    Tissue contents and urinary excretion of taurine after administration of L-cysteine and L-2-oxothiazolidine-4-carboxylate to rats.

    &lt;p&gt;Tissue contents and urinary excretion of taurine were studied in rats after the administration of L-cysteine and its derivatives. Average taurine content in the liver of rats fed a 25% casein diet for 7 days increased 2-fold 2h after the intraperitoneal administration of 5 mmol of L-cysteine per kg of body weight, whereas that in rats fed a 5% casein diet for 2 days increased only slightly. The difference in the liver taurine contents between these two groups was discussed in relation to cysteine dioxygenase. Taurine contents in the heart, brain and blood did not differ significantly between these two groups or between the control and the group of rats which received L-cysteine. The increase in liver taurine concentrations after L-cysteine administration was much higher than that after L-cystine administration, suggesting a difference in their absorption. The intraperitoneal administration of 5 mmol/kg of L-2-oxothiazolidine-4-carboxylate (OTCA) resulted in a 3-fold increase in liver taurine content. The average increase in taurine excretion in the 24-h urine after OTCA administration corresponded to about 6.0% and that in the next 24-h urine to about 2.6% of OTCA administered, suggesting that nearly 10% of OTCA was metabolized to taurine and excreted in the urine.&lt;/p&gt;</p

    Biomechanical Study of Cervical Posterior Decompression

    Study Design Biomechanical study. Purpose We examined the dynamic characteristics of different operative procedures using three-dimensional finite-element models. Overview of Literature Cervical laminoplasty is often selected for patients requiring multilevel spinal decompression. Many laminoplasty techniques have been developed and are continually being improved. Methods Images were extracted for the two intervertebral joints between C3 and C5 in domestic rabbits. The following models were created: preoperative, laminectomy, Z-plasty, open door laminoplasty, French open door laminoplasty, en-bloc laminoplasty, and double-door laminoplasty. The loads required for sagittal rotation of 2.5°, 5°, and 7.5° were measured in vitro and respectively placed at the bottom of the C5 model. Displacement of the plate in the X, Y, and Z coordinates was measured and the rotational angles of the plate were determined. Results The sagittal rotation angle was greater in the laminectomy model than in the preoperative model, but was almost the same in the preoperative and Z-plasty models. Compared with the preoperative model, sagittal rotation angles were smaller in the French open door, open door, en-bloc, and double-door laminoplasty models. Conclusions Laminectomy and various types of laminoplasty are associated with different mechanical features, such as stability and mobility, allowing for selection of the most appropriate surgical procedure for each case

    Control of Dual-Output DC/DC Converters Using Duty Cycle and Frequency

    As part of the integration process of the auxiliary power systems of electric vehicles, plug-in hybrid vehicles and fuel cell vehicles, this study proposes a method to control two different voltage types using two control factors of the rectangular alternating waveforms contained in DC/DC converters, namely the duty cycle and frequency. A prototype circuit consisting of an H-bridge inverter, a transformer, two series resonant filters and two diode bridge circuits was constructed. The H-bridge inverter was connected to the primary side of the transformer and the diode bridge rectifier circuit was connected to the secondary side in parallel. Series resonant filters were inserted between one of the diode bridge circuits and the transformer. Thereafter, the proposed control method was applied to the transformer voltage of the prototype circuit. Although the circuit operation became complex owing to the circulating current flowing between the ground (GND) of the two output circuits, it exhibited ideal static and dynamic characteristics, thereby confirming the possibility of controlling two voltages with the duty cycle and frequency control factors. The results of the efficiency evaluation and loss analysis demonstrated a minimum efficiency of 68.3% and a maximum efficiency of 88.9%.     Gas Chromatographic Determination of Sulfuric Acid and Application to Urinary Sulfate

    A new gas chromatographic method for the determination of sulfate was developed. In this method, sulfate was quantitatively converted to a volatile derivative, dimethyl sulfate, by a two-step procedure. First, sulfate was converted to silver sulfate by reaction with silver oxide, and then to dimethyl sulfate by reaction with methyl iodide. The derivative was analyzed by gas chromatography. Methyl methanesulfonate was used as an internal standard. The method was applied to the determination of total urinary sulfate. Phosphate and chloride ions, which interfered with the present method, were eliminated with the use of basic magnesium carbonate and an excess of silver oxide, respectively. Recovery was over 96% when 5 to 40 mumol/ml of sulfate was added to human urine samples.</p

    Antiresorptive agent-related osteonecrosis of the jaw: Position Paper 2017 of the Japanese Allied Committee on Osteonecrosis of the Jaw

    Antiresorptive agent-related osteonecrosis of the jaw (ARONJ) is an intractable, though rare, complication in cancer patients with bone metastases and patients with osteoporosis who are treated with antiresorptive agents, including bisphosphonates and denosumab. Despite the more than 10 years that have passed since the first cases of bisphosphonate-related osteonecrosis of the jaw (BRONJ) were reported, our understanding of the epidemiology and pathophysiology of ARONJ remains limited, and data supported by evidence-based medicine are still sparse. However, the diagnosis and staging of ARONJ, identification of risk factors, and development of preventive and therapeutic approaches have advanced significantly over the past decade. The Position Paper 2017 is an updated version of the Position Paper 2010 of the Japanese Allied Committee on Osteonecrosis of the Jaw, which now comprises six Japanese academic societies. The Position Paper 2017 describes a new diagnostic definition for ARONJ, as proposed by the American Association of Oral and Maxillofacial Surgeons (AAOMS), summarizes our current understanding of the pathophysiology of ARONJ based on a literature search, and suggests methods for physicians and dentists/oral surgeons to manage the disease. In addition, the appropriateness of discontinuing antiresorptive medications (drug holiday) before, during, and after invasive dental treatments is discussed extensively. More importantly, the manuscript also proposes, for the first time, the importance of interactive communication and cooperation between physicians and dentists/oral surgeons for the successful treatment of ARONJ. The Position Paper 2017 is intended to serve as a guide for improving the management of ARONJ patients in Japan

    The Effect of a Blue Light Filtering Intraocular Lens on Macular Edema

    This study sought to compare the effects of either a blue light-filtering intraocular lens (blue-filtering IOL) or an ultraviolet light-filtering intraocular lens (UV-filtering IOL) on the incidence of angiographic macular edema (ME) 3 and 12 months after implantation. A prospective randomized parallel clinical study was performed at Showa University Hospital. Forty-five cataract patients randomly received either a blue-filtering IOL (n = 21) or a UV-filtering IOL (n = 24), and macular leakage was evaluated by fluorescence angiography. At 3 months, ME was 24% in the blue- and 25% in the UV-filtering IOL group. At 12 months, ME was 5% in the blue- and 21% in the UV-filtering IOL group. The recovery rate in the blue-filtering IOL group was higher than in the UV-filtering IOL group at 12 months after surgery (P = 0.0457). These results suggested that an implanted blue-filtering IOL is more effective for recovery of ME than a UV-filtering IOL

    Normal Values of Diffusion Tensor Magnetic Resonance Imaging Parameters in the Cervical Spinal Cord

    Study DesignProspective study.PurposeWe evaluated the usefulness of diffusion tensor imaging (DTI) in diagnosing patients with cervical myelopathy by determining the accuracy of normal DTI parameter values.Overview of LiteratureDTI can visualize white matter tracts in vivo and quantify anisotropy. DTI is known to be more sensitive than conventional magnetic resonance imaging (MRI) in detecting subtle pathological changes of the spinal cord.MethodsA total of 31 normal subjects (13 men and 18 women; age, 23-87 years; mean age, 46.0 years) were included in this study. The patients had no symptoms of myelopathy or radiculopathy. A Philips Achieva 3-Tesla MRI with SE-type Single Shot EPI was used to obtain diffusion tensor images. Apparent diffusion coefficient (ADC) and fractional anisotropy (FA) values were measured as DTI parameters on axial sections of several cervical levels. Subjects were divided into two groups: >40 years (n=16) and ≤40 years (n=15). A paired t-test was used to compare significant differences between the groups. ADC and FA values were most stable on axial sections.ResultsFor all subjects, mean ADC and FA values were 1.06±0.09×10-3 mm2/sec and 0.68±0.05, respectively. ADC was significantly higher in subjects >40 years of age than in those ≤40 years. There was no significant difference in FA values between the two groups. The mean ADC value was significantly higher in normal subjects >40 years of age than in those ≤40 years.ConclusionsIt is important to consider age when evaluating cervical myelopathy by DTI

    The incidence and distribution of biopsy-proven renal diseases in children

    The precise distribution pattern of pediatric renal diseases has seldom been reported especially for young children. In orderto clarify the incidence and distribution of renal diseases in children, 599 renal biopsy specimens obtained from 547 pediatricpatients (≦15 years old) were analyzed by routine light, electron and immunofluorescence microscopy. Among the total biopsycases, the most common renal disease identified in childhood was IgA glomerulonephritis (IgAGN; 37%). More than 80%of patients with IgAGN were discovered by urinary screening at the school, showing asymptomatic proteinuria and/orhematuria. The other major renal diseases were minor glomerular abnormality (12.2%), Henoch-Schönlein purpura nephritis(HSPN; 10.6%), and thin basement membrane disease (7.1%). One of the major causes of pediatric nephrotic syndrome wereminimal change nephrotic syndrome (58%), whereas the other causes were IgAGN (11.6%), HSPN (8.9%), membranousglomerulonephritis (5.4%), membranoproliferative glomerulonephritis (3.6%). In conclusion, IgAGN was the most commonrenal disease also in children as in adolescence. From the age of seven years, the incidence of IgA-GN was highest amongall the pediatric renal diseases during whole childhood period, and the incidence and number of the patients increased as theygrow. On the other hand, there are also many cases with non-progressive clinical courses, such as minor glomerular abnormalityand thin basement membrane disease in children. The renal biopsy following routine urinary screening at the schoolmay be important for the therapeutic and prognostic guidelines of pediatric group of patients with renal diseases

    Comparisons on Efficacy of Elcatonin and Limaprost Alfadex in Patients with Lumbar Spinal Stenosis and Concurrent Osteoporosis: A Preliminary Study Using a Crossover Design

    Study DesignMulticenter prospective study with a crossover design.PurposeThe objective of this study is to compare the efficacy of limaprost alfadex (LP) and elcatonin (EL) for lumbar spinal stenosis (LSS) patients with concurrent osteoporosis.Overview of LiteratureIt has been increasingly important to improve quality of life by establishing appropriate conservative treatments for LSS patients with concurrent osteoporosis who will presumably continue to increase due to the percentage of the aging elevations, however there is no prospective study.MethodsA total of 19 patients with LSS and concurrent osteoporosis were enrolled in this study. The patients were divided into two groups and compared using a crossover design. The Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) and short-form (SF)-8 health survey scale were used for clinical evaluations.ResultsThere was a significant improvement of buttock-leg pain and numbness in the EL group. A significant improvement of impaired walking function was noted for the LP group according to the JOABPEQ while the rest of the items in the JOABPEQ showed no significant differences. The SF-8 health survey revealed that somatic pains and physical summary scores in the EL group and physical functioning and physical summary scores in the LP group tended to improve but not to any statistically significant extents.ConclusionsConcomitant uses of EL may be useful in patients who do not respond satisfactorily to the treatments of LP for 6-8 weeks
