    Long range planning of radiotherapy facilities in the Netherlands

    The subject of this paper is long range planning or policy development for healthcare in the Netherlands. Especially the co-ordinating function of planning will be discussed. In healthcare different actors or stakeholders are involved. Each of these actors may have their own interests, expectations, goals, and knowledge. The main goal of this paper is to discuss the alleged centralisation versus decentralisation dichotomy in healthcare strategic decision making and planning. The development of facilities for radiotherapy is used as an exemplary case for this explorative research. The radiotherapy case illustrates that a more centralised, comprehensive, and systemic way of planning and strategic decision making might be necessary to take into account and balance the different developments, which are relevant for this specific problem field, at the macro level. This kind of strategic process involves inputs from medical specialists and other experts with various disciplinary backgrounds. All relevant knowledge regarding facts and trends on demographic, social, epidemiological, technological, and therapeutic factors, and also those concerning the basic disciplines (such as biology) should be collected and analysed to obtain the insights needed. This expertise can not be found on a local or even regional level. It requires initiatives from co-ordinating boards like the Health Council, but also professional bodies play an important role, next to initiatives from influential and respected individuals

    Adaptive foraging of pollinators fosters gradual tipping under resource competition and rapid environmental change

    Plant and pollinator communities are vital for transnational food chains. Like many natural systems, they are affected by global change: rapidly deteriorating conditions threaten their numbers. Previous theoretical studies identified the potential for community-wide collapse above critical levels of environmental stressors—so-called bifurcation-induced tipping points. Fortunately, even as conditions deteriorate, individuals have some adaptive capacity, potentially increasing the boundary for a safe operating space where changes in ecological processes are reversible. Our study considers this adaptive capacity of pollinators to resource availability and identifies a new threat to disturbed pollinator communities. We model the adaptive foraging of pollinators in changing environments. Pollinator’s adaptive foraging alters the dynamical responses of species, to the advantage of some—typically generalists—and the disadvantage of others, with systematic non-linear and non-monotonic effects on the abundance of particular species. We show that, in addition to the extent of environmental stress, the pace of change of environmental stress can also lead to the early collapse of both adaptive and nonadaptive pollinator communities. Specifically, perturbed communities exhibit rate-induced tipping points at stress levels within the safe boundary defined for constant stressors. With adaptive foraging, tipping is a more asynchronous collapse of species compared to nonadaptive pollinator communities, meaning that not all pollinator species reach a tipping event simultaneously. These results suggest that it is essential to consider the adaptive capacity of pollinator communities for monitoring and conservation. Both the extent and the rate of stress change relative to the ability of communities to recover are critical environmental boundaries

    Detecting time-fragmented cache attacks against AES using Performance Monitoring Counters

    Cache timing attacks use shared caches in multi-core processors as side channels to extract information from victim processes. These attacks are particularly dangerous in cloud infrastructures, in which the deployed countermeasures cause collateral effects in terms of performance loss and increase in energy consumption. We propose to monitor the victim process using an independent monitoring (detector) process, that continuously measures selected Performance Monitoring Counters (PMC) to detect the presence of an attack. Ad-hoc countermeasures can be applied only when such a risky situation arises. In our case, the victim process is the AES encryption algorithm and the attack is performed by means of random encryption requests. We demonstrate that PMCs are a feasible tool to detect the attack and that sampling PMCs at high frequencies is worse than sampling at lower frequencies in terms of detection capabilities, particularly when the attack is fragmented in time to try to be hidden from detection

    Thematically analysing social network content during disasters through the lens of the disaster management lifecycle

    Social Networks such as Twitter are often used for disseminating and collecting information during natural disasters. The potential for its use in Disaster Management has been acknowledged. However, more nuanced understanding of the communications that take place on social networks are required to more effectively integrate this information into the processes within disaster management. The type and value of information shared should be assessed, determining the benefits and issues, with credibility and reliability as known concerns. Mapping the tweets in relation to the modelled stages of a disaster can be a useful evaluation for determining the benefits/drawbacks of using data from social networks, such as Twitter, in disaster management.A thematic analysis of tweets' content, language and tone during the UK Storms and Floods 2013/14 was conducted. Manual scripting was used to determine the official sequence of events, and classify the stages of the disaster into the phases of the Disaster Management Lifecycle, to produce a timeline. Twenty-five topics discussed on Twitter emerged, and three key types of tweets, based on the language and tone, were identified. The timeline represents the events of the disaster, according to the Met Office reports, classed into B. Faulkner's Disaster Management Lifecycle framework. Context is provided when observing the analysed tweets against the timeline. This illustrates a potential basis and benefit for mapping tweets into the Disaster Management Lifecycle phases. Comparing the number of tweets submitted in each month with the timeline, suggests users tweet more as an event heightens and persists. Furthermore, users generally express greater emotion and urgency in their tweets.This paper concludes that the thematic analysis of content on social networks, such as Twitter, can be useful in gaining additional perspectives for disaster management. It demonstrates that mapping tweets into the phases of a Disaster Management Lifecycle model can have benefits in the recovery phase, not just in the response phase, to potentially improve future policies and activities

    Facilitated engraftment of human hematopoietic cells in severe combined immunodeficient mice following a single injection of Cl²MDP liposomes

    Transplantation of normal and malignant human hematopoietic cells into severe combined immunodeficient (SCID) mice allows for evaluation of long-term growth abilities of these cells and provides a preclinical model for therapeutic interventions. However, large numbers of cells are required for successful engraftment in preirradiated mice due to residual graft resistance, that may be mediated by cells from the mononuclear phagocytic system. Intravenous (i.v.) injection of liposomes containing dichloromethylene diphosphonate (Cl2MDP) may eliminate mouse macrophages in spleen and liver. In this study outgrowth of acute myeloid leukemia (AML) cells and umbilical cord blood (UCB) cells in SCID mice conditioned with a single i.v. injection of Cl2MDP liposomes in addition to sublethal total body irradiation (TBI) was compared to outgrowth of these cells in SCID mice that had received TBI alone. A two- to 10-fold increase in outgrowth of AML cells was observed in four cases of AML. Administration of 107 UCB cells reproducibly engrafted SCID mice that had been conditioned with Cl2MDP liposomes and TBI, whereas human cells were not detected in mice conditioned with TBI alone. As few as 2 x 104 purified CD34+ UCB cells engrafted in all mice treated with Cl2MDP liposomes. In SCID mice treated with macrophage depletion unexpected graft failures were not observed. Histological examination of the spleen showed that TBI and Cl2MDP liposomes i.v. resulted in a transient elimination of all macrophage subsets in the spleen, whereas TBI had a minor effect. Cl2MDP liposomes were easy to use and their application was not associated with appreciable side-effects. Cl2MDP liposome pretreatment in combination with TBI allows for reproducible outgrowth of high numbers of human hematopoietic cells in SCID mice
