    Análise da influência das redes sociais na busca por procedimentos cosméticos entre acadêmicos de medicina

    INTRODUÇÃO: O objetivo deste trabalho foi avaliar a influência da mídia no interesse de acadêmicos de medicina por procedimentos estéticos. MÉTODOS: Empregou-se um questionário online a 322 estudantes de medicina da UNINOVAFAPI em 2021. Às variáveis estudadas aplicou-se o teste qui-quadrado, intervalo de confiança de 95%. RESULTADOS: A maioria da amostra (72%) era do sexo feminino, e 79,5% cogitaram submeter-se a procedimentos influenciados por anúncios. Mais mulheres (52%) compararam suas fotos nas redes (p=0,0005) e se sentem desconfortáveis em publicar fotos sem filtros (46%; p=0,0002) em comparação aos homens. CONCLUSÃO: O uso de mídias sociais se associa à maior probabilidade de estudantes de medicina, principalmente mulheres, submeterem-se a procedimentos estéticos, compararem-se com outras pessoas e se sentirem desconfortáveis sem filtro

    Human immunodeficiency virus/Leishmania infantum in the first foci of urban American visceral leishmaniasis: clinical presentation from 1994 to 2010

    INTRODUCTION: Human immunodeficiency virus (HIV) coinfection with Leishmania infantum or Leishmania donovani, the agents of visceral leishmaniasis (or kala-azar), has become a fatal public health problem in the tropics where kala-azar is endemic. METHODS: The clinical presentation of patients with HIV and L. infantum coinfection is described using two unique databases that together produce the largest case series of patients with kala-azar infected with HIV in South America. First, a retrospective study paired the list of all patients with kala-azar from 1994 to 2004 with another of all patients with HIV/AIDS from the reference hospital for both diseases in the City of Teresina, State of Piauí, Brazil. Beginning in 2005 through to 2010 this information was prospectively collected at the moment of hospitalization. RESULTS: During the study, 256 admissions related to 224 patients with HIV/L. infantum coinfection were registered and most of them were males between 20-40 years of age. Most of the 224 patients were males between 20-40 years of age. HIV contraction was principally sexual. The most common symptoms and signs were pallor, fever, asthenia and hepatosplenomegaly. 16.8% of the cohort died. The primary risk factors associated to death were kidney or respiratory failure, somnolence, hemorrhagic manifestations and a syndrome of systemic inflammation. The diagnosis of HIV and kala-azar was made simultaneously in 124 patients. CONCLUSIONS: The urban association between HIV and kala-azar coinfection in South America is worrisome due to difficulty in establishing the diagnosis and higher mortality among the coinfected then those with either disease independently. HIV/L. infantum coinfection exhibits some singular characteristics and due to its higher mortality it requires immediate assistance to patients and greater research on appropriate combination therapy

    HOSPITALIZAÇÕES DE VÍTIMAS POR QUEIMADURAS NO PIAUÍ NA DÉCADA DE 2013 A 2022: ANÁLISE EPIDEMIOLÓGICA

    <p>Tem-se poucos dados a respeito do perfil dos pacientes queimados no estado do Piauí. Diante disso, aventou-se essa pesquisa a fim de avaliar o perfil epidemiológico dos pacientes internados em hospitais deste estado. Os dados foram coletados através da plataforma DATASUS. Houve 4.186 hospitalizações de vítimas por queimaduras, a maioria do sexo masculino (n=2.705; 64,7%), e no feminino, 1.481 internações (35,3%). A taxa de mortalidade foi de 1,89% (n=79). Houve predomínio em pacientes de 1-4 anos (n=1.099), seguidos pelos de 30-39 anos (n=615). A faixa etária que registrou mais óbitos foi 40-49 anos (n=14), entretanto a taxa de mortalidade foi vertiginosamente maior na faixa de 80 anos ou mais (18,87%). Houve predominância no caráter de urgência (n=4.149). A maioria foi atendida no Meio Norte (n=4.025). A maioria das vítimas era do sexo masculino com uma quantidade de óbitos que ultrapassou duas vezes e meia em comparação ao sexo feminino, e a taxa de mortalidade foi maior em pessoas de cor preta. A maioria das hospitalizações ocorreu em pacientes de 1-4 anos e em adultos de 30-39 anos, mas a taxa de mortalidade foi maior em idosos. A maioria das hospitalizações foi de urgência e ocorreu na macrorregião do Meio Norte. As flutuações anuais sugerem um novo pico de óbitos em 2023. A prevenção e o tratamento adequado das queimaduras devem ser reforçados.</p&gt

    Recurring fibrous dysplasia of anthro maxillary with cranial base invasion

    Introduction: Fibrous dysplasia is an osseous lesion with an unknown etiology. It is characterized by the osseous maturation insufficiency. It may affect any bone, but the affection of craniofacial bones is the most critical for otorhinolaryngology. Maxilla is the most affected facial bone and the orbitary invasion is an uncommon event. The symptoms are unspecific and for its low suspicion and uncommonness, the diagnosis is generally late. The monostotic form presents a slow growth and asymptomatic course and needs to be followed up. The polyostotic type has a progressive behavior and is associated to recurrence and complications. Objective: To present two cases of patients with fibrous dysplasia diagnosis and describe the clinical presentation, radiological findings and the treatment of this pathology. Cases Report: Two cases of fibrous dysplasia are reported, which initially presented unspecific symptomatology, but with characteristic radiologic signs. They were submitted to surgical treatment for resection of the lesions and evolved with frequent recurrences with extensive affection of the facial sinuses, one patient had cranial base invasion and frontal craniotomy was needed for tumoral excision. Final Comments: Fibrous dysplasia is an uncommon osteopathy. The tomography is the choice method for characterization of the tumoral expansion, and helps in the surgical planning. The surgical strategy is indicated for symptomatic lesions, functions alterations or anatomic disorders. O desempenho do IT-LEISH® foi comparado ao da reação de imunofluorescência indireta; e ao da reação imunoenzimática, usando-se antígeno solúvel de Leishmania chagasi e recombinante K39 (rK39). O estudo incluiu 332 pacientes com quadro clínico sugestivo de LV: 213 casos de LV confirmados parasitologicamente; e 119 não-casos, com confirmação de outra etiologia. O teste IT-LEISH® apresentou sensibilidade de 93% e especificidade de 97%. As técnicas RIFI (imunofluorescência indireta), ELISA L. chagasi e ELISA rK39 apresentaram sensibilidade de 88%, 92% e 97% e especificidades de 81%, 77% e 84%, respectivamente.     Polymorphisms in GSTE2 is associated with temephos resistance in Aedes aegypti

    The glutathione S-transferases (GSTs) are enzymes involved in several distinct biological processes. In insects, the GSTs, especially delta and epsilon classes, play a key role in the metabolism of xenobiotics used to control insect populations. Here, we investigated its potential role in temephos resistance, examining the GSTE2 gene from susceptible (RecL) and resistant (RecR) strains of the mosquito Aedes aegypti, vector for several pathogenic arboviruses. Total GST enzymatic activity and the GSTE2 gene expression profile were evaluated, with the GSTE2 cDNA and genomic loci sequenced from both strains. Recombinant GSTE2 and mutants were produced in a heterologous expression system and assayed for enzyme kinetic parameters. These proteins also had their 3D structure predicted through molecular modeling. Our results showed that RecR has a profile of total GST enzymatic activity higher than RecL, with the expression of the GSTE2 gene in resistant larvae increasing six folds. Four exclusive RecR mutations were observed (L111S, I150V, E178A and A198E), which were absent in the laboratory susceptible strains. The enzymatic activity of the recombinant GSTE2 showed different kinetic parameters, with the GSTE2 RecR showing an enhanced ability to metabolize its substrate. The I150V mutation was shown to induce significant changes in catalytic parameters and a 3D modeling of GSTE2 mapped two of the RecR changes (L111S and I150V) near the enzyme's catalytic pocket, also implying an impact on its catalytic activity. Our results reinforce a potential role for GSTE2 in the metabolic resistance phenotype while contributing to the understanding of the molecular basis for the resistance mechanism

The use of an easy-to-use and interpret, sensitive, and specific method that requires no complex infrastructure or specialized professionals, such as direct agglutination test (DAT) and the rK39-based rapid immunochromatographic test may enhance the diagnosis of disease. This study evaluated the performance of a rapid test (DiaMed- IT-LEISH®) and the DAT for the diagnosis of VL in 213 parasitologically confirmed cases and 119 controls with clinical suspicion of VL and confirmation of another etiology. The sensitivities and specificities of the rapid test were 93% and 97%, respectively and those of the DAT were 90% and 96%, respectively. The positive predictive values of the rapid test and the DAT were 98% and 97%, respectively and the negative predictive values were 89% and 84%, respectively. The Kappa index showed agreement between both methods classified as substantial (0.77). This study showed that the DAT and the rapid test can be used to diagnose VL in Brazil, following a pilot study for implementation of the rapid test in the health service
