    Respiratory symptoms and cross-shift lung function in relation to cotton dust and endotoxin exposure in textile workers in Nepal: a cross-sectional study

    Objectives: Inhalation of a cotton-based particulates has previously been associated with respiratory symptoms and impaired lung function. This study investigates the respiratory health of Nepalese textile workers in relation to dust and endotoxin exposure. Methods: A total of 938 individuals from four sectors (garment, carpet, weaving and recycling) of the textile industry in Kathmandu, Nepal completed a health questionnaire and performed spirometry. A subset (n=384) performed cross-shift spirometry. Personal exposure to inhalable dust and airborne endotoxin was measured during a full shift for 114 workers. Results: The overall prevalence of persistent cough, persistent phlegm, wheeze ever, breathlessness ever and chest tightness ever was 8.5%, 12.5%, 3.2%, 6.5% and 12.3%, respectively. Symptoms were most common among recyclers and least common among garment workers. Exposure to inhalable dust significantly predicted persistent cough and chest tightness. Exposure to endotoxin did not have any independent predictive effect. Significant cross-shift reduction in forced expiratory volume in 1ā€…s (FEV1) and forced vital capacity (FVC) were found (p<0.001 for both) being largest for FEV1 in the recyclers (āˆ’143ā€…mL), and least in the garment workers (āˆ’38ā€…mL; p=0.012). Exposure to inhalable dust predicted a cross-shift reduction in FEV1. Conclusions: This study is the first to investigate the respiratory health of Nepalese cotton workers. The measured association between inhalable dust exposure and reporting of respiratory symptoms and across-shift decrement in FEV1 and FVC indicates that improved dust control measures should be instituted, particularly in the recycling and carpet sectors. The possible role of other biologically active agents of cotton dust beyond endotoxin should be further explored

    The Transition to an Energy Sufficient Economy

    Nigeria is an energy-rich nation with a huge energy resource base. The country is the largest reserves holder and largest producer of oil and gas in the African continent. Despite this, only about 40% of its 158 million people have access to modern energy services. Around 80% of its rural population depend on traditional biomass. This paper presents an overview of ongoing research to examine energy policies in Nigeria. The aims are: 1) to identify and quantify the barriers to sustainable energy development and 2) to provide an integrated tool to aid energy policy evaluation and planning. System dynamics modelling is shown to be a useful tool to map the interrelations between critical energy variables with other key sectors of the economy, and for understanding the energy use dynamics (impact on society and the environment). It is found that the critical factors are burgeoning population, lack of capacity utilisation, and inadequate energy investments. Others are lack of suitably trained manpower, weak institutional frameworks, and inconsistencies in energy policies. These remain the key barriers hampering Nigeria\u27s smooth transition from energy poverty to an energy sufficient economy

    Differences in all-cause hospitalisation by ethnic group: a data linkage cohort study of 4.62 million people in Scotland, 2001ā€“2013

    Background: Immigration into Europe has raised contrasting concerns about increased pressure on health services and equitable provision of healthcare to immigrants /ethnic minorities. We assessed hospital use by ethnic group in Scotland. Methods: We anonymously linked Scotland?s Census 2001 records for 4.62 million people, including their ethnic group, to National Health Service general hospitalisation records for 2001-2013. We used Poisson regression to calculate hospitalisation rate ratios (RRs) in 14 ethnic groups, presented as percentages of the White Scottish reference group (RR=100), for males and females separately. We adjusted for age and socio-economic status and compared those born in the United Kingdom or the Republic of Ireland (UK/RoI) with elsewhere. We calculated mean lengths of hospital stay. Results: 9,789,975 hospital admissions were analysed. Compared to the White Scottish, unadjusted RRs for both males and females in most groups were about 50-90, e.g. Chinese males 49 (95% CI 45-53) and Indian females 76 (71-81). The exceptions were White Irish males, 120 (117-124) and females 115 (112-119) and Caribbean females, 103 (85-126). Adjusting for age increased the RRs for most groups towards or above the reference. Socio-economic status had little effect. In many groups, those born outside the UK/RoI had lower admission rates. Unadjusted mean lengths of stay were substantially lower in most ethnic minorities. Conclusions: Use of hospital beds in Scotland by most ethnic minorities was lower than by the White Scottish majority, largely explained by their younger average age. Other countries should use similar methods to assess their own experiences

    Traditional schizophrenia pharmacotherapy remains a subjective trial and error process involving administration, titration and switching of drugs multiple times until an adequate response is achieved. Despite this time-consuming and costly process, not all patients show an adequate response to treatment. As a consequence, relapse is a common occurrence and early intervention is hampered. Here, we have attempted to identify candidate blood biomarkers associated with drug response in 121 initially antipsychotic-free recent-onset schizophrenia patients treated with widely-used antipsychotics, namely olanzapine (n=40), quetiapine (n=23), risperidone (n=30) and a mixture of these drugs (n=28). Patients were recruited and investigated as two separate cohorts to allow biomarker validation. Data analysis showed the most significant relationship between pre-treatment levels of heart-type fatty acid binding protein (H-FABP) and response to olanzapine (p=0.008, F=8.6, Ī²=70.4 in the discovery cohort and p=0.003, F=15.2, Ī²=24.4 in the validation cohort, adjusted for relevant confounding variables). In a functional follow-up analysis of this finding, we tested an independent cohort of 10 patients treated with olanzapine and found that baseline levels of plasma H-FABP and expression of the binding partner for H-FABP, fatty acid translocase (CD36), on monocytes predicted the reduction of psychotic symptoms (p=0.040, F=6.0, Ī²=116.3 and p=0.012, F=11.9, Ī²=-0.0054, respectively). We also identified a set of serum molecules changed after treatment with antipsychotic medication, in particular olanzapine. These molecules are predominantly involved in cellular development and metabolism.
