Preparation of some new benzo[d]thiazole derivatives
In this work, four new benzo[d]thiazole derivatives were synthesized successfully from vanillin. Nitration of vanillin gave nitrovanillin followed by cyclization reaction with o-aminothiophenol under microwave irradiation in 4 minutes to give nitroaromatic compound 3. The reduction to convert the nitro group to amino group was optimized. It was found that Fe/ con. HCl in ethanol was the best condition for this case about both yield (~95 %) and simple procedure to give compound 4 as a salt. Acetylation occurs at both phenolic hydroxyl group and amino group of the salt 4 to form N,O-acetyl compound 5. Under mild hydrolysis 5 produces N-acetyl compound 6. Childhood Obesity as Interactions of Environmental and Genetic Factors: A Community – Based Study on Primary School Children of Hanoi, Vietnam
Objective: Obesity is a complex disease that involves interactions between environmental and genetic factors in its pathogenesis. The aims of this present study were to analyse the overall effects of some nutritional, physical and genetic factors with childhood obesity, and to build predictive models of childhood obesity.
Material and Methods: A case-control study was conducted on 559 Hanoi primary school children (278 obese cases and 281 normal controls). Genetic analysis was performed to evaluate genotype on the rs6265 gene BDNF, rs6548238 gene TMEM18, rs6499640 gene FTO, and rs17782313 gene MC4R. Univariate and multivariate regression analysis were conducted to compare the two groups of children, so as to draw conclusions regarding influencing factors and to establish predictive models of obesity for Hanoi primary school children.
Results: Factors increasing the risk of obesity found included: urban living areas (odds ratio (OR)=1.5), caesarean section (OR=2.1), high birth weight (OR=1.9-3.1), parental overweight and obesity (OR=2.1-5.1), short night sleeping times (OR=2.6), characteristics of child feeding; including early weaning (OR=2.1), gluttony (OR=19.1), no snacking (OR=2.4), controlled eating (OR=2.4) fat food hobbies (OR=2.7), and CC rs6548238-TMEM18 genotype (OR=1.8). A predictive model of childhood obesity for Hanoi primary school children was built, based on the BMI of parents, gluttonous characteristics, unrestricted eating with ROC=0.871.
Conclusion: Childhood obesity develops as a result of interaction between eating habits, lifestyle characteristics of the child and genetic characteristics inherited from over nutritious parents. Encouraging parents to manage both their own weight and their children’s eating habits such as allowing children to consume food without undue restriction and reducing gluttony can serve as effective interventions to diminish the prevalence of childhood obesity within the community
Prognostic Values of Serum Lactate-to-Bicarbonate Ratio and Lactate for Predicting 28-Day IN-Hospital Mortality in Children With Dengue Shock Syndrome
This study aimed to assess the clinical utility of blood lactate-to-bicarbonate (L/B) ratio, as a prognostic factor for 28-day in-hospital mortality in children with dengue shock syndrome (DSS), admitted to the pediatric intensive care unit (PICU). This single-center retrospective study was conducted at a tertiary children hospital in southern Vietnam from 2013 to mid-2022. Prognostic models for DSS mortality were developed, using a predefined set of covariates in the first 24 hours of PICU admission. Area under the curves (AUCs), multivariable logistic and Least Absolute Shrinkage and Selection Operator (LASSO) regressions, bootstrapping and calibration slope were performed. A total of 492 children with DSS and complete clinical and biomarker data were included in the analysis, and 26 (5.3%) patients died. The predictive values for DSS mortality, regarding lactate showing AUC 0.876 (95% CI, 0.807-0.944), and that of L/B ratio 0.867 (95% CI, 0.80-0.934) (P values of both biomarkers \u3c .001). The optimal cutoff point of the L/B ratio was 0.25, while that of lactate was 4.2 mmol/L. The multivariable model showed significant clinical predictors of DSS fatality including severe bleeding, cumulative amount of fluid infused and vasoactive-inotropic score (\u3e30) in the first 24 hours of PICU admission. Combined with the identified clinical predictors, the L/B ratio yielded higher prognostic values (odds ratio [OR] = 8.66, 95% confidence interval [CI], 1.96-38.3; P \u3c .01) than the lactate-based model (OR = 1.35, 95% CI, 1.15-1.58; P \u3c .001). Both the L/B and lactate models showed similarly good performances. Considering that the L/B ratio has a better prognostic value than the lactate model, it may be considered a potential prognostic biomarker in clinical use for predicting 28-day mortality in PICU-admitted children with DSS
Multiple Wolbachia strains provide comparative levels of protection against dengue virus infection in Aedes aegypti.
The insect bacterium Wolbachia pipientis is being introgressed into Aedes aegypti populations as an intervention against the transmission of medically important arboviruses. Here we compare Ae. aegypti mosquitoes infected with wMelCS or wAlbB to the widely used wMel Wolbachia strain on an Australian nuclear genetic background for their susceptibility to infection by dengue virus (DENV) genotypes spanning all four serotypes. All Wolbachia-infected mosquitoes were more resistant to intrathoracic DENV challenge than their wildtype counterparts. Blocking of DENV replication was greatest by wMelCS. Conversely, wAlbB-infected mosquitoes were more susceptible to whole body infection than wMel and wMelCS. We extended these findings via mosquito oral feeding experiments, using viremic blood from 36 acute, hospitalised dengue cases in Vietnam, additionally including wMel and wildtype mosquitoes on a Vietnamese nuclear genetic background. As above, wAlbB was less effective at blocking DENV replication in the abdomen compared to wMel and wMelCS. The transmission potential of all Wolbachia-infected mosquito lines (measured by the presence/absence of infectious DENV in mosquito saliva) after 14 days, was significantly reduced compared to their wildtype counterparts, and lowest for wMelCS and wAlbB. These data support the use of wAlbB and wMelCS strains for introgression field trials and the biocontrol of DENV transmission. Furthermore, despite observing significant differences in transmission potential between wildtype mosquitoes from Australia and Vietnam, no difference was observed between wMel-infected mosquitoes from each background suggesting that Wolbachia may override any underlying variation in DENV transmission potential
Ventilator-associated respiratory infection in a resource-restricted setting: impact and etiology.
BACKGROUND: Ventilator-associated respiratory infection (VARI) is a significant problem in resource-restricted intensive care units (ICUs), but differences in casemix and etiology means VARI in resource-restricted ICUs may be different from that found in resource-rich units. Data from these settings are vital to plan preventative interventions and assess their cost-effectiveness, but few are available. METHODS: We conducted a prospective observational study in four Vietnamese ICUs to assess the incidence and impact of VARI. Patients ≥ 16 years old and expected to be mechanically ventilated > 48 h were enrolled in the study and followed daily for 28 days following ICU admission. RESULTS: Four hundred fifty eligible patients were enrolled over 24 months, and after exclusions, 374 patients' data were analyzed. A total of 92/374 cases of VARI (21.7/1000 ventilator days) were diagnosed; 37 (9.9%) of these met ventilator-associated pneumonia (VAP) criteria (8.7/1000 ventilator days). Patients with any VARI, VAP, or VARI without VAP experienced increased hospital and ICU stay, ICU cost, and antibiotic use (p < 0.01 for all). This was also true for all VARI (p < 0.01 for all) with/without tetanus. There was no increased risk of in-hospital death in patients with VARI compared to those without (VAP HR 1.58, 95% CI 0.75-3.33, p = 0.23; VARI without VAP HR 0.40, 95% CI 0.14-1.17, p = 0.09). In patients with positive endotracheal aspirate cultures, most VARI was caused by Gram-negative organisms; the most frequent were Acinetobacter baumannii (32/73, 43.8%) Klebsiella pneumoniae (26/73, 35.6%), and Pseudomonas aeruginosa (24/73, 32.9%). 40/68 (58.8%) patients with positive cultures for these had carbapenem-resistant isolates. Patients with carbapenem-resistant VARI had significantly greater ICU costs than patients with carbapenem-susceptible isolates (6053 USD (IQR 3806-7824) vs 3131 USD (IQR 2108-7551), p = 0.04) and after correction for adequacy of initial antibiotics and APACHE II score, showed a trend towards increased risk of in-hospital death (HR 2.82, 95% CI 0.75-6.75, p = 0.15). CONCLUSIONS: VARI in a resource-restricted setting has limited impact on mortality, but shows significant association with increased patient costs, length of stay, and antibiotic use, particularly when caused by carbapenem-resistant bacteria. Evidence-based interventions to reduce VARI in these settings are urgently needed
Childhood obesity is a High-risk factor for hypertriglyceridemia: A case-control study in Vietnam
Objectives: To investigate the relationship between dyslipidemia and obesity status among Viet-namese adolescents.
Methods: In this case-control study, 282 adolescents (6–11 years), including 88 obese cases and 194 normal-weight controls, were recruited from a population-based cross-sectional study from two provinces in Vietnam. The anthropometric, blood lipid, and other laboratory test results of the study subjects were analyzed.
Results: Obese children tended to have more visceral fat (Pearson’s r = 0.795, p < 0.0001) than subcutaneous fat (Pearson’s r = 0.754, p < 0.0001), and this difference was associated with an increase in blood triglyceride level (Pearson’s r = 0.232, p < 0.05) and a strikingly high rate of hypertriglyceridemia (38.6%). We also found that birth weight and parental body mass index were related to the status of obesity among the study subjects. However, only birth weight was significantly higher in the obese group than in the normal weight group. These findings indicate the effect of prenatal nutrition on childhood obesity. Furthermore, high-birth weight children had a surprisingly high rate of obesity.
Conclusion: Together, our data suggest that obesity increased the risk for hypertriglyceridemia, which was, at least partially, due to prenatal nutrition
Physiological properties of new species of Acidithiobacillus isolated from abandoned Tin mine in Ha Thuong, Thai Nguyen province: Research article
Acidophilic bacteria are able to tolerate acidic environment and also contribute to the lowering of environmental pH value, implying potential applications in metal-leaching technology extracting metals from tailings and electronic wastes. In this study, we conducted a sampling campaign in abandoned Tin mine in Ha Thuong, Thai Nguyen province, to isolate acidophilic bacteria and to study physiological characteristics of the isolated bacteria. As a result, two acidophilic bacteria were successfully isolated and identification by 16S rDNA gene sequences showed that the two bacteria are similar to Acidithiobacillus ferrooxidans and Acidithiobacillus thiooxidans (98% and 94 % of similarity, respectively). Both strains are tolerant of pH in the range of 3 and have the ability to grow optimally at temperatures of 30°C.Vi khuẩn ưa axit có ý nghĩa ứng dụng trong công nghệ tách rút kim loại từ quặng đuôi và ngay cả từ rác thải linh kiện điện tử. Trong nghiên cứu này, chúng tôi đã tiến hành thu mẫu ở mỏ thiếc bỏ hoang ở Hà Thượng, Thái Nguyên nhằm phân lập được nhóm vi khuẩn ưa axit và qua đó nghiên cứu một số đặc điểm sinh lý của vi khuẩn này. Kết quả chúng tôi đã phân lập được hai chủng vi khuẩn ưa axit. Định dạng bằng nhận diện trình tự gen 16S rADN cho thấy hai vi khuẩn này có độ tương đồng là 98% với vi khuẩn Acidithiobacillus ferrooxidans và 94% Acidithiobacillus thiooxidans. Cả hai chủng vi khuẩn đều có tính chịu pH trong khoảng 3 và có khả năng sinh trưởng tối ưu ở nhiệt độ 30oC
