    Consensus guidelines for digital scholarship in academic promotion

See: http://creativecommons.org/licenses/by/4.0/ Introduction: As scholarship moves into the digital sphere, applicant and promotion and tenure (P&T) committee members lack formal guidance on evaluating the impact of digital scholarly work. The P&T process requires the appraisal of individual scholarly impact in comparison to scholars across institutions and disciplines. As dissemination methods evolve in the digital era, we must adapt traditional P&T processes to include emerging forms of digital scholarship. Methods: We conducted a blended, expert consensus procedure using a nominal group process to create a consensus document at the Council of Emergency Medicine Residency Directors Academic Assembly on April 1, 2019. Results: We discussed consensus guidelines for evaluation and promotion of digital scholarship with the intent to develop specific, evidence-supported recommendations to P&T committees and applicants. These recommendations included the following: demonstrate scholarship criteria; provide external evidence of impact; and include digital peer-review roles. As traditional scholarship continues to evolve within the digital realm, academic medicine should adapt how that scholarship is evaluated. P&T committees in academic medicine are at the epicenter for supporting this changing paradigm in scholarship. Conclusion: P&T committees can critically appraise the quality and impact of digital scholarship using specific, validated tools. Applicants for appointment and promotion should highlight and prepare their digital scholarship to specifically address quality, impact, breadth, and relevance. It is our goal to provide specific, timely guidance for both stakeholders to recognize the value of digital scholarship in advancing our field. [West J Emerg Med. 2020;21(4)882-890.]

    Regularizing cosmological singularities by varying physical constants

    Varying physical constant cosmologies were claimed to solve standard cosmological problems such as the horizon, the flatness and the Λ\Lambda-problem. In this paper, we suggest yet another possible application of these theories: solving the singularity problem.     A roadmap to develop dementia research capacity and capability in Pakistan: a model for low- and middle-income countries

    Objective To produce a strategic roadmap for supporting the development of dementia research in Pakistan. Background While global research strategies for dementia research already exist, none is tailored to the specific needs and challenges of low- and middle-income countries (LMIC) like Pakistan. Methods We undertook an iterative consensus process with lay and professional experts to develop a Theory of Change-based strategy for dementia research in Pakistan. This included Expert Reference Groups (ERGs), strategic planning techniques, a “research question” priority survey, and consultations with Key Opinion Leaders. Results We agreed on ten principles to guide dementia research in Pakistan, emphasizing pragmatic, resource sparing, real-world approaches to support people with dementia, both locally and internationally. Goals included capacity/capability building. Priority research topics included raising awareness and understanding of dementia, and improving quality of life. Conclusion This roadmap may be a model for other LMIC health ecosystems with emerging dementia research cultures

    The lack of effective therapeutic agents specifically tailored for chronic pancreatitis (CP) has hampered clinical care and negatively impacted patients' lives. New mechanistic insights now point to novel therapies, which involve both recently developed and/or repurposed agents. This working group focused on 2 main outcomes for CP: pain and progression of disease. The goal is to frame the essential aspects of trial design including patient-centered outcomes, proposed methods to measure the outcomes of pain and progression, and study design considerations for future trials to facilitate rapid drug development for patients with CP

    Src Dependent Pancreatic Acinar Injury Can Be Initiated Independent of an Increase in Cytosolic Calcium

    Several deleterious intra-acinar phenomena are simultaneously triggered on initiating acute pancreatitis. These culminate in acinar injury or inflammatory mediator generation in vitro and parenchymal damage in vivo. Supraphysiologic caerulein is one such initiator which simultaneously activates numerous signaling pathways including non-receptor tyrosine kinases such as of the Src family. It also causes a sustained increase in cytosolic calcium- a player thought to be crucial in regulating deleterious phenomena. We have shown Src to be involved in caerulein induced actin remodeling, and caerulein induced changes in the Golgi and post-Golgi trafficking to be involved in trypsinogen activation, which initiates acinar cell injury. However, it remains unclear whether an increase in cytosolic calcium is necessary to initiate acinar injury or if injury can be initiated at basal cytosolic calcium levels by an alternate pathway. To study the interplay between tyrosine kinase signaling and calcium, we treated mouse pancreatic acinar cells with the tyrosine phosphatase inhibitor pervanadate. We studied the effect of the clinically used Src inhibitor Dasatinib (BMS-354825) on pervanadate or caerulein induced changes in Src activation, trypsinogen activation, cell injury, upstream cytosolic calcium, actin and Golgi morphology. Pervanadate, like supraphysiologic caerulein, induced Src activation, redistribution of the F-actin from its normal location in the sub-apical area to the basolateral areas, and caused antegrade fragmentation of the Golgi. These changes, like those induced by supraphysiologic caerulein, were associated with trypsinogen activation and acinar injury, all of which were prevented by Dasatinib. Interestingly, however, pervanadate did not cause an increase in cytosolic calcium, and the caerulein induced increase in cytosolic calcium was not affected by Dasatinib. These findings suggest that intra-acinar deleterious phenomena may be initiated independent of an increase in cytosolic calcium. Other players resulting in acinar injury along with the Src family of tyrosine kinases remain to be explored. © 2013 Mishra et al

    Gravitational collapse of massless scalar field and radiation fluid

    Several classes of conformally-flat and spherically symmetric exact solutions to the Einstein field equations coupled with either a massless scalar field or a radiation fluid are given, and their main properties are studied. It is found that some represent the formation of black holes due to the gravitational collapse of the matter fields. When the spacetimes have continuous self-similarity (CSS), the masses of black holes take a scaling form MBH(PP)γM_{BH} \propto (P - P^{*})^{\gamma}, where γ=0.5\gamma = 0.5 for massless scalar field and γ=1\gamma = 1 for radiation fluid. The reasons for the difference between the values of γ\gamma obtained here and those obtained previously are discussed. When the spacetimes have neither CSS nor DSS (Discrete self-similarity), the masses of black holes always turn on with finite non-zero values.Comment: Two figures have been removed, and the text has been re-written.     Loop Quantum Cosmology: Recent Progress

    Aspects of the full theory of loop quantum gravity can be studied in a simpler context by reducing to symmetric models like cosmological ones. This leads to several applications where loop effects play a significant role when one is sensitive to the quantum regime. As a consequence, the structure of and the approach to classical singularities are very different from general relativity: The quantum theory is free of singularities, and there are new phenomenological scenarios for the evolution of the very early universe including inflation.     Dimensional Dependence of Black Hole Formation in Self-Similar Collapse of Scalar Field

    We study classical and quantum self-similar collapses of a massless scalar field in higher dimensions, and examine how the increase in the number of dimensions affects gravitational collapse and black hole formation. Higher dimensions seem to favor formation of black hole rather than other final states, in that the initial data space for black hole formation enlarges as dimension increases. On the other hand, the quantum gravity effect on the collapse lessens as dimension increases.     Energy and directional signatures for plane quantized gravity waves

    Solutions are constructed to the quantum constraints for planar gravity (fields dependent on z and t only) in the Ashtekar complex connection formalism. A number of operators are constructed and applied to the solutions. These include the familiar ADM energy and area operators, as well as new operators sensitive to directionality (z+ct vs. z-ct dependence). The directionality operators are quantum analogs of the classical constraints proposed for unidirectional plane waves by Bondi, Pirani, and Robinson (BPR). It is argued that the quantum BPR constraints will predict unidirectionality reliably only for solutions which are semiclassical in a certain sense. The ADM energy and area operators are likely to have imaginary eigenvalues, unless one either shifts to a real connection, or allows the connection to occur other than in a holonomy. In classical theory, the area can evolve to zero. A quantum mechanical mechanism is proposed which would prevent this collapse.Comment: 54 pages; LaTe

    Association of various reproductive rights, domestic violence and marital rape with depression among Pakistani women

    <p>Abstract</p> <p>Background</p> <p>Depression among women is common in developing countries. Gender inequality can contribute to women's risk for depression. Lack of reproductive and sexual rights is an important marker of gender inequality and women do not have the freedom to express their reproductive and sexual needs in many parts of the world. Therefore we designed this study to determine the association of depression with lack of various reproductive rights and domestic violence among married women in Karachi, Pakistan.</p> <p>Methods</p> <p>A case-control study with 152 cases and 152 controls, which included women 15-48 years, recruited from two teaching hospitals from 1<sup>st </sup>June 2007 through 31<sup>st </sup>August 2007. The SRQ was administered to all subjects. A cut off score of 8 was used to confirm cases of depression diagnosed by physicians, and to exclude cases of depression from the controls. Self-administered questionnaire was used to assess the risk factors.</p> <p>Results</p> <p>61% of the cases and 43% of the controls were ever abused by spouse and the frequency of marital rape was 33% in cases and 13% in controls. After adjusting for the effects of other variables in the model, less than 18 years of age at marriage (OR 2.00; 95% CI = 1.07, 3.7), decision for marriage by parents (OR 3.51; 95% CI = 1.67, 7.37), abuse by in laws (OR 4.91; 95% CI = 2.66, 9.06), ≤ 3 hours per day spent with husband (OR 2.33; 95% CI = 1.34, 4.08), frequency of intercourse ≤ 2 times per week (OR 1.85; 95% CI = 1.06, 3.22) and marital rape (OR 3.03; 95% CI = 1.50, 6.11) were associated with depression among women.</p> <p>Conclusion</p> <p>In our study depression in married women was associated with younger age at marriage, lack of autonomy in marriage decisions, marital rape and domestic abuse by in-laws. Efforts should be directed towards creating awareness about the reproductive and sexual rights of women in Pakistan. Physicians should be trained to screen and identify women who may be at risk for psychological distress as a result of denial of reproductive rights so that they can support positive mental health outcomes through individual, family or marital counseling.</p
