    Integrating the prevention of mother-to-child transmission of HIV into primary healthcare services after AIDS denialism in South Africa: perspectives of experts and health care workers - a qualitative study

    Integrating Prevention of Mother-to-Child Transmission (PMTCT) programmes into routine health services under complex socio-political and health system conditions is a priority and a challenge. The successful rollout of PMTCT in sub-Saharan Africa has decreased Human Immunodeficiency Virus (HIV), reduced child mortality and improved maternal health. In South Africa, PMTCT is now integrated into existing primary health care (PHC) services and this experience could serve as a relevant example for integrating other programmes into comprehensive primary care. This study explored the perspectives of both experts or key informants and frontline health workers (FHCWs) in South Africa on PMTCT integration into PHC in the context of post-AIDS denialism using a Complex Adaptive Systems framework. METHODS: A total of 20 in-depth semi-structured interviews were conducted; 10 with experts including national and international health systems and HIV/PMTCT policy makers and researchers, and 10 FHCWs including clinic managers, nurses and midwives.     Preserving data privacy and security in Australian my health record system: A quality health care implication

    Australian My Health Record (MyHR) system must enable efficient availability of meaningful, accurate, complete and up-to-date health data. However, the major challenge must be to ensure the security of the clinical information of the MyHR. The foremost question that remains unanswered is ‘are current information security settings adequate to protect MyHR?’. To build an adequate security setup and increase the uptake of the MyHR system, it is imperative to show the MyHR is safe to use. In addressing this issue and implementing the adoption of the initiative, we determine and systematically analyse the existing threats to the system. We assess strengths of various solutions against possible threats and discuss the development and implementation process of the proposed model

    Objetivo: verificar las estimativas de confiabilidad y validez de constructo de la escala "Evaluación del producto del cuidar en enfermería" (APROCENF) y su aplicabilidad. Métodos: este estudio de validación incluyó en su muestra 40 (confiabilidad inter-evaluadores) y 172 (validez de constructo) evaluaciones efectuadas por enfermeros al final del turno de trabajo en nueve unidades de internación de un hospital universitaria del sudeste brasileño. Los datos fueron recolectados entre febrero y septiembre de 2014 de forma interrumpida. Fueron calculados los coeficientes alfa de Cronbach y correlación de Spearman (consistencia interna), la correlación intraclase y Kappa ponderado (confiabilidad inter-evaluadores)y el análisis factorial exploratorio fue utilizado con extracción por componentes principales y rotación varimax (validación de constructo). Resultados: la consistencia interna reveló alfa de0,85,correlación entre ítems variando de 0,13 a 0,61 y ítem-total de 0,43 a 0,69. La equivalencia inter-evaluadores fue alcanzada y todos los ítems evidenciaron cargas factoriales significantes. Conclusión: esta investigación mostró evidencias de confiabilidad y validez del constructo del instrumento para evaluación del producto del cuidar en enfermería. Su aplicación en la práctica de enfermería posibilita identificar mejoras necesarias en el proceso productivo, contribuyendo para decisiones gerenciales y asistenciales.Objectives: to verify the reliability and construct validity estimates of the "Assessment of nursing care product" scale (APROCENF) and its applicability. Methods: this validation study included a sample of 40 (inter-rater reliability) and 172 (construct validity) assessments performed by nurses at the end of the work shift at nine inpatient services of a teaching hospital in the Brazilian Southeast. The data were collected between February and September/2014 with interruptions. Cronbach's alpha and Spearman's correlation coefficients were calculated, as well as the intraclass correlation and the weighted kappa index (inter-rater reliability). Exploratory factor analysis was used with principal component extraction and varimax rotation (construct validity). Results: the internal consistency revealed an alpha coefficient of 0.85, item-item correlation ranging between 0.13 and 0.61 and item-total correlation between 0.43 and 0.69. Inter-rater equivalence was obtained and all items evidenced significant factor loadings. Conclusion: this research evidenced the reliability and construct validity of the scale to assess the nursing care product.     Integrating organisation of healthcare services, workers' wellbeing, and quality of care:An introduction to the system-based perspective of healthy healthcare

    The current chapter introduces Healthy Healthcare, an integrated perspective involving quality of care, workers' wellbeing, and organisation of healthcare services, for a system-based understanding of healthcare practice. Healthy Healthcare is based on three main conditions, herein termed pillars, of healthcare delivery: (a) quality of care; (b) workers' wellbeing; and (c) organisation of healthcare. This perspective is important to develop research approaches and to incorporate evidence-based practice and knowledge into Healthy Healthcare. The current volume provides perspectives on Healthy Healthcare based on research from different disciplines and different countries. This chapter introduces Healthy Healthcare with a brief presentation of the modern context of healthcare practice and a description and explanation of the system. It concludes with a brief outline of the volume's contents.</p
