Long-Term Efficacy and Safety of Chronic Globus Pallidus Internus Stimulation in Different Types of Primary Dystonia
Background: Deep brain stimulation (DBS) of the globus pallidus internus (GPi) offers a very promising therapy for medically intractable dystonia. However, little is known about the long-term benefit and safety of this procedure. We therefore performed a retrospective long-term analysis of 18 patients (age 12-78 years) suffering from primary generalized (9), segmental (6) or focal (3) dystonia (minimum follow-up: 36 months). Methods: Outcome was assessed using the Burke-Fahn-Marsden (BFM) scores (generalized dystonia) and the Tsui score (focal/segmental dystonia). Follow-up ranged between 37 and 90 months (mean 60 months). Results: Patients with generalized dystonia showed a mean improvement in the BFM movement score of 39.4% (range 0 68.8%), 42.5% (range -16.0 to 81.3%) and 46.8% (range-2.7 to 83.1%) at the 3- and 12-month, and long-term follow-up, respectively. In focal/ segmental dystonia, the mean reduction in the Tsui score was 36.8% (range 0-100%), 65.1% (range 16.7-100%) and 59.8% (range 16.7-100%) at the 3- and 12-month, and long-term follow-up, respectively. Local infections were noted in 2 patients and hardware problems (electrode dislocation and breakage of the extension cable) in 1 patient. Conclusion: Our data showed Gpi-DBS to offer a very effective and safe therapy for different kinds of primary dystonia, with a significant long-term benefit in the majority of cases. Objective estimation of visual Acuity with preferential looking
Purpose: A novel Preferential Looking (PL) procedure that uses quantitative analysis of visual scanning parameters is presented.
Methods: Nine adult subjects were presented with a set of 14 visual stimuli (stimuli included 3 uniform grey fields and 1 field with black and white square wave gratings) spanning the range of spatial frequencies from 1.5 cycles/degree to 35.1 cycles/degree (1.3 logMAR to -0.07 logMAR). A remote gaze-tracking system was used to monitor the subject's eye movements and the relative fixation time (RFT) on the grating target. Subsequently, a four alternative forced-choice psychophysical test (4AFC) was performed with the same visual stimuli.
Results: For visual stimuli for which the gratings' positions in the 4AFC test were identified correctly in 100% of the trials ("reliably discriminated"), the mean RFT was 72.5% ± 9.0%. For stimuli for which the spatial frequencies were higher than the subject's psychophysically determined VA threshold ("non-discriminated"), the mean RFT was 25.3% ± 8.5%. Using three repeated trials at each spatial frequency and a VA detector that is based on the conditional probability density functions of the RFT, the average VA was underestimated by 0.06 logMAR (range: 0.00 logMAR to 0.20 logMAR).
Conclusions: In adults, automated quantitative analysis of visual scanning patterns can be used to estimate objectively and rapidly (210 seconds) VA with a mean error of 0.06 logMAR. The novel approach may form the basis for PL procedures that are more objective and accurate than the traditional clinical PL procedures

Mapping soil erosion risk and safety factors of the Massanutten Trail system
Public mountain biking and hiking trails can pose challenges to trail-user safety. The purpose of this project is to improve the overall safety factors on the Massanutten Western Slope, in eastern Rockingham County, Virginia, where a 15-plus mile trail system has been made available to a broad range of users. Owned by Massanutten Resort, the trail system is in a remote, forested area frequented by local off-road cyclists, runners, hikers, as well as seasonal tourists and is maintained by the Shenandoah Valley Bicycle Coalition (SVBC). This multifaceted project, which integrates ESRI ArcGIS, Trimble Pathfinder, USDA Soil Survey Geographic (SSURGO) data, and the National Land Cover Database (NLCD), has the common goal of increasing the overall safety of the trail system through assessing both environmental impact in trail construction and use, as well as accessibility for first responders. Trail erosion risk was modeled in ArcGIS based on the variables outlined in the Revised Universal Soil Loss Equation (RUSLE). The factors of soil erodibility, slope, and vegetation cover type were used to assess erosion risk and identify areas where current trails are likely to erode and where future construction should be avoided by SVBC. Field collection of GPS locational data using a survey-grade field rover was undertaken to update trail marker signage, provide accurate locational maps for rescue efforts, and test the validity of the erosion model through ground-truthing. Cellular strength data was collected to identify areas along the trails where cellular coverage may be weak. Ultimately, first responders will be provided with maps of the trail system labeled with geographic coordinates and that identify access points
Deep brain stimulation in schizophrenia
Deep brain stimulation (DBS) has successfully advanced treatment options of putative therapy-resistant
neuropsychiatric diseases. Building on this strong foundation more and more mental disorders in the
stadium of therapy-resistance are considered as possible indications for DBS. Especially schizophrenia
with its associated severe and difficult to treat symptoms is gaining attention. This attention demands
critical questions regarding the assumed mechanisms of DBS and its possible influence on the supposed
pathophysiology of schizophrenia. Here we synoptically compare current approaches and theories of
DBS and discuss the feasibility of DBS in schizophrenia as well as the transferability from other
psychiatric disorders successfully treated with DBS. For this we consider recent advances in animal
models of schizophrenic symptoms, results regarding the influence of DBS on dopaminergic
transmission as well as data concerning neural oscillation and synchronization. In conclusion the use of
DBS for some symptoms of schizophrenia seems to be a promising approach, but the lack of a
comprehensive theory of the mechanisms of DBS as well as its impact on schizophrenia might void the
use of DBS in schizophrenia at this point
Comparing Chandra and SIRTF Observations for Obscured Starbursts and AGN at High Redshift
Tracking the star formation rate to high redshifts requires knowledge of the
contribution from both optically visible and obscured sources. The dusty,
optically-obscured galaxies can be located by X-ray and infrared surveys. To
establish criteria for selecting such sources based only on X-ray and infrared
surveys, we determine the ratio of infrared to X-ray brightness that would be
observed by SIRTF and Chandra for objects with the same spectral shapes as
nearby starbursts if seen at high redshift. The parameter IR/X is defined as
IR/X = (flux density observed in SIRTF MIPS 24 m filter in mJy)/(total
flux observed within 0.5-2.0 keV in units of 10^-16 ergs\s\cm^2). Based on
observations of NGC 4038/39 (``The Antennae''), NGC 3690+IC 694 (Arp 299 or Mkn
171), M 82, and Arp 220, nine starburst regions are compared using mid-infrared
spectra taken by the Infrared Space Observatory (ISO) and X-ray spectra
obtained with Chandra . The IR/X are determined as they would appear for 1<z<3.
The mean IR/X over this redshift range is 1.3 and is not a significant function
of redshift or luminosity, indicating that SIRTF surveys reaching 0.4 mJy at 24
m should detect the same starbursts as deep CXO surveys detect at a flux
of 0.3x10^-16 ergs/s/cm^2. The lower bound of IR/X for starbursts is about 0.2,
suggesting that objects with IR/X smaller than this have an AGN X-ray component
in addition to the starburst. Values of IR/X for the obscured AGN within NGC
1068, the Circinus galaxy, and NGC 6240 are also determined for comparison
although interpretation is complicated by the circumnuclear starbursts in these
galaxies. Any sources found in surveys having IR/X>4 would not match any of the
objects considered.Comment: accepted for publication in Ap

Nonclassical Recrystallization
Applications in the fields of materials science and nanotechnology increasingly demand monodisperse nanoparticles in size and shape. Up to now, no general purification procedure exists to thoroughly narrow the size and shape distributions of nanoparticles. Here, we show by analytical ultracentrifugation (AUC) as an absolute and quantitative high-resolution method that multiple recrystallizations of nanocrystals to mesocrystals is a very efficient tool to generate nanocrystals with an excellent and so-far unsurpassed size-distribution (PDIc=1.0001) and shape. Similar to the crystallization of molecular building blocks, nonclassical recrystallization removes “colloidal” impurities (i.e., nanoparticles, which are different in shape and size from the majority) by assembling them into a mesocrystal. In the case of nanocrystals, this assembly can be size- and shape-selective, since mesocrystals show both long-range packing ordering and preferable crystallographic orientation of nanocrystals. Besides the generation of highly monodisperse nanoparticles, these findings provide highly relevant insights into the crystallization of mesocrystals. © 2020 The Authors. Published by Wiley-VCH Gmb
