    The present invention relates to a container, system, and method for creating parenteral solutions at a site, remote from sterile environments. The system includes a flexible container that is empty except for a prepackaged amount of a solute that is housed in the interior of the container. The container includes at least one port and a sterilizing filter in communication with an interior of the port. The container is so constructed and arranged that a fluid flow path is created from the port through the filter and into the interior of the container. A sterile water source including means for establishing fluid flow from the sterile water source into the port is provided. Accordingly, sterile water can flow from the sterile water source through the filter into the container where it is mixed with the solute to create a parenteral solution that can then be infused into a patient.     The present invention provides a system and method for creating on site, remote from a sterile environment, parenteral solutions in large volume parenteral containers for intravenous administration to a patient. In an embodiment, this system comprises an empty large volume container including at least one port for accessing an interior of the container. The port includes a sterilizing filter for sterilizing a fluid fed through the port into the container. A second container is provided including a solute and having means for coupling the second container to the large volume container and thereby providing fluid communication therebetween allowing the solute to be received within the interior of the container. A sterile water source is also provided including means for placing the sterile water source in fluid communication with the port and allowing water to flow from the sterile water source into the interior of the container. This allows the solute, and sterile water that has been fed through the filter, to create a parenteral solution in the large volume parenteral container

    Optimization of the Ugi Reaction Using Parallel Synthesis and Automated Liquid Handling

    The optimization of a Ugi reaction involving the mixing of furfurylamine, benzaldehyde, boc-glycine and t-butylisocyanide is described. Triplicate runs of 48 parallel experiments are reported, varying concentration, solvent and the excess of some of the reagents. The isolation of the product was achieved by a simple filtration and wash procedure. The highest yield obtained was 66% from 0.4 M methanol with 1.2 eq. of imine. This is significantly above the 49% yield obtained from the initial reaction under equimolar concentration at 0.4 M in methanol. Methanol solutions with reagent concentrations of 0.4M or 0.2M gave superior yields while all solvent systems at 0.07M performed poorly. At 0.2M, methanol and ethanol/methanol (60/40) mixtures were statistically equally good while THF/methanol (60/40) was poor and acetonitrile/methanol (60/40) was intermediate. Good reproducibility of the precipitate yields was obtained in these replicate experiments, allowing for subtle interaction effects to be positively identified

    Complement factor I deficiency: a potentially treatable cause of fulminant cerebral inflammation

    Objective To raise awareness of complement factor I (CFI) deficiency as a potentially treatable cause of severe cerebral inflammation. Methods Case report with neuroradiology, neuropathology, and functional data describing the mutation with review of literature. Results We present a case of acute, fulminant, destructive cerebral edema in a previously well 11-year-old, demonstrating massive activation of complement pathways on neuropathology and compound heterozygote status for 2 pathogenic mutations in CFI which result in normal levels but completely abrogate function. Conclusions Our case adds to a very small number of extant reports of this phenomenon associated with a spectrum of inflammatory histopathologies including hemorrhagic leukoencephalopathy and clinical presentations resembling severe acute disseminated encephalomyelitis. CFI deficiency can result in uncontrolled activation of the complement pathways in the brain resulting in devastating cerebral inflammation. The deficit is latent, but the catastrophic dysregulation of the complement system may be the result of a C3 acute phase response. Diagnoses to date have been retrospective. Diagnosis requires a high index of suspicion and clinician awareness of the limitations of first-line clinical tests of complement activity and activation. Simple measurement of circulating CFI levels, as here, may fail to diagnose functional deficiency with absent CFI activity. These diagnostic challenges may mean that the CFI deficiency is being systematically under-recognized as a cause of fulminant cerebral inflammation. Complement inhibitory therapies (such as eculizumab) offer new potential treatment, underlining the importance of prompt recognition, and real-time whole exome sequencing may play an important future role

    Soil quality assessment of an agroforestry system following long-term management in the Ozark Highlands

    The Soil Management Assessment Framework (SMAF) is a quantitative soil quality (SQ) evaluation tool that is widely applied to assess soil response to specific agricultural management practices over time. Considering the reported SQ benefits of agroforestry (AF) systems and the potential usefulness of SMAF, the objective of this study was to evaluate the effects of tree species (pecan [Carya illinoinensis (Wangenh.) K. Koch] and northern red oak [Quercus rubra L.]), soil fertility source (poultry litter [PL] and inorganic N fertilizer [control]), and soil depth (0–15 and 15–30 cm) on SMAF-derived SQ indices after 17 yr of management at an AF site in northwest Arkansas. Averaged across soil depth, soil organic C scores under red oak with PL application had a lower score (0.48) than red oak fertilized with inorganic N (0.60) and pecan receiving long-term PL applications (0.60), which did not differ from pecan with inorganic N fertilizer application (0.51). Averaged across soil depth, the soil quality index (SQI) for pecan receiving PL applications was 1.1 times greater than that under red oak receiving PL and soils under pecan receiving inorganic N fertilizer. Soil quality assessments use in AF are novel, as SMAF has not been used to identify soil health in these systems, although specific tree crop codes need to be developed in SMAF. Results of this study demonstrate that soils planted under various tree species respond dissimilarly to fertilizer sources and that management may improve overall SQ
