    INTERACTIONAL UNFOLDING OF VOCABULARY EXPLANATIONS IN MEANING AND FLUENCY CONTEXTS

    In the field of foreign and second language education, there is great deal of research on vocabulary teaching and learning. However, there is relatively limited research dealing with the vocabulary explanations during ongoing classroom interaction. This article aims to provide vocabulary explanation sequences in meaning and fluency contexts (Seedhouse, 2004). To this end, a foreign language education classroom including 13 students was recorded for a classroom hour, and the interaction in the classroom was transcribed. The classroom interactions were analyzed by using conversation analysis methodology, and vocabulary explanation examples were described in detail. As a result of the analysis, two vocabulary explanation sequences are provided in meaning and fluency contexts based on the students’ responses. Results show that when the students display understanding in their native language, the teacher asks follow–up questions before closing the turn; however, when they display understanding in the target language, the teacher closes the turn without asking further questions.Keywords: conversation analysis, vocabulary explanation, meaning and fluency contexts, classroom interaction

    The Role of Store Environment Cues on Store Personality and Store Image

    Purpose – This paper aims to explain the effects of color, lighting, and music as store environment cues on the perceptions among consumers of store personality, store image, and on their own store preferences. Design/Methodology/Approach – An experimental approach was employed to determine these effects, using a between-subjects factorial design including two levels of color, lighting, and music. The data were collected from 600 participants through face-to-face surveys. A video design of a fictional store environment was shown to the subjects and survey questions were applied to eight different experimental groups by providing manipulation control. Results and implications – The results demonstrate that consumer perceptions of store image, store personality, and store preferences differ with changing color, lighting, and music variables. Additionally, these variables differ in their interactions with each other and in their total interactions. Limitations – While the present study sheds light on the influence of color, lighting, and music cues of the fashion store environment on consumer perceptions of store personality and image, as well as their store preferences, interactions between store environment cues and other attributes in stores should also be explored. Originality – This study examines the impact of store environment variables in their different aspects on consumer perceptions of store image and store personality, as well as consumer store preferences, by focusing on Mehrabian and Russell’s (1974) Stimulus-Organism-Response (SOR) model as its theoretical background

    Design and development of a multi-functional laparoscopic device

    This study presents a design of a multifunctional laparoscopic appendectomy device that includes three surgical instruments commonly used in laparoscopic appendicitis surgeries: endoloop, endobag and scissors. It collects these three independent surgical tools in a single laparoscopic appendectomy device. These days there is a trend of moving to multi-functional surgery devices during minimally invasive surgery. The main reasons behind the minimal invasive surgery are to avoid changing the devices several times during the operation, to reduce the time spent in operation, to increase the efficiency of the operation, to facilitate the follow-up of the camera and devices, and to leave trocars to be used for other surgical instruments. The multi-functional appendectomy device that, we present here, provides these benefits. The standard trocar entries are appropriate for its usage. The presented multifunctional laparoscopic appendectomy device offers more practical use in comparison to individual devices. On the other hand, development of these multi-functional surgery devices can be directly enhanced to the robotic surgery devices

    Paracervical block with 1% lidocaine for pain control during intrauterine device insertion: a prospective, single-blinded, controlled study

    In this prospective controlled study we aimed to investigate efficacy of paracervical block with 1% Lidocaine for pain control and demographic variables which may affect pain perception during intrauterine device insertion in Turkish women. Data from 95 women assigned to paracervical block (n=34), placebo (n=30) and no treatment (n=31) arms and asked to grade the pain level they  felt during tenaculum placement, intrauterine device insertion and 5 minutes after the procedure using a visual pain scale. Demographic variables were also recorded. Pain scores were found to be lower in paracervical block group when compared to other 2 groups during tenaculum placement (p=0.00), intrauterine device insertion (p=0.00) and 5 minutes after the procedure (p=0.00). Level of pain was unrelated to mode of previous deliveries and current breastfeeding. Paracervical block is an easy, safe and effective way of pain control during intrauterine device insertion. Lack of vaginal birth history is not a reason to draw back from intrauterine device use

    Çok fonksiyonlu laparoskopik apendektomi cihazı

    Bu çalışma laparoskopik apendisit cerrahisinde kullanılan üç adet cerrahi aleti; endoskopik düğüm halkası (endoloop), endoskopik spesimen torbası (endobag) ve makası, bir tek laparoskopik el aletinde toplayan, çok fonksiyonlu bir laparoskopik apendektomi aletinin tasarımını sunmaktadır. Minimal invaziv ameliyatlarda çok fonksiyonlu ameliyat cihazlarına yönelimin en büyük nedeni operasyon sırasında sık alet değiştirmelerin önüne geçmek, ameliyat süresini kısaltmak, işlemin güvenirliğini arttırmak, kamera ile aletlerin takibini kolaylaştırmak ve diğer cerrahi aletlerin kullanılabilmesi için trokar veya trokar yeri bırakmaktır. Tasarladığımız çok fonksiyonlu apendektomi cihazı yukarıda saydığımız bu faydaları sağlamaktadır. Kullanımı için standart trokar girişleri uygundur. Önerdiğimiz çok-fonksiyonlu cerrahi alet belirttiğimiz üç aleti tek tek kullanmaktan daha pratik kullanım sunmaktadır. Çok fonksiyonlu cerrahi cihazların geliştirilmesindeki diğer bir motivasyon olarak bu cihazları robotik cerrahide kullanmak ulaşmak istediğimiz hedefler arasında yer almaktadır

    Educational Family Health Centers' in Family Medicine Resident Training Experience of Şişli Hamidiye Etfal Training and Research Hospital

    Aile hekimliği uzmanlık eğitimi Türkiye’de 1984 yılındabaşlamıştır. 2010 yılına kadar aile hekimliği uzmanlık öğrencilerinin 3 yıllık eğitim sürecinin tamamı klinik rotasyonlardan oluşmaktayken; 2010 yılından bu yana belirtilenamaçları uygulayabilmek ve yeterlilikleri kazanabilmek için,18 ay klinik rotasyon, 18 ay ise saha eğitimi olacak şekildedüzenleme yapılmıştır. Saha eğitiminin ihtiyacını karşılamak üzere Sağlık Bakanlığı tarafından 2014 yılında ‘EğitimAile Sağlığı Merkezleri’’ (E-ASM) kurulmasıyla ilgili mevzuat çıkarılmıştır. Aile hekimliği uzmanlığı saha eğitimindeönemli bir boşluğu doldurması beklenen E-ASM’lerin eğitimhizmetinin yanında diğer aile sağlığı merkezlerinin verdiğihizmetleri vermesi de beklenmektedir. E- ASM’lerin sayısıhalen yetersiz olup arttırılması için eğitim kurumlarınınfarkındalığının artırılması, malzeme temini, tadilat ve tamirat ihtiyaçlarının kolaylaştırılması gerekmektedir.Sağlık Müdürlükleri tarafından alt yapının oluşturulmasıve destek personelinin sağlanması; Aile hekimliği eğitimkurumları tarafından ise eğitici görevlendirilmesi ve eğitimgörecek uzmanlık öğrencisi planlanması alternatif olarakdüşünülmelidir. Nüfus kaydının ve hizmet planlamalarınınasistan eğitimine imkân verecek şekilde düzenlenmesi, görev yapacak eğitici, asistan ve diğer personelin ödemelerinin hak kaybına neden olmayacak şekilde yapılandırılması, sahada yapılacak araştırmalarla ilgili izin süreçlerininkolaylaştırılması gerekmektedir. Until 2010, 3-year education process of family medicine residency students consisted of clinical rotations. Since 2010, in order to implement the stated objectives and gain the qualifications, 18 months of clinical rotation and 18 months of field training have been arranged. In order to meet the need for field training, the Ministry of Health issued legislation on the establishment of Education Family Health Centers (E-FHC) in 2014. It is expected that the E-FHCs, which are anticipated to fill a significant gap in family medicine speciality field training, in addition to educational services they have to provide services provided by other family health centers. The number of E-FHCs is still insufficient and to increase it, the awareness of educational institutions should be increased, material procurement, modification and repair needs should be facilitated. The establishment of infrastructure by the Health Directorates and the provision of support staff, as well as the planning of the trainer by the family medicine education institutions and the planning of the specialist for the training should be considered as an alternative. It is necessary to arrange population registration and service planning in a way that will allow residence training, to make trainers, residences and other personnels to be employed in such a way that they do not cause any loss of right, and to facilitate the process of permits related to the researches in the field. In this paper, it is aimed to contribute to the improvement of E-FHC process by sharing the experiences about opening and operating of two separate E-FHCs of the Health Sciences University Şişli Hamidiye Etfal Training and Research Hospital Family Medicine Clinic which started to serve in two buildings as of August 2016

    Assessment the effect of diabetes education on self-care behaviors and glycemic control in the Turkey Nursing Diabetes Education Evaluating Project (TURNUDEP): a multi-center study

    Background: Diabetes education in Turkey is provided by diabetes nurse educators in almost all healthcare organizations. However, the education is not standardized in terms of learning content, duration, and methods. This multi-center study was performed to assess the self-care behaviors and glycemic control following education provided to the patients with type 2 diabetes mellitus by diabetes nurse educators. Methods: This was a descriptive and cross-sectional study and included 1535 patients admitted to 28 public hospitals for the treatment of type 2 diabetes mellitus. The education was assessed by using a Patient Identification Form and Self-care Scale. Results: The proportion of individuals who received diabetes education within the last year was 78.5%, with 46.7% of them having received it once. Of the patients, 84.8% reported that they received diabetes education individually. It was found that the proportion of individuals who received education about oral antidiabetics (78.5%) and glucose testing at home (78.5%) was higher than the proportion of individuals who received education about exercise (58.8%) and foot care (61.6%). The status of diabetes education, education intervals, and the correlation of the education method with self-care and glycemic control was evaluated. Self-care and glycemic control levels were better among the patients who received diabetes education thrice or more and in patients who received education both individually and in a group (p < 0.05). Conclusions: Approximately three-quarters of individuals with type 2 diabetes mellitus received education by diabetes nurse educators in Turkey. Diabetes education is positively correlated with self-care and glycemic control levels among patients with type 2 diabetes mellitus. Efforts for generalization and standardized education for all diabetes patients are necessary. © 2022, The Author(s)

    The frequency of Duchenne muscular dystrophy/Becker muscular dystrophy and Pompe disease in children with isolated transaminase elevation: results from the observational VICTORIA study

    IntroductionElevated transaminases and/or creatine phosphokinase can indicate underlying muscle disease. Therefore, this study aims to determine the frequency of Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) in male children and Pompe disease (PD) in male and female children with isolated hypertransaminasemia.MethodsThis multi-center, prospective study enrolled patients aged 3–216 months with serum alanine transaminase (ALT) and/or aspartate transaminase (AST) levels &gt;2× the upper limit of normal (ULN) for ≥3 months. Patients with a known history of liver or muscle disease or physical examination findings suggestive of liver disease were excluded. Patients were screened for creatinine phosphokinase (CPK) levels, and molecular genetic tests for DMD/BMD in male patients and enzyme analysis for PD in male and female patients with elevated CPK levels were performed. Genetic analyses confirmed PD. Demographic, clinical, and laboratory characteristics of the patients were analyzed.ResultsOverall, 589 patients [66.8% male, mean age of 63.4 months (standard deviation: 60.5)] were included. In total, 251 patients (188 male and 63 female) had CPK levels above the ULN. Of the patients assessed, 47% (85/182) of male patients were diagnosed with DMD/BMD and 1% (3/228) of male and female patients were diagnosed with PD. The median ALT, AST, and CPK levels were statistically significantly higher, and the questioned neurological symptoms and previously unnoticed examination findings were more common in DMD/BMD patients than those without DMD/BMD or PD (p &lt; 0.001).DiscussionQuestioning neurological symptoms, conducting a complete physical examination, and testing for CPK levels in patients with isolated hypertransaminasemia will prevent costly and time-consuming investigations for liver diseases and will lead to the diagnosis of occult neuromuscular diseases.
