Leczenie migreny przewlekłej — zalecenia opracowane przez Grupę Ekspertów Polskiego Towarzystwa Bólów Głowy i Sekcji Badania Bólu Polskiego Towarzystwa Neurologicznego na podstawie międzynarodowych zaleceń i najnowszej literatury
Migrena przewlekła jest chorobą, w której ból głowy występuje przez co najmniej 15 dni w miesiącu, z których co najmniej 8 jest dniami, kiedy ból głowy spełnia kryteria rozpoznania migreny i występuje co najmniej przez kolejne 3 miesiące. W praktyce chorzy cierpią na migrenę przewlekłą wiele miesięcy i lat, a częstość tej choroby wynosi około 3% populacji ogólnej. Wiąże się z tym znaczący wzrost absencji w pracy i szkole, a w przypadku wykonywania pracy lub uczestniczenia w zajęciach dydaktycznych — słaba wydajność w tych czynnościach. Jakość życia znacząco spada. Konsekwencje długofalowe mają implikacje ekonomiczne i socjalne. Pacjenci cierpiący na migrenę przewlekłą także znacznie częściej niż cierpiący na migrenę epizodyczną poszukują pomocy lekarskiej i konsultacji specjalistów. Ponadto liczba przyjmowanych, a więc także i kupowanych leków przeciwmigrenowych i ogólnie przeciwbólowych, zarówno doraźnych jak i profilaktycznych, jest wyraźnie ponadprzeciętna. Migrena przewlekła stanowi więc znaczne obciążenie w rozumieniu zarówno medycznym, jak i ekonomicznym, zarówno dla chorych, jak i dla społeczeństwa, a równocześnie nie jest prawidłowo leczona. W niniejszym opracowaniu przedstawiono stanowisko i zalecenia ekspertów Polskiego Towarzystwa Bólów Głowy w sprawie postępowania w migrenie przewlekłej oparte na międzynarodowych rekomendacjach
The role of microparticles in pathomechanisms of diabetic retinopathy : analysis of intercellular communication mechanisms in endothelial aging : case control study in patients with metabolic syndrome, diabetes type 1 and type 2
The project is proposed to explain the role of specific circulating microparticles (MPs) as conveyors in trafficking bio-active molecules in type 1 (T1DM) and type 2 (T2DM) diabetic patients with risk of diabetic retinopathy (DR) and in patients with metabolic syndrome (MS). The possible role of miRNAs as modulators of these processes (in switching on/off mechanism on the molecular level) is proposed. An increased number of MPs with respect to glucose concentrations and levels of proangiogenic factors in vivo (patients’ plasma) is expected. The relationship between age of patents and MP content (cell membrane glycoproteins, phosphatidylserine or miRNA profile) is possible. MPs will be obtained from T1DM (n = 30) T2DM (n = 30), MS (n = 30) and controls (n = 30). Retinopathy in diabetic patients will be assessed by imaging method. Biological profile of MPs will be assessed in vitro by means of flow cytometry, molecular biology methods and cell proliferation assays

Retrospective analysis of the treatment of BRCA1 and BRCA2 mutation carriers – the experience of a single-center tertiary institution
Introduction.Breast cancer gene 1 and 2 (BRCA1/2) mutation carriers are at a higher risk of developing breast cancer. There are several established risk-reducing therapies. Our study aimed to characterize the BRCA1/2 mutation carriers, and to evaluate the implemented treatment methods.
Material and methods.A retrospective analysis of clinical records of 96 female patients hospitalized from October 2019 to December 2022 in the Breast Cancer Unit in Lodz, Poland.
Results.Out of 85 BRCA1 and 11 BRCA2 mutation carriers, 96.88% received nipple-sparing or skin-sparing, unilateral or bilateral risk-reducing mastectomies. Out of all the patients, 36 developed 38 breast cancers. One patient was diagnosed with breast cancer 2 years after a bilateral risk-reducing mastectomy. The most common breast cancer subtype was triple-negative breast cancer (73.68%). The patients could receive surgery, chemotherapy, endocrine therapy and radiotherapy. 18 patients had neoadjuvant chemotherapy, in 6 of these patients a complete pathological response (ypT0N0) was achieved.
Conclusions.Oncoplastic bilateral risk-reducing mastectomies are effective and safe procedures
RR-Lyrae-type pulsations from a 0.26-solar-mass star in a binary system
RR Lyrae pulsating stars have been extensively used as tracers of old stellar
populations for the purpose of determining the ages of galaxies, and as tools
to measure distances to nearby galaxies. There was accordingly considerable
interest when the RR Lyr star OGLE-BLG-RRLYR-02792 was found to be a member in
an eclipsing binary system4, as the mass of the pulsator (hitherto constrained
only by models) could be unambiguously determined. Here we report that
RRLYR-02792 has a mass of 0.26 M_sun and therefore cannot be a classical RR
Lyrae star. Through models we find that its properties are best explained by
the evolution of a close binary system that started with 1.4 M_sun and 0.8
M_sun stars orbiting each other with an initial period of 2.9 days. Mass
exchange over 5.4 Gyr produced the observed system, which is now in a very
short-lived phase where the physical properties of the pulsator happen to place
it in the same instability strip of the H-R diagram occupied by RR Lyrae stars.
We estimate that samples of RR Lyr stars may contain a 0.2 percent
contamination with systems similar to this one, implying that distances
measured with RR Lyrae stars should not be significantly affected by these
binary interlopers
