    Electrical and Mechanical Properties of new Recyclable Power Cable Insulation Materials based upon Polyethylene Blends

    Chemically crosslinked polyethylene (XLPE) has been used as electrical insulation for power cables since the 1970s due to its favourable combination of electrical and mechanical properties. However, as the electrical engineering community has become increasingly aware of the life cycle environmental impacts, XLPE has come under scrutiny for its lack of recyclability and the high process energies used in its manufacture. Although technologies are being developed to facilitate the re-use of XLPE at the end of its initial service life, the use of this material is inferior to fully recyclable and low process energy alternatives. In this investigation, we concentrated on the use of binary blends of linear and branched polyethylene (LPE / BPE) as potential replacement materials for XLPE, since such systems have the potential to combine comparable mechanical properties and enhanced breakdown strength with good recyclability. We compare the thin film AC ramp breakdown behaviour of blends as a function of temperature up to 97 oC. These consist of the same BPE in virgin and crosslinked states and in a blend with 20wt% LPE. These data are augmented with dynamic mechanical analysis. In concert, these data indicate that with appropriate morphological control the blended thermoplastic material exhibits superior properties to XLPE under conventional operating conditions and may even be suitable for higher temperature operation than XLPE. The paper will discuss the importance of polymer blending and blend physical properties in the context of the process requirements and the implications for cable manufacture and on cable electrical and environmental performance in comparison with XLPE

    X-ray spectral evolution in the ultraluminous X-ray source M33 X-8

    The bright ultraluminous X-ray source (ULX), M33 X-8, has been observed several times by XMM–Newton, providing us with a rare opportunity to ‘flux bin’ the spectral data and search for changes in the average X-ray spectrum with flux level. The aggregated X-ray spectra appear unlike standard sub-Eddington accretion state spectra which, alongside the lack of discernible variability at any energy, argues strongly against conventional two-component, sub-Eddington models. Although the lack of variability could be consistent with disc-dominated spectra, sub-Eddington disc models are not sufficiently broad to explain the observed spectra. Fits with a ∼ Eddington accretion rate slim disc model are acceptable, but the fits show that the temperature decreases with flux, contrary to expectations, and this is accompanied by the appearance of a harder tail to the spectrum. Applying a suitable two-component model reveals that the disc becomes cooler and less advection dominated as the X-ray flux increases, and this is allied to the emergence of an optically thick Comptonization medium. We present a scenario in which this is explained by the onset of a radiatively driven wind from the innermost regions of the accretion disc, as M33 X-8 exceeds the Eddington limit. Furthermore, we argue that the direct evolution of this spectrum with increasing luminosity (and hence radiation pressure) leads naturally to the two-component spectra seen in more luminous ULXs

    The ultraluminous state revisited : fractional variability and spectral shape as diagnostics of super-Eddington accretion

    Although we are nearing a consensus that most ultraluminous X-ray sources (ULXs) below 1041 erg s−1 represent stellar mass black holes accreting in a super-Eddington ‘ultraluminous’ accretion state, little is yet established of the physics of this extreme accretion mode. Here, we use a combined X-ray spectral and timing analysis of an XMM–Newton sample of ULXs to investigate this new accretion regime. We start by suggesting an empirical classification scheme that separates ULXs into three classes based on the spectral morphologies observed by Gladstone et al.: a singly peaked broadened disc class, and two-component hard ultraluminous and soft ultraluminous regimes, with the spectra of the latter two classes dominated by the harder and softer component, respectively. We find that at the lowest luminosities (LX < 3 × 1039 erg s−1) the ULX population is dominated by sources with broadened disc spectra, whilst ULXs with two-component spectra are seen almost exclusively at higher luminosities, suggestive of a distinction between ∼Eddington and super-Eddington accretion modes. We find high levels of fractional variability are limited to ULXs with soft ultraluminous spectra, and a couple of the broadened disc sources. Furthermore, the variability in these sources is strongest at high energies, suggesting it originates in the harder of the two spectral components. We argue that these properties are consistent with current models of super-Eddington emission, where a massive radiatively driven wind forms a funnel-like geometry around the central regions of the accretion flow. As the wind provides the soft spectral component this suggests that inclination is the key determinant in the observed two-component X-ray spectra, which is very strongly supported by the variability results if this originates due to clumpy material at the edge of the wind intermittently obscuring our line-of-sight to the spectrally hard central regions of the ULX. The pattern of spectral variability with luminosity in two ULXs that straddle the hard/soft ultraluminous regime boundary is consistent with the wind increasing at higher accretion rates, and thus narrowing the opening angle of the funnel. Hence, this work suggests that most ULXs can be explained as stellar mass black holes accreting at and above the Eddington limit, with their observed characteristics dominated by two variables: accretion rate and inclination

    Effect of a national primary care pay for performance scheme on emergency hospital admissions for ambulatory care sensitive conditions: controlled longitudinal study.

    OBJECTIVE: To estimate the impact of a national primary care pay for performance scheme, the Quality and Outcomes Framework in England, on emergency hospital admissions for ambulatory care sensitive conditions (ACSCs). DESIGN: Controlled longitudinal study. SETTING: English National Health Service between 1998/99 and 2010/11. PARTICIPANTS: Populations registered with each of 6975 family practices in England. MAIN OUTCOME MEASURES: Year specific differences between trend adjusted emergency hospital admission rates for incentivised ACSCs before and after the introduction of the Quality and Outcomes Framework scheme and two comparators: non-incentivised ACSCs and non-ACSCs. RESULTS: Incentivised ACSC admissions showed a relative reduction of 2.7% (95% confidence interval 1.6% to 3.8%) in the first year of the Quality and Outcomes Framework compared with ACSCs that were not incentivised. This increased to a relative reduction of 8.0% (6.9% to 9.1%) in 2010/11. Compared with conditions that are not regarded as being influenced by the quality of ambulatory care (non-ACSCs), incentivised ACSCs also showed a relative reduction in rates of emergency admissions of 2.8% (2.0% to 3.6%) in the first year increasing to 10.9% (10.1% to 11.7%) by 2010/11. CONCLUSIONS: The introduction of a major national pay for performance scheme for primary care in England was associated with a decrease in emergency admissions for incentivised conditions compared with conditions that were not incentivised. Contemporaneous health service changes seem unlikely to have caused the sharp change in the trajectory of incentivised ACSC admissions immediately after the introduction of the Quality and Outcomes Framework. The decrease seems larger than would be expected from the changes in the process measures that were incentivised, suggesting that the pay for performance scheme may have had impacts on quality of care beyond the directly incentivised activities

    The ultraluminous X-ray source NGC 5643 ULX1: a large stellar mass black hole accreting at super-Eddington rates?

    A sub-set of the brightest ultraluminous X-ray sources (ULXs), with X-ray luminosities well above 1040 erg s−1, typically have energy spectra which can be well described as hard power laws, and short-term variability in excess of ∼10 per cent. This combination of properties suggests that these ULXs may be some of the best candidates to host intermediate-mass black holes (IMBHs), which would be accreting at sub-Eddington rates in the hard state seen in Galactic X-ray binaries. In this work, we present a temporal and spectral analysis of all of the available XMM–Newton data from one such ULX, the previously poorly studied 2XMM J143242.1−440939, located in NGC 5643. We report that its high-quality EPIC spectra can be better described by a broad, thermal component, such as an advection-dominated disc or an optically thick Comptonizing corona. In addition, we find a hint of a marginal change in the short-term variability which does not appear to be clearly related to the source unabsorbed luminosity. We discuss the implications of these results, excluding the possibility that the source may be host an IMBH in a low state, and favouring an interpretation in terms of super-Eddington accretion on to a black hole of stellar origin. The properties of NGC 5643 ULX1 allow us to associate this source to the population of the hard/ultraluminous ULX class

    The role of screening blood tests in patients with arterial disease attending vascular outpatients

    Objective:to evaluate the benefits of a policy of performing screening blood tests in new patients with arterial disease referred to the vascular outpatients department.Methods:clinical audit over a 12-month period of all new referrals with arterial disease to the vascular outpatients department at the Leicester General Hospital.Results:two hundred and seventy-two patients had at least one blood test performed at their outpatient visit. All of these patients had a full blood count performed, of which 21 results (21%) were abnormal. Further investigation of patients with abnormal results revealed one case of bladder cancer, one case of leukaemia and one patient with polycythaemia. Urea and electrolytes were measured in 269 patients (99%). Of these, 26 (10%) were expectedly abnormal in patients with known renal impairment. A further 27 patients (10%) were identified to have some degree of unrecognised renal impairment. Serum non-fasting glucose was measured in 252 patients (93%). There were 11 unexpectedly raised results, but further investigation of these patients only diagnosed one of these patients as diabetic. Serum cholesterol was measured in 201 patients (74%). One hundred and thirty-two patients (66%) had an abnormally raised serum cholesterol level. Of these, only 12 patients (6%) were known to have hyperlipidaemia.Conclusions:screening new patients with arterial disease in vascular outpatients does identify significant abnormalities, in particular renal impairment and hyperlipidaemia. Correction of these abnormalities may reduce the morbidity associated with contrast induced nephrotoxic acute renal failure, and also contribute to secondary prevention of vascular events associated with raised lipids

    Edge wetting of an Ising three-dimensional system

    The effect of edge on wetting and layering transitions of a three-dimensional spin-1/2 Ising model is investigated, in the presence of longitudinal and surface magnetic fields, using mean field (MF) theory and Monte Carlo (MC) simulations. For T=0, the ground state phase diagram shows that there exist only three allowed transitions, namely: surface and bulk transition, surface transition and bulk transition. However, there exist a surface intra-layering temperature TLsT_{L}^{s}, above which the surface and the intra-layering surface transitions occur. While the bulk layering and intra-layering transitions appear above an other finite temperature TLb(TLs)T_{L}^{b} (\ge T_{L}^{s}). These surface and bulk intra-layering transitions are not seen in the perfect surfaces case. Numerical values of TLsT_{L}^{s} and TLbT_{L}^{b}, computed by Monte Carlo method are found to be smaller than those obtained using mean field theory. However, the results predicted by the two methods become similar, and are exactly those given by the ground state phase diagram, for very low temperatures. On the other hand, the behavior of the local magnetizations as a function of the external magnetic field, shows that the transitions are of the first order type. TLsT_{L}^{s} and TLbT_{L}^{b} decrease when increasing the system size and/or the surface magnetic field. In particular, TLbT_{L}^{b} reaches the wetting temperature TwT_{w} for sufficiently large system sizes.Comment: 11 Pages latex, 12 Figures P

    Repetitive task training for improving functional ability after stroke

    &lt;p&gt;&lt;b&gt;Objectives:&lt;/b&gt; The objective of this review was to determine if repetitive task training after stroke improves global, upper, or lower limb function and if treatment effects are influenced by the amount, type, or timing of practice.&lt;/p&gt; &lt;p&gt;&lt;b&gt;Search Strategy:&lt;/b&gt; We searched the Cochrane Stroke Trials Register (to October 2006); The Cochrane Library, MEDLINE, EMBASE, CINAHL, AMED, SportDiscus, Science Citation Index, Index to Theses, ZETOC, PEDro, and OT Seeker (all to September 2006); and OT search (to March 2006). We also searched for unpublished/non-English language trials; combed conference proceedings and reference lists; requested information on bulletin boards; and contacted trial authors.&lt;/p&gt; &lt;p&gt;&lt;b&gt;Selection Criteria:&lt;/b&gt; Selection criteria included randomized/quasirandomized trials in adults after stroke, of interventions that included an active motor sequence performed repetitively within a single training session, a clear functional goal, and a quantifiable level of practice.&lt;/p&gt; &lt;p&gt;&lt;b&gt;Data Collection and Analysis:&lt;/b&gt; Two authors independently screened abstracts, extracted data, and appraised trial quality. Further information was obtained from study authors. Results from individual trials were combined using meta-analytic techniques appropriate to the data extracted and the level of between-trial heterogeneity.&lt;/p&gt; &lt;p&gt;&lt;b&gt;Main Results:&lt;/b&gt; Fourteen trials with 17 intervention-control pairs and 659 participants were included. Primary outcomes showed that treatment effects were statistically significant for walking distance (see the Figure); walking speed (standardized mean difference, 0.29; 95% CI, 0.04 to 0.53); and sit-to-stand (standardized effect estimate, 0.35; 95% CI, 0.13 to 0.56). Treatment effects were of borderline statistical significance for functional ambulation.&lt;/p&gt
