    Availability, scope and quality of monkeypox clinical management guidelines globally : a systematic review

There has been an increase in MPX cases and outbreaks in endemic and non-endemic regions in recent decades. We appraised the availability, scope, quality and inclusivity of clinical management guidelines for MPX globally. Methods For this systematic review, we searched six databases from inception until 14 October 2021, augmented by a grey literature search until 17 May 2022. MPX guidelines providing treatment and supportive care recommendations were included, with no exclusions for language. Two reviewers assessed the guidelines. Quality was assessed using the Appraisal of Guidelines for Research and Evaluation II tool. Results Of 2026 records screened, 14 guidelines were included. Overall, most guidelines were of low-quality with a median score of 2 out of 7 (range: 1–7), lacked detail and covered a narrow range of topics. Most guidelines focused on adults, five (36%) provided some advice for children, three (21%) for pregnant women and three (21%) for people living with HIV. Treatment guidance was mostly limited to advice on antivirals; seven guidelines advised cidofovir (four specified for severe MPX only); 29% (4/14) tecovirimat, and 7% (1/14) brincidofovir. Only one guideline provided recommendations on supportive care and treatment of complications. All guidelines recommended vaccination as post-exposure prophylaxis (PEP). Three guidelines advised on vaccinia immune globulin as PEP for severe cases in people with immunosuppression. Conclusion Our results highlight a lack of evidence-based clinical management guidelines for MPX globally. There is a clear and urgent need for research into treatment and prophylaxis including for different risk populations. The current outbreak provides an opportunity to accelerate this research through coordinated high-quality studies. New evidence should be incorporated into globally accessible guidelines, to benefit patient and epidemic outcomes. A ‘living guideline’ framework is recommended.     Medical students' and facilitators' experiences of an Early Professional Contact course: Active and motivated students, strained facilitators

    Background: Today, medical students are introduced to patient contact, communication skills, and clinical examination in the preclinical years of the curriculum with the purpose of gaining clinical experience. These courses are often evaluated from the student perspective. Reports with an additional emphasis on the facilitator perspective are scarce. According to constructive alignment, an influential concept from research in higher education, the learning climate between students and teachers is also of great importance. In this paper, we approach the learning climate by studying both students' and facilitators' course experiences.\ud \ud In 2001, a new "Early Professional Contact" longitudinal strand through term 1–4, was introduced at the Sahlgrenska Academy, University of Gothenburg, Sweden. General practitioners and hospital specialists were facilitators.\ud \ud The aim of this study was to assess and analyse students' and clinical facilitators' experiences of the Early Professional Contact course and to illuminate facilitators' working conditions.\ud \ud Methods: Inspired by a Swedish adaptation of the Course Experience Questionnaire, an Early Professional Contact Questionnaire was constructed. In 2003, on the completion of the first longitudinal strand, a student and facilitator version was distributed to 86 students and 21 facilitators. In the analysis, both Chi-square and the Mann-Whitney tests were used.\ud \ud Results: Sixty students (70%) and 15 facilitators (71%) completed the questionnaire. Both students and facilitators were satisfied with the course. Students reported gaining [inspiration] for their future work as doctors along with increased confidence in meeting patients. They also reported increased motivation for biomedical studies. Differences in attitudes between facilitators and students were found. Facilitators experienced a greater workload, less reasonable demands and less support, than students.\ud \ud Conclusion: In this project, a new Early Professional Contact course was analysed from both student and facilitator perspectives. The students experienced the course as providing them with a valuable introduction to the physician's professional role in clinical practice. In contrast, course facilitators often experienced a heavy workload and lack of support, despite thorough preparatory education. A possible conflict between the clinical facilitator's task as educator and member of the workplace is suggested. More research is needed on how doctors combine their professional tasks with work as facilitators

    All We Need is Love? Trade-Adjustment, Inequality, and the Role of the Partner

    We examine the impact of Germany’s trade integration with China and Eastern Europe from 1993 through 2008 on earnings inequality between households. We especially focus our attention on the role of assortative mating for the magnitude of this impact. Our analysis demonstrates that the trade shock worked towards an increase of earnings inequality, both in the short and in the medium run. Moreover, we illustrate that mating has become increasingly assortative in Germany, especially among low-skilled workers. The results of a counterfactual experiment reveal that this surge in the assortativeness of mating might deepen the negative impact of future trade shocks on earnings inequality between households and might consequently also reduce potential gains from trade

    Impact of Sentence length on the Readability of Web for Screen Reader Users

    Readability of text is generally believed to be connected to sentence length. Most studies on readability are based on visual reading. Less is known about text readability for users relying on screen readers, such as users who are blind. This study therefore set out to investigate the effect of sentence length on the readability of web texts accessed using screen readers. A controlled within-subjects experiment was performed with twenty-one participants. Participants used a screen reader to read five texts with different sentence lengths. The par-ticipants’ comprehension and perceived workload were measured. The findings reveal that there is a significant effect of sentence length and most participants exhibit the highest comprehension and lowest workload with sentences compris-ing 16-20 words. Implications of these results are that web content providers should strive for sentence length of 16-20 words to maximize readability

    In low-GDP countries, such as Brazil, many older people and people with disabilities rely on the public health system for access to assistive devices, which are important to support them to live healthy and dignified lives. However, many problems limit their access to specialized prescriptions and appropriate devices that satisfy their needs and expectations. This paper explores the challenges, contributions and results from an interdisciplinary collaboration between two different yet related areas of knowledge – Product Design and Rehabilitation – in education, research and innovation in assistive technologies. The combination of product design education and rehabilitation services has mutual benefits from the perspective of research and innovation, thus contributing for the community.     Reference data on hand grip and lower limb strength using the Nintendo Wii balance board: a cross-sectional study of 354 subjects from 20 to 99 years of age

    Abstract Background Accurate assessment of isometric hand grip strength (HGS) and isometric lower limb strength (LS) are often limited to specialized clinics due to high costs and need for specialized equipment and personnel. A mobile and user-friendly device would facilitate a wider use of these measures in the clinical setting. The Nintendo Wii Balance Board (WBB) is a novel and pragmatic tool that has been validated for measuring muscle strength and other clinically relevant physiological variables. However, reference data for HGS and LS are lacking. The purpose of the current study is to establish reference data for HGS and LS in individuals ≄20 years of age using the WBB method, and to characterize the effects of age in these measurements. Method Healthy participants were recruited at various locations and their HGS and LS were tested by six assessors using the WBB. Reference data were analysed and presented in age-groups, while the age-related change in HGS and LS was tested and characterized with linear regression models. Results Three hundred and fifty-four participants between 20 and 99 years of age were tested. Data are presented separately according to gender and the following age categories: 20–29, 30–39, 40–49, 50–59, 60–69, 70–79, and 80+, and presented in absolute values as well as percentiles. The main findings were; (1) Significantly higher HGS and LS among males compared to females and for the dominant limb compared to the non-dominant limb, (2) a significant decline in strength with increasing age, and (3) the rate of decline increased significantly (i.e. it was non-linear) with age for HGS, but not for LS. Conclusion This study reported reference data with percentiles for a novel method for assessing HGS and LS. Data were consistent with previously known effects of age and gender on HGS and LS. The presented data may supplement future trials using the WBB in research or in the clinical setting
