The surgical management of male breast cancer: Time for an easy access national reporting database?
Introduction Male breast cancer is extremely rare with an incidence of less than 1% of all breast cancers. Literature reports a peak of incidence at roughly 71 years of age. Management currently follows the same clinical pathways as female breast cancer as a general rule.
Methods A retrospective search for all patients who were referred and diagnosed with male breast cancer at our centre was undertaken. Patients notes were then explored for demographics, histological staging, multidisciplinary team meeting outcome and treatment.
A literature search including the search terms ‘Male Breast Cancer AND Surgery’ or ‘Male Breast Cancer AND Experience’ were used. Non English language articles, or those without abstracts were excluded.
Results Seven patients were reviewed over 3 years (2006–2009). Mean agea was 69 years and mean lesion size was 15 mm. Histology was invasive ductal carcinoma for all patients. All patients were ER receptor positive. Two patients were HER2 positive. Five patients were offered mastectomy. One patient refused treatment. In follow up at 36 months there were 3 recurrences. 1 patient was lost to follow up. There were 3 mortalities. The literature search identified 72 articles. Articles were subdivided into those that discussed the surgical management of male breast cancer (n = 8), articles that discussed male breast cancer as podium presentations or posters with no full text article publication (n = 13) and finally full text publications of case experience of male breast cancer (n = 21).
Discussion We report a series of seven cases of male breast cancer encountered over three years, evaluating patient demographics as well as treatment and outcomes. In our series patients were managed with mastectomy. New evidence is questioning the role of mastectomy against breast conserving surgery in male patients. Furthermore there is a lack of reporting infrastructure for national data capture of the benefits of surgical modalities. Literature review highlights the varied clinical experience between units that remains reported as podium presentation but not published. The establishment of an online international reporting registry would allow for efficient analysis of surgical outcomes to improve patient care from smaller single centres. This would facilitate large scale meta analysis by larger academic surgical centres
Magnetic and multiferroic properties of dilute Fe-doped BaTiO3 crystals
Combining and coupling both magnetic and electric properties in one single phase multiferroic material has attracted high interest recently to enable a broad range of novel devices and applications. To evaluate one potential route toward new multiferroics, we have studied 0.5% Fe-doped BaTiO3 single crystals and measured the ferroelectric, magnetic, and multiferroic properties. X-ray absorption spectroscopy shows the presence of Fe3+, and magnetic measurements confirmed that this has a significant impact on the magnetic properties. Doping of iron introduces paramagnetism from lone iron atoms as well as what appears to be a weak ferromagnetism. Multiferroicity and magnetoelectric (ME) coupling were observed in the polarization-electric field hysteresis loops with an applied magnetic field, yet there was no direct evidence that ME coupling persists when the sample was in the defect dipole-aligned state
Simultaneous large optical and piezoelectric effects induced by domain reconfiguration related to ferroelectric phase transitions
Electrical switching of ferroelectric domains and subsequent domain wall motion promotes strong piezoelectric activity; however, light scatters at refractive index discontinuities such as those found at domain wall boundaries. Thus, simultaneously achieving large piezoelectric effect and high optical transmissivity is generally deemed infeasible. Here, it is demonstrated that the ferroelectric domains in perovskite Pb(In1/2Nb1/2)O3 Pb(Mg1/3Nb2/3)O3-PbTiO3 domain-engineered crystals can be manipulated by electrical field and mechanical stress to reversibly and repeatably, with small hysteresis, transform the opaque poly-domain structure into a highly transparent mono-domain state. This control of optical properties can be achieved at very low electric fields (less than 1.5 kV cm−1) and is accompanied by a large (>10000 pm V−1) piezoelectric coefficient that is superior to that of linear state-of-the-art materials by a factor of three or more. The coexistence of tunable optical transmissivity and high piezoelectricity paves the way for a new class of photonic devices
An integer optimization model and algorithms to support the cost-revenue study and provisory designing warehouses or other storage objects
An optimization model for the cost–revenue study at the stage of system analysis and preliminary designs of storage objects such as warehouses, containers, packs and similar objects are developed. Our assumptions motivated by warehouses design lead us to a nonlinear integer optimization problem with the only basic constraint. We present algorithmic methods for obtaining the exact solution to the general problem with emphasizing the special case when both the objective and the constraint functions are increasing. The results of the paper may be used in developing software tools intended for supporting designers
