Effects of Co substitution on thermodynamic and transport properties and anisotropic in Ba(FeCo)As single crystals
Single crystalline samples of Ba(FeCo)As with
have been grown and characterized via microscopic, thermodynamic and transport
measurements. With increasing Co substitution, the thermodynamic and transport
signatures of the structural (high temperature tetragonal to low temperature
orthorhombic) and magnetic (high temperature non magnetic to low temperature
antiferromagnetic) transitions are suppressed at a rate of roughly 15 K per
percent Co. In addition, for superconductivity is stabilized,
rising to a maximum of approximately 23 K for and
decreasing for higher values. The phase diagram for
Ba(FeCo)As indicates that either superconductivity can
exist in both low temperature crystallographic phases or that there is a
structural phase separation. Anisotropic, superconducting, upper critical field
data () show a significant and clear change in anisotropy between
samples that have higher temperature structural phase transitions and those
that do not. These data show that the superconductivity is sensitive to the
suppression of the higher temperature phase transition
Sairausvakuutuksen matkakorvaukset liittyvät etenkin erikoissairaanhoitoon : Rekisteritutkimus vuonna 2012 tehdyistä matkoista
Kela maksaa sairaanhoitolain (1224/2004) perusteella korvauksia sairauteen ja raskauteen liittyvien matkojen – esimerkiksi sairaalaan, terveyskeskukseen, yksityiseen terveydenhuollon yksikköön – kustannuksista. Korvauksia maksetaan vastaavasti myös Kelan kuntoutukseen liittyvien matkojen kustannuksista.
Tässä raportissa tarkastellaan vuonna 2012 tehtyjen sairausvakuutuksen korvaamien matkojen ja maksettujen matkakorvausten kohdentumista matkan syyn tai matkakohteen mukaan. Korvauksia ei ole aiemmin tarkasteltu hoitopaikoittain tai sairauksittain, sillä Kelaan ei kerry näistä asioista tietoa korvauksia maksettaessa. Tutkimusaineistona käytettiin laajaa rekisteriaineistoa, jossa Kelan matkakorvausrekisteritietoihin yhdistettiin Terveyden ja hyvinvoinnin laitoksen hoitoilmoitusrekisterin tietoja toteutuneista sairaalahoidon (ml. terveyskeskusten vuodeosastohoito) hoitojaksoista ja poliklinikkakäynneistä. Lisäksi aineisto sisältää tiedot toteutuneesta Kelan kuntoutuksesta. Aineisto sisälsi tiedot kaikista vuonna 2012 tehdyistä sairausvakuutuksen korvaamista matkoista.
Vuonna 2012 tehdyistä 5,8 miljoonasta matkasta maksettiin noin 700 000 henkilölle sairausvakuutuksen korvauksia yhteensä 292 miljoonaa euroa. Tulosten mukaan kaksi kolmasosaa (67 %) maksetuista matkakorvauksista liittyi erikoissairaanhoitoon. Kaikista matkakorvauksista 38 % liittyi johonkin viiteen yliopistosairaalaan tehtyyn matkaan. Lukumääräisesti eniten matkoja liittyi Helsingin yliopistolliseen keskussairaalaan tehtyihin käynteihin, ja nämä matkat muodostivat noin 11 % matkakorvauksista. Sairauksittain tarkasteltuna eniten matkakorvauksia liittyi kasvaimiin, kuten syöpätauteihin.
Kelan kuntoutukseen liittyvien matkojen osuus vuoden 2012 matkakorvauksista oli noin 10 %. Valtaosa Kelan kuntoutuksen matkakorvauksista liittyi vaikeavammaisten kuntoutuksena annettavaan fysioterapiaan. Muiden matkojen, kuten terveyskeskuskäynteihin tai sairausvakuutuksen korvaamaan yksityiseen sairaanhoitoon suuntautuneiden matkojen, osuus matkakorvauksista oli 23 %
Hemodialyysikäynteihin liittyvät sairausvakuutuksen matkakorvaukset 13 miljoonaa euroa vuonna 2012
Tässä tutkimuksessa selvitettiin ensimmäistä kertaa valtakunnallisesti tiettyyn sairauteen tai hoitomuotoon liittyviä sairausvakuutuksen matkakorvauksia. Tarkastelukohteena olivat sairaalassa annettuun eli ns. keskushemodialysiin liittyvistä potilaiden matkoista maksetut sairausvakuutuksen matkakorvaukset vuonna 2012. Hemodialyysihoito eli ns. keinomunuaishoito on elintärkeä hoitomuoto, johon liittyy tyypillisesti 3 käyntikertaa viikossa, joten matkoja on paljon. Suomessa hemodialyysihoidot annetaan lähinnä sairaaloissa. Tutkimusaineistona käytettiin Kelan rekisteritietoja vuonna 2012 tehdyistä matkoista maksetuista matkakorvauksista yhdistettynä Terveyden ja hyvinvoinnin laitoksen hoitoilmoitusrekisterin tietoihin erikoissairaanhoidon käytöstä. Tulosten mukaan sairausvakuutuksen matkakorvaukset vuonna 2012 tehdyistä hemodialyysikäynneistä olivat 13 miljoonaa euroa. Matkat oli tehty pääasiassa taksilla tai invataksilla. Valtaosa hemodialyysimatkoista korvauksia saaneista oli saavuttanut sairausvakuutuksen matkakorvausten vuosittaisen omavastuun. Osa hemodialyysikäynneistä oli kuitenkin tapahtunut ilman sairausvakuutuksen matkakorvauksia, eli täysin potilaan omalla kustannuksella. Eniten korvauksia maksettiin Helsingin yliopistollisessa keskussairaalassa annettuun hemodialyysiin liittyen. Siellä myös hemodialyysikäyntimäärä oli kaikkein suurin, noin kaksinkertainen seuraavaksi eniten käyntejä toteuttaneeseen hoitopaikkaan verrattuna ja yli kymmenkertainen pienimpiin hoitopaikkoihin verrattuna. Preparation for the next major incident: are we ready? Comparing major trauma centres and other hospitals
OBJECTIVES:
A major incident is any emergency requiring special arrangements by the emergency services. All hospitals are required by law to keep a major incident plan (MIP) detailing the response to such events. In 2006 and 2019, we assessed the preparedness and knowledge of key individuals in hospitals across England and found a substantial gap in responding to the MIP. In this report, we compare responses from doctors at major trauma centres (MTCs) and other hospitals (non-MTCs).
METHODS:
We identified trusts in England that received over 30 000 patients through the ED in the fourth quarter of 2016/2017. We contacted the on-call anaesthetic, emergency, general surgery and trauma and orthopaedic registrar at each location and asked three questions assessing their confidence in using their hospital’s MIP: (1) Have you read your hospital’s MIP? (2) Do you know where you can access your hospital’s MIP guidelines? (3) Do you know what role you would play if an MIP came into effect while you are on call?
We compared data from MTCs and non-MTCs using multinomial mixed proportional odds models.
RESULTS:
There was a modest difference between responses from individuals at MTCs and non-MTCs for question 2 (OR=2.43, CI=1.03 to 5.73, p=0.04) but no evidence of a difference between question 1 (OR=1.41, CI=0.55 to 3.63, p=0.47) and question 3 (OR=1.78, CI=0.86 to 3.69, p=0.12). Emergency medicine and anaesthetic registrars showed significantly higher preparedness and knowledge across all domains. No evidence of a systematic difference in specialty response by MTC or otherwise was identified.
CONCLUSIONS:
Confidence in using MIPs among specialty registrars in England remains low. Doctors at MTCs tended to be better prepared and more knowledgeable, but this effect was only marginally significant. We make several recommendations to improve education on major incidents
Secondhand Smoke Exposure and Severity of Attention-Deficit/Hyperactivity Disorder in Preschoolers: A Pilot Investigation
Background: Less is known about the effects of secondhand smoke (SHS) on mental health as compared with other medical disorders.Objective: The aims of this study were to examine the following: 1) the association of SHS exposure with childhood attention-deficit/hyperactivity (ADHD) and disruptive disorders; and 2) the association of maternal recall of a child’s SHS exposure and that child’s exposure as measured by bioassay.Method: Sixty children had their saliva collected and assayed for cotinine when they were 4 years old and again when they were 6 years old. Phone interview data were collected to assess maternal recall of the children’s exposure to SHS at these ages. The children were assessed annually for ADHD and disruptive disorders. Repeated measures analysis of exposure level by child characteristics was performed.Results: Greater ADHD and conduct disorder severity scores were associated with greater child smoke exposure (ADHD severity, P = .043; conduct disorder severity, P = .035). A large proportion of mothers reported that their children had no exposure to SHS, despite high levels of measured cotinine in the children’s saliva.Conclusions: An association between SHS exposure and ADHD and conduct disorder symptoms was found. Children and parents may benefit from parent education regarding the deleterious effects of SHS
