    A Latent Profile Analysis of Rural Women Who Use Drugs and Commit Crimes

    The majority of rural Appalachian women in jail meet criteria for a drug use disorder and need treatment. Using a latent profile analysis of a random sample of rural women in Appalachian jails (N=400) the current study established groups of women based on criminal history, drug use in the commission of crimes, and role of the partner’s drug use in the commission of crimes. Analysis found five distinct profiles of rural women based on involvement of criminal activities as a function of drug use severity. Results suggest that among criminally involved rural women, severity of drug use is a critical factor in the criminal career. Findings can be used to better inform treatment approaches and tailor treatment to meet the needs of this vulnerable population

    Eruption and Interplanetary Evolution of a Stealthy Streamer-Blowout CME Observed by PSP at ∼0.5 AU

    Streamer-blowout coronal mass ejections (SBO-CMEs) are the dominant CME population during solar minimum. Although they are typically slow and lack clear low-coronal signatures, they can cause geomagnetic storms. With the aid of extrapolated coronal fields and remote observations of the off-limb low corona, we study the initiation of an SBO-CME preceded by consecutive CME eruptions consistent with a multi-stage sympathetic breakout scenario. From inner-heliospheric Parker Solar Probe (PSP) observations, it is evident that the SBO-CME is interacting with the heliospheric magnetic field and plasma sheet structures draped about the CME flux rope. We estimate that 18 +/- 11% of the CME's azimuthal magnetic flux has been eroded through magnetic reconnection and that this erosion began after a heliospheric distance of similar to 0.35 AU from the Sun was reached. This observational study has important implications for understanding the initiation of SBO-CMEs and their interaction with the heliospheric surroundings.Peer reviewe

    A phase 2 study of panobinostat with lenalidomide and weekly dexamethasone in myeloma.

    Phase 3 studies combining histone deacetylase inhibitors with bortezomib were hampered by gastrointestinal (GI) intolerance, which was not observed when combined with immunomodulatory drugs. This study is a single-center phase 2 study of panobinostat with lenalidomide and dexamethasone (FRD). Twenty-seven relapsed multiple myeloma patients were enrolled. Twenty-two patients (81%) were lenalidomide refractory and 9 (33%), 14 (52%), and 7 (26%) were refractory to pomalidomide, bortezomib, and carfilzomib, respectively. High-risk molecular findings were present in 17 (63%) patients. Responses included 2 complete responses (CRs), 4 very good partial responses (VGPRs), 5 partial responses (PRs), and 9 minimal responses (MRs) for an overall response rate of 41%, clinical benefit rate of 74%, and a disease control rate of 96%. The median progression-free survival (PFS) was 7.1 months. In the 22 lenalidomide-refractory patients, there were 1 CR, 4 VGPRs, 3 PRs, and 7 MRs, with a median PFS of 6.5 months. Median overall survival was not reached. Grade 3/4 toxicities were primarily hematologic. Gene expression profiling of enrollment tumor samples revealed a set of 1989 genes associated with short (<90 days) PFS to therapy. MAGEA1 RNA and protein expression were correlated with short PFS, and laboratory studies demonstrated a role for MAGE-A in resistance to panobinostat-induced cell death. FRD demonstrates durable responses, even in high-risk, lenalidomide-refractory patients, indicating the essential role of panobinostat in attaining responses. MAGEA1 expression may represent a functional biomarker for resistance to panobinostat. In contrast to PANORAMA 1, there were no significant GI toxicities and primarily expected hematologic toxicities. This trial was registered at www.clinicaltrials.gov as #NCT00742027

    Influence of Large-scale Interplanetary Structures on the Propagation of Solar Energetic Particles: The Multispacecraft Event on 2021 October 9

    An intense solar energetic particle (SEP) event was observed on 2021 October 9 by multiple spacecraft distributed near the ecliptic plane at heliocentric radial distances R ≲ 1 au and within a narrow range of heliolongitudes. A stream interaction region (SIR), sequentially observed by Parker Solar Probe (PSP) at R = 0.76 au and 48° east from Earth (ϕ = E48°), STEREO-A (at R = 0.96 au, ϕ = E39°), Solar Orbiter (SolO; at R = 0.68 au, ϕ = E15°), BepiColombo (at R = 0.33 au, ϕ = W02°), and near-Earth spacecraft, regulated the observed intensity-time profiles and the anisotropic character of the SEP event. PSP, STEREO-A, and SolO detected strong anisotropies at the onset of the SEP event, which resulted from the fact that PSP and STEREO-A were in the declining-speed region of the solar wind stream responsible for the SIR and from the passage of a steady magnetic field structure by SolO during the onset of the event. By contrast, the intensity-time profiles observed near Earth displayed a delayed onset at proton energies ≳13 MeV and an accumulation of ≲5 MeV protons between the SIR and the shock driven by the parent coronal mass ejection (CME). Even though BepiColombo, STEREO-A, and SolO were nominally connected to the same region of the Sun, the intensity-time profiles at BepiColombo resemble those observed near Earth, with the bulk of low-energy ions also confined between the SIR and the CME-driven shock. This event exemplifies the impact that intervening large-scale interplanetary structures, such as corotating SIRs, have in shaping the properties of SEP events
