Predictors of mortality in patients with prosthetic valve infective endocarditis: A nation-wide multicenter study
Background: Our aim was to investigate the clinical and prognostic features of the patients with prosthetic valve endocarditis (PVE) in a multicenter nation-wide study.Methods: The present nation-wide study consisted of 75 consecutive patients with PVE treatedat 13 major hospitals in Turkey from 2005 to 2012.Results: The patients who died during follow-up were significantly older than the survivors and had higher C-reactive protein (CRP), creatinine, poor NYHA functional class and large vegetations. High creatinine level (odds ratio [OR] 2.6, 95% confidence interval [CI] 1.14–6.13), poor functional status (OR 24.5; 95% CI 3.1–196.5) and high CRP (OR 1.02; 95% CI1.00–1.03) measured on admission were independent risk associates for in-hospital mortality.Conclusions: High creatinine level, poor functional status and high CRP measured on admission were independent risk associates for in-hospital mortality, whereas a NYHA class ofIII/IV and high CRP reflected independent risk for stroke/mortality end point

A STUDY ON DEFENSE ACQUISITION MODELS WITH AN EMERGING MARKET PERSPECTIVE. THE CASE OF TURKEY
With the end of the Cold War, the static environment, characteristic of the bipolar world, has changed quickly into a dynamic and complicated environment with new actors. These changes have also affected the countries’ armed forces and new approaches to procurement/acquisition come into progress. After the Cold War, countries have focused on responding to changing threats with their decreasing defense budgets. In this study, acquisition models and their effects on Turkey’s technology acquisition and defense industry are evaluated. The evaluation is performed based on criteria such as contribution to national economy, customization, acquisition cost, time and risk, life cycle cost, and technology acquisition. Major acquisition models in this study are determined as direct procurement, production under license, joint venture, indigenous development, production via international consortium. Finally, it is observed that the best model is indigenous development while the others will serve as technology acquisition for indigenous development
Thermokinetics and synergistic effect analysis of peat-lignite coal co-pyrolysis
In this study, the synergistic effect between peat and lignite coal was investigated in terms of kinetic energy and mass loss trends using a thermogravimetric analyzer. Thermogravimetric non-isothermal studies were conducted at different heating rates under inert nitrogen atmosphere. Kinetic analysis was performed using model-free KAS, OFW, and Starink methods and the average activation energies varied between 88.62 kJ/mol and 204.28 kJ/mol. Differential mass loss trends were determined using experimental and theoretical data. An obvious deviation was obtained at a blend ratio of PT:CL40:60. Thus, results revealed that the PT and CL co-pyrolysis has synergistic effects, especially for sample PT:CL40:60. SEM and BET analyses were performed on the coke sample of peat, coal, and PT:CL40:60 blend ratio samples for further analysis. The surface area of peat and coal was determined to be 416 m2/g and 83.06 m2/g, respectively. It was concluded that the porosity and surface area of coal (187.02 m2/g) increased with the addition of peat. The results are expected to be useful in the design of peat biomass and coal co-pyrolysis systems.</p
The association of the Syntax score II with carotid intima media thickness and epicardial fat tissue
Aim: Syntax score II (SSII) is a highly predictive scoring system, which is used to improve individualized assessment of patients with complex coronary artery disease and facilitates clinical decision making. Surrogate markers [carotid intima-media thickness (CIMT), epicardial fat tissue (EFT)] are also used for risk assessment, but their relation with SSII is not well established.
Method: We enrolled 543 consecutive patients, who underwent coronary angiography for stable angina pectoris and acute coronary syndrome, in the study. SSII was calculated for each patient and the patients were divided into two groups as low SSII group and high SSII group according to their median SSII.
Results: The average age of the patients was 61.4 years and 75% of the patients were male. The multivariate analysis indicated that only EFT (p: 0,035), CIMT (p:0,04) and Hypertension (HT) (p: 0,014) were independently associated with high SSII.
Discussion: EFT and CIMT, the surrogate markers which can be simply and non-invasively determined, are of the independent predictors of high SSII. The inclusion of these parameters in the risk classification may provide additional clinical benefit
