Czy leczenie dietą i pochodną sulfonylomocznika wpływa na dokomórkowy transport glukozy u chorych na cukrzycę typu 2?
Introduction: The normalization of cellular glucose assimilation is the basic aim of therapy in diabetes mellitus. This process should be
accompanied by a proportional increase of the cellular glucose transport (CGT). The level of CGT should react to therapy typically recommended
in Type 2 diabetes mellitus (T2DM), composed of diet and sulfonylurea. In order to explore this clinically significant hypothesis,
a clinical-experimental study was undertaken. Its aim was to determine the clinical pharmacotherapeutic significance of CGT measurements.
Material and methods: CGT testing was performed on peripheral blood lymphocytes. CGT was assessed with 2-[3H(G)] glucose: before,
and after the addition of sulfonylurea or sulfonylurea plus insulin to the incubation medium. Tests were performed at baseline in
28 persons with newly diagnosed, "therapeutically naive" T2DM and in 20 control subjects. In diabetic patients the tests for CGT were
repeated after 3 months of routine diet and sulfonylurea therapy. In addition, the level of GLUT4 expression was assessed by flow cytometry
before and after this therapy.
Results: Before treatment, CGT was significantly decreased in all subjects with T2DM. Incubated in-vitro cells responded directly to the
addition of sulfonylurea with a moderate increase of CGT. This response was augmented by the addition of insulin to sulfonylurea in the
incubation medium. The monitored three-month routine, controlled therapy with diet and sulfonylurea resulted in a significant increase
of CGT process in all types of incubation tests.
Conclusions: The basal and reactive CGT is significantly decreased in lymphocytes of persons with T2DM before the introduction of
therapy. Effective therapy with diet and sulfonylurea normalizes both types of CGT - basal and reactive. It is related to the near normalization
of GLUT4 expression in the studied cells. This phenomenon may be used as a new marker for diabetes mellitus pharmacotherapy.

proporcjonalne zwiększenie dokomórkowego transportu glukozy (CGT, cellular glucose transpor). Wielkość CGT powinna zmieniać się
w odpowiedzi na leczenie cukrzycy typu 2, które obejmuje dietę i stosowanie pochodnych sulfonylomocznika. Aby zbadać tę istotną
z klinicznego punktu widzenia hipotezę, przeprowadzono badanie kliniczno-doświadczalne. Miało ono na celu określenie znaczenia
pomiarów CGT w ocenie skuteczności farmakoterapii.
Materiał i metody: Dokomórkowy transport glukozy oceniono na podstawie poboru glukozy 2-[3H(G)] przez limfocyty krwi obwodowej
badanych osób przed i po dodaniu do medium pochodnej sulfonylomocznika lub pochodnej sulfonylomocznika i insuliny. Pomiar przeprowadzono
wyjściowo u 28 chorych z nowo rozpoznaną cukrzycą typu 2, niestosujących wcześniej żadnego leczenia, i u 20 zdrowych
ochotników stanowiących grupę kontrolną. W grupie chorych na cukrzycę pomiar CGT powtórzono po 3 miesiącach stosowania diety
i pochodnych sulfonylomocznika. Ponadto przed rozpoczęciem terapii i po jej zastosowaniu oceniono poziom ekspresji GLUT4 za pomocą
cytometrii przepływowej.
Wyniki: Przed leczeniem u wszystkich chorych na cukrzycę typu 2 CGT był istotnie zmniejszony. W komórkach inkubowanych in vitro
bezpośrednią odpowiedzią na dodanie do medium pochodnej sulfonylomocznika był umiarkowany wzrost CGT. Odpowiedź ta była
silniejsza, kiedy do inkubowanych komórek poza pochodną sulfonylomocznika dodano również insulinę. Po 3 miesiącach kontrolowanego
leczenia dietą i pochodną sulfonylomocznika stwierdzono istotne zwiększenie CGT we wszystkich inkubowanych próbkach.
Wnioski: Przed zastosowaniem terapii podstawowy i reaktywny CGT w limfocytach osób chorych na cukrzycę typu 2 był istotnie obniżony.
Skuteczne leczenie dietą i pochodną sulfonylomocznika spowodowało normalizację CGT - zarówno podstawowego, jak i reaktywnego.
Powyższe korzystne zmiany były następstwem znacznej poprawy ekspresji GLUT4 w badanych komórkach. To zjawisko może
zostać wykorzystane jako nowy wskaźnik w ocenie farmakoterapii cukrzycy typu 2.

Cellular glucose transport disturbances in the pathogenesis and therapy of type 2 diabetes mellitus
Epidemia cukrzycy typu 2 na świecie wynika z dwóch głównych grup czynników przyczynowych. Jedną z nich jest wpływ czynników środowiskowych, opisywany również jako negatywny wpływ cywilizacji. Szacuje się, że około 30% całkowitej populacji na świecie ma
genetyczną predyspozycję do ujawnienia objawów cukrzycy typu 2 pod wpływem czynników zewnętrznych. Zjawisko to jest związane z komórkową i narządową opornością na insulinę. Druga grupa czynników jest związana z dysfunkcją komórek β trzustki.
W związku z powyższym badania etiologii insulinooporności mają znaczenie zarówno teoretyczne, jak i praktyczne. Mechanizmem,
który prowadzi do wielu zaburzeń metabolicznych, a przede wszystkim do insulinooporności jest spadek dokomórkowego transportu
glukozy. Z tego powodu dokomórkowy transport glukozy powinien być uważany jako potencjalny cel postępowania prewencyjnego
i terapeutycznego.
W opracowaniu przedstawiono postępy badań nad dokomórkowym transportem glukozy w aspekcie patofizjologii insulinooporności
i cukrzycy typu 2. Najnowsze doniesienia naukowe podkreślają kluczową rolę dokomórkowego transportu glukozy jako celu prewencji
i terapii cukrzycy typu 2. W artykule opisano obecny stan wiedzy na temat wpływu diety, wysiłku fizycznego oraz terapii farmakologicznej
na dokomórkowy transport glukozy. (Endokrynol Pol 2010; 61 (3): 292-302)The current world epidemic of type 2 diabetes mellitus results from two general groups of causative factors. One is the influence of strong
pathogenetic environmental pressures - also described as negative civilizational influence - on the very large subpopulation, assessed at
30% of the total world population, which is genetically predisposed to react to this external stress with the symptoms of type 2 diabetes
mellitus. Such a pathogenetic reaction is based on the appearance of cellular and organ resistance to insulin. A second factor involves the
beta cells of the pancreatic islets and their dysfunction.
For these reasons, studies on the aetiology of insulin resistance have significance, both theoretical and practical. There are many biological
deviations that can produce cellular insulin resistance and underutilization of glucose. The mechanism that is always present is the decrease
of cellular glucose transport. For this reason, it should be approached as a potential target for preventive and therapeutic actions.
These pathophysiological and clinical circumstances were the motivation for presenting a review of cellular glucose transport pathophysiology,
which contributes to the aetiology of insulin resistance, cellular underutilization of glucose, and type 2 diabetes mellitus. They
underline the significance of cellular glucose transport as a target for prevention and therapy of type 2 diabetes mellitus and other insulinresistant
conditions.
This review presents comments about the influence on cellular glucose transport of diet, physical exercise, and pharmacotherapeutic
agents, based on the authors’ studies. The review could contribute to an innovative approach to the pathogenesis, prevention, and therapy
of type 2 diabetes mellitus and other conditions related to insulin resistance.

Insulina jako główny regulator komórkowej utylizacji glukozy - etiologiczne aspekty insulinooporności
This review presents the advances in the molecular biology and the pathophysiology of insulin resistance with emphasis on disturbances
in cellular glucose transport. New scientific information about the structure and function of glucotransporters from the GLUT4 and SLGT
families underline their significance in endocrinopathies and metabolic disease pathogenesis as related to insulin resistance. The new
discoveries in this area also contribute to a better understanding of the regulation of insulin receptor and post-receptor reactivity by
hormones and by drugs. They refer to the regulation of glycaemia and to its disturbances in diabetes mellitus, particularly of type 2, to
metabolic syndrome, and, in general, to the pathogenesis of many syndromes and clinical disturbances caused by insulin resistance.
Impairment of cellular glucose transport may be one of the primary aetiological factors in this respect. Therefore, studies of cellular glucotransporters
expression and function promise new clinical and pharmacotherapeutic developments. Progress in this area has already

podkreślają ich rolę w patogenezie insulinooporności prowadzącej do powstania endokrynopatii oraz chorób metabolicznych.
Nowe odkrycia w tej dziedzinie przyczyniają się również do lepszego zrozumienia wielopoziomowego działania insuliny, a także patogenezy
cukrzycy typu 2, zespołu metabolicznego oraz wielu schorzeń spowodowanych insulinoopornością.
Upośledzenie dokomórkowego transportu glukozy może być jednym z podstawowych czynników etiologicznych insulinooporności.
W związku z powyższym badania ekspresji i funkcji glukotransporterów zapowiadają nowe osiągnięcia kliniczne i farmakoterapeutyczne.
Postęp w tym zakresie przyczynił się już do powstania wielu praktycznych rozwiązań, które poprawiają praktykę kliniczną.

Chemical composition and morphology of renal stones
Two issues related to nephrolithiasis are explored: (1) does the chemical composition and morphology of renal calculi in South Poland overlap with the studies from other countries? and (2) are there possibilities to evaluate in vivo chemical composition of stones using computed tomography? The study was conducted on 108 renal stones. X-ray fluorescence, X-ray diffraction and Fourier transformed infrared spectroscopy were used to determine the chemical composition. The morphology of the stones was examined using micro computed tomography. The stone chemical composition in South Poland indicate that calcium oxalate monohydrate was overwhelmingly dominant (84%) followed by hydroxyapatite (8%) and struvite (6%). The occurrence of uric acid stones was very low (2%). The relative frequency of various stone types is similar in South Poland to other industrialized countries. The studied renal stones were characterized by a large variability in the concentrations of both major and trace elements. The maximum/minimum concentration ratio exceeds two orders of magnitude. Significant morphological differences have been observed between different types of stones. The stones were composed of oxalate polyhedrons stuck together or had the phosphate core overlaid with layers oxalate and organic mater. The use of CT to identify stone type seems to be limited

The normalization of cellular glucose assimilation is the basic aim of metabolic therapy in type 2 diabetes mellitus (T2DM). It requires
parallel changes in the process of cellular glucose transport (CGT).
This review presents the pathophysiological and clinical outlines of CGT. Sequentially, the advances in the mechanisms and classification
of CGT and their physiological and molecular base are described. The role of CGT pathogenetic significance in diabetes mellitus is stressed.
Finally, the opinion is expressed that the CGT study is a potentially important approach to clinical interpretation of glucose metabolism

Wyrażono opinię, że badania CGT stanowią nowy, potencjalnie bardzo istotny sposób ujęcia zaburzeń metabolizmu glukozy oraz farmakoterapii.

Badania transportu glukozy i jego insulinozależnej regulacji w ludzkich limfocytach krwi obwodowej
Introduction: We investigated the effects of insulin on glucose transport in human peripheral lymphocytes using flow cytometry. We
hypothesized that lymphocytes could be used as tools to study insulin action at the cellular level and facilitate the investigation of mechanisms
that lead to insulin resistance.
Material and methods: Blood was withdrawn from 25 healthy subjects. The expression of glucose transporter (GLUT) isoforms in plasma
membrane and the rates of glucose transport were determined with and without insulin (10 to 100 mU/L). Anti-CD3 phycoerythrin monoclonal
antibody was used for lymphocyte gating. GLUT1, GLUT3, and GLUT4 isoforms were determined after staining cells with specific
monoclonal antibodies to GLUT1, 3, and 4. Glucose transport was monitored with deoxy-D-glucose, 2-[3H(G)] - 185–370 GBq.
Results: Insulin increased the uptake of deoxy-D-glucose and the expression of GLUT1, GLUT3, and GLUT4 isoforms in the plasma
membrane. The optimal effects were always reached at 50 mU/L of insulin with the increase in GLUT1, 3, and 4 expression of 12%, 44%,
and 38%, respectively. Mean baseline values of deoxy-D-glucose uptake were 3409 ccpm at 15 min., 6587 ccpm at 30 min., and 12525 ccpm
at 60 min. of investigation. The maximal uptake in insulin-stimulated conditions was reached with 50 mU/L of insulin and went up to
12450 ccpm at 15 min., 37482 ccpm at 30 min., and 37916 ccpm at 60 min. of investigation (p < 0.01).
Conclusions: Peripheral blood lymphocytes may become an interesting model system to study the effects of insulin on cellular glucose
transport. Flow cytometry is suitable for this investigation and may be used as a method to estimate the influence of insulin on GLUTprotein
translocation and the dynamics of glucose uptake by lymphocytes.

transport glukozy w limfocytach krwi obwodowej człowieka. Celem badania było również potwierdzenie hipotezy, że limfocyty
mogą stanowić interesujący model badawczy do oceny komórkowej aktywności insuliny ułatwiający wykrycie mechanizmu prowadzącego
do insulinooporności.
Materiał i metody: Krew do badań pobierano na czczo na heparynę od 25 zdrowych ochotników, dotychczas nie leczonych farmakologicznie.
Ekspresję izoform transporterów glukozy (GLUT) w błonie komórkowej oraz tempo transportu glukozy oznaczono bez i w obecności
insuliny (od 10 do 100 mj./l). W celu oznaczenia obecności limfocytów wykorzystano przeciwciała barwione fikoerytryną anty-CD3.
Obecność izoform GLUT ustalono dzięki zastosowaniu przeciwciał monoklonalnych (MoAb) anty: GLUT1, GLUT3 oraz GLUT4. Transport
glukozy monitorowano z użyciem deoksy-D-glukozy, 2-[3H(G)] - 185-370 GBq.
Wyniki: Insulina spowodowała wzrost wychwytu zarówno deoksy-D-glukozy, jak i ekspresji izoform GLUT1, GLUT3 i GLUT4 w błonie
komórkowej. Optymalny efekt reakcji osiągnięto przy stężeniu insuliny 50 mj./l, uzyskując wzrost ekspresji GLUT1, 3 i 4 odpowiednio
o 12%, 44% oraz 38%. Ilość pobranej deoksy-D-glukozy w warunkach podstawowych wynosiła średnio 3409 ccpm w 15 min, 6587 ccpm
w 30 min oraz 12 525 ccpm w 60 min badania. Przy zastosowanej dawce insuliny 50 mj./l uzyskano maksymalne wartości średnie: 12 450 ccpm
w 15 min, 37 482 ccpm w 30 min oraz 37 916 ccpm w 60 min badania (p < 0,01).
Wnioski: Limfocyty krwi obwodowej mogą stanowić interesujący model doświadczalny badań wpływu insuliny na transport glukozy.
Cytometria przepływowa jest cenną metodą pozwalającą na oszacowanie wpływu insuliny na translokację białek GLUT w błonie komórkowej

Combining areal DXA bone mineral density and vertebrae postero-anterior width improves the prediction of vertebral strength
Objective - Areal bone mineral density (aBMD) measured by dual-energy X-ray absorptiometry (DXA) is an important determinant of bone strength (BS), despite the fact that the correlation between aBMD and BS is relatively weak. Parameters that describe BS more accurately are desired. The aim of this study was to determine whether the geometrical corrections applied to aBMD would improve its ability for BS prediction. We considered new parameters, estimated from a single DXA measurement, as well as BMAD (bone mineral apparent density) reported in the literature. Materials and methods - In vitro studies were performed with the L3 vertebrae from 20 cadavers, which were studied with DXA and quantitative computed tomography (QCT). A mechanical strength assessment was carried out. Two new parameters were introduced: vBMD_{min}=\frac{aBMD}{W_{PA}^{min}} and vBMD_{av}=\frac{aBMD}{W_{PA}^{av}} \left ( W^{min}_{PA}-minimal vertebral body width in postero-anterior \left ( PA \right ) view, {W_{PA}}^{av} — average PA vertebral body width\right ). Volumetric BMD measured by QCT (vBMD), aBMD, BMAD, vBMD _{min}, and vBMD_{av} were correlated to ultimate load and ultimate stress \left ( P_{max} \right ) to find the best predictor of vertebrae BS. Results - The coefficients of correlation between P_{max} and vBDM_{min} as well as BMAD, were r = 0.626 (p = 0.005), r = 0.610 (p = 0.006) and r = 0.567 (p = 0.012), respectively. Coefficients for vBMD and aBMD are r = 0.648 (p = 0.003) and r = 0.511 (p = 0.03), respectively. Conclusions. Our results showed that aBMD normalized by vertebrae dimensions describes vertebrae BS better than aBMD alone. The considered indices vBMD_{av}, vBMD_{min} and BMAD can be measured in routine PA DXA and considerably improve BS variability prediction. vBMD_{min} is superior compared to vBMD_{av} and BMAD
Thermographic assessment of skin prick tests in comparison with the routine evaluation methods
Introduction: The skin prick test is still the first and basic procedure in the diagnosis of allergic diseases. The possibility
of using a sensitive thermographic method supported by the mathematical model for the assessment of
skin test results will be highlighted in the studies.
Aim: To compare the proposed approach with routine planimetric and thermographic methods.
Material and methods: A mathematical model of allergic reaction was developed. Simplifying assumptions of the
IgE-mediated skin reaction is the essence of the model. Investigations were performed in a group of 40 patients.
Results: Using the spatio-temporal evolution of temperature distributions, the ratios of the histamine released from
mast cells to the control histamine were determined. The obtained values very well correlate with the standard
evaluation of skin prick tests (correlation coefficient = 0.98).
Conclusions: The proposed method of skin test evaluation presents several advantages. The continuous acquisition
of data provides the monitoring of time course of the allergic response. The transport of mediator and its concentration
were distinctly discriminated, which may be diagnostically useful, especially for abnormal cases. The high
sensitivity of the method enables studying patients regardless of age and skin sensitivity
Metabolic syndrome - controversies between academic debate and real world medical practice
Pojęcie zespołu metabolicznego odnosi się do zjawiska współwystępowania
charakterystycznych, głównie metabolicznych, czynników
ryzyka miażdżycy i jej powikłań. Dotychczas nadal trwają
dyskusje dotyczące kolejnych modyfikacji definicji tego zespołu.
Łączą się one z wieloskładnikowym charakterem czynników etiologicznych
i patogenetycznych.
Nie można podać definicji zespołu metabolicznego opartej na znajomości
etiologii. Stąd wynikają utrudnienia zarówno diagnostyczne,
jak i w formułowaniu planów prewencji i leczenia. Obserwacje
kliniczne wskazują, że liczba czynników ryzyka miażdżycy i jej
powikłań z reguły jest większa niż ujmują to proponowane definicje;
są one dodatkowo ilościowo zmienne. Można sądzić, że ich
określenie za pomocą pojęcia „ryzyka angiometabolicznego” oraz
zwiększenie dążności do indywidualizacji klinicznego postępowania
może być bardziej przydatne w praktyce niż akademickie dyskusje
dotyczące z reguły zbyt uproszczonych kryteriów. Z tego
względu praktycznie istotne jest sprecyzowanie bardziej indywidualnych
zasad postępowania diagnostycznego, prewencyjnego
i leczniczego w formie algorytmu decyzji klinicznych. W opracowaniu
przedstawiono odpowiednie propozycje. Wydaje się, że
obecnie w każdym przypadku zespołu metabolicznego konieczna jest ocena insulinooporności. Do pierwotnych działań prewencyjnych
i leczniczych należy zmniejszenie insulinooporności
- także na drodze farmakologicznej.The expression „metabolic syndrome” points to the coexistence of
selected, mainly metabolic factors, forming the increased global risk of
atherosclerosis and of its complications. However the delineation of
such cluster of metabolic, atherogenic risk continue to be under discussion
resulting in many not always well evidenced definitions. They
are related to selected on different levels of evidence criteria, which
are not equivalent. Primary etiological mechanisms of the metabolic
syndrome are not known. This fact impairs all actual definition. It is
also the cause at many diagnostic, preventive and therapeutic difficulties
in clinical practice. The number of the risk factors of atherosclerosis
and of its complications is almost always greater than the
frames of definitions; the risk factors are also quantitatively different.
Therefore their delineation with the more dynamic concept emphasises
of “angiometabolic” risk could be more practical. It the
need for individualization, which could be more useful than the
academic discussion about the definitions.
It follows that such approach should be based on more individual
