    Who’s challenging who? : a co-produced approach for training staff in learning disability services about challenging behaviour

    Purpose The purpose of this paper is to outline the development, piloting and evaluation of the Who’s Challenging Who? (WCW) training intervention for social care staff to improve their empathy and attitudes towards people with learning disabilities (LD) and challenging behaviour (CB). Design/methodology/approach A phased approach was taken to the development and testing of the intervention. Initially, the existing literature was reviewed, the theoretical background of the intervention was developed, and then the intervention was designed. A pilot study was undertaken, followed by further development, and a large-scale randomised controlled trial (RCT). Findings WCW had a small positive effect on staff empathy 20 weeks after the intervention, and small to moderate effects for other staff reported outcomes (e.g. positive empowerment attitudes and positive work motivation). Being trained by people with LD and CB encouraged staff to reflect on the impact they have on the people they support. The trainers with LD valued their role, and saw benefits beyond this (e.g. friendships). Research limitations/implications It is possible to carry out high-quality RCT evaluations of social care practice, and research should continue to generate evidence in this way, as in healthcare settings. However, there were difficulties in retaining participants. Practical implications People with LD can be actively involved in the co-production and delivery of social care training. Social implications Employment and a fair wage can increase the confidence and empowerment of people with LD. Originality/value This is the first large-scale RCT of an intervention that aimed to improve empathy/change attitudes in social care staff who work with people with LD and CB

    Tartan Artibeus: A Batteryless, Computational Satellite Research Platform

    TTartan Artibeus (TA1) is the first batteryless, computational pocketqube satellite; its open-source hardware and software launched into low-Earth orbit (LEO) in January 2022. TA1 is a 1p (125 cm3) pocketqube built around the Tartan Artibeus Bus (TAB), which connects independently-designed modules into a batteryless, computational satellite. In TA1, TAB incorporates an electrical power supply (EPS) module that harvests solar energy into a supercapacitor, a fault-tolerant command and data handling (C&DH) module, a radio-communication module, and a configurable computational payload module. The open-source hardware [17] and software [18] of TA1 supports independently designed modules oblivious to the batteryless nature of the power system via adherence to TAB’s well-defined communication protocol serviced by a C&DH board. TAB allows the C&DH board to manage independent subsystems for power savings and to provide isolation for reduced impact of faults. The C&DH software supports frequent power cycles via task-based, intermittent execution. These features guarantee forward program progress and free subsystem developers to focus on each payload application. To evaluate the computational nanosatellite design TAB enables, we integrate many subsystems, including a radio module, a GNSS module, and a computing payload. The radio, which is based on OpenLST [33] hardware and software, demonstrates the ease of using existing modules with TAB. To the best of our knowledge, TA1 is the first 1p pocketqube to have a GPS module without COCOM limits. The computing payload includes hardware to accelerate machine inference and can be reprogrammed in orbit. The TA1 mechanical, hardware, and software designs are open source to reduce the barrier to entry for orbital edge computing (OEC) research

    The READ-IT study protocol for a feasibility randomised controlled trial of using a support worker/family carer mediated online reading programme to teach early reading skills to adults with intellectual disabilities

    Background: Many individuals with intellectual disability (ID) have not learnt basic reading skills by the time that they reach adulthood, potentially limiting their access to critical information. READ-IT is an online reading programme developed from the Headsprout® Early Reading (HER®) intervention and supplemented by support strategies tailored for adults with ID. HER® has been successfully used to teach adults with ID to read in a forensic setting by trained staff. The aim of this study is to assess the feasibility of delivering READ-IT to adults with ID by family carers/support workers and will assess whether it would be feasible to conduct a later definitive randomised controlled trial (RCT) of the effectiveness of the programme. The study will aim to contribute to the evidence base on improving outcomes for adults with ID and their caregivers. Methods: This study is a feasibility RCT, with embedded process evaluation. Forty-eight adults with ID will be recruited and allocated to intervention: control on a 1:1 basis. Intervention families will be offered the READ-IT programme immediately, continuing to receive usual practice and control participants will be offered the opportunity to receive READ-IT at the end of the trial follow-up period and will continue to receive usual practice. Data will be collected at baseline and 6 months post-randomisation. Discussion: The results of this study will inform a potential future definitive trial, to evaluate the effectiveness of READ-IT to improve reading skills. Such a trial would have significant scientific impact internationally in the intellectual disability field. Trial registration: ISRCTN1140909

    A Cross-Sectional Survey on Knowledge and Perceptions of Health Risks Associated with Arsenic and Mercury Contamination from Artisanal Gold mining in Tanzania.

    An estimated 0.5 to 1.5 million informal miners, of whom 30-50% are women, rely on artisanal mining for their livelihood in Tanzania. Mercury, used in the processing gold ore, and arsenic, which is a constituent of some ores, are common occupational exposures that frequently result in widespread environmental contamination. Frequently, the mining activities are conducted haphazardly without regard for environmental, occupational, or community exposure. The primary objective of this study was to assess community risk knowledge and perception of potential mercury and arsenic toxicity and/or exposure from artisanal gold mining in Rwamagasa in northwestern Tanzania. A cross-sectional survey of respondents in five sub-villages in the Rwamagasa Village located in Geita District in northwestern Tanzania near Lake Victoria was conducted. This area has a history of artisanal gold mining and many of the population continue to work as miners. Using a clustered random selection approach for recruitment, a total of 160 individuals over 18 years of age completed a structured interview. The interviews revealed wide variations in knowledge and risk perceptions concerning mercury and arsenic exposure, with 40.6% (n=65) and 89.4% (n=143) not aware of the health effects of mercury and arsenic exposure respectively. Males were significantly more knowledgeable (n=59, 36.9%) than females (n=36, 22.5%) with regard to mercury (x²=3.99, p<0.05). An individual's occupation category was associated with level of knowledge (x²=22.82, p=<0.001). Individuals involved in mining (n=63, 73.2%) were more knowledgeable about the negative health effects of mercury than individuals in other occupations. Of the few individuals (n=17, 10.6%) who knew about arsenic toxicity, the majority (n=10, 58.8%) were miners. The knowledge of individuals living in Rwamagasa, Tanzania, an area with a history of artisanal gold mining, varied widely with regard to the health hazards of mercury and arsenic. In these communities there was limited awareness of the threats to health associated with exposure to mercury and arsenic. This lack of knowledge, combined with minimal environmental monitoring and controlled waste management practices, highlights the need for health education, surveillance, and policy changes

    The potential effect of improved provision of rabies post-exposure prophylaxis in Gavi-eligible countries: a modelling study

    Background: Tens of thousands of people die from dog-mediated rabies annually. Deaths can be prevented through post-exposure prophylaxis for people who have been bitten, and the disease eliminated through dog vaccination. Current post-exposure prophylaxis use saves many lives, but availability remains poor in many rabies-endemic countries due to high costs, poor access, and supply. Methods: We developed epidemiological and economic models to investigate the effect of an investment in post-exposure prophylaxis by Gavi, the Vaccine Alliance. We modelled post-exposure prophylaxis use according to the status quo, with improved access using WHO-recommended intradermal vaccination, with and without rabies immunoglobulin, and with and without dog vaccination. We took the health provider perspective, including only direct costs. Findings: We predict more than 1 million deaths will occur in the 67 rabies-endemic countries considered from 2020 to 2035, under the status quo. Current post-exposure prophylaxis use prevents approximately 56 000 deaths annually. Expanded access to, and free provision of, post-exposure prophylaxis would prevent an additional 489 000 deaths between 2020 and 2035. Under this switch to efficient intradermal post-exposure prophylaxis regimens, total projected vaccine needs remain similar (about 73 million vials) yet 17·4 million more people are vaccinated, making this an extremely cost-effective method, with costs of US635perdeathavertedand635 per death averted and 33 per disability-adjusted life-years averted. Scaling up dog vaccination programmes could eliminate dog-mediated rabies over this time period; improved post-exposure prophylaxis access remains cost-effective under this scenario, especially in combination with patient risk assessments to reduce unnecessary post-exposure prophylaxis use. Interpretation: Investing in post-exposure vaccines would be an extremely cost-effective intervention that could substantially reduce disease burden and catalyse dog vaccination efforts to eliminate dog-mediated rabies.     This series is a creation that has patiently been developing throughout my life unbeknownst that the internal struggles I faced would be the contributing factors leading me to create one of my most important photo series thus far. The Essence of Everything is a photographic portrait collection of individuals showcasing their passions which were found in the pursuit of happiness and belonging. Using a large format digital camera, I immersed myself into aspects of everyone’s lives capturing decisive moments that depict the relatable and enlightening glimpses into purpose. As it is when you find your passion that you will find your purpose. There was always a feeling while growing up that I was never doing the thing I was meant to do while growing up. I played sports, had friends, did everything just like everyone else, however I would often disassociate from the real world and get caught up daydreaming. It was then that I was diagnosed with the common learning disability of Attention Deficit Disorder, which also accompanied with it other mental hardships such as severe anxiety and later on depression. This depression is what led me to create a surrealism series of self-portraits capturing the inward experiences outwardly that would inevitably lead to this series. This transpired because it was through those pieces that I finally found my purpose and meaning in life. It is because I found my purpose that I set out to find other likeminded people who have also discovered purpose or a sense of belonging so that I may showcase where they find happiness in life. I discovered my love for photography while in high school and took every photography class possible. Upon entering my college career, I decided photography be a hobby and set it aside to pursue an engineering physics major with hopes of working for NASA. Two years later in a chemistry class I caught myself daydreaming yet again and I heard a voice in my head say this wasn’t right and that I needed to be doing something different. I was left with an unmistakable overwhelming desire to redirect my efforts back towards photography. Having had a previous understanding for photography and then becoming even more versed with various forms of photography I wanted what is supposed to be my best work yet to be made using something I never tried, thus giving birth to my use of the Mamiya 645+ large format digital camera. This equipment would allow me to capture events in the greatest detail possible. Struggling with A.D.D. has been a way of life I became accustomed to, however, the anxiety and depression would prove to be the real hardships. Depression put me in a place I never thought I would be in life and then coupled with the embarrassment of telling family and friends. This dark place I was in helped me see the greater meaning to life and belonging. After finding and knowing exactly where my purpose in life is I knew I wanted to show other people where their purpose is in life and how they found their sense of belonging and place in this world that offered them happiness even through struggles we all face. Showing the very thing that separates us as individuals is precisely what makes us the same.
