    ANALYTIC APPROXIMATE SOLUTION FOR NONLINEAR DYNAMICMODELING OF THE ROTATING ELASTIC 2D BEAM WITH A SINGLE CRACK

    In this paper, the 2D lateral vibration analysis of a rotating cracked beam as a rotary structure is investigated through the Homotopy perturbation analysis and compared with the numerical Newmark-beta (Nβ) algorithm. The structure and crack are modeled as the Euler-Bernoulli (EB) theory and simple torsional spring, respectively. The nonlinear equations of motion are derived using Galerkin and the Assumed Mode Method (AMM). The system’s stability is analyzed through phase plane and time response for different angular velocities of the base, initial values, external disturbances, crack stiffness, and locations. A comparative study presents simulation results for free (first nonlinear frequency) and forced vibration. It is shown that the proposed semi-analytical approach is beneficial as it provides a benchmark for a more precise analysis and further investigation of cracked rotary structures

    Anti-fouling behaviors of surface functionalized high density polyethylene membrane in microfiltration of bovine serum albumin protein

    An essential characteristic for high performance inherently hydrophobic membranes such as microporous high density polyethylene (HDPE) membranes is to have a hydrophilic surface. In this project, wet chemical functionalization as a facile and effective method was developed to give a hydrophilic property to HDPE membranes using polar functional groups. KClO3, K2Cr2O7 and KMnO4 were selected as oxidizing agents. The optimum concentrations and treatment time intervals were determined for each oxidizing agent. Water contact angle and pure water flux measurements were conducted to evaluate the surface hydrophilicity and membrane performance, respectively. The results showed that among different oxidizing agents, 1wt% K2Cr2O7 solution with 60 min immersion time had the highest impact on the pure water flux. The percentage of re-construction phenomenon was about 4.70%, 21.94% and 32.6% for the HDPE membranes treated by KClO3, K2Cr2O7 and KMnO4, respectively. In addition, the attenuated total reflectance spectra-Fourier transform infrared spectroscopy (ATR-FTIR) results confirmed the presence of hydroxyl groups (O–H peak appeared at 3418.78 cm−1) in the membrane modified by KClO3. Bovine serum albumin (BSA) filtration experiments revealed that the total fouling ratio (TFR) and irreversible fouling ratio (IFR) decreased from 88.10% and 42.60% for pristine membrane to 65%, 68% and 72%and 26.60%, 29.30% and 35% for the modified membranes treated by KClO3, K2Cr2O7 and KMnO4, respectively.     Job Burnout, Stress, and Satisfaction among Emergency Nursing Staff after Health System Transformation Plan in Iran

    Introduction: Job burnout, stress, and satisfaction are linked to quality of care, patient outcomes and retention of staff. This study was conducted to determine the mentioned issues among emergency nurses.Methods: This cross-sectional study was conducted on all nurses working in the emergency departments of 10 hospitals in Tehran, Iran, in 2017. Standard questionnaires were used for gathering the data of participants regarding job burnout, stress, and satisfaction. Results: 709 (90%) participants returned the completed questionnaires (58.9% female). The mean age of the nurses was 33 (SD = 7) years. The level of job burnout was moderate in 76.1%, low in 22.5%, and high in 1.4% of the nurses. The level of burnout in the married nursing staff was lower than single nurses (3.78 ± 0.98 versus 4.14 ± 0.58, p = 0.049). The level of job satisfaction was moderate in 61.1%, low in 22.2%, and high in 16.7%. There was a significant correlation between age and job satisfaction (p = 0.027, r = 0.3). Job burnout was directly correlated with job stress (p ≤0.001, r = 0.57) and job burnout was negatively correlated with job satisfaction (p = 0.001, r = -0.41). Conclusion: More than 60% of the studied emergency nurses had moderate levels of job burnout, stress, and satisfaction. Job burnout had a direct correlation with job stress and indirect correlation with job satisfaction. Planning to reduce burnout of the emergency nursing staff seems to be necessary

    ANTIBIOTIC SUSCEPTIBILITY PATTERNS OF ISOLATED BACTERIA FROM BILE FLUIDS OF PATIENTS WITH GALLSTONE DISEASE IN ISFAHAN CITY (IRAN)

    Bacterial infections are one of the important agents in the creation of gallstones in the gallbladder. In recent years the spread of antibiotic-resistant bacteria such as extended-spectrum beta-lactamases (ESBL) is increasing and of concern in hospitalized patients worldwide. The purpose of this study was to investigate the antibiotic susceptibility patterns of isolated bacteria from the bile specimens of patients with chronic and acute cholecystitis who had been operated by single-incision laparoscopic cholecystectomy (SILC) in Isfahan (Iran) using an antibiogram susceptibility test and molecular technique. The bile fluids of 91 patients were obtained from the Al-Zahra hospital and were cultured on specific media for the isolation of Gram-negative and positive bacteria and the disk diffusion test was done to determine the antibiotic susceptibility patterns of isolated bacteria. Finally, bacterial DNA was extracted from the bile samples and polymerase chain reaction (PCR) was performed to investigate extended-spectrum beta-lactamases genes. The bacteria Escherichia coli, Klebsiella pneumoniae, Proteus spp. and Staphylococcus aureus were detected in bile specimens cultured with high frequency, and the results showed that biliary infection increased with aging in patients with gallstone disease operated by SILC. The results showed a high frequency of ESBL genes including TEM, SHV, and CTX-M in isolated bacteria (especially Escherichia coli and Klebsiella spp.). Thus, evaluating the antibiotic susceptibility patterns and screening of ESBLs bacteria in patients with gallstones are essential. Prescribing suitable drugs, designing good strategies, and informing the medical community could decrease bile infection and antibiotic-resistant bacteria in clinical centers and hospitals

Governments have mandated safety equipment like helmets to protect cyclists in the event of an accident, but as any experienced bicyclist will tell you, keeping safe on a bike is largely about avoiding accidents in the first place. To meet this need for better bicycling safety, Cyclic Technologies is developing an antilock braking system (ABS) for bicycles that will help cyclists to avoid accidents by enabling them to stop faster and with more control

    A large-scale review of wave and tidal energy research over the last 20 years

    Over the last two decades, a large body of academic scholarship has been generated on wave and tidal energy related topics. It is therefore important to assess and analyse the research direction and development through horizon scanning processes. To synthesise such large-scale literature, this review adopts a bibliometric method and scrutinises over 8000 wave/tidal energy related documents published during 2003–2021. Overall, 98 countries contributed to the literature, with the top ten mainly developed countries plus China produced nearly two-thirds of the research. A thorough analysis on documents marked the emergence of four broad research themes (dominated by wave energy subjects): (A) resource assessment, site selection, and environmental impacts/benefits; (B) wave energy converters, hybrid systems, and hydrodynamic performance; (C) vibration energy harvesting and piezoelectric nanogenerators; and (D) flow dynamics, tidal turbines, and turbine design. Further, nineteen research sub-clusters, corresponding to broader themes, were identified, highlighting the trending research topics. An interesting observation was a recent shift in research focus from solely evaluating energy resources and ideal sites to integrating wave/tidal energy schemes into wider coastal/estuarine management plans by developing multicriteria decision-making frameworks and promoting novel designs and cost-sharing practices. The method and results presented may provide insights into the evolution of wave/tidal energy science and its multiple research topics, thus helping to inform future management decisions

    A Comparative Analysis of Clinical Characteristics and Laboratory Findings of COVID-19 between Intensive Care Unit and Non-Intensive Care Unit Pediatric Patients: A Multicenter, Retrospective, Observational Study from Iranian Network for Research in Viral

    Introduction: To date, little is known about the clinical features of pediatric COVID-19 patients admitted to intensive care units (ICUs). Objective: Herein, we aimed to describe the differences in demographic characteristics, laboratory findings, clinical presentations, and outcomes of Iranian pediatric COVID-19 patients admitted to ICU versus those in non-ICU settings. Methods: This multicenter investigation involved 15 general and pediatrics hospitals and included cases with confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection based on positive real-time reverse transcription polymerase chain reaction (RT-PCR) admitted to these centers between March and May 2020, during the initial peak of the COVID-19 pandemic in Iran. Results: Overall, 166 patients were included, 61 (36.7%) of whom required ICU admission. The highest number of admitted cases to ICU were in the age group of 1–5 years old. Malignancy and heart diseases were the most frequent underlying conditions. Dyspnea was the major symptom for ICU-admitted patients. There were significant decreases in PH, HCO3 and base excess, as well as increases in creatinine, creatine phosphokinase (CPK), lactate dehydrogenase (LDH), and potassium levels between ICU-admitted and non-ICU patients. Acute respiratory distress syndrome (ARDS), shock, and acute cardiac injury were the most common features among ICU-admitted patients. The mortality rate in the ICU-admitted patients was substantially higher than non-ICU cases (45.9% vs. 1.9%, respectively; p<0.001). Conclusions: Underlying diseases were the major risk factors for the increased ICU admissions and mortality rates in pediatric COVID-19 patients. There were few paraclinical parameters that could differentiate between pediatrics in terms of prognosis and serious outcomes of COVID-19. Healthcare providers should consider children as a high-risk group, especially those with underlying medical conditions

    Global, regional, and national burden of disorders affecting the nervous system, 1990–2021: a systematic analysis for the Global Burden of Disease Study 2021

    BackgroundDisorders affecting the nervous system are diverse and include neurodevelopmental disorders, late-life neurodegeneration, and newly emergent conditions, such as cognitive impairment following COVID-19. Previous publications from the Global Burden of Disease, Injuries, and Risk Factor Study estimated the burden of 15 neurological conditions in 2015 and 2016, but these analyses did not include neurodevelopmental disorders, as defined by the International Classification of Diseases (ICD)-11, or a subset of cases of congenital, neonatal, and infectious conditions that cause neurological damage. Here, we estimate nervous system health loss caused by 37 unique conditions and their associated risk factors globally, regionally, and nationally from 1990 to 2021.MethodsWe estimated mortality, prevalence, years lived with disability (YLDs), years of life lost (YLLs), and disability-adjusted life-years (DALYs), with corresponding 95% uncertainty intervals (UIs), by age and sex in 204 countries and territories, from 1990 to 2021. We included morbidity and deaths due to neurological conditions, for which health loss is directly due to damage to the CNS or peripheral nervous system. We also isolated neurological health loss from conditions for which nervous system morbidity is a consequence, but not the primary feature, including a subset of congenital conditions (ie, chromosomal anomalies and congenital birth defects), neonatal conditions (ie, jaundice, preterm birth, and sepsis), infectious diseases (ie, COVID-19, cystic echinococcosis, malaria, syphilis, and Zika virus disease), and diabetic neuropathy. By conducting a sequela-level analysis of the health outcomes for these conditions, only cases where nervous system damage occurred were included, and YLDs were recalculated to isolate the non-fatal burden directly attributable to nervous system health loss. A comorbidity correction was used to calculate total prevalence of all conditions that affect the nervous system combined.FindingsGlobally, the 37 conditions affecting the nervous system were collectively ranked as the leading group cause of DALYs in 2021 (443 million, 95% UI 378–521), affecting 3·40 billion (3·20–3·62) individuals (43·1%, 40·5–45·9 of the global population); global DALY counts attributed to these conditions increased by 18·2% (8·7–26·7) between 1990 and 2021. Age-standardised rates of deaths per 100 000 people attributed to these conditions decreased from 1990 to 2021 by 33·6% (27·6–38·8), and age-standardised rates of DALYs attributed to these conditions decreased by 27·0% (21·5–32·4). Age-standardised prevalence was almost stable, with a change of 1·5% (0·7–2·4). The ten conditions with the highest age-standardised DALYs in 2021 were stroke, neonatal encephalopathy, migraine, Alzheimer's disease and other dementias, diabetic neuropathy, meningitis, epilepsy, neurological complications due to preterm birth, autism spectrum disorder, and nervous system cancer.InterpretationAs the leading cause of overall disease burden in the world, with increasing global DALY counts, effective prevention, treatment, and rehabilitation strategies for disorders affecting the nervous system are needed

    A review on the progress and research directions of ocean engineering

    This paper reviews research in ocean engineering over the last 50+ years with the aim to (I) understand the technological challenges and evolution in the field, (II) investigate whether ocean engineering studies meet present global demands, (III) explore new scientific/engineering tools that may suggest pragmatic solutions to problems, and (IV) identify research and management gaps, and the way forward. Six major research divisions are identified, namely (I) Ocean Hydrodynamics, (II) Risk Assessment and Safety, (III) Ocean Climate and Geophysics: Data and Models, (IV) Control and Automation in the Ocean, (V) Structural Engineering and Manufacturing for the Ocean, and (VI) Ocean Renewable Energy. As much as practically possible research sub-divisions of the field are also identified. It is highlighted that research topics dealing with ocean renewable energy, control and path tracking of ships, as well as computational modelling of wave-induced motions are growing. Updating and forecasting energy resources, developing computational methods for wave generation, and introducing novel methods for the optimised control of energy converters are highlighted as the potential research opportunities. Ongoing studies follow the global needs for environmentally friendly renewable energies, though engineering-based studies often tend to overlook the longer-term potential influence of climate change. Development and exploitation of computational engineering methods with focus on continuum mechanics problems remain relevant. Notwithstanding this, machine learning methods are attracting the attention of researchers. Analysis of COVID-19 transmission onboard is rarely conducted, and 3D printing-based studies still need more attention from researchers.Peer reviewe
