    Gender Differences in Current Received during Transcranial Electrical Stimulation.

    Low current transcranial electrical stimulation (tCS) is an effective but somewhat inconsistent tool for augmenting neuromodulation. In this study, we used 3D MRI guided electrical transcranial stimulation modeling to estimate the range of current intensities received at cortical brain tissues. Combined T1, T2, and proton density MRIs from 24 adult subjects (12 male and 12 female) were modeled with virtual electrodes placed at F3, F4, C3, and C4. Two sizes of electrodes 20 mm round and 50 mm × 45 mm were examined at 0.5, 1, and 2 mA input currents. The intensity of current received was sampled in a 1-cm sphere placed at the cortex directly under each scalp electrode. There was a 10-fold difference in the amount of current received by individuals. A large gender difference was observed with female subjects receiving significantly less current at targeted parietal cortex than male subjects when stimulated at identical current levels (P < 0.05). Larger electrodes delivered somewhat larger amounts of current than the smaller ones (P < 0.01). Electrodes in the frontal regions delivered less current than those in the parietal region (P < 0.05). There were large individual differences in current levels that the subjects received. Analysis of the cranial bone showed that the gender difference and the frontal parietal differences are due to differences in cranial bone. Males have more cancelous parietal bone and females more dense parietal bone (P < 0.01). These differences should be considered when planning tCS studies and call into question earlier reports of gender differences due to hormonal influences

    Pre- and intraoperative variables affecting early outcomes in elderly patients undergoing pancreaticoduodenectomy

    AbstractBackgroundConflicting data exist regarding the safety of pancreatic resections in elderly patients. In this study we compared early complication and mortality rates between patients younger and older than 80 years of age who underwent pancreaticoduodenectomy using a validated national database.MethodsThe National Surgical Quality Improvement Program (NSQIP) database for 2005–2009 was used for this retrospective analysis. The primary outcome measures for our analysis were 30-day postoperative mortality, major complication rate and overall complication rate.ResultsA total of 6293 patients who underwent PD for any cause were included in the analysis. Of these, 9.4% were aged ≥80 years. The incidence of 30-day mortality was significantly higher in patients aged ≥80 years (6.3%) than in those aged <80 years (2.7%). Older patients were also noted to have higher rates of overall complications and serious complications. On multivariate analysis, age, ASA (American Society of Anesthesiologists) classification, reduced functional status, history of dyspnoea, and need for intraoperative transfusion were risk factors associated with the occurrence of overall complications, serious complications and postoperative mortality.ConclusionsThis study shows that age among other factors is a determinant of postoperative morbidity and mortality following PD

    Scaling in Numerical Simulations of Domain Walls

    We study the evolution of domain wall networks appearing after phase transitions in the early Universe. They exhibit interesting dynamical scaling behaviour which is not yet well understood, and are also simple models for the more phenomenologically acceptable string networks. We have run numerical simulations in two- and three-dimensional lattices of sizes up to 4096^3. The theoretically predicted scaling solution for the wall area density A ~ 1/t is supported by the simulation results, while no evidence of a logarithmic correction reported in previous studies could be found. The energy loss mechanism appears to be direct radiation, rather than the formation and collapse of closed loops or spheres.     A new electroreceptive teleost: Xenomystus nigri (Osteoglossiformes: Notopteridae)

    The African knife fish, Xenomystus nigri , is found to be sensitive to weak electric fields by the method of averaged evoked potentials from the brain. Slow waves and spikes were recorded in or near the lateral line area of the medulla and the torus semicircularis of the mesencephalon in response to long pulses (best > 50 ms) and low frequency sine waves (best ca. 10 Hz) of voltage gradients down to < 10 ÎĽV/cm. Evoked waves in the lateral line area are a sequence of negative and positive deflections beginning with a first peak at ca. 24 ms; in the torus semicircularis the first peak is at ca. 37 ms. Spikes are most likely in the torus between 50 and 80 ms after ON. At each recording locus there is a best axis of the homogeneous electric field and a better polarity. Effects of stimulus intensity, duration and repetition are described.     A bovine CD18 signal peptide variant with increased binding activity to Mannheimia hemolytica leukotoxin [version 1; referees: 2 approved]

    Background: Mannheimia haemolytica is the major bacterial infectious agent of bovine respiratory disease complex and causes severe morbidity and mortality during lung infections. M. haemolytica secretes a protein leukotoxin (Lkt) that binds to the CD18 receptor on leukocytes, initiates lysis, induces inflammation, and causes acute fibrinous bronchopneumonia. Lkt binds the 22-amino acid CD18 signal peptide domain, which remains uncleaved in ruminant species. Our aim was to identify missense variation in the bovine CD18 signal peptide and measure the effects on Lkt binding. Methods: Missense variants in the integrin beta 2 gene (ITGB2) encoding CD18 were identified by whole genome sequencing of 96 cattle from 19 breeds, and targeted Sanger sequencing of 1238 cattle from 46 breeds. The ability of different CD18 signal peptide variants to bind Lkt was evaluated by preincubating the toxin with synthetic peptides and applying the mixture to susceptible bovine cell cultures in cytotoxicity-blocking assays. Results: We identified 14 missense variants encoded on 15 predicted haplotypes, including a rare signal peptide variant with a cysteine at position 5 (C5) instead of arginine (R5). Preincubating Lkt with synthetic signal peptides with C5 blocked cytotoxicity significantly better than those with R5. The most potent synthetic peptide (C5PQLLLLAGLLA) had 30-fold more binding activity compared to that with R5. Conclusions: The results suggest that missense variants in the CD18 signal peptide affect Lkt binding, and animals carrying the C5 allele may be more susceptible to the effects of Lkt. The results also identify a potent class of non-antibiotic Lkt inhibitors that could potentially protect cattle from cytotoxic effects during acute lung infections
