    Use of Mobile Phones as Intelligent Sensors for Sound Input Analysis and Sleep State Detection

    Sleep is not just a passive process, but rather a highly dynamic process that is terminated by waking up. Throughout the night a specific number of sleep stages that are repeatedly changing in various periods of time take place. These specific time intervals and specific sleep stages are very important for the wake up event. It is far more difficult to wake up during the deep NREM (2–4) stage of sleep because the rest of the body is still sleeping. On the other hand if we wake up during the mild (REM, NREM1) sleep stage it is a much more pleasant experience for us and for our bodies. This problem led the authors to undertake this study and develop a Windows Mobile-based device application called wakeNsmile. The wakeNsmile application records and monitors the sleep stages for specific amounts of time before a desired alarm time set by users. It uses a built-in microphone and determines the optimal time to wake the user up. Hence, if the user sets an alarm in wakeNsmile to 7:00 and wakeNsmile detects that a more appropriate time to wake up (REM stage) is at 6:50, the alarm will start at 6:50. The current availability and low price of mobile devices is yet another reason to use and develop such an application that will hopefully help someone to wakeNsmile in the morning. So far, the wakeNsmile application has been tested on four individuals introduced in the final section

    Prevalence and Correlates of Disordered Sleep in Southeast Asian Indians with Type 2 Diabetes

    BackgroundSleep disturbances are common in individuals with diabetes. Patients with diabetes have higher rates of insomnia, excessive daytime sleepiness and increased incidence of restless leg syndrome. The purpose of our study was to investigate the prevalence and determine the predictors of sleep dysfunction in patients with type 2 diabetes in a southeast Asian Indian population.MethodsWe enrolled 120 patients with type 2 diabetes who attended an endocrinology clinic in a tertiary-care hospital. After we collected their demographic data, we recorded their anthropometric measurements. Fasting, postprandial blood glucose values and glycosylated hemoglobin (HbA1c) values were then obtained. Quality of sleep was evaluated in all the patients through the Pittsburgh Sleep Quality Index (PSQI), which is a questionnaire that assesses sleep quality and disturbances over a monthlong period. A Global Sleep Quality score ≥5 discriminates between good and poor sleepers.ResultsThe mean global PSQI score was 7.08 (standard deviation, 3.89), which suggested poor sleep quality in this population. Sixty-nine percent of patients had a global PSQI score ≥5, indicating that they were "poor sleepers." The global PSQI score positively correlated with the duration of diabetes and was also independent of other variables such as age, gender, body mass index, HbA1c, or medications.ConclusionWe found a high prevalence of sleep dysfunction in patients with type 2 diabetes. We also found a significant correlation between duration of diabetes and quality of sleep, independent of other variables. It is important for physicians to address the quality and duration of sleep in patients with type 2 diabetes

    Heart rate variability in non-apneic snorers and controls before and after continuous positive airway pressure

    BACKGROUND: We hypothesized that sympathetic nervous system activity (SNSA) is increased and parasympathetic nervous system activity (PNSA) is decreased during non-rapid eye movement (NREM) sleep in non-apneic, otherwise healthy, snoring individuals compared to control. Moreover, we hypothesized that these alterations in snoring individuals would be more evident during non-snoring than snoring when compared to control. METHODS: To test these hypotheses, heart rate variability was used to measure PNSA and SNSA in 11 normotensive non-apneic snorers and 12 control subjects before and 7-days after adapting to nasal continuous positive airway pressure (nCPAP). RESULTS: Our results showed that SNSA was increased and PNSA was decreased in non-apneic snorers during NREM compared to control. However, these changes were only evident during the study in which snoring was eliminated with nCPAP. Conversely, during periods of snoring SNSA and PNSA were similar to measures obtained from the control group. Additionally, within the control group, SNSA and PNSA did not vary before and after nCPAP application. CONCLUSION: Our findings suggest that long-lasting alterations in autonomic function may exist in snoring subjects that are otherwise healthy. Moreover, we speculate that because of competing inputs (i.e. inhibitory versus excitatory inputs) to the autonomic nervous system during snoring, the full impact of snoring on autonomic function is most evident during non-snoring periods

    The Heavy Vehicle Study: a case-control study investigating risk factors for crash in long distance heavy vehicle drivers in Australia

    Background Heavy vehicle transportation continues to grow internationally; yet crash rates are high, and the risk of injury and death extends to all road users. The work environment for the heavy vehicle driver poses many challenges; conditions such as scheduling and payment are proposed risk factors for crash, yet the precise measure of these needs quantifying. Other risk factors such as sleep disorders including obstructive sleep apnoea have been shown to increase crash risk in motor vehicle drivers however the risk of heavy vehicle crash from this and related health conditions needs detailed investigation. Methods and Design The proposed case control study will recruit 1034 long distance heavy vehicle drivers: 517 who have crashed and 517 who have not. All participants will be interviewed at length, regarding their driving and crash history, typical workloads, scheduling and payment, trip history over several days, sleep patterns, health, and substance use. All participants will have administered a nasal flow monitor for the detection of obstructive sleep apnoea. Discussion Significant attention has been paid to the enforcement of legislation aiming to deter problems such as excess loading, speeding and substance use; however, there is inconclusive evidence as to the direction and strength of associations of many other postulated risk factors for heavy vehicle crashes. The influence of factors such as remuneration and scheduling on crash risk is unclear; so too the association between sleep apnoea and the risk of heavy vehicle driver crash. Contributory factors such as sleep quality and quantity, body mass and health status will be investigated. Quantifying the measure of effect of these factors on the heavy vehicle driver will inform policy development that aims toward safer driving practices and reduction in heavy vehicle crash; protecting the lives of many on the road network

    Validity of Two Consumer Multisport Activity Tracker and One Accelerometer against Polysomnography for Measuring Sleep Parameters and Vital Data in a Laboratory Setting in Sleep Patients

    Two commercial multisport activity trackers (Garmin Forerunner 945 and Polar Ignite) and the accelerometer ActiGraph GT9X were evaluated in measuring vital data, sleep stages and sleep/wake patterns against polysomnography (PSG). Forty-nine adult patients with suspected sleep disorders (30 males/19 females) completed a one-night PSG sleep examination followed by a multiple sleep latency test (MSLT). Sleep parameters, time in bed (TIB), total sleep time (TST), wake after sleep onset (WASO), sleep onset latency (SOL), awake time (WASO + SOL), sleep stages (light, deep, REM sleep) and the number of sleep cycles were compared. Both commercial trackers showed high accuracy in measuring vital data (HR, HRV, SpO2, respiratory rate), r > 0.92. For TIB and TST, all three trackers showed medium to high correlation, r > 0.42. Garmin had significant overestimation of TST, with MAE of 84.63 min and MAPE of 25.32%. Polar also had an overestimation of TST, with MAE of 45.08 min and MAPE of 13.80%. ActiGraph GT9X results were inconspicuous. The trackers significantly underestimated awake times (WASO + SOL) with weak correlation, r = 0.11–0.57. The highest MAE was 50.35 min and the highest MAPE was 83.02% for WASO for Garmin and ActiGraph GT9X; Polar had the highest MAE of 21.17 min and the highest MAPE of 141.61% for SOL. Garmin showed significant deviations for sleep stages (p p = 0.000), r < 0.50. Garmin and Polar overestimated light sleep and underestimated deep sleep, Garmin significantly, with MAE up to 64.94 min and MAPE up to 116.50%. Both commercial trackers Garmin and Polar did not detect any daytime sleep at all during the MSLT test. The use of the multisport activity trackers for sleep analysis can only be recommended for general daily use and for research purposes. If precise data on sleep stages and parameters are required, their use is limited. The accuracy of the vital data measurement was adequate. Further studies are needed to evaluate their use for medical purposes, inside and outside of the sleep laboratory.
