a prevalence of heavy-intensity infection of less than 1% in all sentinel sites. Heavy-intensity infection was defined as at least 400 eggs per gram of feces for S. mansoni infection or as more than 50 eggs per 10 ml of urine for S. haematobium infection. RESULTS: All but one country program (Niger) reached the disease-control target by two treatment rounds or less, which is earlier than projected by current WHO guidelines (5 to 10 years). Programs in areas with low endemicity levels at baseline were more likely to reach both the control and elimination targets than were programs in areas with moderate and high endemicity levels at baseline, although the elimination target was reached only for S. mansoni infection (in Burkina Faso, Burundi, and Rwanda within three treatment rounds). Intracountry variation was evident in the relationships between overall prevalence and heavy-intensity infection (stratified according to treatment rounds), a finding that highlights the challenges of using one metric to define control or elimination across all epidemiologic settings. CONCLUSIONS: These data suggest the need to reevaluate progress and treatment strategies in national schistosomiasis control programs more frequently, with local epidemiologic data taken into consideration, in order to determine the treatment effect and appropriate resource allocations and move closer to achieving the global goals.     Harmonizing semantic annotations for computational models in biology

    Life science researchers use computational models to articulate and test hypotheses about the behavior of biological systems. Semantic annotation is a critical component for enhancing the interoperability and reusability of such models as well as for the integration of the data needed for model parameterization and validation. Encoded as machine-readable links to knowledge resource terms, semantic annotations describe the computational or biological meaning of what models and data represent. These annotations help researchers find and repurpose models, accelerate model composition and enable knowledge integration across model repositories and experimental data stores. However, realizing the potential benefits of semantic annotation requires the development of model annotation standards that adhere to a community-based annotation protocol.Without such standards, tool developers must account for a variety of annotation formats and approaches, a situation that can become prohibitively cumbersome and which can defeat the purpose of linking model elements to controlled knowledge resource terms. Currently, no consensus protocol for semantic annotation exists among the larger biological modeling community. Here, we report on the landscape of current annotation practices among the Computational Modeling in BIology NEtwork community and provide a set of recommendations for building a consensus approach to semantic annotation
