Voices Within Voices: Developing a New Analytical Approach to Vocal Timbre by Examining the Interplay of Emotionally Valenced Vocal Timbres and Emotionally Valenced Lyrics
Aims/Goals This thesis presents a new analytical technique for vocal timbre based on the hypothesis that emotion expressed in vocal timbre impacts emotional perception of lyrics. Background information Vocal timbre is a highly salient musical feature that, arguably, contributes significantly to our emotional experience of a song. Despite this, analytical techniques for vocal timbre remain in their infancy. Today, this is changing as technological developments increasingly allow for vocal timbre to be preserved and studied in a systematic way. The present research capitalises on these developments, using them to facilitate the examination of how emotional vocal timbres impact emotional perception of lyrics. Methodology Since there exists little empirical research on the hypothesis which underlies this analytical technique, and since the experience of vocal timbre could be considered highly subjective, it was necessary to first experimentally test if/how vocal timbre impacts lyric perception. To this end, a reception test was conducted to examine whether vocal timbre on its own has emotional valence, and whether this emotional valence is salient enough to impact emotional perception of words. Results from this test supported the hypothesis, showing that participants were significantly less accurate at identifying the emotional valence of words when these words were sung with a mismatched emotional vocal timbre. The analytical technique itself is multilayered. First, the recording is taken as the basis of analysis. Then, the vocal timbre is described, and its emotional valence is assessed, through Vocal Timbre Features (a system, inspired by the work of van Leeuwen (1999) defined and developed to aid in describing vocal timbre and, potentially, categorising its emotional valence). Observations made by aurally detecting and annotating the Vocal Timbre Features can be confirmed visually through spectrographs. The synergies between emotions identified in the vocal timbre and that conveyed through lyrics can then be assessed using adapted diagrammatic vocabulary sets (inspired by the work of Dennis Smalley (1986, 1997)). Conclusions In summary, this thesis presents a new analytical technique that allows one to analyse vocal timbre in terms of its emotional meaning, and in terms of how this emotional meaning impacts emotional perception of lyrics. It also offers a framework through which one may conduct efficient, aurally based, analyses of vocal timbre more generally. This thesis has also shown that the experience of emotion in vocal timbre, and its impact on lyric perception, may be similar across listeners (i.e., intersubjective)
The relationship between radiation-induced G1arrest and chromosome aberrations in Li-Fraumeni fibroblasts with or without germline TP53 mutations
We previously showed that cultured fibroblasts from patients with the cancer-prone Li-Fraumeni (LF) syndrome, having heterozygous germline TP53 mutations, sustain less ionizing radiation-induced permanent G1 arrest than normal fibroblasts. In contrast, fibroblast strains from LF patients without TP53 mutations showed normal G1 arrest. We have now investigated the relationship between the extent of G1 arrest and the level of structural chromosome damage (mainly dicentrics, rings and acentric fragments) in cells at their first mitosis after G1 irradiation, in 9 LF strains with TP53 mutations, 6 without TP53 mutations and 7 normal strains. Average levels of damage in the mutant strains were 50% higher than in normals, whereas in non-mutant LF strains they were 100% higher. DNA double strand breaks (dsb) are known to act as a signal for p53-dependent G1 arrest and to be the lesions from which chromosome aberrations arise. These results suggest that a minimal level of dsb is required before the signal for arrest is activated and that p53-defective cells have a higher signal threshold than p53-proficient cells. Dsb that do not cause G1 blockage can progress to mitosis and appear as simple deletions or interact to form exchange aberrations. The elevated levels in the non-mutant strains may arise from defects in the extent or accuracy of dsb repair. In LF cells with or without TP53 mutations, the reduced capacity to eliminate or repair chromosomal damage of the type induced by ionising radiation, may contribute to cancer predisposition in this syndrome. © 2001 Cancer Research Campaign http://www.bjcancer.co
Protocol for a multicentre, prospective, population-based cohort study of variation in practice of cholecystectomy and surgical outcomes (The CholeS study)
INTRODUCTION: Cholecystectomy is one of the most common general surgical operations performed. Despite level one evidence supporting the role of cholecystectomy in the management of specific gallbladder diseases, practice varies between surgeons and hospitals. It is unknown whether these variations account for the differences in surgical outcomes seen in population-level retrospective data sets. This study aims to investigate surgical outcomes following acute, elective and delayed cholecystectomies in a multicentre, contemporary, prospective, population-based cohort. METHODS AND ANALYSIS: UK and Irish hospitals performing cholecystectomies will be recruited utilising trainee-led research collaboratives. Two months of consecutive, adult patient data will be included. The primary outcome measure of all-cause 30-day readmission rate will be used in this study. Thirty-day complication rates, bile leak rate, common bile duct injury, conversion to open surgery, duration of surgery and length of stay will be measured as secondary outcomes. Prospective data on over 8000 procedures is anticipated. Individual hospitals will be surveyed to determine local policies and service provision. Variations in outcomes will be investigated using regression modelling to adjust for confounders. ETHICS AND DISSEMINATION: Research ethics approval is not required for this study and has been confirmed by the online National Research Ethics Service (NRES) decision tool. This novel study will investigate how hospital-level surgical provision can affect patient outcomes, using a cross-sectional methodology. The results are essential to inform commissioning groups and implement changes within the National Health Service (NHS). Dissemination of the study protocol is primarily through the trainee-led research collaboratives and the Association of Upper Gastrointestinal Surgeons (AUGIS). Individual centres will have access to their own results and the collective results of the study will be published in peer-reviewed journals and presented at relevant surgical conferences
Acute effects of a standardised extract of Hericium erinaceus (Lion’s Mane mushroom) on cognition and mood in healthy younger adults: a double-blind randomised placebo-controlled study
Introduction: Animal studies have suggested that Lion’s Mane mushroom [Hericium erinaceus (Bull.) Pers.] can enhance cognitive function and mood due to its bioactive metabolites, including erinacines and hericenones. However, despite being an ingredient used both culinarily and therapeutically in the East, and more and more commonly in the West, limited research has focused on the immediate effects of H. erinaceus on the cognitive function and mood of healthy young adults.
Methods: In an acute randomized, placebo-controlled, double-blinded, cross-over intervention study, we investigated the potential benefits of an acute dose of H. erinaceus fruiting body extract (3g of 10:1 extract) on cognitive performance and mood compared to a placebo. Eighteen healthy participants aged 18 to 35 years took part in the study. At baseline and 90 minutes post-consumption of the interventions, cognitive and mood assessments were administered to measure various cognitive abilities such as executive function, working memory, psychomotor skills, attention and information processing speed as well as positive and negative affect.
Results and discussion: The results showed no significant effect of the H. erinaceus fruiting body extract for composite measures of global cognitive function and mood. However, when analysing individual tests, participants exhibited improved performance on the pegboard test at 90 minutes following a single dose of H. erinaceus.
Conclusion: In conclusion, acute consumption of H. erinaceus fruiting body did not demonstrate a significant overall improvement in cognitive performance and mood compared to the placebo and any benefits may be task or domain specific. Further investigations should investigate the effects of chronic supplementation of H. erinaceus fruiting body on cognition and mood in healthy younger adults, as well as establish optimal dosage and the time to peak concentration of H. erinaceus bioactives in the human brain. Additionally, future research should aim to further elucidate potential mechanisms of action to explain potential brain region and cognitive domain specific effects, such as possible regional increases in cerebral blood flow following consumption of H. erinaceus fruiting bodies. It must also be noted that typically only Lion’s mane fruiting bodies are consumed culinarily, where up to 300g of fresh fruiting body are often consumed in the form of mushroom steaks
Population‐based cohort study of outcomes following cholecystectomy for benign gallbladder diseases
Background The aim was to describe the management of benign gallbladder disease and identify characteristics associated with all‐cause 30‐day readmissions and complications in a prospective population‐based cohort. Methods Data were collected on consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing all‐cause 30‐day readmissions and complications were analysed by means of multilevel, multivariable logistic regression modelling using a two‐level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 8909 patients undergoing cholecystectomy from 167 hospitals. Some 1451 cholecystectomies (16·3 per cent) were performed as an emergency, 4165 (46·8 per cent) as elective operations, and 3293 patients (37·0 per cent) had had at least one previous emergency admission, but had surgery on a delayed basis. The readmission and complication rates at 30 days were 7·1 per cent (633 of 8909) and 10·8 per cent (962 of 8909) respectively. Both readmissions and complications were independently associated with increasing ASA fitness grade, duration of surgery, and increasing numbers of emergency admissions with gallbladder disease before cholecystectomy. No identifiable hospital characteristics were linked to readmissions and complications. Conclusion Readmissions and complications following cholecystectomy are common and associated with patient and disease characteristics
Virtual Choirs in Care Homes:The Development and Early Assessment of a New Virtual Reality Choir Intervention
Engaging with music has been shown to have a positive impact on the quality of life of residents in care homes, who are known to be affected by anxiety, depression and loneliness. Based on the known benefits of in-person singing activities, a new Virtual Reality (VR) choir application was developed to facilitate group singing, aiming to improve residents’ wellbeing and sense of community. Co-designed with Alzheimer Scotland, the intervention was tested in two care homes for functionality and to develop an approach towards assessing feasibility. Residents participated in scheduled sessions over a five-week period, in addition to staff engaging in independent ad hoc use of the experience with residents. Data on reactions to the intervention, the quality of life of participants and preferences about the outcome instruments were collected. The VR intervention proved technically successful, user-friendly, and allowed multiple users to sing together. Participants and staff showed strong enthusiasm for the intervention, with residents actively engaging in singing and movement, although some residents found the headsets uncomfortable. This suggests that VR choirs could be a valuable, scalable activity in care homes, especially when in-person facilitators are unavailable. Preliminary observations indicated that the intervention was not detrimental to participants’ health; however, the sample size was very small and a larger feasibility study is required to examine the intervention’s effectiveness, scalability, and cost-effectiveness. This research highlights the challenges associated with measuring the feasibility of VR interventions in residential care settings, and the value of capturing qualitative data in an ecological setting that represents the intended use of the intervention

The Cholecystectomy As A Day Case (CAAD) score: a validated score of preoperative predictors of successful day-case cholecystectomy using the CholeS data set
Background:
Day-case surgery is associated with significant patient and cost benefits. However, only 43% of cholecystectomy patients are discharged home the same day. One hypothesis is day-case cholecystectomy rates, defined as patients discharged the same day as their operation, may be improved by better assessment of patients using standard preoperative variables.
Methods:
Data were extracted from a prospectively collected data set of cholecystectomy patients from 166 UK and Irish hospitals (CholeS). Cholecystectomies performed as elective procedures were divided into main (75%) and validation (25%) data sets. Preoperative predictors were identified, and a risk score of failed day case was devised using multivariate logistic regression. Receiver operating curve analysis was used to validate the score in the validation data set.
Results:
Of the 7426 elective cholecystectomies performed, 49% of these were discharged home the same day. Same-day discharge following cholecystectomy was less likely with older patients (OR 0.18, 95% CI 0.15–0.23), higher ASA scores (OR 0.19, 95% CI 0.15–0.23), complicated cholelithiasis (OR 0.38, 95% CI 0.31 to 0.48), male gender (OR 0.66, 95% CI 0.58–0.74), previous acute gallstone-related admissions (OR 0.54, 95% CI 0.48–0.60) and preoperative endoscopic intervention (OR 0.40, 95% CI 0.34–0.47). The CAAD score was developed using these variables. When applied to the validation subgroup, a CAAD score of ≤5 was associated with 80.8% successful day-case cholecystectomy compared with 19.2% associated with a CAAD score >5 (p < 0.001).
Conclusions:
The CAAD score which utilises data readily available from clinic letters and electronic sources can predict same-day discharges following cholecystectomy
