    Care: a challenge to healthy organizations? A case study of a hospital department

    In this article we study a healthy organization through exploring the dual agenda from the perspective of a human service organization. Looking at the case of a specialized department at a major teaching hospital in Sweden, we analyze whether the concept of the healthy organization needs an added dimension. Quantitative and qualitative methods have been used, collected within the framework of the EU project Quality of Life in a Changing Europe.  We explore the dual agenda from two perspectives — the work-household duality and workforce-organization duality. Using a multi-method approach shows a complex picture, especially with regard to stress and workload intensity.Neste artigo estudamos uma organização saudável através da exploração da agenda dual, na perspetiva de uma organização de serviços a pessoas.     The new Felsenkeller 5 MV underground accelerator

    The field of nuclear astrophysics is devoted to the study of the creation of the chemical elements. By nature, it is deeply intertwined with the physics of the Sun. The nuclear reactions of the proton-proton cycle of hydrogen burning, including the 3He({\alpha},{\gamma})7Be reaction, provide the necessary nuclear energy to prevent the gravitational collapse of the Sun and give rise to the by now well-studied pp, 7Be, and 8B solar neutrinos. The not yet measured flux of 13N, 15O, and 17F neutrinos from the carbon-nitrogen-oxygen cycle is affected in rate by the 14N(p,{\gamma})15O reaction and in emission profile by the 12C(p,{\gamma})13N reaction. The nucleosynthetic output of the subsequent phase in stellar evolution, helium burning, is controlled by the 12C({\alpha},{\gamma})16O reaction. In order to properly interpret the existing and upcoming solar neutrino data, precise nuclear physics information is needed. For nuclear reactions between light, stable nuclei, the best available technique are experiments with small ion accelerators in underground, low-background settings. The pioneering work in this regard has been done by the LUNA collaboration at Gran Sasso/Italy, using a 0.4 MV accelerator. The present contribution reports on a higher-energy, 5.0 MV, underground accelerator in the Felsenkeller underground site in Dresden/Germany. Results from {\gamma}-ray, neutron, and muon background measurements in the Felsenkeller underground site in Dresden, Germany, show that the background conditions are satisfactory for nuclear astrophysics purposes.     Racial differences in systemic sclerosis disease presentation: a European Scleroderma Trials and Research group study

    Objectives. Racial factors play a significant role in SSc. We evaluated differences in SSc presentations between white patients (WP), Asian patients (AP) and black patients (BP) and analysed the effects of geographical locations.Methods. SSc characteristics of patients from the EUSTAR cohort were cross-sectionally compared across racial groups using survival and multiple logistic regression analyses.Results. The study included 9162 WP, 341 AP and 181 BP. AP developed the first non-RP feature faster than WP but slower than BP. AP were less frequently anti-centromere (ACA; odds ratio (OR) = 0.4, P < 0.001) and more frequently anti-topoisomerase-I autoantibodies (ATA) positive (OR = 1.2, P = 0.068), while BP were less likely to be ACA and ATA positive than were WP [OR(ACA) = 0.3, P < 0.001; OR(ATA) = 0.5, P = 0.020]. AP had less often (OR = 0.7, P = 0.06) and BP more often (OR = 2.7, P < 0.001) diffuse skin involvement than had WP.AP and BP were more likely to have pulmonary hypertension [OR(AP) = 2.6, P < 0.001; OR(BP) = 2.7, P = 0.03 vs WP] and a reduced forced vital capacity [OR(AP) = 2.5, P < 0.001; OR(BP) = 2.4, P < 0.004] than were WP. AP more often had an impaired diffusing capacity of the lung than had BP and WP [OR(AP vs BP) = 1.9, P = 0.038; OR(AP vs WP) = 2.4, P < 0.001]. After RP onset, AP and BP had a higher hazard to die than had WP [hazard ratio (HR) (AP) = 1.6, P = 0.011; HR(BP) = 2.1, P < 0.001].Conclusion. Compared with WP, and mostly independent of geographical location, AP have a faster and earlier disease onset with high prevalences of ATA, pulmonary hypertension and forced vital capacity impairment and higher mortality. BP had the fastest disease onset, a high prevalence of diffuse skin involvement and nominally the highest mortality

    Ambient-noise tomography of the wider Vienna Basin region

    We present a new 3-D shear-velocity model for the top 30 km of the crust in the wider Vienna Basin region based on surface waves extracted from ambient-noise cross-correlations. We use continuous seismic records of 63 broad-band stations of the AlpArray project to retrieve interstation Green’s functions from ambient-noise cross-correlations in the period range from 5 to 25 s. From these Green’s functions, we measure Rayleigh group traveltimes, utilizing all four components of the cross-correlation tensor, which are associated with Rayleigh waves (ZZ, RR, RZ and ZR), to exploit multiple measurements per station pair. A set of selection criteria is applied to ensure that we use high-quality recordings of fundamental Rayleigh modes. We regionalize the interstation group velocities in a 5 km × 5 km grid with an average path density of ∼20 paths per cell. From the resulting group-velocity maps, we extract local 1-D dispersion curves for each cell and invert all cells independently to retrieve the crustal shear-velocity structure of the study area. The resulting model provides a previously unachieved lateral resolution of seismic velocities in the region of ∼15 km. As major features, we image the Vienna Basin and Little Hungarian Plain as low-velocity anomalies, and the Bohemian Massif with high velocities. The edges of these features are marked with prominent velocity contrasts correlated with faults, such as the Alpine Front and Vienna Basin transfer fault system. The observed structures correlate well with surface geology, gravitational anomalies and the few known crystalline basement depths from boreholes. For depths larger than those reached by boreholes, the new model allows new insight into the complex structure of the Vienna Basin and surrounding areas, including deep low-velocity zones, which we image with previously unachieved detail. This model may be used in the future to interpret the deeper structures and tectonic evolution of the wider Vienna Basin region, evaluate natural resources, model wave propagation and improve earthquake locations, among others

    The Effect of Organizational Factors on Long-term Sick Leave in Swedish Municipalities The aim with this study was to explain the variation in long-term sick leave within the public sector among workers in different municipalities. The findings show a strong correlation between anxiety for reorganization at the workplace and the level of long-term sick leave. In addition to organizational anxiety, multivariate analysis shows that there is significantly more long-term sick leave in municipalities with greater problems of sick leave already in the mid-1990s. However, there are still few leads about what kinds of organizational changes that at least indirectly and independently from previous problems with sick leave in the municipality trigger local variation in long-term sick leave within the public sector domain.     Reorganization and Sick Leave : The Ombudsman s Report on Organizational Change at Local Government Workplaces The aim was to analyze why certain reorganizations at public sector workplaces seem to generate both organizational anxiety and long-term sick leave. The study was based on a survey with employees conducted 2000 and a new survey directed to the ombudsman at 88 of these workplaces in 2003. The findings showed strong correlations between organizational anxiety in 2000, and the degree of long-term sick leave in 2003. Apart form the effect of organizational anxiety, multivariate analysis showed that both the number of reorganizations during the previous five year period and how the process of reorganization was handled by the workplace management had great separate impact on the reported illness at the workplace. The model of the reorganization was of less importance in this regard. The most significant effects instead occurred when several reorganizations were carried out at the workplace or when the process was characterized by indistinct and uncompromising workplace management. Resultaten uppvisar ett starkt samband mellan organisatorisk oro år 2000 och långtidssjukskrivning/förtidspensionering 2003. Även om personaldemokratiska inslag tenderade att reducera den organisatoriska oron, hade omorganisationens syfte ringa betydelse för ohälsan. De allra viktigaste orsakerna utgjordes i stället av organisatorisk utmattning orsakad av att flera omorganisationer utförts i rad, och processer som kännetecknas av arbetsledningens kompromisslöshet, otydlighet eller pseudodemokratiska inslag.
