    The usability of Homelab, a digital self-service at a Dutch general practice, for diagnostic tests: pilot study with a questionnaire

    Background: eHealth potentially can make health care more accessible and efficient and help reduce the workload in primary health care. Homelab is an eHealth tool implemented in a general practice environment, and it offers relatively simple laboratory diagnostic tests without the referral of the general practitioner. After logging in this eHealth tool, patients select and order a diagnostic test based on their symptoms. The test results are presented online to the general practitioner and the patient. Objective: This study aims to evaluate the use, usability, and user characteristics of Homelab. Further, it aims to evaluate whether Homelab replaces an appointment with the general practitioner. Methods: Homelab has been implemented since May 2021 as a pilot in a Dutch general practice. The number of requests and the ordered diagnostic packages are monitored. After using Homelab, patients are invited to complete a short questionnaire. The questionnaire contains demographic questions and assesses usability using the System Usability Scale (10 items). In addition, questions about requesting an appointment with the general practitioner without Homelab are included. All data were anonymous. Results: The questionnaire was filled by 74 individual patients. The mean age of the patients was 40.33 (SD 12.11) years, and half of them were females (39/74, 53%). The majority of the patients were highly educated (56/74, 76%) and employed (53/74, 72%). Approximately 81% (60/74) of the patients reported that they would use Homelab again in the future and 66% (49/74) reported that they would have gone to the general practitioner if they had not used Homelab. The usability of Homelab was perceived higher by the younger age group (mean 73.96, SD 14.74) than by the older age group (mean 61.59, SD 14.37). In total, 106 test packages were ordered over 1 year, and the most requested diagnostic package was "Am I still healthy? I want to do my annual health checkup." Homelab was used the most during the months of the COVID-19 lockdown. Conclusions: The use of Homelab, a digital self-service for ordering diagnostic tests, was monitored in this study, and its usability was perceived as above average. Our findings showed that patients are willing to use Homelab in the future and they would use it most of the time as a replacement for regular consultations.     Background Given the pressure on modern healthcare systems, eHealth can offer valuable opportunities. However, understanding the potential and challenges of eHealth in daily practice can be challenging for many general practitioners (GPs) and their staff. Objectives To critically appraise five widely used eHealth applications, in relation to safe, evidence-based and high-quality eHealth. Using these applications as examples, we aim to increase understanding of eHealth among GPs and highlight the opportunities and challenges presented by eHealth. Discussion eHealth applications can support patients while increasing efficiency for GPs. A three-way division (inform, monitor, track; interaction; data utilisation) characterises many eHealth applications, with an increasing degree of complexity depending on the domain. All applications provide information and some have extra functionalities that promote interaction, while data analysis and artificial intelligence may be applied to support or (fully) automate care processes. Applications in the inform domain are relatively easy to use and implement but their impact on clinical outcomes may be limited. More demanding applications, in terms of privacy and ethical aspects, are found in the data utilisation domain and may potentially have a more significant impact on care processes and patient outcomes. When selecting and implementing eHealth applications, we recommend that GPs remain critical regarding preconditions on safe, evidence-based and high-quality eHealth, particularly in the case of more complex applications in the data utilisation domain.Prevention, Population and Disease management (PrePoD)Public Health and primary car

    Stoma-free Survival After Rectal Cancer Resection With Anastomotic Leakage: Development and Validation of a Prediction Model in a Large International Cohort

    OBJECTIVE: This study aimed to develop and validate a prediction model (STOMA-score) for one-year stoma-free survival in rectal cancer (RC) patients with anastomotic leakage (AL). BACKGROUND: AL after RC resection often results in a permanent stoma. METHODS: This international retrospective cohort study (TENTACLE-Rectum) encompassed 216 participating centres, and included patients who developed AL after RC surgery between 2014-2018. Clinically relevant predictors for one-year stoma-free survival were included in uni- and multivariable logistic regression models. The STOMA-score was developed and internally validated in a cohort of patients operated between 2014-2017, with subsequent temporal validation in a 2018 cohort. The discriminative power and calibration of the models' performance were evaluated. RESULTS: This study included 2499 AL patients; 1954 in the development cohort and 545 in the validation cohort. Baseline characteristics were comparable. One-year stoma-free survival was 45.0% in the development cohort and 43.7% in the validation cohort. The following predictors were included in the STOMA-score: sex, age, ASA-classification, body mass index, clinical M-disease, neoadjuvant therapy, abdominal- and transanal approach, primary defunctioning stoma, multivisceral resection, clinical setting in which AL was diagnosed, postoperative day of AL diagnosis, abdominal contamination, anastomotic defect circumference, bowel wall ischemia, anastomotic fistula, retraction and reactivation leakage. The STOMA-score showed good discrimination and calibration (c-index 0.71, 95%CI 0.66-0.76). CONCLUSION: The STOMA-score consists of eighteen clinically relevant factors and estimates the individual risk for one-year stoma-free survival in patients with AL after RC surgery, which may improve patient counselling and give guidance when analyzing efficacy of different treatment strategies in future studies.Nynke G.     Prevention, early diagnosis and treatment of complications after colorectal surgery

    Dit proefschrift richt zich op het voorkomen, vroeg diagnosticeren en behandelen van complicaties na gastro-intestinale chirurgie. Het voorkomen hebben we onderzocht door patiënten met bloedarmoede (anemie) die een operatie voor darmkanker ondergaan, te behandelen met oraal of intraveneus ijzer om ze zo fit mogelijk voor de operatie te krijgen (FIT-trial). Anemie is een veelvoorkomend probleem en zien we bij 1 op de 3 patiënten met de diagnose darmkanker. In dit proefschrift hebben we gevonden dat deze patiënten meer profijt hebben van intraveneus ijzer, dan van oraal ijzer om de bloedarmoede te herstellen. Tevens zagen we minder complicaties en IC-opnames na de operatie in patiënten met een milde anemie na behandeling met intraveneus ijzer.Vroege diagnostisering hebben we onderzocht door in de eerste dagen na de operatie ontstekingswaardes in het bloed te bekijken en daarna een CT-scan te maken als deze verhoogd waren. We vonden dat dit een effectieve en veilig methode is en bijna geen patiënten gemist werden. Hierbij hebben we met name naar een ernstige complicatie gekeken; de naadlekkage. Hierbij lekt darminhoud in de buik bij de nieuwe darmaansluiting (naad) die tijdens de operatie is aangelegd en kan serieuze consequenties hebben, zoals heroperatie of IC-opname.Met de klassieke behandeling (aanleggen stoma) geneest slechts de helft van de naadlekkages en houden veel patiënten op lange termijn een stoma. In dit proefschrift hebben we een innovatieve behandeling onderzocht, waarbij vacuümsponsen werden geplaatst in de holte die ontstaat door naadlekkage. Deze zuigt de holte schoon, waardoor deze vervolgens gesloten kan worden met hechtingen (EVASC-therapie). Uit dit proefschrift bleek dat tot 90% kan genezen en dat veel minder patiënten een stoma hield, met name als de behandeling snel startte.Samenvattend, dragen de resultaten van dit proefschrift bij aan de kennis over complicaties rondom darmkankeroperaties en worden deze deels ook al gebruikt in de huidige praktijk om de zorg te verbeteren

    The transanal approach is a new and exciting addition to the surgeons' repertoire to deal with complications after colorectal surgery. Improved exposure, accessibility, and visibility greatly facilitate adequate dissection of the affected area with potential increase in effectiveness and reduced morbidity. An essential component in salvaging anastomotic leaks of low colorectal, coloanal, or ileoanal anastomoses is early diagnosis and early treatment, especially when starting with endoscopic vacuum therapy, followed by early surgical closure (endoscopic vacuum-assisted surgical closure). Redo surgery using a transanal minimally invasive surgery platform for chronic leaks after total mesorectal excision surgery or surgical causes of pouch failure successfully mitigates limited visibility and exposure by using a bottom-up approach
