    Hermeneutical considerations involving the nuances of words in translation have a bearing in interpreting philosophical concepts. Stambaugh highlighted the eternal in Nietzsche as well as the meaning of the Same in the doctrine of the Eternal Recurrence of the Same. As translation of the German word das Gleiche, she provided important considerations regarding the nuances of das Gleiche and the sense of how the English word ā€œthe sameā€ could capture as well as leave out some meanings in the original. This paper builds on Stambaughā€™s observations by providing a linguistic analysis of das Gleiche to open up to how it does mean immediacy and likeness as Stambaugh already observed, but the said analysis propels the discussion further by also pointing to the fact that das Gleiche also refers to what is imminent and forthcoming. This reconsidered meaning of das Gleiche as imminent and open to futurity will provide a fresh understanding of Nietzscheā€™s doctrine. The paper ends by featuring Higginsā€™s reading of eternal recurrence as present-oriented akin to the temporal experience of music to show that this insight in Higgins could also be developed further by showing the futurity and imminence of music as hermeneutical key to Nietzscheā€™s doctrine

    We have studied the effects of hypocapnia on cerebrovascular changes in two MAC-equivalent anaesthetic regimens, using the transcranial Doppler technique as an index of cerebral blood flow (CBF) in 24healthy ASA I patients undergoing spinal surgery. Eight of the patients were subjected to carbon dioxide reactivity challenges in the awake state. Before surgery, the other 16 patients received, in random order, either 1.15% isoflurane in oxygen or 0.5% isoflurane with 70% nitrous oxide. Carbon dioxide reactivity was calculated for each group as the increase in flow velocity per kPa change in PƉCO2 (cm sāˆ’1 kPaāˆ’1). It was significantly greater for the isoflurane group (14.09 (SD 2.44) cm sāˆ’1 kPaāˆ’1) and significantly less for the isofluraneā€”nitrous oxide group (7.95 (1.32) cm s-āˆ’1 kPaāˆ’1) compared with the awake group (11.24 (0.95) cm sāˆ’1 kPaāˆ’1). We conclude that cerebrovascular responsiveness to changes in arterial carbon dioxide concentration is influenced markedly by the anaesthetic procedure.     We studied cerebral pressure autoregulation and carbon dioxide reactivity during propofol-induced electrical silence of the electroencephalogram (EEG) in 10 patients. Anaesthesia was induced with propofol 2.5 mg kgāˆ’1, fentanyl 3 Ī¼g kgāˆ’1 and vecuronium 0.1 mg kgāˆ’1, and a propofol infusion of 250-300 Ī¼g kgāˆ’1 mināˆ’1 was used to induce EEG silence. Cerebral pressure autoregulation was tested by increasing mean arterial pressure (MAP) by 24 (SEM 5) mm Hg from baseline with an infusion of phenylephrine and simultaneously recording middle cerebral artery blood flow velocity (vmca) using transcranial Doppler. Carbon dioxide reactivity was tested by varying Paco2 between 4.0 and 7.0 kPa and recording vmca simultaneously. Although absolute carbon dioxide reactivity was reduced, relative carbon dioxide reactivity was within normal limits for all patients studied (mean 8.5 (SEM 0.8) cm sāˆ’1 kPaāˆ’1 and 22 (2)% kPaāˆ’1, respectively). No significant change in vmca (34 (2) and 35 (2) cm sāˆ’1) was observed with the increase in MAP (77 (4) to 101 (4) mm Hg) during autoregulation testing.     Described here are the results of a NASA field experiment conducted in the Superior National Forest near Ely, Minnesota, during the summers of 1983 and 1984. The purpose of the experiment was to examine the use of remote sensing to provide measurements of biophysical parameters in the boreal forests. Leaf area index, biomass, net primary productivity, canopy coverage, overstory and understory species composition data are reported for about 60 sites, representing a range of stand density and age for aspen and spruce. Leaf, needle, and bark high-resolution spectral reflectance and transmittance data are reported for the major boreal forest species. Canopy bidirectional reflectance measurements are provided from a helicopter-mounted Barnes Multiband Modular Radiometer (MMR) and the Thematic Mapper Simulator (TMS) on the NASA C-130 aircraft

    There are many causes for headaches after childbirth. Even though postdural puncture headache (PDPH) has to be considered in a woman with a history of difficult epidural anaesthesia, preā€eclampsia should always be excluded as an important differential diagnosis. We report a case with signs of lateā€onset preā€eclampsia where administration of an epidural blood patch (EBP) was associated with eclampsia.     The Jesuit priest, Roque J. Ferriols is well known in Ateneo de Manila University as the first philosophy professor to teach philosophy in Filipino. Since 1969 until he retired in 2014, he taught all of his philosophy classes, both in the undergraduate and graduate levels, in Filipino. His example was followed by many Ateneo philosophy faculty. Since around the 1990s the number of philosophy classes taught in English and in Filipino had a 50/50 percentage ratio. This ratio continued on to the present time. As the tradition of teaching philosophy in Filipino nears its 50th anniversary in 2019, Ferriols continues to be appreciated as the trailblazer of the tradition. Even if Ferriols never taught philosophy of language, his teaching pedagogy and his writings reflect important insights relevant to the philosophy of language. This paper seeks to draw these insights from three different texts written years apart from each other but all within Ferriolsā€™s teaching career. The texts are A Memoir of Six Years, an essay written for Philippine Studies in April 1975; Sinauna Bilang Potensyal, the final chapter of his Pambungad sa Metapisika published in 1991; and a few sections of his memoirs, Sulyap sa Aking Pinanggalingan published in 2016. In so doing, this paper seeks to spearhead critical discussions of Ferriolsā€™s philosophy of language stressing on how these insights come from the context of pedagogy and as such how these insights reflect the context, experience, lifeworld and historicity of the learner towards a deeper understanding of the human being qua linguistic being and human being qua human being. Thus, the paper seeks to show that the conscious and deliberate awareness of language and culture often seen in the celebration of Buwan ng Wika distracts us from the reality and nature of language and culture, and obscures the role of language and culture in human life. The main emphasis of this paper is the insight that the reality, nature, and role of language and culture could be found in the actual lived experience of the world as linguistic reality. In other words, we do not experience the world ā€œthrough the mediumā€ of language and culture. What really happens is that our experience of the world is language and culture. This paper does not pretend to be exhaustive in that it cannot fully follow the logical and phenomenological implications of the insights it brings to the readerā€™s attention. Its task is to make thematic these insights and note that these insights are conscious to Ferriols as a philosophy teacher so that these themes and insights could be further mulled over and reflected upon by other writers and scholars seeking to elucidate on the subject matter

    In his book Pambungad sa Metapisika, Roque J. Ferriols, SJ, stresses that metaphysics is a practicum. Metaphysics makes one aware of reality, and this awareness drives the person to affirm, respect, and work and move in accord to and within it. This response to reality is what Ferriols calls ā€œpagbigkas sa meron.ā€ Ferriols also devotes a chapter of his book to the thought of the French paleontologist, Pierre Teilhard de Chardin, SJ. The paper explores how this understanding of Teilhardā€™s thought leads to the practicum identified above as ā€œpagbigkas sa meron.ā€ The paper begins with an exposition of the key concepts found in Ferriolsā€™s book relevant to Teilhardā€™s ā€œplanetisation,ā€ or the furling back upon itself of a ā€œbundleā€ of potential species around the surface of the earth, a movement that could lead to the destruction of humankind itself but which could also occasion unity or what he called the ā€œbrotherhood of persons.ā€ Then it argues that Teilhardā€™s concept of planetisation implies an act of ā€œpagbigkas sa meron.

    Corynebacterium pseudodiphtheriticum has rarely been reported to cause disease in humans, despite its common presence in the flora of the upper respiratory tract. We report here a case of exudative pharyngitis with pseudomembrane possibly caused by C. pseudodiphtheriticum in a 4-year-old girl. The case initially triggered clinical and laboratory suspicion of diphtheria. Because C. pseudodiphtheriticum can be easily confused with Corynebacterium diphtheriae in Gram stain, clarification of its role in the pathogenesis of exudative pharyngitis in otherwise healthy persons is of public health importance. Simple and rapid screening tests to differentiate C. pseudodiphtheriticum from C. diphtheriae should be performed to prevent unnecessary concern in the community and unnecessary outbreak control measures

    Effect of age on intraoperative cerebrovascular autoregulation and near-infrared spectroscopy-derived cerebral oxygenation

    Background Age is an important risk factor for perioperative cerebral complications such as stroke, postoperative cognitive dysfunction, and delirium. We explored the hypothesis that intraoperative cerebrovascular autoregulation is less efficient and brain tissue oxygenation lower in elderly patients, thus, increasing the vulnerability of elderly brains to systemic insults such as hypotension. Methods We monitored intraoperative cerebral perfusion in 50 patients aged 18-40 and 77 patients >65 yr at two Swiss university hospitals. Mean arterial pressure (MAP) was measured continuously using a plethysmographic method. An index of cerebrovascular autoregulation (Mx) was calculated based on changes in transcranial Doppler flow velocity due to changes in MAP. Cerebral oxygenation was assessed by the tissue oxygenation index (TOI) using near-infrared spectroscopy. End-tidal CO2, O2, and sevoflurane concentrations and peripheral oxygen saturation were recorded continuously. Standardized anaesthesia was administered in all patients (thiopental, sevoflurane, fentanyl, atracurium). Results Autoregulation was less efficient in patients aged >65 yr [by 0.10 (se 0.04; P=0.020)] in a multivariable linear regression analysis. This difference was not attributable to differences in MAP, end-tidal CO2, or higher doses of sevoflurane. TOI was not significantly associated with age, sevoflurane dose, or Mx but increased with increasing flow velocity [by 0.09 (se 0.04; P=0.028)] and increasing MAP [by 0.11 (se 0.05; P=0.043)]. Conclusions Our results do not support the hypothesis that older patients' brains are more vulnerable to systemic insults.     We demonstrate experimentally the guiding of cold and slow ND3 molecules along a thin charged wire over a distance of ~0.34 m through an entire molecular beam apparatus. Trajectory simulations confirm that both linear and quadratic high-field-seeking Stark states can be efficiently guided from the beam source up to the detector. A density enhancement up to a factor 7 is reached for decelerated beams with velocities ranging down to ~50 m/s generated by the rotating nozzle technique
