    Entrepreneurship in Missouri

    There is a well-based belief that entrepreneurial activity and economic growth are positively related. This paper uses various measures of entrepreneurial activity and business formation to compare Missouri’s record to that of the national average and to its neighboring states. Based on our analysis, the average small business in Missouri looks similar to those in other states. But the climate in which small businesses are created and generate jobs is much different in Missouri relative to other states. Two indices of entrepreneurship, each based on different criteria, rank Missouri far down the list of states when it comes to entrepreneurial activity. The data on business formation corroborates this picture of deficient entrepreneurial activity: between 2005 and 2013, the pace at which Missouri created new establishments pales in comparison to the national average, and to the average of the neighboring states. The outcome of not creating new businesses also is evident in slower jobs growth. How to encourage more entrepreneurship in Missouri? An oft-suggested approach is to use enact legislation or pass special incentives, such as tax abatements, to attract specific businesses or industries. Trying to pick winners in business is an ill-advised policy to promote economic growth. Public policy aimed at encouraging entrepreneurship should consider improving the economic and social environment within which all entrepreneurs will operate. This can be done by improving the tax climate, labor market conditions, and especially the educational attainment of the labor force

    Background-Limited Imaging in the Near-Infrared with Warm InGaAs Sensors: Applications for Time-Domain Astronomy

    We describe test observations made with a customized 640 x 512 pixel Indium Gallium Arsenide (InGaAs) prototype astronomical camera on the 100" DuPont telescope. This is the first test of InGaAs as a cost-effective alternative to HgCdTe for research-grade astronomical observations. The camera exhibits an instrument background of 113 e-/sec/pixel (dark + thermal) at an operating temperature of -40C for the sensor, maintained by a simple thermo-electric cooler. The optical train and mechanical structure float at ambient temperature with no cold stop, in contrast to most IR instruments which must be cooled to mitigate thermal backgrounds. Measurements of the night sky using a reimager with plate scale of 0.4 arc seconds / pixel show that the sky flux in Y is comparable to the dark current. At J the sky brightness exceeds dark current by a factor of four, and hence dominates the noise budget. The sensor read noise of ~43e- falls below sky+dark noise for exposures of t>7 seconds in Y and 3.5 seconds in J. We present test observations of several selected science targets, including high-significance detections of a lensed Type Ia supernova, a type IIb supernova, and a z=6.3 quasar. Deeper images are obtained for two local galaxies monitored for IR transients, and a galaxy cluster at z=0.87. Finally, we observe a partial transit of the hot JupiterHATS34b, demonstrating the photometric stability required over several hours to detect a 1.2% transit depth at high significance. A tiling of available larger-format sensors would produce an IR survey instrument with significant cost savings relative to HgCdTe-based cameras, if one is willing to forego the K band. Such a camera would be sensitive for a week or more to isotropic emission from r-process kilonova ejecta similar to that observed in GW170817, over the full 190 Mpc horizon of Advanced LIGO's design sensitivity for neutron star mergers.Comment: 13 pages, 12 figures, submitted to A

    Impact of scribing history and physical notes and procedure reports on endoscopist efficiency during routine procedures: a proof-of-concept study

    BACKGROUND: Efficiency is an important aspect of endoscopic practice that has received limited study. We evaluated the impact of scribing electronic pre-procedure history and physical examinations, and electronic procedure reports on endoscopist efficiency. METHODS: We used a stopwatch to measure the time between the procedures (scope out to scope in), pre-procedure patient assessment time, and procedure report generation time for 180 consecutive procedures performed by a single endoscopist with or without a scribe for recording history and physical and procedure reports. Schedulers were unaware of whether a scribe would be present. RESULTS: Mean times for recording the pre-procedure history and physical and procedure reports were reduced by 34% (p = 0.001) and 71% (p < 0.0001), respectively, when scribes were used. The mean time saved by the endoscopist from scribing the history and the physical and procedure reports was 2.12 and 1.59 min, respectively. When both processes were scribed, the endoscopist spent 42% (p = 0.033) longer in the recovery area (absolute mean increase 1.01 min) compared with when no scribes were utilized. The total time saved per 6.5-h procedure block with both scribes averaged to 41.7 min. CONCLUSION: The use of scribes to record history and physical examination notes and procedure reports saved enough endoscopist time to allow additional procedures or longer procedures, or to free the time for other tasks

    Faster colonoscope withdrawal time without impaired detection using EndoRings

    Background and study aims:  Mucosal exposure devices on the colonoscope tip have improved detection. We evaluated detection and procedure times in colonoscopies performed with EndoRings. Patients and methods:  We had 14 endoscopists in a university practice trial EndoRings. We compared detection and procedure times to age- and indication-matched procedures by the same endoscopists. Results:  There were 137 procedures with EndoRings. The adenoma detection rate was 44 % with EndoRings vs. 39 % without ( P  = 0.39). Mean adenomas per colonoscopy (standard deviation) was 1.2 (2.3) with EndoRings vs. 0.9 (1.6) without ( P  = 0.055). Mean insertion time with EndoRings was 6.2 (3.2) minutes vs. 6.6 (6.7) minutes without ( P  = 0.81). Mean withdrawal time with EndoRings in all patients with or without polypectomy was 12.2 (5.3) minutes and 16.1 (10.3) minutes without ( P  = 0.0005). Conclusion:  EndoRings may allow faster withdrawal during colonoscopy without any reduction in detection. Prospective trials with mucosal exposure devices targeting procedure times as primary endpoints are warranted

    Thermal Stability and Rehybridization of Carbon Bonding in Tetrahedral Amorphous Carbon

    We preform a quantitative investigation of the energetics of thermally induced sp3 → sp2 conversion of carbon-carbon bonds in tetrahedral amorphous carbon (ta-C) films by using near edge x-ray absorption fine structure (NEXAFS) and Raman spectroscopy. We investigate the evolution of the bonding configuration in ta-C thin films subjected to high temperature annealing in flowing Argon gas using a rapid thermal annealing furnace over the range of 200-1000 ºC. We observe no substantial change in bonding structure below 600 ºC, and by 1000 ºC a significant increase in the sp2 bonding in the film is observed. No oxygen bonding is detected in the NEXAFS spectra, but we do observe an isosbestic point, demonstrating that the thermally driven sp3 → sp2 conversion reaction occurs without passing through an intermediate transition state. This allows us to use NEAFS spectra of thermally annealed ta-C films to quantitatively determine that the activation energy for directly converting the sp3-bonded carbon to the s

    The Phoenix Deep Survey: Extremely Red Galaxies and Cluster Candidates

    We present the results of a study of a sample of 375 Extremely Red Galaxies (ERGs) in the Phoenix Deep Survey, 273 of which constitute a subsample which is 80% complete to K_s = 18.5 over an area of 1160 arcmin^2. The angular correlation function for ERGs is estimated, and the association of ERGs with faint radio sources explored. We find tentative evidence that ERGs and faint radio sources are associated at z > 0.5. A new overdensity-mapping algorithm has been used to characterize the ERG distribution, and identify a number of cluster candidates, including a likely cluster containing ERGs at 0.5 < z < 1. Our algorithm is also used in an attempt to probe the environments in which faint radio sources and ERGs are associated. We find limited evidence that the I - K_s > 4 criterion is more efficient than R - K_s > 5 at selecting dusty star-forming galaxies, rather than passively evolving ERGs.Comment: 14 emulateapj pages, 15 figures, 1 table, accepted for publication in Astronomical Journal.     Patients’ perceived needs for medical services for non-specific low back pain: A systematic scoping review

    Background: An improved understanding of patients’ perceived needs for medical services for low back pain (LBP) will enable healthcare providers to better align service provision with patient expectations, thus improving patient and health care system outcomes. Thus, we aimed to identify the existing literature regarding patients’ perceived needs for medical services for LBP. Methods: A systematic scoping review was performed of publications identified from MEDLINE, EMBASE, CINAHL and PsycINFO (1990–2016). Descriptive data regarding each study, its design and methodology were extracted and risk of bias assessed. Aggregates of patients’ perceived needs for medical services for LBP were categorised. Results: 50 studies (35 qualitative, 14 quantitative and 1 mixed-methods study) from 1829 were relevant. Four areas of perceived need emerged: (1) Patients with LBP sought healthcare from medical practitioners to obtain a diagnosis, receive management options, sickness certification and legitimation for their LBP. However, there was dissatisfaction with the cursory and superficial approach of care. (2) Patients had concerns about pharmacotherapy, with few studies reporting on patients’ preferences for medications. (3) Of the few studies which examined the patients’ perceived need of invasive therapies, these found that patients avoided injections and surgeries (4) Patients desired spinal imaging for diagnostic purposes and legitimation of symptoms. Conclusions: Across many different patient populations with data obtained from a variety of study designs, common themes emerged which highlighted areas of patient dissatisfaction with the medical management of LBP, in particular, the superficial approach to care perceived by patients and concerns regarding pharmacotherapy. Patients perceive unmet needs from medical services, including the need to obtain a diagnosis, the desire for pain control and the preference for spinal imaging. These issues need to be considered in developing approaches for the management of LBP in order to improve patient outcomes

    Influence of Surface Passivation on the Friction and Wear Behavior of Ultrananocrystalline Diamond and Tetrahedral Amorphous Carbon Thin Films

    Highly sp3-bonded, nearly hydrogen-free carbon-based materials can exhibit extremely low friction and wear in the absence of any liquid lubricant, but this physical behavior is limited by the vapor environment. The effect of water vapor on friction and wear is examined as a function of applied normal force for two such materials in thin film form: one that is fully amorphous in structure (tetrahedral amorphous carbon, or ta-C) and one that is polycrystalline with sp3 to disordered sp2 bonding is observed, no crystalline graphite formation is observed for either film. Rather, the primary solid-lubrication mechanism is the passivation of dangling bonds by OH and H from the dissociation of vapor-phase H2O. This vapor-phase lubrication mechanism is highly effective, producing friction coefficients as low as 0.078 for ta-C and 0.008 for UNCD, and wear rates requiring thousands of sliding passes to produce a few nanometers of wear
