    Routine prophylactic antibiotic use in the management of snakebite

    BACKGROUND: Routine antibiotic prophylaxis following snakebite is not recommended but evidence suggests that it may be common practice in Zimbabwe. This study set out to determine and describe the extent of this practice at Parirenyatwa Hospital, a large teaching hospital in Zimbabwe METHODS: A retrospective case review (1996 to 1999 inclusive) of all cases of snakebite was undertaken at Parirenyatwa Hospital. Cases with a diagnosis of snakebite, presenting within 24 hours of the bite and with no complications or concurrent illness were defined as "routine prophylactic antibiotic use". RESULTS: From 78 cases which satisfied the inclusion criteria, 69 (88.5%) received antibiotics. Ten different antibiotics from 6 different classes were used with penicillins the most commonly prescribed (benzylpenicillin in 29% of cases, alone or in combination). Over 40% of antibiotics were given parenterally although all patients were conscious on admission. The total cost of antibiotics used was estimated at US$522.98. CONCLUSION: Routine prophylactic use of antibiotics in snakebite at Parirenyatwa Hospital is common practice. This may highlight the lack of a clearly defined policy leading to wasteful inappropriate antibiotic use which is costly and may promote bacterial antibiotic resistance. Further work is required to investigate the reasons for this practice and to design appropriate interventions to counter it

    Topography-derived variables provide insight into habitat occupancy of a cryptic snake, Bitis atropos

    Understanding species’ habitat use is fundamental for conservation and management. However, quantifying habitat use for small cryptic species is limited by imperfect detection during field surveys and the lack of habitat data at meaningful spatial scales. Topographically-derived habitat variables from digital elevation models (DEMs) have the potential to overcome these limitations. Here we used DEM-derived topographic variables as fine-scale proxies for abiotic conditions to study site-occupancy patterns of the berg adder (Bitis atropos), a small-bodied cryptic viper. We carried out seven repeated field surveys across 219 hectares in a mountainous protected area in north-eastern South Africa to estimate snake detection probability and occupancy using maximum likelihood methods. Although snakes occurred across a third of the surveyed habitat, they were only detected 40% of the time during the springtime when detection was highest. Results showed that these snakes preferred north-west facing, mid and upper slopes, which are exposed to afternoon sun and presumably higher ambient energy. Our results demonstrate the value of using DEM-derived topographic variables for ecological studies where habitat data are either unavailable or inappropriate, thereby providing valuable insights into habitat use of cryptic and difficult to detect species

    Phylogeography of the widespread African puff adder (Bitis arietans) reveals multiple Pleistocene refugia in southern Africa

    Evidence from numerous Pan-African savannah mammals indicates that open-habitat refugia existed in Africa during the Pleistocene, isolated by expanding tropical forests during warm and humid interglacial periods. However, comparative data from other taxonomic groups are currently lacking. We present a phylogeographic investigation of the African puff adder (Bitis arietans), a snake that occurs in open-habitat formations throughout sub-Saharan Africa. Multiple parapatric mitochondrial clades occur across the current distribution of B. arietans, including a widespread southern African clade that is subdivided into four separate clades. We investigated the historical processes responsible for generating these phylogeographic patterns in southern Africa using species distribution modelling and genetic approaches. Our results show that interior regions of South Africa became largely inhospitable for B. arietans during glacial maxima, whereas coastal and more northerly areas remained habitable. This corresponds well with the locations of refugia inferred from mitochondrial data using a continuous phylogeographic diffusion model. Analysis of data from five anonymous nuclear loci revealed broadly similar patterns to mtDNA. Secondary admixture was detected between previously isolated refugial populations. In some cases, this is limited to individuals occurring near mitochondrial clade contact zones, but in other cases, more extensive admixture is evident. Overall, our study reveals a complex history of refugial isolation and secondary expansion for puff adders and a mosaic of isolated refugia in southern Africa. We also identify key differences between the processes that drove isolation in B. arietans and those hypothesized for sympatric savannah mammals
