    Cardio-Vascular Complications of Hydroxychloroquine Use: Can Impact COVID-19 Pandemic

    As the coronavirus pandemic is on the rise, many compounds with anti-viral properties are under investigation. Hydroxychloroquine (HCQ) being the daily debated daily during this COVID-19 pandemic is an immunomodulatory drug which has been used for indications like malaria, systemic lupus erythematosus and arthritis. Although some researchers have claimed its effectiveness against coronavirus, it results in proarrhythmic effects and drug-induced long QT syndrome. These cardiac issues while using hydroxychloroquine, have limited its use against coronavirus. A literature search was performed, and general safety information of this drug was collected. It can be concluded that this drug leads to cardiovascular events, heart disease, hypotension, tachycardia, and QT interval prolongation, sometimes in combination with other drugs and should be prescribed to the patients only after thoroughly estimating its benefit risk ratio

    Synthesis, Characterization, In Vitro and In Vivo Screening of Unsymmetrical Borole Complexes of 2-Hydroxy-N-Phenylbenzamide and its Derivatives

    Biochemical aspects, synthesis and characterization of some boron complexes of 2-hydroxy-N-phenylbenzamide (HOPhBenz) and its semicarbazone (HOPhBenz.SCZH) and thiosemicarbazone (HOPhBenz.TSCZH) are described. These derivatives were prepared by the reactions of 2-isopropoxy-4-methyl-1,3,2-dioxaborolane, and 2-isopropoxy-4-methyl-1,3,2-dioxaborinane with 2-hydroxy-N-phenylbenzamide, 1-[(2-hydroxyphenyl)-1-N-phenylamino]hydrazinecarboxamide (HOPhBenz.SCZH), and 1-[(2-hydroxyphenyl)-1-N-phenylamino]hydrazinecarbothioamide(HOPhBenz.TSCZH) in a 1:1 molar ratio. In order to assess the increase of the inhibitor potency, (HOPhBenz), (HOPhBenz.SCZH), (HOPhBenz.TSCZH) and their boron complexes have been tested in vitro against a number of pathogenic fungi and bacteria at different concentrations and were found to possess remarkable fungicidal and bactericidal properties. The testicular sperm density, testicular sperm morphology, sperm motility, density of cauda epididymal spermatozoa and fertility in mating trials and biochemical parameters of reproductive organs are discussed

    Are preeclampsia and small for gestational age baby could be predicted by placental location?

    Objective: Preeclampsia is one of the major cause of maternal and perinatal mortality and morbidity. The pathophysiology is complex and involves multiple organs. The purpose of this study was to find out whether the placental laterality as a predictor of preeclampsia and small for gestational age baby.Methods: This was prospective observational study conducted from February 2015 to December 2015, in a tertiary care hospital of Delhi. 347 antenatal patients attending obstetrics OPD without high risk factors were enrolled. After enrolment 50 patients were lost to follow up and 27 not delivered in our hospital. Ultrasonography for placental localization was done at 18-24 weeks of pregnancy. Patients were followed till delivery for pregnancy outcomes such as preeclampsia, small for gestation age (SGA) baby and mode of delivery. Placenta locations were divided into lateral (either right or left) and central (anterior, posterior or fundal).Results: Out of 347, a total of 270 patients were analysed, 39 (14.4%) had lateral placenta and among them 17 (43.5%) developed preeclampsia and 24 (61.5%) had small for gestational age baby (p<0.001). 231 (85.5%) had central placenta and among them 49 (21.2%) developed preeclampsia and 63 (27.2%) had small for gestational age baby (p <0.001).Conclusions: Laterally located placenta had significant association with preeclampsia and small for gestational age babies

    Background: Herbal drugs are used in treatment of diseases since decades. Major contributing factor for their use is easy availability, less expensive and more belief of common population because of relatively less side effects compared to allopathic medicines. Medicines of natural origin or functional foods in the prevention of disease are the need of hour. Hence, the present review focused on activity of four drugs viz. Withania somnifera,Allium sativum,Curcuma longa andAzadirachta indica and role in different clinical complications.Methods: A thorough review of all the articles, research as well as reviews available regarding the concerned topic was performed. MEDLINE database was searched and English language articles were preferably selected.Results: Withania somnifera, Allium sativum, Curcuma longa andAzadirachta indicahave shown alleviation in inflammation, diabetes and cancer states. The herbal drugs have shown beneficial effects in the prevention and treatment of these disorders. Conclusion from these facts:Utilizing this concept, it can be assumed that herbal drugs play an intricate role in safeguarding the health of individuals from life-threatening complications. However, validation and reproducibility of results in clinical trails should be there in order to confirm the safety andefficacy of these herbal drugs

    Comparative evaluation of levetiracetam and valproic acid as monotherapy on cognitive impairment in patients of epilepsy

    Background: Cognitive decline with AEDs (Anti-epileptic drugs) is associated with learning and memory deficits especially in the younger age group. The data regarding the impact of levetiracetam and valproic acid as monotherapy on cognition in epileptic patients is scarce. The present study was done for evaluation of cognitive decline associated with the use of AEDs.Methods: Present study was a prospective study on 60 patients on AEDs for a period of 12 weeks. Patients were enrolled from the Department of Neurology, Swami Rama Himalayan University, Dehradun, Uttarakhand, India and divided into group A (levetiracetam) and group B (valproic acid) with 30 patients in each group. Permission from the institutional ethics committee and written informed consent was taken from all the patients. They were analyzed for cognitive impairment using MMSE and MoCA scales at baseline and 12 weeks.Results: The mean duration of disease was 2.13±1.1 years and 2.08±1.1 years and mean age of the patients was 14.67±1.9 years in group A and 16.20±1.6 years in group B. GTCS was present in 31 patients (52%) followed by partial seizures in 29 patients (48%). The mean change in the MMSE scores from baseline to 12 weeks was significant in group A 1.30±1.1 (p value <0.05) and change group B was -0.20±1.4 not statistically significant. The mean change was observed in MoCA scores from baseline to 12 weeks was significant in both groups A and B by 1.17±1.1 and -0.70±1.1 respectively (P value <0.05).Conclusions: Patients on levetiracetam showed cognitive improvement, whereas patients on valproic acid showed a decline in the MMSE and MoCA scores

    Evaluation of levetiracetam and valproic acid as monotherapy on quality of life in patients of generalized tonic clonic epilepsy

    Background: Quality of life plays an important role in patients of epilepsy and is the most neglected part during management. The antiepileptic drugs treatment results in seizure control but adversely affect the quality of life in patients.Methods: An observational analytical study was conducted in the Department of pharmacology with Department of Neurology of Himalayan Institute of Medical Sciences, Dehradun over 12 months. 85 patients fulfilling the inclusion criteria with diagnosis of generalized tonic clonic seizures (GTCS) were enrolled and divided into two groups based on physicians discretion and followed up for 12 weeks. Patients were evaluated for quality of life by QOLIE-10 self administered questionnaire at 0 and 12 weeks, assessed for seizure control and drug related adverse effects.Results: 85 patients were enrolled and divided into two treatment arms as per physician discretion, levetiracetam (41) and valproic acid group (44). Study drugs showed significant improvement in quality of life, levetiracetam showed mean change that was significantly greater than valproic acid (p=0.003) at 12 weeks. Patients who failed to achieve seizure control at 6 weeks were 17% patients in levetiracetam and 20% in valproic acid group, reason being non-adherence which was 17% and 20% respectively. Adverse events recorded with Levetiracetam (10), most common being increased sleep and with valproic acid (18), with most common being increased sleep and weight gain.Conclusions: Levetiracetam treatment resulted in better quality of life, with similar seizure control but decreased number of adverse effect then Valproic acid

    Electromagnetic interference shielding performance of carbon nanostructure reinforced, 3D printed polymer composites

    We report the electrical, mechanical and electromagnetic interference (EMI) shielding performance of polypropylene random copolymer (PPR)/multi-wall carbon nanotube (MWCNT) nanocomposites enabled via customized fused filament fabrication process. The electro-conductive PPR/MWCNT filament feedstocks were fabricated via shear-induced melt-blending process that allows 3D printing of nanoengineered composites even at higher MWCNT loading (up to 8 wt%). The uniform dispersion of MWCNTs in PPR matrix confirmed via Raman spectroscopy and scanning electron microscopy facilitates better mechanical, electrical and EMI shielding performance. The results furthermore show enhanced shielding properties and higher attenuation for the nanocomposites printed in 90° direction (~ − 37 dB for 8 wt% MWCNT loading). Effective interfacial adhesion between the beads with lesser extent of voids (confirmed via micro-computed tomography) endorsed low transmission loss in nanocomposites printed in 90° direction compared to samples printed in 0° direction. Surface architected structure (frustum shape) reveals higher specific shielding effectiveness (maximum ~ − 40 dBg−1cm3, + 38%) over the plain structure. The realization of excellent shielding effectiveness (~ 99.9% attenuation) of additive manufacturing-enabled PPR/MWCNT nanocomposites demonstrates their potential for lightweight and strong EMI shields
