    Doxapram versus placebo in preterm newborns: a study protocol for an international double blinded multicentre randomized controlled trial (DOXA-trial)

    Abstract Background Apnoea of prematurity (AOP) is one of the most common diagnoses among preterm infants. AOP often leads to hypoxemia and bradycardia which are associated with an increased risk of death or disability. In addition to caffeine therapy and non-invasive respiratory support, doxapram might be used to reduce hypoxemic episodes and the need for invasive mechanical ventilation in preterm infants, thereby possibly improving their long-term outcome. However, high-quality trials on doxapram are lacking. The DOXA-trial therefore aims to investigate the safety and efficacy of doxapram compared to placebo in reducing the composite outcome of death or severe disability at 18 to 24 months corrected age. Methods The DOXA-trial is a double blinded, multicentre, randomized, placebo-controlled trial conducted in the Netherlands, Belgium and Canada. A total of 396 preterm infants with a gestational age below 29 weeks, suffering from AOP unresponsive to non-invasive respiratory support and caffeine will be randomized to receive doxapram therapy or placebo. The primary outcome is death or severe disability, defined as cognitive delay, cerebral palsy, severe hearing loss, or bilateral blindness, at 18–24 months corrected age. Secondary outcomes are short-term neonatal morbidity, including duration of mechanical ventilation, bronchopulmonary dysplasia and necrotising enterocolitis, hospital mortality, adverse effects, pharmacokinetics and cost-effectiveness. Analysis will be on an intention-to-treat principle. Discussion Doxapram has the potential to improve neonatal outcomes by improving respiration, but the safety concerns need to be weighed against the potential risks of invasive mechanical ventilation. It is unknown if the use of doxapram improves the long-term outcome. This forms the clinical equipoise of the current trial. This international, multicentre trial will provide the needed high-quality evidence on the efficacy and safety of doxapram in the treatment of AOP in preterm infants.     Enclosed please find a copy of Analysis and Intelligent Design’s interim report for the embedded fingerprint based user authentication system. We have currently looked at the 3 different parts of supporting efficient fingerprint based user authentication in an embedded system and 2 different hardware options. We are going to put more emphasis on image processing and fingerprint classification than on authentication and the DSP starter kit and fingerprint development kit have been ordered as the hardware component at $796.51. A final report on the chosen design will be delivered on April 9 th, with a poster presentation on April 5 th

    Barriers and facilitators to help-seeking by men: A rapid review of systematic reviews to support intervention design.

    Help-seeking in the context of healthcare involves seeking help for an existing health condition or to prevent future conditions. Men can be less likely than women to seek help for their health. This rapid review identifies the barriers and facilitators to men seeking help for various health concerns. Four databases were searched in April, 2022, using terms related to men, health and help-seeking. Systematic reviews published after 2009 were eligible for inclusion. Duplicates were removed before 5690 studies were screened for inclusion; 29 reviews were included and underwent data extraction and quality assessment. The majority of reviews satisfied most quality criteria. Data were synthesised narratively to identify themes. The studies included men from various ethnic and cultural backgrounds, and examined help-seeking for various health concerns and behaviours. Barriers to help-seeking by men included: stigma associated with help-seeking (especially for sexual health); the need to preserve masculinity; feeling fear related to the process of help-seeking and anticipated assessments or treatments; having poor health literacy specific to symptom identification, and knowing when help is needed and where to access it; having little support from others around them to seek help; and having healthcare services that are inaccessible or insensitive to the needs of men. Facilitators to help-seeking behaviour included: the removal of fear by ensuring men are comfortable and understand the need to seek help; improving health literacy by increasing awareness of health conditions impacting men; having supportive family and peers who encourage help-seeking; and having services that are accessible and sensitive to the unique needs of men. Overall, the identified barriers and facilitators can be used to design interventions that accept the common perception of help-seeking being unmasculine, while also challenging that notion and making help-seeking appear attractive and essential for men to live their best life

    Harmonizing Quality Improvement Metrics Across Global Trial Networks to Advance Paediatric Clinical Trials Delivery.

    BackgroundDespite global efforts to improve paediatric clinical trials, significant delays continue in paediatric drug approvals. Collaboration between research networks is needed to address these delays. This paper is a first step to promote interoperability between paediatric networks from different jurisdictions by comparing drivers for, and content of, metrics about clinical trial conduct.MethodsThree paediatric networks, Institute for Advanced Clinical Trials for Children, the Maternal Infant Child and Youth Research Network and conect4children, have each developed metrics to address delays and create efficiencies. We identified the methodology by which each network identified metrics, described the metrics of each network, and mapped consistency to come to consensus about core metrics that networks could share.ResultsMetric selection was driven by site quality improvement in one network (11 metrics), by network performance in one network (13 metrics), and by both in one network (five metrics). The domains of metrics were research capacity/capability, site identification/feasibility, trial start-up, and recruitment/enrolment. The network driven by site quality improvement did not have indicators for capacity/capability or identification/feasibility. Fifteen metrics for trial start up and conduct were identified. Metrics related to site approvals were found in all three networks. The themes for metrics can inform the development of 'shared' metrics.ConclusionWe found disparity in drivers, methodology and metrics. Tackling this disparity will result in a unified approach to addressing delays in paediatric drug approvals. Collaborative work to define inter-operable metrics globally is outlined

    Risk axis: Implementation of structured clinical guidelines for the assessment and management of risk in psychiatry

    Introduction: Mental health clinicians routinely assess risk in three domains: suicide, violence and self neglect. While the published work on the assessment of these risks has increased, concerns are often expressed that assessment alone does not lead to management of the identified risk. Not all axes of multiaxial classificatory systems are used by clinicians to the same extent, despite their proven usefulness. It is possible that a dedicated axis for risk management as a part of a multiaxial classificatory system could align risk assessment with management and increase utility of multiaxial classificatory systems. Methods: Based on our earlier work we developed a comprehensive risk management axis (RMA), which was then further refined by conducting a pilot study and then used to collect data retrospectively from the case notes of 100 patients consecutively discharged from a general psychiatric ward. Results: Common association between risk factors and three risk domains were as follows. Violence: active symptoms of major mental illness, employment problems, and major mental illness; Suicide: history of abuse, clinically significant depressive symptoms, and alcohol and drug use; Self neglect: neglect of home, personal safety, noncompliance with treatment, male gender, and low income. We also investigated whether the presence of a particular risk factor acted as a barrier or prompt for clinicians to address management factors in the RMA. Discussion: It is possible to assess risk across the three domains and to derive management plans by using the proposed RMA. Minimal clinical background is necessary for collecting data retrospectively. It should be possible to apply this axis prospectively
