    Concordancia de dos métodos inmunológicos para la determinación de Metabolitos de Benezodiacepinas en orina

    Debido a la gran cantidad de test comerciales disponibles para la determinación de metabolitos de benzodiacepinas en orina en el mercado, es que decidimos evaluar la concordancia de 2 test inmunológicos, disponibles en el laboratorio de Toxicología de nuestro hospital. Las muestras de orinas recibidas en nuestro laboratorio fueron analizadas en paralelo por ambos métodos. Se aplicaran los test estadisticos de Kappa y Chi cuadrado para su evaluación. La utilidad de estos test ante la sospecha diagnóstica de consumo de benzodiacepinas en determinadas patologías, amerita la disponibilidad de estos test inmunológicos que pueden proveer en corto tiempo, un resultado que permita al profesional médico una oportuna toma de decisiones frente a la problemática con la cual arriba el paciente. En este trabajo pudimos comprobar que ambos métodos presentan muy buena concordancia (test Kappa=0,88), por lo cual son ambos test indistintamente aplicables para la casuística que presenta el Laboratorio de Toxicología

    Innovative approach for self-management and social welfare of children with cystic fibrosis in Europe: Development, validation and implementation of an mHealth tool (MyCyFAPP)

    Introduction: For the optimal management of children with cystic fibrosis, there are currently no efficient tools for the precise adjustment of pancreatic enzyme replacement therapy, either for advice on appropriate dietary intake or for achieving an optimal nutrition status. Therefore, we aim to develop a mobile application that ensures a successful nutritional therapy in children with cystic fibrosis. Methods and analysis: A multidisciplinary team of 12 partners coordinate their efforts in 9 work packages that cover the entire so-called ‘from laboratory to market’ approach by means of an original and innovative co-design process. A cohort of 200 patients with cystic fibrosis aged 1–17 years are enrolled. We will develop an innovative, clinically tested mobile health application for patients and health professionals involved in cystic fibrosis management. The mobile application integrates the research knowledge and innovative tools for maximising self-management with the aim of leading to a better nutritional status, quality of life and disease prognosis. Bringing together different and complementary areas of knowledge is fundamental for tackling complex challenges in disease treatment, such as optimal nutrition and pancreatic enzyme replacement therapy in cystic fibrosis. Patients are expected to benefit the most from the outcomes of this innovative project. Ethics and dissemination: The project is approved by the Ethics Committee of the coordinating organisation, Hospital Universitari La Fe (Ref: 2014/ 0484). Scientific findings will be disseminated via journals and conferences addressed to clinicians, food scientists, information and communications technology experts and patients.     El rol de los voluntariados en la restauración ecológica del centro argentino

    La modificación de los paisajes en las montañas del Centro argentino ha sido muy extensa debido al aumento en la frecuencia de incendios, la ganadería, la invasión por especies exóticas, la minería, el avance de las urbanizaciones y las actividades turísticas. Tal vez impulsados por una larga historia académica y un número importante de activistas, desde fines de la década del 1990, un creciente número de voluntarios se ha involucrado en pequeñas restauraciones modelo con un alto valor educativo que intentan revertir los daños ambientales causados por las modificaciones humanas. Si bien este movimiento fue apoyado en parte por una red de organizaciones gubernamentales y no gubernamentales dedicadas a la investigación y conservación, el esfuerzo individual de las personas para revertir los deterioros ambientales ha sido fundamental para el logro de objetivos de restauración ecológica. Durante los voluntariados se trabaja para revertir la degradación ambiental, se forman amistades, se aprende sobre los beneficios de restaurar los ecosistemas, las dificultades que implica y se forjan nuevos proyectos. Describimos algunas de las experiencias relacionadas a la restauración de bosques nativos, poblaciones animales silvestres de importancia regional, restauración de riberas de arroyos y ríos, control de especies exótica invasoras y restauración de bosque nativo en urbanizaciones. En la actualidad, el mayor desafío en la región es lograr que la restauración ecológica no sea solo una actividad recreativa ejecutada principalmente por voluntarios, sino una actividad masiva y profesionalizada, guiada por políticas de estado y principios científicos sólidos.Fil: Renison, Daniel.     Real-world characteristics and outcome of patients treated with single-agent ibrutinib for chronic lymphocytic leukemia in Spain (IBRORS-LLC Study)

    Background: Ibrutinib demonstrated remarkable efficacy and favorable tolerability in patients with untreated or relapsed/refractory (R/R) chronic lymphocytic leukemia (CLL), including those with high-risk genetic alterations. The IBRORS-CLL study assessed the characteristics, clinical management and outcome of CLL patients receiving ibrutinib in routine clinical practice in Spain. Patients: Observational, retrospective, multicenter study in CLL patients who started single-agent ibrutinib as first-line treatment or at first or second relapse between January 2016 and January 2019. Results: A total of 269 patients were included (median age: 70.9 years; cardiovascular comorbidity: 55.4%, including hypertension [47.6%] and atrial fibrillation [AF] [7.1%]). Overall, 96.7% and 69% of patients underwent molecular testing for del(17p)/TP53 mutation and IGHV mutation status. High-risk genetic features included unmutated IGHV (79%) and del(17p)/TP53 mutation (first-line: 66.3%; second-line: 23.1%). Overall, 84 (31.2%) patients received ibrutinib as first-line treatment, and it was used as second- and third-line therapy in 121 (45.0%) and 64 (23.8%) patients. The median progression-free survival and overall survival were not reached irrespective of del(17p)/TP53, or unmutated IGHV. Common grade ≥3 adverse events were infections (12.2%) and bleeding (3%). Grade ≥3 AF occurred in 1.5% of patients. Conclusion: This real-world study shows that single-agent ibrutinib is an effective therapy for CLL, regardless of age and high-risk molecular features, consistent with clinical trials. Additionally, single-agent ibrutinib was well tolerated, with a low rate of cardiovascular events. This study also emphasized a high molecular testing rate of del(17p)/TP53 mutation and IGHV mutation status in clinical practice according to guideline recommendations

    © 2019 Ferrata Storti Foundation.It has been postulated that monitoring measurable residual disease (MRD) could be used as a surrogate marker of progression-free survival (PFS) in chronic lymphocytic leukemia (CLL) patients after treatment with immunochemotherapy regimens. In this study, we analyzed the outcome of 84 patients at 3 years of follow-up after first-line treatment with fludarabine, cyclophosphamide and rituximab (FCR) induction followed by 36 months of rituximab maintenance thearpy. MRD was assessed by a quantitative four-color flow cytometry panel with a sensitivity level of 10-4. Eighty out of 84 evaluable patients (95.2%) achieved at least a partial response or better at the end of induction. After clinical evaluation, 74 patients went into rituximab maintenance and the primary endpoint was assessed in the final analysis at 3 years of follow-up. Bone marrow (BM) MRD analysis was performed after the last planned induction course and every 6 months in cases with detectable residual disease during the 36 months of maintenance therapy. Thirty-seven patients (44%) did not have detectable residual disease in the BM prior to maintenance therapy. Interestingly, 29 patients with detectable residual disease in the BM after induction no longer had detectable disease in the BM following maintenance therapy. After a median followup of 6.30 years, the median overall survival (OS) and PFS had not been reached in patients with either undetectable or detectable residual disease in the BM, who had achieved a complete response at the time of starting maintenance therapy. Interestingly, univariate analysis showed that after rituximab maintenance OS was not affected by IGHV status (mutated vs. unmutated OS: 85.7% alive at 7.2 years vs. 79.6% alive at 7.3 years, respectively). As per protocol, 15 patients (17.8%), who achieved a complete response and undetectable peripheral blood and BM residual disease after four courses of induction, were allowed to stop fludarabine and cyclophosphamide and complete two additional courses of rituximab and continue with maintenance therapy for 18 cycles. Surprisingly, the outcome in this population was similar to that observed in patients who received the full six cycles of the induction regimen. These data show that, compared to historic controls, patients treated with FCR followed by rituximab maintenance have high-quality responses with fewer relapses and improved OS. The tolerability of this regime is favorable. Furthermore, attaining an early undetectable residual disease status could shorten the duration of chemoimmunotherapy, reducing toxicities and preventing long-term side effects. The analysis of BM MRD after fludarabine-based induction could be a powerful predictor of post-maintenance outcomes in patients with CLL undergoing rituximab maintenance and could be a valuable tool to identify patients at high risk of relapse, influencing further treatment strategies.This work was funded by Roche Farma, S.A., Madrid, Spai

    Bronchopulmonary infection-colonization patterns in Spanish cystic fibrosis patients: Results from a national multicenter study

    Background: Clinical and demographical knowledge on Spanish cystic fibrosis (CF) patients is incomplete as no national registry exists. CF-microbiology has not been studied at national level. The results of the first Spanish multicenter study on CF microbiology are presented.Methods: 24 CF-Units for adult (n = 12) and pediatric (n = 12) patients from 17 hospitals provided sputa and clinical data from 15 consecutive patients. Cultures and susceptibility testing were performed. Colonization impact on pulmonary function was assessed.Results: 341 patients [mean (SD) age 21 (11) years, 180 >= 18 years, mean (SD) FEV1 = 68 (25)%] were included. Pseudomonas aeruginosa was reported as chronic, intermittent or absent in 46%, 22% and 32% of patients, respectively. The annual prevalence was 62%. Positive P. aeruginosa and methicillin-resistant Staphylococcus aureus cultures were significantly associated with lower FEV1 (p = 18 years, mean (SD) FEV1 = 68 (25)%] were included.
