    Using health check data to understand risks for dementia and cognitive impairment among Torres Strait Islander and Aboriginal peoples in northern Queensland—a data linkage study

    Objective: High rates of dementia are evident in First Nations populations, and modifiable risk factors may be contributing to this increased risk. This study aimed to use a longitudinal dataset to gain insights into the long-term risk and protective factors for dementia and cognitive impairment not dementia (CIND) in a Torres Strait Islander and Aboriginal population in Far North Queensland, Australia. Study Design and Setting: Probabilistic data linkage was used to combine baseline health check data obtained in 1998/2000 and 2006/2007 for 64 residents in remote communities with their results on a single dementia assessment 10–20 years later (2015–2018). The relationship between earlier measures and later CIND/dementia status was examined using generalized linear modeling with risk ratios (RRs). Due to the small sample size, bootstrapping was used to inform variable selection during multivariable modeling. Results: One third of participants (n = 21, 32.8%) were diagnosed with dementia (n = 6) or CIND (n = 15) at follow-up. Secondary school or further education (RR = 0.38, 95% CI 0.19–0.76, p = 0.006) and adequate levels of self-reported physical activity (RR = 0.26, 95% CI 0.13–0.52, p < 0.001) were repeatedly selected in bootstrapping and showed some evidence of protection against later CIND/dementia in final multivariate models, although these had moderate collinearity. Vascular risk measures showed inconclusive or unexpected associations with later CIND/dementia risk. Conclusions: The preliminary findings from this small study highlighted two potential protective factors for dementia that may be present in this population. A tentative risk profile for later CIND/dementia risk is suggested, although the small sample size limits the applicability of these findings

    Dementia Risk Models in an Australian First Nations Population: Cross-Sectional Associations and Preparation for Follow-Up

    Background: Reducing the burden of dementia in First Nations populations may be addressed through developing population specific methods to quantify future risk of dementia. Objective: To adapt existing dementia risk models to cross-sectional dementia prevalence data from a First Nations population in the Torres Strait region of Australia in preparation for follow-up of participants. To explore the diagnostic utility of these dementia risk models at detecting dementia. Methods: A literature review to identify existing externally validated dementia risk models. Adapting these models to cross-sectional data and assessing their diagnostic utility through area under the receiver operating characteristic curve (AUROC) analyses and calibration using Hosmer-Lemeshow Chi2. Results: Seven risk models could be adapted to the study data. The Aging, Cognition and Dementia (AgeCoDe) study, the Framingham Heart Study (FHS), and the Brief Dementia Screening Indicator (BDSI) had moderate diagnostic utility in identifying dementia (i.e., AUROC >0.70) before and after points for older age were removed. Conclusion: Seven existing dementia risk models could be adapted to this First Nations population, and three had some cross-sectional diagnostic utility. These models were designed to predict dementia incidence, so their applicability to identify prevalent cases would be limited. The risk scores derived in this study may have prognostic utility as participants are followed up over time. In the interim, this study highlights considerations when transporting and developing dementia risk models for First Nations populations

    A Magellan M2FS Spectroscopic Survey of Galaxies at 5.5<z<6.8: Program Overview and a Sample of the Brightest Lyman-alpha Emitters

    We present a spectroscopic survey of high-redshift, luminous galaxies over four square degrees on the sky, aiming to build a large and homogeneous sample of Lyα\alpha emitters (LAEs) at z≈5.7z\approx5.7 and 6.5, and Lyman-break galaxies (LBGs) at 5.5<z<6.85.5<z<6.8. The fields that we choose to observe are well-studied, such as SXDS and COSMOS. They have deep optical imaging data in a series of broad and narrow bands, allowing efficient selection of galaxy candidates. Spectroscopic observations are being carried out using the multi-object spectrograph M2FS on the Magellan Clay telescope. M2FS is efficient to identify high-redshift galaxies, owing to its 256 optical fibers deployed over a circular field-of-view 30 arcmin in diameter. We have observed ∼2.5\sim2.5 square degrees. When the program is completed, we expect to identify more than 400 bright LAEs at z≈5.7z\approx5.7 and 6.5, and a substantial number of LBGs at z≥6z\ge6. This unique sample will be used to study a variety of galaxy properties and to search for large protoclusters. Furthermore, the statistical properties of these galaxies will be used to probe cosmic reionization. We describe the motivation, program design, target selection, and M2FS observations. We also outline our science goals, and present a sample of the brightest LAEs at z≈5.7z\approx5.7 and 6.5. This sample contains 32 LAEs with Lyα\alpha luminosities higher than 1043^{43} erg s−1^{-1}. A few of them reach ≥3×1043\ge3\times10^{43} erg s−1^{-1}, comparable to the two most luminous LAEs known at z≥6z\ge6, `CR7' and `COLA1'.     HST observations of the Local Group dwarf galaxy Leo I

    We present deep HST F555W (V) and F814W (I) observations of a central field in the Local Group dwarf spheroidal (dSph) galaxy Leo I. The resulting color-magnitude diagram (CMD) reaches I \simeq 26 and reveals the oldest ~10-15 Gyr old turnoffs. Nevertheless, a horizontal branch is not obvious in the CMD. Given the low metallicity of the galaxy, this likely indicates that the first substantial star formation in the galaxy may have been somehow delayed in Leo I in comparison with the other dSph satellites of the Milky Way. The subgiant region is well and uniformly populated from the oldest turnoffs up to the 1 Gyr old turnoff, indicating that star formation has proceeded in a continuous way, with possible variations in intensity but no big gaps between successive bursts, over the galaxy's lifetime. The structure of the red-clump of core He-burning stars is consistent with the large amount of intermediate-age population inferred from the main sequence and the subgiant region. In spite of the lack of gas in Leo I, the CMD clearly shows star formation continuing until 1 Gyr ago and possibly until a few hundred Myrs ago in the central part of the galaxy.Comment: 26 pages with 8 figures (fig 2 not available electronically).     Primary care biomarkers and dementia in people of the Torres Strait, Australia: extended data analysis

    Objective: Dementia disproportionately affects First Nations populations. Biomarkers collected in primary care may assist with determining dementia risk. Our previous underpowered study showed some suggestive associations between baseline biomarkers with follow-up dementia or cognitive impairment. The current study extended this work with a larger linked dataset. Study design and setting: Probabilistic data linkage was used to combine four baseline datasets with one follow-up assessment of dementia status 0–20 years later in a First Nations population in Australia. Mixed Effects Generalized Linear Regression models were used to test associations between baseline measures and follow-up status, accounting for repeated measures within individuals. Results: Linked data were available for 88 individuals, with 101–279 baseline observations, depending on the type of measure. Higher urinary albumin to creatine ratio was associated with greater risk of cognitive impairment/dementia, whereas body weight and key lipid markers were negatively associated. There was no clear trend when these associations were examined by timing of measurement (i.e., ≤10 years or >10 years before a dementia assessment). Conclusions: The results of this study support findings from our previous work and indicate that microalbuminuria can be an early indicator of dementia risk in this population. The weight and lipid profile findings reflect the mixed results in the published literature and require further investigation and interpretation

    Investigating the purpose of an online discussion group for health professionals: a case example from forensic occupational therapy

    Background: Thousands of health-related online discussion groups are active world-wide however, very little is known about the purpose and usefulness of such groups. In 2003 an online discussion group called ‘forensic occupational therapy' was established in the United Kingdom. This group was examined to gain an understanding of the purpose and use of online discussion groups for health professionals who may be practically and geographically isolated from others in similar areas of practice. Methods: Following a case study design, descriptive characteristics on members' locations and number of posts were collected from the forensic occupational therapy online discussion group. Eight years of posts (2003-2011) were examined using a theoretical thematic analysis process to identify and describe the purposes for which members were using the group. Results: Members from 20 countries contributed to the discussion group; the vast majority of posts being from members in the United Kingdom. Activity within the group was consistently high for the first five years however, activity within the group declined in the final three years. Six purposes for which members use the online discussion group were identified: seeking and giving advice, networking, requesting and sharing material resources, service development, defining the role of occupational therapists, and student learning. Conclusions: Findings suggest that health professionals in specialized and often isolated areas of practice are keen to connect with colleagues and learn from each other's experiences. The main purposes for which the online discussion group was used could be summarized as communication, information sharing and networking; though activity within the group declined significantly during the last three years of the data collection period. This raises questions about the sustainability of online discussion groups within the rapidly developing social media environment
