    Financial monitoring of the port industry companies on the basis of risk-oriented approach

    The paper determines that the need in financial monitoring of Ukrainian companies of the port sector is caused by financial consequences related to the need to protect the environment, the existence of a dominant part of payments in foreign currency (foreign exchange risk associated with currency fluctuations), the possibility of cash outflows into the shadow sector of the economy and the possibility to use transport for smuggling. In addition, in the recent years there is a tendency of diminished financial stability of the seaports of Ukraine, which is a signal to determine the factors that have an impact on this situation, the consequences of risky operations and the optimal structure of sources for the formation of assets. An important aspect of financial monitoring is the necessity to identify the data about the objecton the basis of insignificant and relevant information, which makes it possible to avoid non-essential features and parameters and to speed up the decision making process at the lowest cost of expenditures and time. The study offers a methodical approach to financial monitoring of the port industry companies on the basis of the risk-based approach that takes into account the conditions for the functioning of port companies and their business characteristics, making it possible to implement the appropriate measures to prevent and avoid risky financial transactions and, consequently, to increase/preserve the competitiveness of the port sector companies. Regarding the need in further studies of the practical realization of financial monitoring of the port sector companies, there is a choice and substantiation of aggregate indicators of the financial monitoring system, their listing with a view to specific features, types and scale of activities

    Risk Assessment of Water Transport Enterprises by Modeling Direct and Indirect Threats

    Introduction. The methods of traffic modeling by water transport and assessing the risks associated with it is needed to identify the issues of the past period, proposing methods for assessing not only direct but also indirect risks to form the preconditions for preventing them in the postwar reconstruction. The coordination of different transport type’s actions of transport requires an assessment of risks impact of the previous stages of mixed transportation on the formation of the following risks’ stages. Existing methods of assessing such impact need to be improved. Aim and tasks. The aim of this study is creation of methodological approach to risk management in water transport based on a mathematical model for assessing the impact of both direct and indirect risks. The tasks are: to prove that the additive approach of taking risks into account leads to the systematic deviation appearance from the result; take into account the impact on the risk of cargo transportation. Results. It has proved that the calculation of risk as an additive function leads to a systematic deviation from the relevant result. It stated that the risk of each of the next stages of transportation depends on the risks of the previous stages. To increase risk analysis relevance in water transport, the use of an oriented graph in a multidimensional parameter space proposed. It stated that in order to calculate the integrated risk, it is essential building not only the risk matrix but also the risk incidence matrix to take into account their relation to business entities. It established the impact of even minor risks could take the form of a catastrophe, which leads to cargo flows reorientation. It established that: for calculation of integral risk, it is crucial consider direct and indirect influences of risks; risk calculation for water transport also requires risk analysis in related modes of transport. Conclusions. It was established that, when calculating integral risk, it is necessary to consider direct and indirect influences on risks and that the risk calculation for water transport also requires risk analysis in related modes of transport. The proposed approach significantly increases the relevance of water transport risk analysis and allows for managing changes in transportation routes in real time

    Investment attractiveness of the port industry in crisis conditions

    Historically, the maritime industry in Ukraine, which previously was subordinated to the Ministry of Transport of Ukraine and whose successor is the Ministry of Infrastructure of Ukraine, was complex of different organizations (shipping companies, ports, ship-repairing and shipbuilding factories and other related and auxiliary enterprises). They had their own organizational structures, goals and tasks and in some way interacted with each other. Such organization was aimed, first of all, at achieving target production indicators formed by the state authorities. In fact, such structure of maritime industry can be regarded as vertically integrated. When the issue of increasing the economic efficiency of the main component of the maritime industry (of the ports) was raised, the main efforts were aimed at minimizing the costs of their functioning. This led to the creation of horizontally integrated (in whole or in part) companies in the maritime industry. The driving factor for their implementation should be the transformation of organizational and economic mechanisms of regulation of the port’s activity from the point of view of a clearly defined anti-crisis character. Therefore, the goal of the study was to develop and justify the feasibility of using a model that will determine the effectiveness of the strategy of investment into the development of the organizational and economic mechanism of regulation of the port activity. The calculations of determining the best variant of an investment project under different external conditions are presented. In calculations based on certain criteria (appropriate levels of capitalization and effective dividend policies), we took into account the factors related to the expectations of the crisis in Ukraine, Ukraine’s unpreparedness to enter the new technological phase, the loss of scientific and technical personnel, inflationary phenomena and instability of the national and world currencies. In addition, within the framework of the developed model, the authors propose an algorithm that makes it possible to select the characteristics and determine the parameters of optimization of the organizational and economic mechanism of regulation of the port activity under the conditions of long-term continuation of the crisis situation

    A rare case of a severe course of systemic onset of juvenile idiopathic arthritis associated with MEFV gene mutations in a 12-year-old girl

    Juvenile idiopathic arthritis (JIA) is a common rheumatic disease in children and adolescents. MEFV (Mediterranean fever, FMF) gene mutations are observed in systemic-onset JIA, that in addition to increasing the risk of JIA development, worsen the disease prognosis. We reported a rare case of a severe systemic-onset JIA associated with MEFV gene mutations in a 12-year-old girl. The patient had an aggressive disease course and resistance to conventional immunosuppressive agents. This case confirms the difficulties of diagnostic and treatment of systemic JIA (sJIA) associated with FMF. Currently, there are no established criteria for the definition or differential diagnosis of arthritis associated with FMF. The severe prognosis of JIA associated with FMF should motivate clinicians to initiate aggressive therapy early

    A case report of bullous emphysema in an adolescent: congenitalmalformation or outcome of bronchopulmonary dysplasia?

    Pulmonary emphysema belongs to the group of chronic obstructive pulmonary diseases, and inpediatric pulmonology is one of the complex diagnoses that require a careful differential diagnosis. Thearticle describes the possible causes of the formation and clinical manifestations of pulmonaryemphysema in children. We present a clinical case of bullous emphysema in a teenager. This case showsthat a detailed examination using such a modern diagnostic method as high-resolution computedtomography played a crucial role in establishing the patient's correct diagnosis. However, establishing thenature of this pathological process in the child was very difficult. Perhaps an earlier diagnosis couldprevent severe irreversible changes in a teenager's lungs would avoid developing the diffusebronchopulmonary process

    An unusual case of foreign body aspiration in 6-year-old boy

    Foreign body aspiration (FBA) is a common cause of mortality and morbidity in children. FBA isnot always immediately diagnosed, partly because this diagnosis is not frequently in mind, but alsobecause FBA does not have a specific clinical manifestation. FBA can be misdiagnosed as asthma, upperrespiratory tract infection, pneumonia, or croup. Sometimes, FBA can be completely asymptomatic. Wepresent the case with a delay in diagnosing FBA and discuss the appropriate management of suspectedforeign body aspiration in a 6-year-old boy. This case illustrates the need for prompt action and a highlevel of suspicion in patients with prolonged symptoms and no resolution with standard treatment.Diagnosis should always be based on a combination of data obtained from the clinical history, physicalexamination, and radiographic findings. However, the normal appearance of a chest X-ray does notexclude the possibility of FBA

    complexity of early diagnosis of congenital ataxia: a case report

    Ataxia-telangiectasia (AT) is a rare form of phakomatoses with multisystem lesions that are characterized by a specific neuro-cutaneous syndrome. AT is a multisystem disease that includes progressive clinical manifestations of cerebral ataxia, oculocutaneous telangiectasia, and increased susceptibility to cancer due to ionizing radiation sensitivity. Lack of awareness of this disease by medical providers could ultimately lead to a delay in diagnosis and increase morbidity in patients. This case study presents the history of a female adolescent patient, who was eventually placed in protective custody with congenital AT. She was clinically observed for fifteen years. This paper demonstrates the complexity of early diagnosis of AT in children. The importance of a comprehensive evaluation of neural and immunological systems, timely genetic testing, and aggressive treatment of infectious diseases is paramount in the formulation of an appropriate treatment plan. Early diagnosis and management significantly improve the prognosis and quality of life for these patients

    Сlinical features of chronic bronchitis and genetic risk factors for the development of chronic obstructive pulmonary disease in adolescent smokers

    The aim is to study the clinical features of the course of CB in adolescent smokers and to study the genetic risk factors for the development of COPD. Materials and methods: There were examined 40 adolescent smokers with CB, 30 never-smokers adolescents with CB and 37 healthy adolescents smokers (control group). The study included the collection of anamnesis, objective examination. calculation of the smoking index and the «pack/year», molecular genetic investigations. Results: It was proved that smoking leads to the development of chronic bronchitis as early as adolescence and affects its course, increasing the frequency and duration of exacerbations. We identified an association of the 2G/2G genotype of MMP1 gene with the development of chronic bronchitis in adolescent smokers. The TT genotype of CYP1A1 gene may be considered as a possible sustainability factor for the development of chronic bronchitis in adolescent smokers. Conclusions: The study of candidate genes for COPD in childhood and adolescence will facilitate the early detection of high-risk groups in the formation of this pathology, which will allow doctors to take the necessary preventive measures

    Specific features of the oral microbiome in young children with laryngopharyngeal reflux and its role the development of recurrent respiratory diseases

    The aim: To examine the composition of the oral microbiome in young children with laryngopharyngeal reflux (LPR) and its role the development of recurrent respiratory diseases. Materials and methods: There were examined 38 children with physiological gastroesophageal reflux (GER), 18 children with LPR who had a medical history of recurrent bronchitis and 17 healthy children (control group). The study included the collection of anamnesis, objective examination. The qualitative and quantitative microbial composition of the upper respiratory tract was performed obtained by oropharyngeal deep swab. Salivary pepsin level and IL-8 were determined by enzyme-linked immunosorbent assay. Results: This research showed significant alterations in the oral microbiome of patients with GER and LPR as compared to healthy control. We found that gram-negative microbiota such as Klebsiella pneumoniae, Escherichia coli, Proteus vulgaris, Proteus spp. and Candida albicans were identified in children with GER and LPR compared to the healthy control. At the same time, the amount of such a representative of the normal microbiome as Streptococcus viridans in children with LPR was sharply reduced. There were established a much higher mean salivary pepsin level of the patients with LPR than in the GER and control group. We found the association between high pepsin levels, saliva IL-8 levels and frequency of respiratory pathology in children with LPR. Conclusions: Our study confirms that increased levels of pepsin in saliva are a risk factor for recurrent respiratory diseases in children with LPR
