    Global Health Law: A Definition and Grand Challenges

    It has been only recently that scholars have engaged in a serious discussion of public health law. This academic discourse examines the role of the state and civil society in health promotion and disease prevention within the country. There is an important emerging literature on the international dimensions of health, but no similar systematic definition and exposition of a field we call global health law. In this article we aim to fill this gap by defining global health law and characterizing the grand challenges. Given the rapid and expanding globalization that is a defining feature of today\u27s world, the need for a coherent system of international health law and governance has never been greater. We begin with a discussion of the health hazards posed by contemporary globalization on human health and the consequent urgent need for global health law to facilitate effective multilateral cooperation in advancing the health of populations equitably. We then offer a definition of the emerging field of global health law. After explicating the central features identified in our definition, we turn to an examination of the grand challenges – legal, political, and social – to reaching the full potential of global health law to advance human health in just and effective ways. Our definition of global health law follows, and the remainder of this section explains the salient aspects of the definition: Global health law is the study of the legal norms, processes, and institutions needed to create the conditions for people throughout the world to attain the highest possible level of physical and mental health. The field seeks to facilitate health-promoting behaviour among the key actors that significantly influence the public\u27s health, including international organizations, governments, businesses, foundations, the media, and civil society. Global health law should stimulate investment in research and development, mobilize resources, set priorities, coordinate activities, monitor progress, create incentives, and enforce standards. The field should be guided by the value of social justice, and seek equitable distribution of health services, particularly to benefit the world’s poorest populations. The domain of global health law primarily is concerned with (1) formal sources of public international law, including, for example, treaties establishing the authority and responsibility of states for the health of their populations and duties of international cooperation, and (2) formal subjects of international law, including states, individuals, and public international organizations. However, to be an effective global health governance strategy, global health law must evolve beyond its traditional confines of formal sources and subjects of international law. It must foster more effective collective global health action among governments, businesses, civil society and other actors. Accordingly, our definition of global health law is prescriptive as well as descriptive: it sets out the sort of international legal framework needed, but still unavailable, to empower the world community to advance global health in accordance with the value of social justice

    Intranasal melanoma treated with radiation therapy in three dogs

    Three dogs were investigated for chronic unilateral nasal discharge. In all cases CT imaging showed an intranasal mass causing turbinate lysis and no evidence of metastasis. Cytology in cases 1 (a 14-year-old neutered male crossbreed dog) and 2 (a five-year-old neutered male German Shepherd dog) demonstrated a pleomorphic cell population with variable intracellular pigment suspicious of melanocytic neoplasia. Histopathology with immunohistochemistry (Melan-A and vimentin, plus PNL-2 in one case) confirmed the diagnosis of melanoma in all dogs. All dogs were treated with megavoltage radiotherapy using linear accelerators. Cases 1 and 3 (a nine-year-old neutered female beagle dog) received a hypofractionated (4 × 8 Gy) protocol and case 2 received a definitive (12 × 4 Gy) protocol. Complete remission was demonstrated on repeat CT scan five months after diagnosis in case 1 and seven months in case 2. Stable disease was documented on CT at four months for case 3; however, clinical signs in this dog remained controlled for 10 months in total. Case 1 died of unrelated causes five months after diagnosis, case 2 was euthanased due to the development of seizures 13 months after diagnosis, and case 3 was lost to follow-up 12 months after diagnosis. Melanoma should be considered as a rare differential diagnosis for primary nasal neoplasia in the dog and radiation therapy can be used as effective local therapy

    Momentum Space Integral Equations for Three Charged Particles: Diagonal Kernels

    It has been a long-standing question whether momentum space integral equations of the Faddeev type are applicable to reactions of three charged particles, in particular above the three-body threshold. For, the presence of long-range Coulomb forces has been thought to give rise to such severe singularities in their kernels that the latter may lack the compactness property known to exist in the case of purely short-range interactions. Employing the rigorously equivalent formulation in terms of an effective-two-body theory we have proved in a preceding paper [Phys. Rev. C {\bf 61}, 064006 (2000)] that, for all energies, the nondiagonal kernels occurring in the integral equations which determine the transition amplitudes for all binary collision processes, possess on and off the energy shell only integrable singularities, provided all three particles have charges of the same sign, i.e., all Coulomb interactions are repulsive. In the present paper we prove that, for particles with charges of equal sign, the diagonal kernels, in contrast, possess one, but only one, nonintegrable singularity. The latter can, however, be isolated explicitly and dealt with in a well-defined manner. Taken together these results imply that modified integral equations can be formulated, with kernels that become compact after a few iterations. This concludes the proof that standard solution methods can be used for the calculation of all binary (i.e., (in-)elastic and rearrangement) amplitudes by means of momentum space integral equations of the effective-two-body type.Comment: 36 pages, 2 figures, accepted for publication in Phys.     Probing Solar Convection

    In the solar convection zone acoustic waves are scattered by turbulent sound speed fluctuations. In this paper the scattering of waves by convective cells is treated using Rytov's technique. Particular care is taken to include diffraction effects which are important especially for high-degree modes that are confined to the surface layers of the Sun. The scattering leads to damping of the waves and causes a phase shift. Damping manifests itself in the width of the spectral peak of p-mode eigenfrequencies. The contribution of scattering to the line widths is estimated and the sensitivity of the results on the assumed spectrum of the turbulence is studied. Finally the theoretical predictions are compared with recently measured line widths of high-degree modes.Comment: 26 pages, 7 figures, accepted by MNRA

    Demographic Features, Beliefs And Socio–Psychological Impact Of Acne Vulgaris Among Its Sufferers In Two Towns In Nigeria

    There is paucity of reports in the demographic knowledge, belief and socio-psychological impact of acne vulgaris sufferers towards the disorder in a black population. This is the first study from Nigeria designed to address this issue. A total of 174 facial acne sufferers completed a self-administered questionnaire, which contained several items mentioning different areas in their belief, knowledge, perception, severity, socio-psychological impact and medication attention. The findings were discussed and compared to those of the Caucasians. The occurrence of the disorder was higher in females (65.0%) compared to the males (35.0%). About 54.0% of the female subjects indicated increase in severity of the disorder during their pre-menstrual period. Also 64.9% of acne sufferers indicated increase in severity during the rainy season, while 93.1% of the population implicated stress to perpetuate the severity of the disorder. Most (75.7%) of the acne sufferers believed that it is caused by oily diet, 40.8% thought that it is hereditary, while barely 5.2% had at sometime sought doctor's attention. Non- prescription products used by acne sufferers were cleansers and cream/lotions. Psychological abnormalities experienced by the sufferers included social inhibition, depression and anxiety. Pain and discomfort are the psychosomatic symptoms. No major differences were found in the beliefs, misconception and socio-psychological impact of acne sufferers in a black population (Nigeria) compared to the Caucasians. There is need to improve the understanding of the disorder in Nigeria through health education programmes

    The Relationship between the UniProt Knowledgebase (UniProtKB) and the IntAct Molecular Interaction Databases

    IntAct provides a freely available, open source database system and analysis tools for protein interaction data. All interactions are derived from literature curation or direct user submission and all experimental information relating to binary protein-protein
interactions is entered into the IntAct database by curators, via a web-based editor. Interaction information is added to the SUBUNIT comment and the RP line of the relevant publication within the UniProtKB entry. There may be a single INTERACTION comment present within a UniProtKB entry, which conveys information relevant to binary protein-protein interactions. This is automatically derived from the IntAct database and is updated on a triweekly basis. Interactions can be derived by any appropriate experimental method but must be confirmed by a second interaction if resulting from a single yeast2hybrid experiment. For large-scale experiments, interactions are considered if a high confidence score is assigned by the authors. The INTERACTION line contains a direct link to IntAct that provides detailed information for the experimental support. These lines are not changed manually and any discrepancy is reported to IntAct for updates. There is also a database crossreference line within the UniProtKB entry i.e.: DR IntAct _UniProtKB AC, which directs the user to additional interaction data for that molecule.

    Post-Newtonian Theory for Precision Doppler Measurements of Binary Star Orbits

    The determination of velocities of stars from precise Doppler measurements is described here using relativistic theory of astronomical reference frames so as to determine the Keplerian and post-Keplerian parameters of binary systems. We apply successive Lorentz transformations and the relativistic equation of light propagation to establish the exact treatment of Doppler effect in binary systems both in special and general relativity theories. As a result, the Doppler shift is a sum of (1) linear in c−1c^{-1} terms, which include the ordinary Doppler effect and its variation due to the secular radial acceleration of the binary with respect to observer; (2) terms proportional to c−2c^{-2}, which include the contributions from the quadratic Doppler effect caused by the relative motion of binary star with respect to the Solar system, motion of the particle emitting light and diurnal rotational motion of observer, orbital motion of the star around the binary's barycenter, and orbital motion of the Earth; and (3) terms proportional to c−2c^{-2}, which include the contributions from redshifts due to gravitational fields of the star, star's companion, Galaxy, Solar system, and the Earth. After parameterization of the binary's orbit we find that the presence of periodically changing terms in the Doppler schift enables us disentangling different terms and measuring, along with the well known Keplerian parameters of the binary, four additional post-Keplerian parameters, including the inclination angle of the binary's orbit, ii. We briefly discuss feasibility of practical implementation of these theoretical results, which crucially depends on further progress in the technique of precision Doppler measurements.Comment: Minor changes, 1 Figure included, submitted to Astrophys.

    Near Infrared Adaptive Optics Imaging of QSO Host Galaxies

    We report near-infrared (primarily H-band) adaptive optics (AO) imaging with the Gemini-N and Subaru Telescopes, of a representative sample of 32 nearby (z<0.3) QSOs selected from the Palomar-Green (PG) Bright Quasar Survey (BQS), in order to investigate the properties of the host galaxies. 2D modeling and visual inspection of the images shows that ~36% of the hosts are ellipticals, \~39% contain a prominent disk component, and ~25% are of undetermined type. 30% show obvious signs of disturbance. The mean M_H(host) = -24.82 (2.1L_H*), with a range -23.5 to -26.5 (~0.63 to 10 L_H*). At <L_H*, all hosts have a dominant disk component, while at >2 L_H* most are ellipticals. "Disturbed" hosts are found at all M_H(host), while "strongly disturbed" hosts appear to favor the more luminous hosts. Hosts with prominent disks have less luminous QSOs, while the most luminous QSOs are almost exclusively in ellipticals or in mergers (which presumably shortly will be ellipticals). At z<0.13, where our sample is complete at B-band, we find no clear correlation between M_B(QSO) and M_H(host). However, at z>0.15, the more luminous QSOs (M_B<-24.7), and 4/5 of the radio-loud QSOs, have the most luminous H-band hosts (>7L_H*), most of which are ellipticals. Finally, we find a strong correlation between the "infrared-excess", L_IR/L_BB, of QSOs with host type and degree of disturbance. Disturbed and strongly disturbed hosts and hosts with dominant disks have L_IR/L_BB twice that of non-disturbed and elliptical hosts, respectively. QSOs with "disturbed" and "strongly-disturbed" hosts are also found to have morphologies and mid/far-infrared colors that are similar to what is found for "warm" ultraluminous infrared galaxies, providing further evidence for a possible evolutionary connection between both classes of objects.Comment: 80 pages, accepted for publication in ApJ Supp
