    Performance of the resurfaced hip. Part 1: the influence of the prosthesis size and positioning on the remodelling and fracture of the femoral neck

    Hip resurfacing is an established treatment for osteoarthritis in young active patients. Failure modes include femoral neck fracture and prosthesis loosening, which may be associated with medium-term bone adaptation, including femoral neck narrowing and densification around the prosthesis stem.Finite element modelling was used to indicate the effects of prosthesis sizing and positioning on the bone remodelling and fracture strength under a range of normal and traumatic loads, with the aim of understanding these failure modes better.The simulations predicted increased superior femoral neck stress shielding in young patients with small prostheses, which required shortening of the femoral neck to give an acceptable implant–bone interface. However, with a larger prosthesis, natural femoral head centre recreation in the implanted state was possible; therefore stress shielding was restricted to the prosthesis interior, and its extent was less sensitive to prosthesis orientation. With valgus orientation, the implanted neck strength was, at worst, within 3 per cent of its intact strength.The study suggests that femoral neck narrowing may be linked to a reduction in the horizontal femoral offset, occurring if the prosthesis is excessively undersized. As such, hip resurfacing should aim to reproduce the natural femoral head centre, and, for valgus prosthesis orientation, to avoid femoral neck fracture

    Observing the CMB at High-l using the VSA and AMI

    We discuss two experiments - the Very Small Array (VSA) and the Arcminute MicroKelvin Imager (AMI) - and their prospects for observing the CMB at high angular multipoles. Whilst the VSA is primarily designed to observe primary anisotropies in the CMB, AMI is designed to image secondary anisotropies via the Sunyaev-Zel'dovich effect.     Blood-feeding in the young adult filarial worms litomosoides sigmodontis

    In this study with the filarial model Litomosoides sigmodontis, we demonstrate that the worms ingest host red blood cells at a precise moment of their life-cycle, immediately after the fourth moult. The red blood cells (RBC) were identified microscopically in live worms immobilized in PBS at 4 degrees C, and their density assessed. Two hosts were used: Mongolian gerbils, where microfilaraemia is high, and susceptible BALB/c mice with lower microfilaraemia. Gerbils were studied at 12 time-points, between day 9 post-inoculation (the worms were young 4th stage larvae) and day 330 p.i. (worms were old adults). Only the very young adult filarial worms had red blood cells in their gut. Haematophagy was observed between days 25 and 56 p.i. and peaked between day 28 and day 30 p.i. in female worms. In males, haematophagy was less frequent and intense. Similar kinetics of haematophagy were found in BALB/c mice, but frequency and intensity tended to be lower. Haematophagy seems useful to optimize adult maturation. These observations suggest that haematophagy is an important step in the life-cycle of L. sigmodontis. This hitherto undescribed phenomenon might be characteristic of other filarial species including human parasites

    A common garden design reveals population-specific variability in potential impacts of hybridisation between populations of farmed and wild Atlantic salmon, Salmo salar L

    Released individuals can have negative impacts on native populations through various mechanisms; including competition, disease transfer and introduction of maladapted gene-complexes. Previous studies indicate that the level of farmed Atlantic salmon introgression in native populations is population-specific. However few studies have explored the potential role of population diversity or river characteristics, such as temperature, on the consequences of hybridisation. We compared freshwater growth of multiple families derived from two farmed, five wild, and two F1 hybrid salmon populations at three contrasting temperatures (7°C, 12°C, and 16°C) in a common garden experiment. As expected, farmed salmon outgrew wild salmon at all temperatures, with hybrids displaying intermediate growth. However, differences in growth were population-specific and some wild populations performed better than others relative to the hybrid and farmed populations at certain temperatures. Therefore, the competitive balance between farmed and wild salmon may depend both on the thermal profile of the river and the genetic characteristics of the respective farmed and wild strains. While limited to F1 hybridisation, the present study shows the merits in adopting a more complex spatially resolved approach to risk management of local populations

    Polarization Observations with the Cosmic Background Imager

    We describe polarization observations of the CMBR with the Cosmic Background Imager, a 13 element interferometer which operates in the 26-36 GHz band from Llano de Chajnantour in northern Chile. The array consists of 90-cm Cassegrain antennas mounted on a steerable platform which can be rotated about the optical axis to facilitate polarization observations. The CBI employs single mode circularly polarized receivers which sample multipoles from â„“~400 to â„“~4250. The instrumental polarization of the CBI was calibrated with 3C279, a bright polarized point source which was monitored with the VLA

    pTcINDEX: a stable tetracycline-regulated expression vector for Trypanosoma cruzi

    BACKGROUND: Trypanosoma cruzi is a protozoan pathogen of major medical importance in Latin America. It is also an early diverging eukaryote that displays many unusual biochemical features. The completion of the T. cruzi genome project has highlighted the need to extend the range of techniques available to study gene function. To this end we report the development of a stable tetracycline-dependent expression vector applicable to this parasite and describe in detail the parameters of the system. RESULTS: We first produced T. cruzi cell lines that constitutively expressed bacteriophage T7 RNA polymerase and the tetracycline repressor protein from a multicopy episome. An integrative vector with an inducible expression site under the control of a tetracycline-regulatable T7 promoter (pTcINDEX) was targeted to the transcriptionally silent ribosomal RNA spacer region of these parasites and transformants selected using a T7 RNA polymerase-dependent hygromycin resistance gene. To test the system we used two marker proteins, luciferase and red fluorescent protein (RFP), and an endogenous parasite protein (a mitochondrial superoxide dismutase). In each case we found that induction was both time and dose-dependent. Luciferase mRNA could be induced by at least 100-fold, and luciferase activity up to 60-fold, within 24 hours of the addition of tetracycline. When we examined RFP induction by confocal microscopy and fluorescence activated cell sorter, we observed very high levels of expression (>1000-fold increase in fluorescence intensity), although this was not synchronous throughout clonal populations. Induction of superoxide dismutase resulted in an 18-fold increase in cellular activity. The observation that a tagged version of the enzyme was correctly targeted to the mitochondrion demonstrates that our expression system may also provide a high-throughput strategy for subcellular localisation. CONCLUSION: Our results show that pTcINDEX represents a valuable addition to the genetic tools available for T. cruzi. The vector system is sufficiently flexible that it should have widespread uses including inducible expression of tagged proteins, generation of conditional knockout cell lines and the application of dominant-negative approaches

    Pain management for chronic musculoskeletal conditions : the development of an evidence-based and theory-informed pain self-management course

    Objective: To devise and test a self-management course for chronic pain patients based on evidence and underpinned by theory using the Medical Research Council (MRC) framework for developing complex interventions. Design: We used a mixed method approach. We conducted a systematic review of the effectiveness of components and characteristics of pain management courses. We then interviewed chronic pain patients who had attended pain and self-management courses. Behavioural change theories were mapped onto our findings and used to design the intervention. We then conducted a feasibility study to test the intervention. Setting: Primary care in the inner city of London, UK. Participants: Adults (18 years or older) with chronic musculoskeletal pain. Outcomes: Related disability, quality of life, coping, depression, anxiety, social integration and healthcare resource use. Results: The systematic reviews indicated that group-based courses with joint lay and healthcare professional leadership and that included a psychological component of short duration (<8 weeks) showed considerable promise. The qualitative research indicated that participants liked relaxation, valued social interaction and course location, and that timing and good tutoring were important determinants of attendance. We used behavioural change theories (social learning theory and cognitive behaviour approaches (CBA)) to inform course content. The course addressed: understanding and accepting pain, mood and pain, unhelpful thoughts and behaviour, problem solving, goal setting, action planning, movement, relaxation and social integration/reactivation. Attendance was 85%; we modified the recruitment of patients, the course and the training of facilitators as a result of testing. Conclusions: The MRC guidelines were helpful in developing this intervention. It was possible to train both lay and non-psychologists to facilitate the courses and deliver CBA. The course was feasible and well received

    In a recent manuscript (arXiv:0710.4917v2), Jones-Smith et al. attempt to use the well-established box-counting technique for fractal analysis to "demonstrate conclusively that fractal criteria are not useful for authentication". Here, in response to what we view to be an extremely simplistic misrepresentation of our earlier work by Jones-Smith et al., we reiterate our position regarding the potential of fractal analysis for artwork authentication.     Fidelity in complex behaviour change interventions : a standardised approach to evaluate intervention integrity

    Objectives: The aim of this study was to (1) demonstrate the development and testing of tools and procedures designed to monitor and assess the integrity of a complex intervention for chronic pain (COping with persistent Pain, Effectiveness Research into Self-management (COPERS) course); and (2) make recommendations based on our experiences. Design: Fidelity assessment of a two-arm randomised controlled trial intervention, assessing the adherence and competence of the facilitators delivering the intervention. Setting: The intervention was delivered in the community in two centres in the UK: one inner city and one a mix of rural and urban locations. Participants: 403 people with chronic musculoskeletal pain were enrolled in the intervention arm and 300 attended the self-management course. Thirty lay and healthcare professionals were trained and 24 delivered the courses (2 per course). We ran 31 courses for up to 16 people per course and all were audio recorded. Interventions: The course was run over three and a half days; facilitators delivered a semistructured manualised course. Outcomes: We designed three measures to evaluate fidelity assessing adherence to the manual, competence and overall impression. Results: We evaluated a random sample of four components from each course (n=122). The evaluation forms were reliable and had good face validity. There were high levels of adherence in the delivery: overall adherence was two (maximum 2, IQR 1.67–2.00), facilitator competence exhibited more variability, and overall competence was 1.5 (maximum 2, IQR 1.25–2.00). Overall impression was three (maximum 4, IQR 2.00–3.00). Conclusions: Monitoring and assessing adherence and competence at the point of intervention delivery can be realised most efficiently by embedding the principles of fidelity measurement within the design stage of complex interventions and the training and assessment of those delivering the intervention. More work is necessary to ensure that more robust systems of fidelity evaluation accompany the growth of complex interventions

    Setting UBVRI Photometric Zero-Points Using Sloan Digital Sky Survey ugriz Magnitudes

    We discuss the use of Sloan Digital Sky Survey (SDSS) ugriz point-spread function (PSF) photometry for setting the zero points of UBVRI CCD images. From a comparison with the Landolt (1992) standards and our own photometry we find that there is a fairly abrupt change in B, V, R, & I zero points around g, r, i ~ 14.5, and in the U zero point at u ~ 16. These changes correspond to where there is significant interpolation due to saturation in the SDSS PSF fluxes. There also seems to be another, much smaller systematic effect for stars with g, r > 19.5. The latter effect is consistent with a small Malmquist bias. Because of the difficulties with PSF fluxes of brighter stars, we recommend that comparisons of ugriz and UBVRI photometry should only be made for unsaturated stars with g, r and i in the range 14.5 - 19.5, and u in the range 16 - 19.5. We give a prescription for setting the UBVRI zero points for CCD images, and general equations for transforming from ugriz to UBVRI.Comment: 13 pages. 6 figures.
