    Training intensity and shoulder musculoskeletal physical quality responses in competitive swimmers

    Context: Shoulder pain is the main cause of missed or modified training in competitive swimmers. Shoulder musculoskeletal maladaptations occur to some extent as a consequence of training loads during swimming that may increase the risk of shoulder injury. Further evidence is needed to understand the training intensities at which these maladaptations occur. Objective: To determine the acute effect of training intensity on the shoulder musculoskeletal physical qualities associated with shoulder injury in competitive swimmers. Design: Cross-sectional study. Setting: Indoor swimming pool. Patients or Other Participants: Sixteen asymptomatic national- and regional-level swimmers (7 females, 9 males; age = 14.6 ± 3.9 years, height = 160.5 ± 12.7 cm, mass = 55.3 ± 12.5 kg). Main Outcome Measure(s): Bilateral active shoulder-rotation range of motion (ROM), joint position sense, latissimus dorsi length, combined elevation test, and shoulder-rotation isometric peak torque and handgrip peak force normalized to body weight were measured before and immediately after low- and high-intensity swim-training sessions. The intensity of the sessions was determined by the distance swum over or at the pace threshold and confirmed by the swimmer's rating of perceived exertion. Results: After the high-intensity training session, shoulder external-rotation ROM (dominant side: P < .001, change = −7.8°; d = 1.10; nondominant side: P = .002, change = −6.5°, d = 1.02), internal-rotator isometric peak torque (dominant side: P < .001, change = −11.4%, d = 0.42; nondominant side: P = .03, change = −6.6%, d = 0.20), and external-rotator isometric peak torque (dominant side: P = .004, change = −8.7%, d = 0.27; nondominant side: P = .02, change = −7.6%, d = 0.25) were reduced. No changes were found in any of the outcome measures after the low-intensity session. Conclusions: Shoulder active external-rotation ROM and rotation isometric peak torque were decreased immediately after a high-intensity training session, possibly increasing the risk of injury during subsequent training. Monitoring these variables may help practitioners adjust and manage training loads to decrease the risk of shoulder injury

    Cumulative effects of a week’s training loads on shoulder physical qualities and wellness in competitive swimmers

    Background: Competitive swimmers are exposed to high training loads, which can contribute to the development of shoulder pain. There is a lack of research investigating the interactions between the accumulation of training loads and factors associated to shoulder pain in swimmers. Purpose: The primary objective was to analyze the changes in shoulder physical qualities and wellness factors over a week of training in competitive swimmers. A secondary objective was to compare the changes in these variables between different swim-training volumes performed during the week. Design: Cross-sectional. Methods: Thirty-one national and regional-level swimmers were included (18 females, 13 males; age= 15.5 ± 2.2 years). Active shoulder external rotation (ER) range of motion (ROM), shoulder-rotation isometric torque, and wellness factors using the Hooper questionnaire were measured twice over the week: a baseline measurement (before Monday´s training session) and a follow-up during the week. Participants were divided into a high-volume group (HVG) and low-volume group (LVG) based on the day follow-up was performed. HVL (n= 15) was tested at the end of the training week (after Saturday´s session) and LVG (n= 16) during the week (after Thursday or Friday´s session). Rating of perceived exertion (RPE) of the whole week was recorded after the follow-up session. Results: At follow-up, the LVG averaged a volume of 26.2 ± 2.2 km, whereas the HVG averaged a volume of 37.5 ± 3.7 km. LVG and HVG participants decreased active shoulder ER ROM on dominant (p= 0.002; p= 0.006) and nondominant sides (p= 0.001; p= 0.004), displayed increased muscular soreness (p= 0.001; p= 0.007) and worsened overall wellness (p< 0.001; p= 0.010). Fatigue (p= 0.008) and poor sleep quality were increased (p= 0.023) in HVG, but not in LVG. There were no changes in shoulder-rotation torque and stress in any group. Regarding between-groups differences, only weekly RPE was higher (p= 0.004) in HVG. Conclusions: The accumulation of training loads over the week negatively affect physical and wellness factors. Greater swim-volumes were associated with an increase perception of training loads. The regular monitoring of multiple factors to assess swimmers’ response to training might be necessary

    Power grip, pinch grip, manual muscle testing or thenar atrophy - which should be assessed as a motor outcome after carpal tunnel decompression? A systematic review

    <p>Abstract</p> <p>Background</p> <p>Objective assessment of motor function is frequently used to evaluate outcome after surgical treatment of carpal tunnel syndrome (CTS). However a range of outcome measures are used and there appears to be no consensus on which measure of motor function effectively captures change. The purpose of this systematic review was to identify the methods used to assess motor function in randomized controlled trials of surgical interventions for CTS. A secondary aim was to evaluate which instruments reflect clinical change and are psychometrically robust.</p> <p>Methods</p> <p>The bibliographic databases Medline, AMED and CINAHL were searched for randomized controlled trials of surgical interventions for CTS. Data on instruments used, methods of assessment and results of tests of motor function was extracted by two independent reviewers.</p> <p>Results</p> <p>Twenty-two studies were retrieved which included performance based assessments of motor function. Nineteen studies assessed power grip dynamometry, fourteen studies used both power and pinch grip dynamometry, eight used manual muscle testing and five assessed the presence or absence of thenar atrophy. Several studies used multiple tests of motor function. Two studies included both power and pinch strength and reported descriptive statistics enabling calculation of effect sizes to compare the relative responsiveness of grip and pinch strength within study samples. The study findings suggest that tip pinch is more responsive than lateral pinch or power grip up to 12 weeks following surgery for CTS.</p> <p>Conclusion</p> <p>Although used most frequently and known to be reliable, power and key pinch dynamometry are not the most valid or responsive tools for assessing motor outcome up to 12 weeks following surgery for CTS. Tip pinch dynamometry more specifically targets the thenar musculature and appears to be more responsive. Manual muscle testing, which in theory is most specific to the thenar musculature, may be more sensitive if assessed using a hand held dynamometer – the Rotterdam Intrinsic Handheld Myometer. However further research is needed to evaluate its reliability and responsiveness and establish the most efficient and psychometrically robust method of evaluating motor function following surgery for CTS.</p

    Gluonic and leptonic decays of heavy quarkonia and the determination of αs(mc)\alpha_s(m_c) and αs(mb)\alpha_s(m_b)

    QCD running coupling constant αs(mc)\alpha_s(m_c) and αs(mb)\alpha_s(m_b) are determined from heavy quarkonia cc‾c\overline{c} and bb‾b\overline{b} decays. The decay rates of V→3gV\rightarrow 3g and V→e+e−V\rightarrow e^+ e^- for V=J/ψV=J/\psi and Υ\Upsilon are estimated by taking into account both relativistic and QCD radiative corrections. The decay amplitudes are derived in the Bethe-Salpeter formalism, and the decay rates are estimated by using the meson wavefunctions which are obtained with a QCD-inspired inter-quark potential. For the V→3gV\rightarrow 3g decay we find the relativistic correction to be very large and to severely suppress the decay rate. Using the experimental values of ratio R_g\equiv \frac {\Gamma (V\longrightarrow 3g)}% {\Gamma (V\longrightarrow e^{+}e^{-})}\approx 10,~32 for V=J/ψ, ΥV=J/\psi, ~\Upsilon respectively, and the calculated widths , we find αs(mc)=0.29±0.02\alpha_{s}(m_c)=0.29\pm 0.02 and αs(mb)=0.20±0.02\alpha_s(m_b)=0.20\pm 0.02. These values for the QCD running coupling constant are substantially enhanced, as compared with the ones obtained without relativistic corrections, and are consistent with the QCD scale parameter ΛMS‾(4)\Lambda_{\overline {MS}}^{(4)}% \approx 200MeV.     Mutation at the Evi1 locus in Junbo mice causes susceptibility to otitis media

    Otitis media ( OM), inflammation of the middle ear, remains the most common cause of hearing impairment in children. It is also the most common cause of surgery in children in the developed world. There is evidence from studies of the human population and mouse models that there is a significant genetic component predisposing to OM, yet nothing is known about the underlying genetic pathways involved in humans. We identified an N-ethyl-N-nitrosourea-induced dominant mouse mutant Junbo with hearing loss due to chronic suppurative OM and otorrhea. This develops from acute OM that arises spontaneously in the postnatal period, with the age of onset and early severity dependent on the microbiological status of the mice and their air quality. We have identified the causal mutation, a missense change in the C-terminal zinc finger region of the transcription factor Evi1. This protein is expressed in middle ear basal epithelial cells, fibroblasts, and neutrophil leukocytes at postnatal day 13 and 21 when inflammatory changes are underway. The identification and characterization of the Junbo mutant elaborates a novel role for Evi1 in mammalian disease and implicates a new pathway in genetic predisposition to OM

    Targeted prevention of common mental health disorders in university students: randomised controlled trial of a transdiagnostic trait-focused web-based intervention

    Background: A large proportion of university students show symptoms of common mental disorders, such as depression, anxiety, substance use disorders and eating disorders. Novel interventions are required that target underlying factors of multiple disorders.&lt;p&gt;&lt;/p&gt; Aims: To evaluate the efficacy of a transdiagnostic trait-focused web-based intervention aimed at reducing symptoms of common mental disorders in university students.&lt;p&gt;&lt;/p&gt; Method: Students were recruited online (n = 1047, age: M = 21.8, SD = 4.2) and categorised into being at high or low risk for mental disorders based on their personality traits. Participants were allocated to a cognitive-behavioural trait-focused (n = 519) or a control intervention (n = 528) using computerised simple randomisation. Both interventions were fully automated and delivered online (trial registration: ISRCTN14342225). Participants were blinded and outcomes were self-assessed at baseline, at 6 weeks and at 12 weeks after registration. Primary outcomes were current depression and anxiety, assessed on the Patient Health Questionnaire (PHQ9) and Generalised Anxiety Disorder Scale (GAD7). Secondary outcome measures focused on alcohol use, disordered eating, and other outcomes.&lt;p&gt;&lt;/p&gt; Results: Students at high risk were successfully identified using personality indicators and reported poorer mental health. A total of 520 students completed the 6-week follow-up and 401 students completed the 12-week follow-up. Attrition was high across intervention groups, but comparable to other web-based interventions. Mixed effects analyses revealed that at 12-week follow up the trait-focused intervention reduced depression scores by 3.58 (p&#60;.001, 95%CI [5.19, 1.98]) and anxiety scores by 2.87 (p = .018, 95%CI [1.31, 4.43]) in students at high risk. In high-risk students, between group effect sizes were 0.58 (depression) and 0.42 (anxiety). In addition, self-esteem was improved. No changes were observed regarding the use of alcohol or disordered eating.&lt;p&gt;&lt;/p&gt; Conclusions This study suggests that a transdiagnostic web-based intervention for university students targeting underlying personality risk factors may be a promising way of preventing common mental disorders with a low-intensity intervention

    Pseuduscalar Heavy Quarkonium Decays With Both Relativistic and QCD Radiative Corrections

    We estimate the decay rates of ηc→2γ\eta_c\rightarrow 2\gamma, ηc′→2γ\eta_c'\rightarrow 2\gamma, and J/ψ→e+e−J/\psi\rightarrow e^+ e^-, ψ′→e+e−\psi^\prime\rightarrow e^+e^-, by taking into account both relativistic and QCD radiative corrections. The decay amplitudes are derived in the Bethe-Salpeter formalism. The Bethe-Salpeter equation with a QCD-inspired interquark potential are used to calculate the wave functions and decay widths for these ccˉc\bar{c} states. We find that the relativistic correction to the ratio R≡Γ(ηc→2γ)/Γ(J/ψ→e+e−)R\equiv \Gamma (\eta_c \rightarrow 2\gamma)/ \Gamma (J/ \psi \rightarrow e^+ e^-) is negative and tends to compensate the positive contribution from the QCD radiative correction. Our estimate gives Γ(ηc→2γ)=(6−7) keV\Gamma(\eta_c \rightarrow 2\gamma)=(6-7) ~keV and Γ(ηc′→2γ)=2 keV\Gamma(\eta_c^\prime \rightarrow 2\gamma)=2 ~keV, which are smaller than their nonrelativistic values. The hadronic widths Γ(ηc→2g)=(17−23) MeV\Gamma(\eta_c \rightarrow 2g)=(17-23) ~MeV and Γ(ηc′→2g)=(5−7) MeV\Gamma(\eta_c^\prime \rightarrow 2g)=(5-7)~MeV are then indicated accordingly to the first order QCD radiative correction, if αs(mc)=0.26−0.29\alpha_s(m_c)=0.26-0.29. The decay widths for bbˉb\bar b states are also estimated. We show that when making the assmption that the quarks are on their mass shells our expressions for the decay widths will become identical with that in the NRQCD theory to the next to leading order of v2v^2 and αs\alpha_s.Comment: 14 pages LaTex (2 figures included

    Oxyanion doping strategies to enhance the ionic conductivity in Ba2In2O5

    In this paper we report the successful incorporation of phosphate and sulphate groups into the ionic conductor, Ba2In2O5, with the samples analysed through a combination of X-ray diffraction, NMR, TGA, Raman spectroscopy and conductivity measurements. The results show that such oxyanion incorporation leads to a conversion from an ordered brownmillerite-type structure to a disordered perovskite-type, and hence increases the conductivity at temperatures < 800â—‹C. In wet atmospheres, there is evidence for a significant enhancement of the conductivity through a protonic contribution.\u

    ARPES: A probe of electronic correlations

    Angle-resolved photoemission spectroscopy (ARPES) is one of the most direct methods of studying the electronic structure of solids. By measuring the kinetic energy and angular distribution of the electrons photoemitted from a sample illuminated with sufficiently high-energy radiation, one can gain information on both the energy and momentum of the electrons propagating inside a material. This is of vital importance in elucidating the connection between electronic, magnetic, and chemical structure of solids, in particular for those complex systems which cannot be appropriately described within the independent-particle picture. Among the various classes of complex systems, of great interest are the transition metal oxides, which have been at the center stage in condensed matter physics for the last four decades. Following a general introduction to the topic, we will lay the theoretical basis needed to understand the pivotal role of ARPES in the study of such systems. After a brief overview on the state-of-the-art capabilities of the technique, we will review some of the most interesting and relevant case studies of the novel physics revealed by ARPES in 3d-, 4d- and 5d-based oxides.Comment: Chapter to appear in "Strongly Correlated Systems: Experimental Techniques", edited by A.
