    Building leadership capacity and future leaders in operational research in low-income countries: why and how?

    Very limited operational research (OR) emerges from programme settings in low-income countries where the greatest burden of disease lies. The price paid for this void includes a lack of understanding of how health systems are actually functioning, not knowing what works and what does not, and an inability to propose adapted and innovative solutions to programme problems. We use the National Tuberculosis Control Programme as an example to advocate for strong programme-level leadership to steer OR and build viable relationships between programme managers, researchers and policy makers. We highlight the need to create a stimulating environment for conducting OR and identify some of the main practical challenges and enabling factors at programme level. We focus on the important role of an OR focal point within programmes and practical approaches to training that can deliver timely and quantifiable outputs. Finally, we emphasise the need to measure successful OR leadership development at programme level and we propose parameters by which this can be assessed. This paper 1) provides reasons why programmes should take the lead in coordinating and directing OR, 2) identifies the practical challenges and enabling factors for implementing, managing and sustaining OR and 3) proposes parameters for measuring successful leadership capacity development in OR

    Pycnodysostosis: a distinctive brittle bone disease?

    Pycnodysostosis is a rare disorder that is inherited as an autosomal recessive trait usually diagnosed at an early age with an estimated incidence of 1.7 per 1 million births. It is a rare clinical entity, first described in 1962 by Maroteaux and Lamy. The disease has also been named Toulouse-Lautrec syndrome, after the French artist Henri de Toulouse-Lautrec, who suffered from the disease. Pycnodysostosis is a lysosomal storage disorder of the bone caused by a mutation in the gene that codes the enzyme Cathepsin K causing osteosclerosis. However, the diagnosis is sometimes late, made as a result of bone fracture, given the severe bone fragility resulting from increased bone density. Oral and maxillofacial manifestations of this disease are very common

    Urinary trypsinogen-2 dipstick test for point-of-care screening of acute pancreatitis

    Background: Acute pancreatitis (AP) is associated with high mortality in its severe form. Conventional laboratory tests used in its diagnosis are fraught with multiple shortcomings. Early institution of intravenous fluid resuscitation can reduce morbidity and mortality. Measurement of urinary trypsinogen-2 using a bedside urine dipstick test may prove useful in early identification of AP.Methods: Patients with symptoms consistent with AP, attending the emergency department, at a tertiary care hospital in southern India, between November 2014 and November 2016, were included in a prospective observational study. The patients underwent routine investigations and additionally were tested with a urinary trypsinogen-2 dipstick test (UTT). The diagnostic performance and the time to reporting of the different investigations were compared with those of UTT. Final diagnosis of AP, made by clinicians, served as the standard.Results: The sensitivities of serum amylase, serum lipase, UTT, ultrasonography (USG) and contrast-enhanced computed tomography (CECT) were 97.1%, 94.1%, 92.7%, 98.3% and 100%, respectively. The respective specificities were 92.4%, 98.5%, 98.5%, 100% and 100%. The average time required to obtain the test report was about half hour from admission in case of UTT, compared to about 3 hours for serum amylase/lipase, 4 hours for USG and 6 hours for CECT.Conclusions: The results indicate that UTT test, due to its high performance indices, simplicity and faster availability of reports, can serve as an ideal screening test for AP and help in early institution of treatment

With a view to make a beginning in this direction, the Central Marine Fisheries Research Institute has been demonstrating the methods of homestead production of selected groups of aquarium fishes to the inland fish farmers of Chellanam, a coastal village near Cochin adopted by the Institute for promoting various fisheries activities and the details are reported in this communication

    Time series analysis of marine fish landings in India

    Catch data for the period 1950-91 on all India landings for three important marine fish species and total landing were analysed to develop suitable ARIMA models for time series forecast. An initial recurssive regression analysis showed that the models suitable are ARIMA (2, 2, 1) for total landing, ARlMA (1, 2, 1) for penaeid prawn landings and ARIMA (3, 2, 1) for both catfish and silverbellies landings. The parameters of these models were then estimated by a maximum likelihood estimation procedure following the algorithm of Godolphin (1984). A test by using Chi-square given by Lung and Box (1978) showed that the fitted models are adequate to explain the data. With the fitted models forecasts for the next year were computed along with their confidence limits
