    Obliterated cavum septi pellucidi: Clinical significance and role of fetal magnetic resonance

    Introduction: The objective of this study was to describe a cohort of fetuses with an ultrasound prenatal diagnosis of obliterated cavum septi pellucidi (oCSP) with the aim to explore the rate of associated malformations, the progression during pregnancy and the role of fetal magnetic resonance imaging (MRI).Material and methods: This was a retrospective multicenter international study of fetuses diagnosed with oCSP in the second trimester with available fetal MRI and subsequent ultrasound and/or fetal MRI follow-up in the third trimester. Where available, postnatal data were collected to obtain information on neurodevelopment.Results: We identified 45 fetuses with oCSP at 20.5 weeks (interquartile range 20.1-21.1). oCSP was apparently isolated at ultrasound in 89% (40/45) and fetal MRI found additional findings in 5% (2/40) of cases, including polymicrogyria and microencephaly. In the remaining 38 fetuses, fetal MRI found a variable amount of fluid in CSP in 74% (28/38) and no fluid in 26% (10/38). Ultrasound follow-up at or after 30 weeks confirmed the diagnosis of oCSP in 32% (12/38) while fluid was visible in 68% (26/38). At follow-up MRI, performed in eight pregnancies, there were periventricular cysts and delayed sulcation with persistent oCSP in one case. Among the remaining cases with normal follow-up ultrasound and fetal MRI findings, the postnatal outcome was normal in 89% of cases (33/37) and abnormal in 11% (4/37): two with isolated speech delay, and two with neurodevelopmental delay secondary to postnatal diagnosis of Noonan syndrome at 5 years in one case and microcephaly with delayed cortical maturation at 5 months in the other.Conclusions: Apparently isolated oCSP at mid-pregnancy is a transient finding with the visualization of the fluid later in pregnancy in up to 70% of cases. At referral, associated defects can be found in around 11% of cases at ultrasound and 8% at fetal MRI indicating the need for a detailed evaluation by expert physicians when oCSP is suspected

    Arrhythmogenic Phenotype in Dilated Cardiomyopathy: Natural History and Predictors of Life-Threatening Arrhythmias

    BACKGROUND-\u2014Patients with dilated cardiomyopathy (DCM) may present with ventricular arrhythmias early in the disease course, unrelated to the severity of left ventricular dysfunction. These patients may be classified as having an arrhythmogenic DCM (ARDCM). We investigated the phenotype and natural history of patients with AR-DCM. METHODS AND RESULTS-\u2014Two hundred eighty-five patients with a recent diagnosis of DCM (median duration of the disease 1 month, range 0 to 7 months) and who had Holter monitoring at baseline were comprehensively evaluated and followed for 107 months (range 29 to 170 months). AR-DCM was defined by the presence of 651 of the following: unexplained syncope, rapid nonsustained ventricular tachycardia ( 655 beats, 65150 bpm), 651000 premature ventricular contractions/24 hours, and 6550 ventricular couplets/ 24 hours, in the absence of overt heart failure. The primary end points were sudden cardiac death (SCD), sustained ventricular tachycardia (SVT), or ventricular fibrillation (VF). The secondary end points were death from congestive heart failure or heart transplantation. Of the 285 patients, 109 (38.2%) met criteria for AR-DCM phenotype. AR-DCM subjects had a higher incidence of SCD/SVT/VF compared with non\u2013AR-DCM patients (30.3% vs 17.6%, P=0.022), with no difference in the secondary end points. A family history of SCD/SVT/VF and the AR-DCM phenotype were statistically significant and cumulative predictors of SCD/SVT/VF. CONCLUSIONS-\u2014One-third of DCM patients may have an arrhythmogenic phenotype associated with increased risk of arrhythmias during follow-up. A family history of ventricular arrhythmias in DCM predicts a poor prognosis and increased risk of SCD

    Arrhythmias in Dilated Cardiomyopathy: Diagnosis and Treatment

    In patients with dilated cardiomyopathy (DCM), it is possible to find a broad range of bradyrhythmias and tachyarrhythmias. Bradyrhythmias and supraventricular arrhythmias can frequently occur in some familial forms such as lamin A/C mutations. Nonsustained ventricular arrhythmias (VA) are observed in about 40% of patients with DCM, but their prognostic role is not clear, and conflicting data have been published in the last 30 years. Multiple mechanisms can explain atrial and ventricular tachyarrhythmias in DCM. Reentry is associated with slow conduction across surviving muscle bundles within regions of interstitial fibrosis, but other mechanisms can be involved, as nonuniform anisotropy of impulse propagation, ion channel dysfunction, and reduced gap junction function

While the main focus is on echocardiography, the applications of cardiac magnetic resonance imaging and computed tomography are also described. Throughout, a clinically oriented approach is employed: detailed attention is paid to differential diagnosis and numerous high-quality images depict the main features of the various types of cardiomyopathy. Consideration is also given to the genetics of cardiomyopathies, with analysis of genotype-phenotype relationships. The main milestones, such as the early diagnosis and the systematic follow-up, the proper etiological characterization at the onset of the disease, and a multidisciplinary approach to the patients together with the optimal medical and device treatment and the rigorous tailored follow-up, accompanied these prognostic improvements. However, DCM still remains the most common cause of heart transplantation and one of the leading causes of cardiac death in the western world. Therefore, proper prognostic stratification and systematic follow-up are the cornerstones to optimize medical management and improve outcomes of patients with DCM

    The last years witnessed an important progress in the diagnosis and treatment of dilated cardiomyopathy (DCM), improving prognosis and life expectancy. However, some important issues in clinical management remain unresolved. The role of genetic testing, the arrhythmic stratification, and the therapeutic approach still represent areas of uncertainty. The way for improving care of DCM should go through better understanding of the etiological basis of the disease, appropriate risk stratification, and development of new therapies. The abovementioned issues represent the most important and demanding challenges for the next future research on DCM
