The snacking habits of white preschool ·children
Three-day estimated dietary records were kept for 194 white 3- and 4-year-old children to deterMine and evaluate the extent, nature and quality of their snacking. All but 1 child ate between meals, with morning and afternoon snacking being favoured in terms of frequency and quantity. Soft drinks were .consumed most frequently, followed by fresh fruits and fruit juices, sweets and chocolates, milk and sugar. Between-meal eating contributed more than one-third of the average day's energy and approximately one-quarter of most vitamins and minerals to the children's diets. Foods eaten between meals were, however, significantly less nutrient-dense than mealtime foods. Non-basic foods supplied more energy to the diet than _ any of the five basic food groups, but minimal quantities of micronutrients. Sugar consumption, mostly in the form of sugary foods and drinks, was high, but was not consumed exclusively between meals. Such children should be encouraged to make more use of basic commodities, particularly when snacking
Prevalence and determinants of stunting and overweight in 3-year-old black South African children residing in the Central Region of Limpopo Province, South Africa
OBJECTIVES: To determine the prevalence of stunting, wasting and overweight and their determinants in 3-year-old children in the Central Region of Limpopo Province, South Africa. DESIGN: Prospective cohort study. SETTING: Rural villages in the Central Region of the Limpopo Province, South Africa. SUBJECTS: One hundred and sixty-two children who were followed from birth were included in the study. Anthropometric measurements and sociodemographic characteristics of the children were recorded. RESULTS: Height-for-age Z-scores were low, with a high prevalence of stunting (48%). The children also exhibited a high prevalence of overweight (22%) and obesity (24%). Thirty-one (19%) children were both stunted and overweight. Gaining more weight within the first year of life increased the risk of being overweight at 3 years by 2.39 times (95% confidence interval (CI) 1.96-4.18) while having a greater length at 1 year was protective against stunting (odds ratio (OR) 0.41; 95% CI 0.17-0.97). Having a mother as a student increased the risk for stunting at 3 years by 18.21 times (95% CI 9.46-34.74) while having a working mother increased the risk for overweight by 17.87 times (95% CI 8.24-38.78). All these factors also appeared as risks or as being protective in children who were both overweight and stunted, as did living in a household having nine or more persons (OR 5.72; 95% CI 2.7-12.10). CONCLUSION: The results of this study highlight the importance of evaluating anthropometric status in terms of both stunting and overweight. Furthermore, it is important to realise the importance of normal length and weight being attained at 1 year of age, since these in turn predict nutritional status at 3 years of age

The fast and forceful kicking strike of the secretary bird
The study of animal locomotion has uncovered principles that can be applied to bio-inspired robotics, prosthetics and rehabilitation medicine, while also providing insight into musculoskeletal form and function [1, 2, 3, 4]. In particular, study of extreme behaviors can reveal mechanical constraints and trade-offs that have influenced evolution of limb form and function [1, 2]. Secretary birds (Sagittarius serpentarius; Figure 1A) are large terrestrial birds of prey endemic to sub-Saharan Africa, which feed on snakes, lizards and small mammals [5]. They frequently kick and stamp on the prey’s head until it is killed or incapacitated, particularly when dispatching larger lizards and venomous snakes [5]. The consequences of a missed strike when hunting venomous snakes can be deadly [5], so the kicking strikes of secretary birds require fast yet accurate neural control. Delivery of fast, forceful and accurate foot strikes that are sufficient to stun and kill prey requires precision targeting, demanding a high level of coordination between the visual and neuromuscular systems
Cost analysis of violence-related medical imaging in a Free State tertiary trauma unit
Background: Violence is a leading public health problem worldwide. Beyond the pain and suffering, violence has a significant economic impact on a country’s health, policing and judicial services. Because of the lack of current and comprehensive data in South Africa, local violence-related economic impact studies are largely estimations. Violence-related imaging expenditure, as a component of a public hospital’s expenditure, is yet to be determined.
Objectives: The goals of this study were to measure the violence-related patient burden on Pelonomi Tertiary Hospital’s (PTH) trauma and radiology services, determine the imaging-component cost of violence-related injuries and calculate the financial burden violence has on the hospital’s expenditures.
Method: From the PTH’s trauma unit patient registry, 1380 patients with violence-related injuries were consecutively sampled for 6 months ending 31 December 2017. Imaging investigations were documented and categorised according to the South African National Department of Health’s 2017 Uniform Patient Fee Schedule (UPFS). Descriptive analysis and cost calculations were performed using the 2017 UPFS tariff schedule and hospital-specific health efficiency indicators – patient-day equivalent and expenditure per patient-day equivalent.
Results: Violence-related injuries accounted for 50.64% of all trauma department visits and received a total of 5475 imaging investigations. Violence-related imaging investigations represented 14.81% of all investigations performed by the radiology department in the study period. Overall violence-related admission costs amounted to R35 410 241.85 (8.33% of the hospital’s total expenditure), of which 20.08% (R7 108 845.00) was attributed to imaging investigations.
Conclusion: Violence-related admissions had a high patient and financial burden on PTH. The pinnacle of healthcare cost saving is violence prevention; however, the cost-conscious radiologist could assist with cost saving if responsible and ethical imaging practices are followed
Diet, nutrition and the prevention of type 2 diabetes
Objectives: The overall objective of this study was to evaluate and provide evidence
and recommendations on current published literature about diet and lifestyle in the
prevention of type 2 diabetes.
Design: Epidemiological and experimental studies, focusing on nutritional
intervention in the prevention of type 2 diabetes are used to make disease-specific
recommendations. Long-term cohort studies are given the most weight as to strength
of evidence available.
Setting and subjects: Numerous clinical trials and cohort studies in low, middle and
high income countries are evaluated regarding recommendations for dietary
prevention of type 2 diabetes. These include, among others, the Finnish Diabetes
Prevention Study, US Diabetes Prevention Program, Da Qing Study; Pima Indian
Study; Iowa Women’s Health Study; and the study of the US Male Physicians.
Results: There is convincing evidence for a decreased risk of diabetes in adults who are
physically active and maintain a normal body mass index (BMI) throughout
adulthood, and in overweight adults with impaired glucose tolerance who lose weight
voluntarily. An increased risk for developing type 2 diabetes is associated with
overweight and obesity; abdominal obesity; physical inactivity; and maternal diabetes.
It is probable that a high intake of saturated fats and intrauterine growth retardation
also contribute to an increased risk, while non-starch polysaccharides are likely to be
associated with a decreased risk. From existing evidence it is also possible that omega-3 fatty acids, low glycaemic index foods and exclusive breastfeeding may play a
protective role, and that total fat intake and trans fatty acids may contribute to the risk.
However, insufficient evidence is currently available to provide convincing proof.
Conclusions: Based on the strength of available evidence regarding diet and lifestyle in the prevention of type 2 diabetes, it is recommended that a normal weight status in the lower BMI range (BMI 21–23) and regular physical activity be maintained throughout adulthood; abdominal obesity be prevented; and saturated fat intake be less than 7% of the total energy intake
Isotopic Production Cross Sections in Proton-Nucleus Collisions at 200 MeV
Intermediate mass fragments (IMF) from the interaction of Al,
Co and Au with 200 MeV protons were measured in an angular range
from 20 degree to 120 degree in the laboratory system. The fragments, ranging
from isotopes of helium up to isotopes of carbon, were isotopically resolved.

Differential skewing of donor-unrestricted and γδ T cell repertoires in tuberculosis-infected human lungs
Unconventional T cells that recognize mycobacterial antigens are of great interest as potential vaccine targets against tuberculosis (TB). This includes donor-unrestricted T cells (DURTs), such as mucosa-associated invariant T cells (MAITs), CD1-restricted T cells, and γδ T cells. We exploited the distinctive nature of DURTs and γδ T cell receptors (TCRs) to investigate the involvement of these T cells during TB in the human lung by global TCR sequencing. Making use of surgical lung resections, we investigated the distribution, frequency, and characteristics of TCRs in lung tissue and matched blood from individuals infected with TB. Despite depletion of MAITs and certain CD1-restricted T cells from the blood, we found that the DURT repertoire was well preserved in the lungs, irrespective of disease status or HIV coinfection. The TCRδ repertoire, in contrast, was highly skewed in the lungs, where it was dominated by Vδ1 and distinguished by highly localized clonal expansions, consistent with the nonrecirculating lung-resident γδ T cell population. These data show that repertoire sequencing is a powerful tool for tracking T cell subsets during disease
Wavelet signatures of -splitting of the Isoscalar Giant Quadrupole Resonance in deformed nuclei from high-resolution (p,p) scattering off Nd
The phenomenon of fine structure of the Isoscalar Giant Quadrupole Resonance
(ISGQR) has been studied with high energy-resolution proton inelastic
scattering at iThemba LABS in the chain of stable even-mass Nd isotopes
covering the transition from spherical to deformed ground states. A wavelet
analysis of the background-subtracted spectra in the deformed 146,148,150Nd
isotopes reveals characteristic scales in correspondence with scales obtained
from a Skyrme RPA calculation using the SVmas10 parameterization. A semblance
analysis shows that these scales arise from the energy shift between the main
fragments of the K = 0, 1 and K = 2 components.Comment: 7 pages, 6 figure
The role of the C8 proton of ATP in the regulation of phosphoryl transfer within kinases and synthetases
<p>Abstract</p> <p>Background</p> <p>The kinome comprises functionally diverse enzymes, with the current classification indicating very little about the extent of conserved regulatory mechanisms associated with phosphoryl transfer. The apparent <it>K</it><sub>m </sub>of the kinases ranges from less than 0.4 μM to in excess of 1000 μM for ATP. It is not known how this diverse range of enzymes mechanistically achieves the regulation of catalysis via an affinity range for ATP varying by three-orders of magnitude.</p> <p>Results</p> <p>We have demonstrated a previously undiscovered mechanism in kinase and synthetase enzymes where the overall rate of reaction is regulated via the C8-H of ATP. Using ATP deuterated at the C8 position (C8D-ATP) as a molecular probe it was shown that the C8-H plays a direct role in the regulation of the overall rate of reaction in a range of kinase and synthetase enzymes. Using comparative studies on the effect of the concentration of ATP and C8D-ATP on the activity of the enzymes we demonstrated that not only did C8D-ATP give a kinetic isotope effect (KIE) but the KIE's obtained are clearly not secondary KIE effects as the magnitude of the KIE in all cases was at least 2 fold and in most cases in excess of 7 fold.</p> <p>Conclusions</p> <p>Kinase and synthetase enzymes utilise C8D-ATP in preference to non-deuterated ATP. The KIE obtained at low ATP concentrations is clearly a primary KIE demonstrating strong evidence that the bond to the isotopically substituted hydrogen is being broken. The effect of the ATP concentration profile on the KIE was used to develop a model whereby the C8H of ATP plays a role in the overall regulation of phosphoryl transfer. This role of the C8H of ATP in the regulation of substrate binding appears to have been conserved in all kinase and synthetase enzymes as one of the mechanisms associated with binding of ATP. The induction of the C8H to be labile by active site residues coordinated to the ATP purine ring may play a significant role in explaining the broad range of <it>K</it><sub>m </sub>associated with kinase enzymes.</p
