    AV Nodal Reentrant Tachycardia Causing Inappropriate ICD Shocks In A Patient With Arrhythmogenic RV Dysplasia

    We report a patient with an implantable cardioverter defibrillator (ICD) for arrhythmogenic right ventricular dysplasia (ARVD) who received inappropriate shocks for atrioventricular node reentry tachycardia (AVRNT). Patient had multiple shocks for tachycardia with EGM characteristics of very short VA interval and CL of 300 msec. An electrophysiologic (EP) study reproducibly induced typical AVNRT with similar features. The slow AV nodal pathway ablation resolved the ICD shocks. Despite increasingly sophisticated discrimination algorithms available in modern ICDs, the ability to differentiate SVT from VT can be challenging. Our patient received inappropriate shocks for AVNRT. When device interrogation alone is not conclusive, an EP study may be necessary to determine the appropriate therapeutic course

    A 47 year old man with episodes of attacks of pain, redness and mild blurring of vision was investigated for narrow angle glaucoma in view of shallow anterior chambers and a cupped optic disc. The history was reviewed following a spontaneous attack in hospital, which had features other than acute glaucoma. A diagnosis of cluster headache was made on the basis of tests. Cluster headache has been defined as unilateral intense pain, involving the eye and head on one side, usually associated with flushing, nasal congestion and lacrimation; the attacks recurring one or more times daily and lasting 20 - 120 minutes. Such attacks commonly continue for weeks or months and are separated by an asymptomatic period of months to years. This episodic nature, together with unilaterality and tendency to occur at night, closely mimics narrow angle glaucoma. Further, if patients have shallow anterior chambers and disc cupping, the differentiation becomes more difficult yet critical. Resource to provocative tests is often the only answer as the following case report demonstrates

    Deontologists are not always trusted over utilitarians : revisiting inferences of trustworthiness from moral judgments

    Recent research has looked at how people infer the moral character of others based on how they resolve sacrificial moral dilemmas. Previous studies provide consistent evidence for the prediction that those who endorse outcome-maximizing, utilitarian judgments are disfavored in social dilemmas and are seen as less trustworthy in comparison to those who support harm-rejecting deontological judgments. However, research investigating this topic has studied a limited set of sacrificial dilemmas and did not test to what extent these effects might be moderated by specific features of the situation described in the sacrificial dilemma (for instance, whether the dilemma involves mortal or non-mortal harm). In the current manuscript, we assessed the robustness of previous findings by exploring how trust inference of utilitarian and deontological decision makers is moderated by five different contextual factors (such as whether the sacrificial harm is accomplished by an action or inaction), as well as by participants’ own moral preferences. While we find some evidence that trust perceptions of others are moderated by dilemma features, we find a much stronger effect of participants’ own moral preference: deontologists favored other deontologists and utilitarians favored utilitarians

    The freedom to excel: Belief in free will predicts better academic performance

    © 2015 Elsevier Ltd. Increasing evidence supports the importance of beliefs in predicting positive outcomes in life. We examined the performance implications of the belief in free will as an abstract, philosophical belief that views the self as free from internal and external constraints and capable of choosing and directing one's own path. In Study 1 (N = 116, undergraduates), belief in free will was associated with higher performance on an academic proofreading task. In Study 2 (N = 614, undergraduates), we examined performance in real academic settings, and the belief in free will measured at the beginning of the semester predicted better course and semester grades at the end of the semester. Importantly, we found support for the distinctive contribution of the belief in free will in comparison to well-established predictors of academic performance - trait self-control and implicit theories. We conclude that individual differences in the endorsement of the belief in free will are a significant and unique predictor of academic achievement.Link_to_subscribed_fulltex
