Methodical investigation of the reproductibility, sensitivity and specificity of live 3D echocardiography analysis of endocardium motion with respect to the evaluation of the synchronicity of the cardiac wall movement
In this work the synchronous movement of the cardiac wall using 3D-
echocardiography is examined. To determine if the methology is well suited for
the analysis of endocardium motion, persons with normal cardiac wall movement
(the control group) and patients with reduced cardiac wall movement and bi-
ventricular cardiac pacemaker (the interventions group) were examined. The
data acquisition took place with the help of the trans-thoracic
echocardiography which was equipped with a matrixprobe. The left ventricle was
reconstructed subsequently, and analyzed. The evaluation happened by means of
a semiautomatic outlining program. Additionally however also a mode where
manual corrections could be used was accepted. The 4D analysis program for the
left ventricle computed the so-called bags of the left ventricles from the
wire frame of the endocardium. From these bags the regional partial volume and
the regional partial ejection volume were determined. With the help of the
expert system MATLAB 7.0 statistical parameters were computed. Additionally
volume time curves were provided. For the statistical evaluation four
parameters were analysed. For the investigation of the reproductibility of the
cardiac wall motion four data records from the control group and 12 data
records from the group of interventions were evaluated by two examiners and
compared with one another. Here the selected parameters were evaluated by
means of t-test in pairs and the appropriate volume time curves were compared
with one another. There were no significant differences between the groups of
the two examiners, except within one of the parameters. With the help of
standard statisitical analyses significant differences between the two groups
could be determined. Also in the quantitative evaluation by means of the
provided volume time curves clearly visible differences could be shown. For
the quantitative evaluation of the sensitivity the four consulted statistic
parameters from the group of interventions in the intrinsic mode were compared
with the optimal mode of the pacemaker by means of t-test and evaluated
statistically. Via the comparison of the volume time curves of the partial
ejection volumes and the partial impact volumes of the group of interventions
in the intrinsic pacemaker mode with the optimal mode of the pacemaker a
qualitative evaluation was done. While some of the proposed parameters did not
exhibit significant differences between the control and the intervention
group, one of the tested paramters showed to be sensitve. While the results of
this study are promising, it must also be said, that with the current state of
the art analysis programs it remains a very challenging task to use live 3D
echocardiography in the clinical routine. Despite its limitations, this method
already showed its high potential
ProGram 2.0: The New Version of an E-learning Tool for Very Advanced Learners of German
Autonomous and accessible learning are two important didacticmethodological
aspects to address e-learning novelties in foreign language
acquisition. The implementation of both, combined with electronic resources
is created by modernising the existing e-learning tool ProGram for very
advanced German learners. ProGram is designed to improve the academic
writing skills, especially for students of philology studies and is adapted to new
technical and design standards. A special feature of the tool is the intelligent
corrector to foster autonomous learning. The interim version has been tested
to check the relevance of the Internet-based programme. The results show that
the participants, especially advanced users, were satisfied with the design and
content of the tool. A mobile version was viewed positively by only half of the
participants. Further steps will include the adaptation of a responsive design
which facilitates the usage on a mobile device.

First epileptic seizure and epilepsies in adulthood. Abridged version ofthe S2k guideline of the German Society for Neurology in cooperationwith the German Society for Epileptology
Holtkamp M, May T, Berkenfeld R, et al. Erster epileptischer Anfall und Epilepsien im Erwachsenenalter. Kurzfassung S2k-Leitlinie der Deutschen Gesellschaft für Neurologie in Zusammenarbeit mit der Deutschen Gesellschaft für Epileptologie. Clinical Epileptology. 2024.The new S2k guideline "First epileptic seizure and epilepsies in adulthood" provides recommendations on clinically relevant issues in five major topics: management of first epileptic seizures, pharmacotherapy, epilepsy surgery, complementary and supportive treatment, and psychosocial aspects. For the topic management of first epileptic seizures, the guideline provides recommendations on identifying the two major differential diagnoses, syncope and psychogenic non-epileptic seizure. The importance of additional examinations such as EEG, MRI and cerebrospinal fluid for syndromic classification and etiological allocation is discussed. Recommendations on neuropsychological and psychiatric screening tests are also given. The topic pharmacotherapy issues recommendations on antiseizure medication in monotherapy for focal, generalized and unclassified epilepsies; patient groups with special challenges such as the aged, women of childbearing potential and people with mental retardation are emphasized. Further issues are indications for measuring serum concentrations of antiseizure medication and possible risks of switching manufacturers. In the topic epilepsy surgery, indications for presurgical assessment and the multiple therapeutic approaches, such as resection, laser ablation, and neurostimulation are presented. Recommendations on postoperative management of patients, including rehabilitation and psychosocial counselling, are given. The topic complementary and supportive therapeutic approaches comprises recommendations on the diagnostics and treatment of common psychiatric comorbidities of epilepsy, such as anxiety disorder, depression and psychosis. Another important issue is the management of psychogenic non-epileptic seizures as a neuropsychiatric differential diagnosis or comorbidity of epileptic seizures. Furthermore, recommendations on the potential role of ketogenic diet and on acupuncture, homeopathy and other complementary approaches are made. The recommendations on psychosocial aspects comprise practical issues, such as fitness to drive a car, training and occupation, medical rehabilitation, sport, transition, patients' self-help, education programs for patients and next of kin, adherence, advise on SUDEP
